Risk factors for hepatocellular carcinoma in cirrhosis due to nonalcoholic fatty liver disease: A multicenter, case-control study

AIM To identify risk factors associated with hepatocellular carcinoma(HCC), describe tumor characteristics and treatments pursed for a cohort of individuals with nonalcoholic steatohepatitis(NASH) cirrhosis. MeTHODS We conducted a retrospective case-control study of a well-characterized cohort of patients among five liver transplant centers with NASH cirrhosis with(cases) and without HCC(controls).ReSULTS Ninety-four cases and 150 controls were included. Cases were significantly more likely to be male than controls(67% vs 45%, P er age(61.9 years vs 58 years, P = 0.002). In addition, cases were more likely to have had complications of end stage liver disease(83% vs 71%, P = 0.032). On multivariate analysis, the strongest association with the presence of HCC were male gender(OR 4.3, 95%CI: 1.83-10.3, P = 0.001) and age(OR = 1.082, 95%CI: 1.03-1.13, P = 0.001). Hispanic ethnicity was associated with a decreased prevalence of HCC(OR = 0.3, 95%CI: 0.09-0.994, P = 0.048). HCC was predominantly in the form of a single lesion with regional lymph node(s) and distant metastasis in only 2.6% and 6.3%, respectively. Fifty-nine point three percent of individuals with HCC underwent locoregional therapy and 61.5% underwent liver transplantation for HCC. CONCLUSION Male gender, increased age and non-Hispanic ethnicity are associated with HCC in NASH cirrhosis. NASH cirrhosis associated HCC in this cohort was characterized by early stage disease at diagnosis and treatment with locoregional therapy and transplant.

Rice Hull Mulch Affects Germination of Bittercress and Creeping Woodsorrel in Container Plant Culture

Mulches are commonly used to control weeds in container nursery crops, especially in sites where preemergence herbicides are either not labeled or potentially phytotoxic to the crop. Parboiled rice hulls have been shown to provide effective weed control when applied 1.25 to 2.5 cm deep over the container substrate surface. The objective of this research was to determine if weed seed placement, above or below the mulch layer, affects flexuous bittercress or creeping woodsorrel establishment. Seeds of both species were placed either above or below rice hull mulch layers 0, 0.6, 1.3, or 2.5 cm deep in nursery containers with a 80 pine bark: 20 sphagnum peat moss substrate. establishment of both weeds decreased with increasing mulch depth. establishment of both species was generally greater from beneath the mulch compared to when seed were applied above the mulch. Light penetration through varying depths of rice hulls was determined with a spectroradiometer. Photosynthetically active radiation (PAR) decreased exponentially with increasing rice hull depth, and was less than 1 μmol·m-2·s-1 beneath depths greater than 1 cm. Germination of both species was determined in Petri dishes placed beneath varying densities of shade cloth. Flexuous bittercress germination responded quadratically to decreasing light level, but still germinated (13%) in complete darkness after 3 weeks. Creeping woodsorrel germination was not affected by light level and was high (92%) after 3 weeks. The role of light exclusion by rice hulls as a mechanism for controlling buried weed seed is discussed. Water retention immediately after irrigation, and for 24 hr following irrigation, was determined for a 2.5 cm layer of rice hulls, sphagnum peat moss, and pine bark. Rice hulls retained less water, and dried more quickly than peat moss or pine bark. Background： Attention deficit hyperactivity disorder （ADHD） is a common childhood disorder that affects approximately 11% of children in the United States. Research supports that a single session of exercise benefits cognitive performance by children, and a limited number of studies have demonstrated that these effects can also be realized by children with ADHD. The purpose of this study was to examine the effect of acute exercise on cognitive performance by children with and without ADHD. Methods： Children with and without ADHD were asked to perform cognitive tasks on 2 days following treatment conditions that were assigned in a random, counterbalanced order. The treatment conditions consisted of a 30-min control condition on 1 day and a moderate intensity exercise condition on the other day. Results： exercise significantly benefited performance on all three conditions of the Stroop Task, but did not significantly affect performance on the Tower of London or the Trail Making Test. Conclusion： children with and without ADHD realize benefits in speed of processing and inhibitory control in response to a session of acute exercise, but do not experience benefits in planning or set shifting.

Screening for asymptomatic chlamydia and gonorrhea in adolescent males in an urban pediatric emergency department

AIM To determine the prevalence of Chlamydia trachomatis(CT) and Neisseria gonorrhea(GC) in young men seeking care in the emergency department(eD) for non-sexually transmitted infection(STI) related *** This was a prospective, cross-sectional study in an urban eD. The main outcome was the rate of positive CT and GC on urine nucleic acid amplification testing in males aged 16-21 presenting with non-STI related complaints. ReSULTS Two hundred and eighty-four patients were enrolled, 271 were included in the final data analysis [age range 16-21, median: 18(quartiles 16-18, 19-21)]. Overall, 17(6.3%, 95%CI: 4%-10%) tested positive for CT and 0%(95%CI: 0%-2%) were found to have GC. The proportion of sexually active subjects was 71%(95%CI: 65%-76%) and 2%(95%CI: 0.6%-4%) reported sex with men. Previous STI testing was reported in 46%(95%CI: 43%-54%) and 13%(95%CI: 8%-20%) of those patients previously tested had a history of STI. Of the patients who tested positive for CT in the eD, 88%(95%CI: 64%-98%) were successfully followed up. CONCLUSION The prevalence of CT infection found by screening was 6.3%. Screening and follow-up from the eD was successful. The findings justify routine STI screening in male adolescents presenting to the eD with non-STI related complaints.
