Physical activity after total knee arthroplasty: A critical review

Total knee arthroplasty(TKA) is the most commonly performed elective surgery in the United States. TKA typically improves functional performance and reduces pain associated with knee osteoarthritis. Little is known about the influence of TKA on overall physical activity levels. Physical activity, defined as "any bodily movement produced by skeletal muscles that results in energy expenditure", confers many health benefits but typically decreases with endstage osteoarthritis. The purpose of this review is to describe the potential benefits(metabolic, functional, and orthopedic) of physical activity to patients undergoing TKA, present results from recent studies aimed to determine the effect of TKA on physical activity, and discuss potential sources of variability and conflicting results for physical activity outcomes. Several studies utilizing self-reported outcomes indicate that patients perceive themselves to be more physically active after TKA than they were before surgery. Accelerometry-based outcomes indicate that physical activity for patients after TKA remains at or below pre-surgical levels. Several different factors likely contributed to these variable results, including the use of different instruments, duration of follow-up, and characteristics of the subjects studied. Comparison to norms, however, suggests that daily physical activity for patients following TKA may fall short of healthy agematched controls. We propose that further study of the relationship between TKA and physical activity needs to be performed using accelerometry-based outcome measures at multiple post-surgical time points.

Novel biologics that redirect cytotoxic T lymphocytes （CTLs） to kill tumor cells bearing a tumor associated antigen hold great promise in the clinic. However, the ability to safely and potently target CD3 on CTL toward tumor associated antigens （TAA） expressed on tumor cells remains a challenge of both technology and biol- ogy. Herein we describe the use of a Half DVD-Ig format that can redirect CTL to kill tumor cells. Notably, Half DVD-Ig molecules that are monovalent for each speci- ficity demonstrated reduced non-specific CTL activation and conditional CTL activation upon binding to TAA compared to intact tetravalent DVD-Ig molecules that are bivalent for each specificity, while maintaining good drug like properties and appropriate PK properties.

The purpose of this study was to examine four urovirulence factors (UVFs) and their phylogenetic classes and compare their levels of resistance to antimicrobial drug classes in 159 canine uropathogenic e. coli (UPeC) isolates submitted to the Clinical bacteriology and Mycology laboratory of The University of Tennessee Veterinary Medical Center in 2007. Multiplex polymerase chain reaction was used to detect the following UVFs: cytotoxic necrotizing factor (cnf), hemolysin (hly), S-fimbrial adhesion gene (sfa), and Pilus associated with pyelonephritis gene G allele III (pap);polymerase chain reaction was used to determine phylogenetic group. In vitro susceptibility to antimicrobial classes was evaluated, and resistance was compared to UVF presence as well as phylogenetic class. UVFs were presented in 48% of UPeC and were negatively correlated with antimicrobial resistance. Of the 159 samples, the number of UVFs expressed per isolate was 0 = 82 (52%), 1 = 24 (15%), 2 = 2 (1%), 3 = 18 (11%), and 4 = 33 (21%). The following UVFs were expressed: sfa (33%), hly (24%), cnf (25%), and pap (18%). Presence of all four UVFs, hly, cnf, and sfa together, and sfa alone, was associated with less resistance (P lass B2 was the most common phylogenetic class and contained the highest number of UVFs (P ean antimicrobial resistance was the highest in class A (P e lowest in B2 (P enetic class A contained the lowest number of UVFs.

BACKGROUND Individuals with heart failure(HF) frequently experience limitations in mobility, but specific aspects of these limitations are not well understood. This study investigated the association of HF severity, based on the New York Heart Association(NYHA) classes, with digital mobility outcomes(DMOs) and handgrip strength in older inpatients with *** For this explorative analysis, hospital admission and discharge data from an ongoing, prospective cohort study were used. The sample included older participants with HF and a sub-sample of heart-healthy individuals. Participants were equipped with a wearable inertial measurement unit(IMU) system during mobility performance(balancing, sit-to-stand transfer,walking). We analyzed the association between 17 DMOs and HF severity with multiple linear regression *** The total sample included 61 older participants(65–97 years of age, 55.7% female). Of all DMOs, only sway path in a semi-tandem stance position(m/s^(2)) showed a relevant association with NYHA classes(admission: β =-0.28, P = 0.09;discharge:β =-0.39, P = 0.02). Handgrip strength showed a trend towards a significant association(admission: β =-0.15, P = 0.10;discharge:β =-0.15, P = 0.19).CONCLUSIONS This is to our best knowledge the first analysis on the association of HF severity and IMU-based DMOs. Sway path and handgrip strength may be the most promising parameters for monitoring mobility aspects in treatment of HF.

