Percutaneous endoscopic colostomy （PEC） is increasingly proposed as an alternative to surgery to treat various disorders, including acute colonic pseudo-obstruction, chronic intestinal pseudo-obstruction and relapsing sigmoid volvulus. We report on a severe complication that occurred two months after PEC placement. A 74-year-old man with a history of chronic intestinal pseudo-obstruction evolving since 8 years was readmitted to our hospital and received PEC to provide long-standing relief. The procedure was uneventful and greatly improved the patient＇s quality of life. Two months later, the patient developed acute stercoral peritonitis. At laparotomy, the colostomy flange was embedded in the abdominal wall but no pressure necrosis was found at the level of the colonic wall. This complication was likely related to inadvertent traction of the colostomy tube. Subtotal colectomy with terminal ileostomy was performed. We review the major features of 60 cases of PEC reported to date, including indications and complications.

Classical first order finite elements are well known to exhibit locking(both in shear and over the volumetric deformation) when integrated exactly. One of the most used finite element techniques to removed locking is the SRI(Selective Reduced Integration). However this technique is usually applied to remove the volumetric locking only. Reduced Integration(RI) technique used to remove all types of locking has the drawback of developing hourglassing mesh instabilities. EAS(Enhanced Assumed Strains) formalism enables us to have a locking-free response with weaker mesh instabilities. In this updated enhanced Lagrangian framework, hourglassing hardly appears. Thus there is no major need to use higher order spatial integration rules or stabilization techniques. The aim of this paper is to describe an updated EAS finite element formalism used to model the thermomechanical behavior of metal during processes such as sheet forming or impact. The features of this formalism used jointly with and updated Lagrangian approach and a hyperelastic anisothermal constitutive formulation will be described.

Managing familial pancreatic cancer(FPC)is challenging for gastroenterologists,surgeons and ***-risk individuals(HRI)for pancreatic cancer(PC)(FPC or with germline mutations)are a heterogeneous group of subjects with a theoretical lifetime cumulative risk of PC over 5%.Screening is mainly based on annual magnetic resonance imaging(MRI)and endoscopic ultrasound(EUS).The goal of screening is to identify early-stage operable cancers or high-risk precancerous lesions(pancreatic intraepithelial neoplasia or intraductal papillary mucinous neoplasms with high-grade dysplasia).In the literature,target lesions are identified in 2%-5%of HRI who undergo *** appears to provide better identification of small solid lesions(0%-46%of HRI)and chronicpancreatitis-like parenchymal changes(14%-77%of HRI),while MRI is probably the best modality to identify small cystic lesions(13%-49%of HRI).There are no specific studies in HRI on the use of contrast-enhanced harmonic *** can also be used to obtain tissue ***,there is still limited evidence on the accuracy of imaging procedures used for screening or agreement on which patients to *** cost-effectiveness of screening is also *** new EUS-related techniques,such as searching for DNA abnormalities or protein markers in pancreatic fluid,appear to be promising.

Racecadotril Efficacy in the Symptomatic Treatment of Adult Acute Diarrhoea: A Systematic Review and Meta-Analysis

The efficacy of racecadotril (RC), an intestinal antisecretory drug acting via an enkephalinase inhibition, was reviewed in paediatric acute diarrhoea but not yet in adults. Objective: To estimate the effectiveness of RC in the symptomatic treatment of acute diarrhoea in adults. Data Sources: A systematic review of MedLine, Cochrane Controlled Trials Register, DARE, and Embase (up to November 2013). Additional studies were identified by contacting clinical experts and the manufacturer. Study Selection and Appraisal: Randomized Controlled Trials performed in adults suffering from acute diarrhoea using RC in one treatment arm. Independent extraction of articles using predefined data fields, and methodological quality measurement assessment. All randomised trials performed in adults suffering from acute diarrhoea with RC as the studied group. Statistics: The main efficacy endpoint was diarrhoea duration defined as time to recovery compared between groups by survival techniques and converted into hazard ratio (HR). We exclusively used a random-effect meta-analytic model. Constipation proportion was the main safety endpoint, evaluated between treatments by the Relative Risks (RR). Results: Twelve randomised trials (2619 patients) met inclusion criteria. Duration of diarrhoea was much shorter in the RC group, the proportion of patients having recovered at any time of the treatment period was 65% higher in the RC group, compared with placebo (HR = 1.65 [1.38-1.97], p < 0.00001, n= 1001). Duration of diarrhoea was similar in the RC and loperamide groups (HR = 1.08 [0.95-1.22], p = 0.24, n = 1618). The proportions of constipated patients were similar in the RC and placebo groups 0.95 [0.24-3.68], p = 0.97), however, about 3 times more constipated patients were found in the loperamide group compared with the RC group (RR = 0.34 [0.22-0.51], p to placebo, RC is characterized by a clinically relevant earlier remission of diarrhoea. Mechanical Buckling Analysis of Composite Panels with/without Cutouts

