Sojucktang (SJT) has long been used for the treatment of endometrial diseases in Korea. However, the mechanisms responsible for the SJT-induced apoptosis in endometrial cancer cells remain unclear. In the present study, SJT was demonstrated to show cytotoxic effect and induce apoptotic cell death via mitochondrial regulation in KLE endometrial cancer cells. Linderae Radix, Glycyrrhizae Radix, Zedoariae Rhizoma, Trogopterorum Faeces and Agelicae Gigantis Radix were found to be the potent constituent herbs of SJT to significantly decrease the viability of KLE cells by a tetra zolium salt (XTT) assay. Apoptotic bodies were observed in SJT-treated KLE cells by 4′-6-diamidino-2-phenylindole (DAPI) and TdT-mediated-dUTP nick-end labeling (TUNEL) assay. SJT also increased sub-G1 DNA contents of the cell cycle undergoing apoptosis in a dose-dependent manner. Furthermore, it was observed that SJT activated caspase-3 and cleaved poly (ADP-ribose) polymerase (PARP), and decreased mitochondrial membrane potential in a dose-dependent manner. Taken together, this study shows that SJT exerts anti-tumor activity against KLE endometrial cancer cells via mitochondrial dependent apoptosis induction.

AIM:To investigate the efficacy of hepatic arterial infusion chemotherapy(HAIC) using floxuridine(FUDR) in patients with advanced hepatocellular carcinoma(HCC) confined to the ***:Thirty-four patients who had advanced HCC with unresectability or unsuccessful previous therapy in the absence of extrahepatic metastasis were treated with intra-arterial FUDR chemotherapy at ourhospital between March 2005 and May *** the 34 patients,9 patients were classified as Child class C,and 18 patients had portal vein tumor thrombus(PVTT).One course of chemotherapy consisted of continuous infusion of FUDR(0.3 mg/kg during day 1-14) and dexamethasone(10 mg on day 1,4,7 and 11),and this treatment was repeated every 28 ***:Two patients(5.9%) displayed a complete response,and 12 patients(35.3%) had a partial *** tumor control rate was 61.8%.The median overall survival times were 15.3 mo,12.4 mo and 4.3 mo for the patients who were classified as Child class A,Child class B and Child class C,respectively(P = 0.0392).The progression-free survival was 12.9 mo,7.7 mo and 2.6 mo for the patients who were classified as Child class A,Child class B and Child class C,respectively(P = 0.0443).The cumulative survival differed significantly according to the Child-Pugh classification and the presence of *** addition to hepatic reserve capacity and PVTT,the extent of HCC was an independent factor in determining a poor *** most common adverse reactions to HAIC were mucositis,diarrhea and peptic ulcer disease,but most of these complications were improved by medical treatment and/or a delay of ***:The present study demonstrates that intra-arterial FUDR chemotherapy is a safe and effective treatment for advanced HCC that is recalcitrant to other therapeutic modalities,even in patients with advanced cirrhosis.

Objective Cardiovascular complications contribute to postoperative morbidity and mortality in elderly hip fracture patients. Limited data are available regarding which preoperative risk factors predict cardiovascular course following hip fracture surgery （HFS）. We used high sensitive troponin I （hs-TnI） assays and clinical parameters to identify preoperative risk factors associated with major adverse cardiac events （MACE） in elderly hip frac^u＇e patients. Method From August 2014 to November 2016, 575 patients with hip fracture were enrolled in a retrospective, single-center registry. A total of 262 of these patients underwent HFS and hs-TnI assays. MACE was defined as postoperative all-cause deaths, heart failure （HF）, new-onset atrial fibrillation （AF）, myocardial infarction （MI） and cardiovascular re-hospitalization that occurred within 90 days postoperative. Results Of 262 HFS patients, MACE developed following HFS in 65 （24.8%）. Patients with MACE were older and had higher rates of renal insufficiency, coronary artery disease, prior HF, low left ventricular ejection fraction and use of beta blockers; higher levels of hs-Tnl and N-terminal pro-brain natriuretic peptide （NT-proBNP） and higher revised cardiac risk index. A preoperative hs-TnI≥ 6.5 ng/L was associated with high risk of postoperative HF, new-onset AF and MACE. In multivariable analysis, pre-operative independent predictors for MACE were age 〉 80 years [adjusted hazard ratio （HR）： 1.79, 95% confident interval （CI）： 1.03-3.13, P = 0.04], left ventricular ejection fraction （LVEF）〈 50% （adjusted HR： 3.17, 95% CI： 1.47-5.82, P 〈 0.01） and hs-TnI 〉 6.5 ng/L （adjusted HR： 3.75, 95% CI： 2.09~5.17, P 〈 0.01）. Conclusion In elderly patients with hip fracture who undergo HFS, a preoperative assessment of hs-TnI may help the risk refinement of cardiovascular complications.

