One-year follow-up study of Helicobacter pylori eradication rate with ^(13)C-urea breath test after 3-d and 7-d rabeprazole-based triple therapy

AIM: To investigate the long-term role of a 3-d rabeprazole-based triple therapy in patients with Helicobacter pylori (H pylori)-infected active peptic ulcers. METHODS: We prospectively studied 115 consecutive patients with H pylori-infected active peptic ulcers. H pylori infection was confirmed if any two of H pylori DNA, histology, and rapid urease test were positive. Patients were assigned to either an open-labeled 3-d course of oral amoxicillin 1 000 mg b.i.d., clarithromycin 500 mg b.i.d., and rabeprazole 20 mg b.i.d., or 7-d course of oral amoxicillin 1 000 mg b.i.d., clarithromycin 500 mg b.i.d., and rabeprazole 20 mg b.i.d. Subsequently, all patients received oral rabeprazole 20 mg once daily until the 8th wk. Three months after therapy, all patients were followed-up endoscopically for the peptic ulcer, H pylori DNA, histology, and rapid urease test. One year after therapy, H pylori infection was tested using the 13C-urea breath test. RESULTS: The ulcer healing rates 3 mo after therapy were 81.0% vs 75.4% for the 3-d and 7-d groups [intention-to-treat (ITT) analysis, P=0.47] respectively, and 90.4% vs 89.6% for the 3-d and 7-d groups [per-protocol (PP) analysis, P=0.89] respectively. The eradication rates 3 mo after therapy were 75.9% vs 73.7% for the 3-d and 7-d groups (ITT, P=0.79) respectively, and 84.6% vs 87.5% for the 3-d and 7-d groups (PP, P=0.68) respectively. One year after therapy, seventy-five patients returned to receive the 13C-urea breath test, and the eradication rates were 78.4% vs 81.6% in 3-d and 7-d groups (PP, P=0.73) respectively. CONCLUSION: Our study showed the eradication rates against H pylori infection 3 and 12 mo after triple therapy were not different between the 3-d and 7-d rabeprazole-based groups. Therefore, the 3-d rabeprazole-based triple therapy may be an alternative treatment for peptic ulcers with H pylori infection.

Application of a wire-guided side-viewing duodenoscope in total esophagectomy with colonic interposition

Therapeutic endoscopic retrograde cholangiopancreatography (ERCP) is the mainstay treatment for bile duct disease. The procedure is difficult per se, especially when a side-viewing duodenoscope is used, and when the patient has altered anatomical features, such as colonic interposition. Currently, there is no consensus on the standard approach for therapeutic ERCP in patients with total esophagectomy and colonic interposition. We describe a novel treatment design that involves the use of a side-viewing duodenoscope to perform therapeutic ERCP in patients with total esophagectomy and colonic interposition. A gastroscope was initially introduced into the interposed colon and a radio-opaque standard guidewire was advanced to a distance beyond the papilla of Vater, before the gastroscope was withdrawn. A sideviewing duodenoscope was then introduced along the guidewire under fluoroscopic guidance. After cannulation into the papilla of Vater, endoscopic retrograde chol-angiography (ERC) revealed a filling defect (maximum diameter: 15 cm) at the distal portion of the common bile duct (CBD). This defect was determined to be a stone, which was successfully retrieved by a Dormia basket after complete sphincterotomy. With this treatment design, it is possible to perform therapeutic ERCP in patients with colonic interposition, thereby precluding the need for percutaneous drainage or surgery.

Gallstone ileus:Report of two cases and review of the literature

Gallstone ileus is a rare disease and accounts for 1%-4% of all cases of mechanical intestinal obstruction. It usually occurs in the elderly with a female predominance and may result in a high mortality rate. Its diagnosis is difficult and early diagnosis could reduce the mortality. Surgery remains the mainstay of treatment. We report two cases of gallstone ileus. The first was a 78-year old woman who had a 2-d history of vomiting and epigastralgia. Plain abdominal film suggested small bowel obstruction clinically attributed to adhesions. Later on, gallstone ileus was diagnosed by abdominal computed tomography (CT) based on the presence of pneumobilia, bowel obstruction, and an ectopic stone within the jejunum. She underwent emergent laparotomy with a one-stage procedure of enterolithotomy, cholecystectomy and fistula repair. The second case was a 76-year old man with a 1-wk history of epigastralgia. Plain abdominal film showed two round calcified stones in the right upper quadrant. Fistulography confirmed the presence of a cholecystoduodenal fistula and gallstone ileus was also diagnosed by abdominal CT. We attempted to remove the stones endoscopically, but failed leading to an emergent laparotomy and the same one-stage procedure as for the first case. The postoperative courses of the two cases were uneventful. Inspired by these 2 cases we reviewed the literature on the cause, diagnosis and treatment of gallstone ileus.