A simplified analytical solution suitable for simple stacking sequences was developed using the Euler buck- ling theory, the structure＇s equations of equilibrium and laminate panel mathematical formulation. Comparing these results with numerical results reveals the accuracy of the method and even more, allows us to validate the nu- merical analysis. Therefore, two important results are obtained： a simplified analytical solution for the buckling problem and validation of the numerical results. Another important and novel finding is related to the influence of the angle ply orientation and of the cutouts, on the buckling load. Under symmetrical boundary conditions and loading case, rectangular panels with elliptical cutouts, give better results for 90~ oriented plies than for 0 oriented ones. With a compression load applied in the X direction, and with material properties 10 times better in X direction than in Y direction, the best results are obtained when the load is aligned with the Y direction associated to the ma- terial reference frame. Moreover, panels with cutouts seem to behave better than panels without cutouts under cer- tainply orientation angles.

In this paper, the behavior analysis of two cells chopper connected to a nonlinear load is reported. Thus, this is done in order to highlight the way to chaos. Furthermore, throughout the study of these dynamical behaviors of this complex switched system some basic dynamical properties, such as Poincare section, first return map, bifurcation diagram, power spectrum, and strange attractor are investigated. The system examined in Matlab-Simulink. Analyses of simulation results show that this system has complex dynamics with some interesting characteristics.

The objective of this study was to evaluate the capacity of the live yeast (LY) Saccharomyces cerevisiae in optimizing ruminal pH and in understanding its mode of action during induced acidosis in dairy cow. Two non-lactating cannulated cows were used and offered twice daily a control diet (CD) consisting of 51% corn silage and 49% concentrates or a LY diet (LYD) composed of CD supplemented with 4 g of LY per cow and per day. Measurements of pH and redox potential (Eh) were continuously made at 1 h interval during an experimental period of 9 h per day. Samples of ruminal fluid were also taken at 2 h intervals for analyses of volatile fatty acids (VFA) and lactic acid. Oxygen partial pressure (logPO2) in the ruminal milieu was calculated from the Nernst equation, using either O2-H2O or lactate-propionate redox couples. The results showed an increase of 0.2 unit in ruminal pH when LYD was fed, which was accompanied by a mean difference in Eh of -20 mV with respect to CD. The logPO2 decreased significantly by 0.8 log unit for LYD when compared to CD. Concentrations of VFA and proportion of propionate were higher with LYD (114.4 mM and 17.1% total VFA) compared to CD (102.4 mM and 15.4% total VFA). Proportion of butyrate decreased (from 15.8% to 14.2% total VFA). Lactate concentration decreased by 55% on average. It is proposed that the stabilization of ruminal pH (>6) is the outcome of the LY ability to scavenge oxygen. In doing so, it increased the reducing capacity of the milieu favoring the production of total VFA and depressing the level of lactate. The LY oxygen-scavenging ability was put forward to account for the transformation of lactate (pKa = 3.86) into propionate (pKa = 4.87) under reduced ruminal conditions.

Racecadotril in the Treatment of Acute Diarrhoea in Adults. An Individual Patient Data Based Meta-Analysis

Racecadotril is an antidiarrhoeal drug with a pure intestinal antisecretory mechanism of action. Aim: To assess racecadotril efficacy, whatever its dose, versus placebo in adult acute diarrhoea. Methods: Individual Patient Data meta-analysis following multilevel mixed models testing of the significance of the treatment effect adjusted for baseline covariates. Diarrhoea duration was the common main criteria. Results: Four randomized clinical trials (n = 669) were identified with raw data. The clinical global impression evaluated at baseline by the physician was found to be the essential predictor influencing the outcome. As compared to placebo, the 100 mg dose, the minimum effective dose, induced a 80% increase of the recovered patient proportion at anytime (Hazard Ratio = 1.8 [1.3, 2.5], p < 0.001), a 60% increase of the responder proportion i.e. recovery within 3 days (p < 0.001), a 47% reduction of abdominal pain and nausea and an overall 33% decrease of sick days (p < 0.001). In conclusion, as compared to placebo, racecadotril induced several significant effects, such as reducing the diarrhoea duration, the number of stools and associated symptoms, leading to less lost productivity.
