Neurodevelopmental Outcomes of Extremely Premature Infants with Extremely Low and Very Low Birth Weight at Three Years of Age: A Retrospective Study

Introduction: The purpose of this retrospective study is to identify medical conditions impacting neurodevelopmental outcomes of extremely low birth weight and very low birth weight preterm infants at three years of age. Methods: Infants born in Banner Diamond Children’s University Medical Center, receiving services in the Newborn Intensive Care Unit, and attending Neonatal Developmental Follow-Up Clinic were identified. Participants received developmental assessment and follow-up from August 2012 through December 2018. Relevant clinical conditions during initial hospital stay and up to three years of age were obtained by reviewing medical and developmental records. Bayley Scales of Infant Toddler Development (Bayley III) was used to evaluate skill development at 6, 9, 12, 18, 24, 30, 36 months. Results: Data analysis did not reveal significant p-values;it did demonstrate that some predictor variables impact neurodevelopmental outcomes in cognitive, language and motor skill development. Conclusion: This retrospective study reports significant association between birth weight and low cognitive scores. Correlations were also found between gestational age and Total Language, and the longer an infant stayed in the NICU, the poorer the Total Language Scaled Scores at 8 to 12 months, 15 to 18 months, and 24 to 36 months. Birth weight was found to be the greatest predictor of poor motor scores.

Comparison of methods for deriving phenotypes from incomplete observation data with an application to age at puberty in dairy cattle

Corn and Soybean Growth as Affected by Wastewater-Derived Struvite-Phosphorus Sources and Irrigation Water Types

Struvite (MgNH4PO4·6H2O) produced synthetically from a stock solution of known phosphorus (P) and nitrogen (N) concentrations has been shown to be an effective, alternative fertilizer-P source for various crops, but little is known about the potential agronomic effectiveness of struvite created from an actual municipal wastewater source. The objective of this study was to evaluate the effects of soil [i.e., Creldon silt loam (Oxyaquic Fragiudalf) and Calloway silt loam (Aquic Fraglossudalf) series], fertilizer-P source [i.e., synthetically produced electrochemically precipitated struvite (ECSTsyn), real-wastewater-derived ECST (ECSTreal), chemically precipitated struvite (CPST), monoammonium phosphate (MAP), and an unamended control (UC)], and irrigation water type (i.e., tapwater and struvite-removed wastewater) on corn (Zea mays L.) and soybean [Glycine max (L.) Merr.] growth and N, P, and magnesium (Mg) uptake in a 60-day, greenhouse potted-plant study. Crop growth and N, P, and Mg uptakes for the struvite treatments (i.e., CPST, ECSTsyn, and ECSTreal) were generally similar to or at least 1.2 times greater than MAP. The ECSTsyn material commonly had up to five times greater N, P, and Mg uptake in corn and soybean than any other fertilizer-P source. Struvite-removed wastewater resulted in at least 1.3 times lower dry matter and N, P, and Mg uptake than tapwater. Similar corn and soybean results from the struvite fertilizers among the various soil-water type combinations compared to MAP suggest that struvite generates similar crop responses as at least one widely used, commercially available, multi-nutrient fertilizer-P source.

Robotic Knee Tracking Control to Mimic the Intact Human Knee Profile Based on Actor-Critic Reinforcement Learning

We address a state-of-the-art reinforcement learning(RL)control approach to automatically configure robotic pros-thesis impedance parameters to enable end-to-end,continuous locomotion intended for transfemoral amputee ***,our actor-critic based RL provides tracking control of a robotic knee prosthesis to mimic the intact knee *** is a significant advance from our previous RL based automatic tuning of prosthesis control parameters which have centered on regulation control with a designer prescribed robotic knee profile as the *** addition to presenting the tracking control algorithm based on direct heuristic dynamic programming(dHDP),we provide a control performance guarantee including the case of constrained *** show that our proposed tracking control possesses several important properties,such as weight convergence of the learning networks,Bellman(sub)optimality of the cost-to-go value function and control input,and practical stability of the human-robot *** further provide a systematic simulation of the proposed tracking control using a realistic human-robot system simulator,the OpenSim,to emulate how the dHDP enables level ground walking,walking on different terrains and at different *** results show that our proposed dHDP based tracking control is not only theoretically suitable,but also practically useful.