BACKGROUND Liver cancer is among the top five most common cancers globally. Lipid-lowering drugs such as statins can lower the risk of liver cancer, but may also cause liver damage. LipoCol Forte capsules(LFC), a red yeast rice product, have demonstrated significant antihypercholesterolemic effects and a good safety profile in clinical *** To evaluate whether LFC lowers the risk of liver cancer in adults in this propensity score-matched, nationwide, population-based cohort *** We used data from Taiwan’s National Health Insurance Research Database, which includes electronic medical records for up to 99.99% of Taiwan’s population. LFC users and LFC non-users were matched 1:1 by propensity scores between January 2010 and December 2017. All had followup data for at least 1 year. Statistical analyses compared demographic distributions including sex, age, comorbidities, and prescribed medications. Cox regression analyses estimated adjusted hazard ratios(aHRs) after adjusting for potential *** We enrolled 33231 LFC users and 33231 non-LFC users(controls). No significant differences between the study cohorts were identified regarding comorbidities and medications [standardized mean difference(SMD) < 0.05]. At follow-up, the overall incidence of liver cancer was significantly lower in the LFC cohort compared with controls [aHR 0.91;95% confidence interval(CI): 0.86-0.95;P < 0.001]. The risk of liver cancer was significantly reduced in both females(aHR 0.87;95%CI: 0.8-0.94;P < 0.001) and males(aHR 0.93;95%CI: 0.87-0.98;P < 0.01) in the LFC cohort compared with their counterparts in the non-LFC cohort. The antitumor protective effects applied to patients with comorbidities(including hypertension, ischemic stroke, diabetes mellitus, hyperlipidemia, hepatitis B infection and hepatitis C infection). Fecal calprotectin correlated with endoscopic remission for Asian inflammatory bowel disease patients

AIM: To evaluate the correlation between fecal calprotectin(f C), C-reactive protein(CRP), and endoscopic disease score in Asian inflammatory bowel disease(IBD) ***: Stool samples were collected and assessed for calprotectin levels by Quantum Blue Calprotectin High Range Rapid test. Crohn's disease endoscopic index of severity(CDEIS) and ulcerative colitis endoscopic index of severity(UCEIS) were used for endoscopic lesion scoring. RESULTS: A total of 88 IBD patients [36 patients with Crohn's disease(CD) and 52 with ulcerative colitis(UC)] were enrolled. For CD patients, f C correlated with CDEIS(r = 0.465, P = 0.005) and CRP(r = 0.528, P = 0.001). f C levels in UC patients correlated with UCEIS(r = 0.696, P < 0.0001) and CRP(r = 0.529, P = 0.0005). Calprotectin could predict endoscopic remission(CDEIS < 6) with 50% sensitivity and 100% specificity(AUC: 0.74) in CD patients when using 918 μg/g as the cutoff. When using 191 μg/g as the cut-off in UC patients, calprotectin could be used for predicting endoscopic remission(UCEIS < 3) with 88% sensitivity and 75% specificity(AUC: 0.87). CONCLUSION: f C correlated with both CDEIS and UCEIS. f C could be used as a predictor of endoscopic remission for Asian IBD patients.

Eco-Environmental Impact Assessment of Pre-Leisure Beach Nourishment on the Benthic Invertebrate Community at Anping Coast

In recent years, owing to global warming and the rising sea levels, beach nourishment and groin building have been increasingly employed to protect coastal land from shoreline erosion. These actions may degrade beach habitats and reduce biomass and invertebrate density at sites where they were employed. We conducted an eco-environmental evaluation at the Anping artificial beach-nourishment project area. At this site, sand piles within a semi-enclosed spur groin have been enforced by use of eco-engineering concepts since 2003. Four sampling sites were monitored during the study period from July 2002 to September 2008. The environmental impact assessment and biological investigations that we conducted are presented here. The results from this study indicate that both biotic （number of species, number of individual organisms, and Shannon-Wiener diversity） and abiotic parameters （suspended solids, biological oxygen demand, chemical oxygen demand, dissolved inorganic nitrogen, dissolved inorganic phosphorus, total phosphorus, total organic carbon, median diameter, and water content） showed significant differences before and after beach engineering construction. Biological conditions became worse in the beginning stages of the engineering but improved after the restoration work completion. This study reveals that the composition of benthic invertebrates changed over the study period, and two groups of organisms, Bivalvia and Gastropoda, seemed to be particularly suitable to this habitat after the semi-enclosed artificial structures completion.