BACKGROUND The role of multidetector computed tomography(MDCT)in patients with acute infectious colitis is still *** To examine the usefulness of MDCT in distinguishing the etiology of acute infectious *** Overall,244 patients who met the criteria for acute infectious colitis and visited the Hospital from February 2015 to 2018 were retrospectively enrolled and divided into two groups(bacterial:204,viral:40)according to causes of acute colitis,based on stool *** MDCT parameters,including wall thickening,submucosal edema,mucosal enhancement,serosa involvement,empty colon sign,small bowel involvement,comb sign,continuous distribution,accordion sign,mucosal thickening,and lymph node enlargement,were constructed in a blinded *** MDCT parameters of wall thickening(OR:13.60;95%CI:5.80–31.88;P<0.001),submucosal edema(OR:36.08;95%CI:13.54–96.13;P<0.001),mucosal enhancement(OR:22.55;95%CI:9.28–54.81;P<0.001),serosal involvement(OR:14.50;95%CI:3.33–63.23;P<0.001),empty colon sign(OR:6.68;95%CI: 2.44–18.32;P < 0.001), continuous distribution (OR: 24.09;95%CI: 9.38–61.90;P < 0.001), accordionsign (OR: 9.02;95%CI: 1.12–72.35;P = 0.038), mucosal thickening (OR: 46.41;95%CI: 10.38–207.51;P< 0.001), and lymph node enlargement (OR: 4.39;95%CI: 1.22–15.72;P = 0.023) were significantlyassociated with bacterial colitis. At least one positive finding in four CT outcomes (submucosaledema, mucosal enhancement, continuous distribution, mucosal thickening) in summer showed ahigh probability of bacterial colitis (sensitivity, 41.67;specificity, 92.50;OR: 24.95).CONCLUSIONMDCT provides many clues that can be useful in suggesting a specific etiology of acute infectiouscolitis.

An 87-year-old male with old myocardial infarction was referred to the hospital due to left femur neck fracture and intermittent dizziness. Significant 2:1 atrioventricular block was demonstrated on initial electrocardiogram (Figure 1). Transthoracic echocardiography showed reduced left ventricular function (LVEF = 47%) with regional wall motion abnormality of right coronary artery (RCA) and left circumflex coronary artery (LCx) artery. Coronary angiography showed patent previous proximal RCA and distal LCx stents. Temporary pacemaker was inserted initially and the patient was planned for permanent pacemaker insertion after the surgery.

AIM： To clarify the mechanism underlying the antidiabetic activities of cortex cinnamomi extract （CCE）. METHODS： To induce in vivo diabetes, mice were injected with streptozotocin （STZ） via a tail vein （100 mg STZ/kg body weight）. To determine the effects of CCE, mice were administered CCE twice daily for 7 d by oral gavage starting 1 wk before the STZ injection. Blood glucose and plasma insulin concentration were measured as an index of diabetes. Also, to induce cytotoxicity of RINm5F cells, we treated with cytokines （IL-1β （2.0 ng/mL） and IFN-γ （100 U/mL））. Cell viability and nitric oxide production were measured colorimetrically. Inducible nitric oxide synthase （iNOS） mRNA and protein expression were determined by RT-PCR and Western blotting, respectively. The activation of NF-κβ was assayed by using gel mobility shift assays of nuclear extracts. RESULTS： Treatment of mice with STZ resulted in hyperglycemia and hypoinsulinemia, which was further evidenced by immunohistochemical staining of islets. However, the diabetogenic effects of STZ were completely prevented when mice were pretreated with CCE. The inhibitory effect of CCE on STZ-induced hyperglycemia was mediated through the suppression of iNOS expression. In rat insulinoma RINm5F cells,CCE completely protected against interleukin-1β and interferon-y-mediated cytotoxicity. Moreover, RINm5F cells incubated with CCE showed significant reductions in interleukin-1β and interferon-y-induced nitric oxide production and in iNOS mRNA and protein expression, and these findings correlated well with in vivo observations. CONCLUSION： The molecular mechanism by which CCE inhibits iNOS gene expression appears to involve the inhibition of NF-κβ activation, These results reveal the possible therapeutic value of CCE for the prevention of diabetes mellitus progression,