Reptile sex determination is attracting much attention because the great diversity of sex-determination and dosage compensation mechanisms permits us to approach fundamental questions about mechanisms of sex chromosome turnover. Recent studies have made significant progress in better understanding diversity and conservation of reptile sex chromosomes, with however no reptile master sex determination genes identified. Here we describe an integrated genomics and cytogenetics pipeline,combining probes generated from the microdissected sex chromosomes with transcriptome and genome sequencing to explore the sex chromosome diversity in non-model Australian reptiles. We tested our pipeline on a turtle, two species of geckos, and a monitor lizard. Genes identified on sex chromosomes were compared to the chicken genome to identify homologous regions among the four species. We identified candidate sex determining genes within these regions, including conserved vertebrate sex-determining genes pdgfa,pdgfra amh and wt1, and demonstrated their testis or ovary-specific expression. All four species showed gene-by-gene rather than chromosome-wide dosage compensation. Our results imply that reptile sex chromosomes originated by independent acquisition of sex-determining genes on different autosomes,as well as translocations between different ancestral macro-and *** discuss the evolutionary drivers of the slow differentiation and turnover of reptile sex chromosomes.

The prone transpsoas approach is a relatively new technique to correct segmental kyphosis and global sagittal imbalance in a minimally invasive fashion. Here, we provide a detailed case report using the prone transpsoas approach to address adjacent segment disease and flatback deformity. This technique allows considerable restoration of segmental lordosis with lateral interbody placement and posterior decompression and fusion using a single position approach. Our experience with the surgical technique and the advantages and challenges unique to this approach are discussed.

AIM: To establish a novel, sensitive and high-throughput gelatinolytic assay to define new inhibitors and compare domain deletion mutants of gelatinase B/matrix metalloproteinase (MMP)-9. MeTHODS: Fluorogenic Dye-quenched (DQ)TM-gelatin was used as a substrate and biochemical parameters (substrate and enzyme concentrations, DMSO solvent concentrations) were optimized to establish a highthroughput assay system. Various small-sized libraries (ChemDiv, InterBioScreen and ChemBridge) of hetero-cyclic, drug-like substances were tested and compared with prototypic inhibitors. ReSULTS: First, we designed a test system with gelatin as a natural substrate. Second, the assay was validated by selecting a novel pyrimidine-2,4,6-trione (barbitu- rate) inhibitor. Third, and in line with present structural data on collagenolysis, it was found that deletion of the O-glycosylated region significantly decreased gelatinolytic activity (kcat/kM ± 40% less than full-length MMP-9). CONCLUSION: The DQTM-gelatin assay is useful in high-throughput drug screening and exosite targeting. We demonstrate that flexibility between the catalytic and hemopexin domain is functionally critical for gelatinolysis.

Fortification of pork loins with docosahexaenoic acid(DHA) and its effect on flavour

Pork is traditionally low in docosahexanoic acid （DHA, C22：6n-3） and deficient in omega-3 fats for a balanced human diet. DHA as triglycerides was commercially prepared from the microalgae Schizochytfium and injected into fresh pork loins. Treatments of a mixed brine control （CON）, 3.1% sunflower oil in mixed brine （SF） and a 3.1% DHA oil in mixed brine （DHA） were injected into pork loins at 10 mL/100 g and grilled at 205℃. After cooking, the CON and SF pork loins contained 0.03 to 0.05 mg DHA/g of pork and the DHA injected loins contained approximately 1.46 mg DHA/g. This also changed the fatty acid profile of omega-6：omega-3 from, 5 to 1 in the CON pork, to a ratio of 1.7 to 1 in DHA pork. The appearance, odor, oxidation rates and sensory taste, as judged by a trained panel, determined the DHA injected meat to be, ＇slightly desirable＇ and gave lower ＇off flavour＇ scores, relative to the CON and SF injected pork. Pork can be fortified with DHA oil to 146 mg/100 g serving, which would meet half the recommended daily omega 3 fatty acid requirements for adult humans and would be desirable in taste.