Shortened dual antiplatelet therapy in contemporary percutaneous coronary intervention era

Percutaneous coronary intervention with stenting is followed by a duration of dual antiplatelet therapy(DAPT)to reduce stent thrombosis and avoid target lesion *** period of DAPT recommended in international guidelines following drug-eluting stent implantation is 12 mo for most patients with acute coronary syndrome,and 6 mo for patients with chronic coronary syndrome or high bleeding *** new generation of drug-eluting stents have metallic platforms with thinner struts,associated with significantly less stent *** DAPT has been investigated with these stents,with evidence from randomised clinical trials for some individual stents showing non-inferior safety and efficacy *** has to be balanced by the effect of DAPT on secondary prevention of systemic cardiovascular disease especially in high-risk *** review will outline the current evidence for individual stents with regards to DAPT duration for both acute coronary syndrome and chronic coronary syndrome and discuss further directions for research and personalised medicine in this contemporary percutaneous coronary intervention era.

Direct heuristic dynamic programming based on an improved PID neural network

In this paper, an improved PID-neural network （IPIDNN） structure is proposed and applied to the critic and action networks of direct heuristic dynamic programming （DHDP）. As one of online learning algorithm of approximate dynamic programming （ADP）, DHDP has demonstrated its applicability to large state and control problems. Theoretically, the DHDP algorithm requires access to full state feedback in order to obtain solutions to the Bellman optimality equation. Unfortunately, it is not always possible to access all the states in a real system. This paper proposes a solution by suggesting an IPIDNN configuration to construct the critic and action networks to achieve an output feedback control. Since this structure can estimate the integrals and derivatives of measurable outputs, more system states are utilized and thus better control performance are expected. Compared with traditional PIDNN, this configuration is flexible and easy to expand. Based on this structure, a gradient decent algorithm for this IPIDNN-based DHDP is presented. Convergence issues are addressed within a single learning time step and for the entire learning process. Some important insights are provided to guide the implementation of the algorithm. The proposed learning controller has been applied to a cart-pole system to validate the effectiveness of the structure and the algorithm.

BACKGROUND:Pharmacology and toxicology are core content knowledge for *** students should demonstrate understanding of general pharmacology and basic treatment of *** objective of this study was to measure the knowledge of the 4th-year medical students(MS4)on these topics over 3 ***:A multiple-choice exam(15 questions)was administered to MS4 students in spring of 2010,2011,and *** were developed by medical toxicologists to evaluate basic knowledge in three areas:pharmacologic effects(PE),treatment of poisoning(TOP),and pharmacokinetics(PK).The students were grouped by intended specialtiesinto pharmacologic intense(anesthesia,emergency medicine,internal medicine,pediatrics,and psychiatry),less pharmacologic intense specialties(dermatology,OB/GYN,ophthalmology,pathology,physical medicine and rehabilitation,radiology,and surgery)and by completion of a pharmacology or toxicology *** group scores were compared using ***:Totally 332 of 401(83%)students completed the *** scores were stable over the three years,higher for students completing a toxicology rotation and for students entering a pharmacologically intense ***:The external validity is limited to a single medical school with incomplete participation and content was limited by the survey *** results over the three-year period and correlation of performance with completing a toxicology rotation and intent to enter a pharmacology intensive specialty suggest this survey may correlate with toxicology *** of required core courses focused on toxicology may improve core content knowledge in fourth year medical students.

We are currently faced with the question of how the CoV-2 severity may change in the years *** analysis of immunological and epidemiological data on endemic human coronaviruses(HCoVs)shows that infection-blocking immunity wanes rapidly,but disease-reducing immunity is *** model,incorporating these components of immunity,recapitulates both the current severity of CoV-2 and the benign nature of HCoVs,suggesting that once the endemic phase is reached and primary exposure is in childhood,CoV-2 may be no more virulent than the common cold.

近似动态规划的基本思想是通过近似计算代价函数,从而避免动态规划中的"维数灾"问题。随机选取初值使得近似动态规划方法需要多次的学习才能最终收敛,极大地限制了在实际系统中的应用。针对上述问题,提出一种基于改进PID神经网络的直接启发式动态规划算法,将初始执行网络与PID控制器之间建立起一种等价关系,因此可以利用已经设计好的PID控制器来指导其初值选取,从而使算法收敛性大大提高。改进的神经网络与常规PID神经网络相比,结构简单且具有更好的扩展性,性能上具有更强的鲁棒性。对4机2区系统的静止无功补偿器附加阻尼控制进行仿真测试,仿真结果表明基于改进PID神经网络的直接启发式动态规划算法和初值选取方法的有效性,并且在部分状态反馈和延时两种情况下有着很好的控制效果。