Controlling the properties of piezoelectric thin films is a key aspect for designing highly efficient flexible electromechanical devices. In this stud）~ the crystallographic phenomena of PbZr1-xTixO3 （PZT） thin films caused by distinguished interfacial effects are deeply investigated by overlooking views, including not only an experimental demonstration but also ab initio modeling. The polymorphic phase balance and crystallinity, as well as the crystal orientation of PZT thin films at the morphotropic phase boundary （MPB）, can be stably modulated using interfacial crystal structures. Here, interactions with MgO stabilize the PZT crystallographic system well and induce the texturing influences, while the PZT film remains quasi-stable on a conventional A1203 wafer. On the basis of this fundamental understanding, a high-output flexible energy harvester is developed using the controlled-PZT system, which shows significantly higher performance than the unmodified PZT generator. The voltage, current, and power densities are improved by 556%, 503%, and 822%, respectively, in comparison with the previous flexional single-crystalline piezoelectric device. Finally, the improved flexible generator is applied to harvest tiny vibrational energy from a real traffic system, and it is used to operate a commercial electronic unit. These results clearly indicate that atomic-scale designs can produce significant impacts on macroscopic applications.

BACKGROUND： Previous studies have analyzed cerebral activation and lateralization of cognitive functions, as well as cerebellar function with reference to high-level cognitive processing. However, there has been very little research on systematization and diversification. In particular, there are no reports on cerebellar lateralization, although reliable results have been reported on cerebral lateralization. OBJECTIVE： This study analyzed cerebellar activation and lateralization in relation to verbal and visuospatial tasks using functional magnetic resonance imaging （fMRI）. DESIGN, TIME AND SETTING： A block design for fMRI observation was performed at the fMRI Laboratory, Brain Science Research Center, Korea Advanced Institute of Science and Technology from May 2006 to September 2008. PARTICIPANTS： Sixteen healthy, male, college students, aged （23.3 ± 0.5） years, and 16 healthy, male, college students, aged （21.5 ± 2.3） years, participated in the study, respectively. METHODS： Verbal and visuospatial tasks were presented while functional brain images were acquired using a 3T fMRI system. Verbal analogy testing required the subject to select the word with the same relationship as one of the given words. Verbal antonym testing required the subject to select the word with a different meaning among four words. Visuospatial tasks involved selecting a shape that corresponded to a given figure with four examples, as well as selecting a development figure of a diagram. MAIN OUTCOME MEASURES： Changes in cerebellar activation and lateralization between two cognition tasks. RESULTS： Bilateral hemisphere Iobules VI and IX, right hemisphere Iobule VIII, bilateral hemisphere Iobules Crus I, and vermis Iobule IV, V, and Vl were closely related to verbal tasks in comparison to visuospatial tasks. AIM To examine the association between the timing of endo-scopy and the short-term outcomes of acute variceal bleeding in cirrhotic *** This retrospective study included 274 consecutive patients admitted with acute esophageal variceal bleeding of two tertiary hospitals in Korea. We adjusted confounding factors using the Cox proportional hazards model and the inverse probability weighting （IPW） method. The primary outcome was the mortality of patients within 6 *** A total of 173 patients received urgent endoscopy （ i.e. , ≤ 12 h after admission）, and 101 patients received non-urgent endoscopy （〉 12 h after admission）. The 6-wk mortality rate was 22.5% in the urgent endoscopy group and 29.7% in the non-urgent endoscopy group, and there was no significant difference between the two groups before （P = 0.266） and after IPW （P = 0.639）. The length of hospital stay was statistically different between the urgent group and non-urgent group （P = 0.033）; however, there was no significant difference in the in-hospital mortality rate between the two groups （8.1%vs 7.9%, P = 0.960）. In multivariate analyses, timing of endoscopy was not associated with 6-wk mortality （hazard ratio, 1.297; 95% confdence interval, 0.806-2.089; P = 0.284）.CONCLUSION In cirrhotic patients with acute variceal bleeding, the timing of endoscopy may be independent of short-term mortality.