Refinement of circular-polarization based on multilayer film structure

Circular-polarization discrimination appears in many antennas＇ applications. A compensation approach based on multilayer film structure is proposed to improve the axial ratio of the magnitude of the two perpendicular modes of the lump ports. The goal is to widen the beamwidth of radiation that has an axial ratio less than 3 dB and thus reducing the complexity at the receiver. A transfer matrix method was developed to represent the multilayer film and characterize its performance. Simulation using high frequency structure simulator shows that a crossed dipole, as an example, can achieve a beamwidth of more than 30° at the frequency of 12.45 GHz after compensation. Finally, conclusions and future work about this compensation method are presented.

Identifying Indicator Species in Habitats Created by Coastal Structures

In this paper, three approachs were developed to find indicator species from the habitat created by coastal structures. These approachs consist of a model of species co-occurrence probability, a model of k-environmental factor probability and a composite model. Simultaneously, a case study was conducted in Hsinchu Fishing Port of north-western Taiwan. Based on the aforementioned models, three primary producer species, Ahnfeltiopsis flabelliformis, Chondrus ocellatus and Sarcodia montagneana, were chosen as the indicator species which had the highest co-occurrence probabilities and showed greater tolerance to more critical environment. It is imperative to understand how the three species under particular co-occurring conditions and environmental factors influence the composition of sessile assemblages in coastal water. The results indicate that for the purpose of increasing biodiversity, these models are feasible to find indicator species of artificial structures, and to help make it possible to design coastal structures based on biological considerations. This study provides an innovative approach for further advanced application in the artificial habitat of coast management.

Microdomain atomic structure of Zr_(50)Pd_(40)Al_(10) metallic glasses and its formation mechanism

Zr-based Zr_50Pd_40Al_10 metallic glasses has not only crystalline phases of about 5 nm in diameter but also amorphous phases. In this work, the radial distribution functions(RDFs) of amorphous structure of Zr_50Pd_40Al_10 metallic glasses were firstly measured by electron diffraction, and then Reverse Monte Carlo(RMC) optimization accompanied by density functional theory(DFT) calculations. The amorphous structure has not only short-range order but also good medium-range order. In the RDFs of its amorphous structure, the first and the second peaks are located at 2.96 ? and 4.79 ?, respectively. Partial radical distribution functions(PRDFs) show that the contributions of the first and the second nearest-neighbor distances of various atom pairs to the G(r) peak values, and the first nearest-neighbor distances of Pd–Zr and Zr–Zr atom pairs are the sources of main G(r) peak values between 2? and 6?. The competition mechanism for generating the Pd_25Zr_55 Al_20 amorphous phase and the intermetallic crystalline phase Pd_11Zr_9 is associated with the differences of atomic radius, the proportion, and the melting point of different atoms, as well as the heat of mixing between atoms, leading to an equilibrium state of the two phases. Accordingly, a composite system with intertwined nanocrystals and amorphous phases is in turn formed, and improves the stability of the material.

Cavitation is often triggered when the fluid pres- sure is lower than the vapor pressure at a local thermo- dynamic state. The present article reviews recent progress made toward developing modeling and computational strat- egies for cavitation predictions under both isothermal and cryogenic conditions, with an emphasis on the attached cav- ity. The review considers alternative cavitation models along Reynolds-averaged Navier-Stokes and very lager eddy simu- lation turbulence approaches to ensure that the computational tools can handle flows of engineering interests. Observing the substantial uncertainties associated with both modeling and experimental information, surrogate modeling strategies are reviewed to assess the implications and relative impor- tance of the various modeling and materials parameters. The exchange between static and dynamic pressures under the influence of the viscous effects can have a noticeable impact on the effective shape of a solid object, which can impact the cavitation structure. The thermal effect with respect to evaporation and condensation dynamics is examined to shed light on the fluid physics associated with cryogenic cav- itation. The surrogate modeling techniques are highlighted in the context of modeling sensitivity assessment. Keywords
