Background: Abdominal parietal hernia, a temporary or permanent exit of viscera through an anatomically pre-existing zone of weakness, is a frequent pathology in surgery. So, the management of emergency hernias surgery should include some complications most often up after 30 days of the operation. Aim: To analyze the factors contributing to morbidity and mortality after 30 days of emergency hernia surgery in adults in the surgical departments of Bujumbura hospitals. Methodology: This is a prospective study over a period of one year that included all hernias operated on in emergency from January 2022 to February 2023. Results: During the period, 251 patients were admitted to the operating room for abdominal parietal hernias, including 49 for emergency hernia surgery. There were 43 men (87.76%) and 6 women (12.24%), i.e. a sex ratio of 7.1. The average age was 49.6 years, with extremes of 18 and 84 years. The occupation of strength (farmer, labourer, mechanic, mason, mason’s helper) represented 75.51% of the cases. Inguino-scrotal hernia was preponderant (65.31%) followed by inguinal hernia (25.58%), umbilical hernia (4.08%);femoral hernia represented 4.08%. Hernial strangulation represented 89.80% and engorged hernia 10.20%. Morbidity was minor, 2.04% of complications (suppuration, hematoma, urinary retention). No deaths were found. Altemeir stage and occupation were statistically related to morbi-mortality of emergency hernia surgery in adults at 30 days postoperative (p = 0.0028 and p = 0.0284 respectively). Conclusion: Abdominal parietal hernias are frequent, dominated by groin hernias. The high frequency of strangulation calls for awareness of cold hernia cures.

Thermo-Mechanical Properties Study of Stabilized Soil Bricks to Sugar Cane Molasses and Cassava Starch Binders

The current study deals Swith thermo-mechanical properties of stabilized soil small bricks with the help of organic binders of sugar cane molasses and cassava starch. Different formulations of soil concrete have been suggested after the geotechnical characterization of samples of soil was taken. From these, it arises that the studied soil is the most plastically clay (of type A3) according to GTR classification. Samples made of small bricks and measured out at 4%, 6% and 8% of binders (molasses, starch or molasses + starch) have been warmed up to different temperatures (100°C, 150°C, 200°C and 250°C) for the rising of the thermic behavior under different conditions and submitted to crushing testings for the estimation of characteristic resistances to the compression. According to the mechanical behavior, we note an improvement of resistances for small bricks measured 4%, 6% and 8%, of molasses respectively of 32.44%, 32.06% and 23.43% against the value of reference for small bricks without molasses. In the same way, the binder (molasses + starch) also reveals an improvement of resistance to the compression of 13.27%, 26.17% and 26.17%. On the contrary, the stabilization with the starch binder did not bring a significative improvement. According to the thermic influence, the heating at 100°C of stabilized small bricks at 4%, 6% and 8% of molasses, reveals a significative improvement of resistances. Moreover, the stabilization with the starch reveals on the contrary a good behavior for heatings at 150°C and 250°C. In short, for the binder (molasses + starch), it is the heating at 200°C that shows some improvements of remarkable resistances. Different analyses of realized statistics also show the effectivity of obtained results. For all realized formulations, the measuring out at 6% of binders (molasses, or molasses + starch) seems as optimal in front of the best thermo-mechanical revealed properties.

Microbial starch-binding domains (SBD) and granule-bound starch synthase I (GBSSI) are proteins which are accumulated in potato starch granules. The efficiency of SBD and GBSSI for targeting active luciferase reporter proteins to granules during starch biosynthesis was compared. GBSSI or SBD sequences were fused to the N- or C-terminus of the luciferase (LUC) gene, via an artificial Pro-Thr encoding linker sequence. The genes were introduced into an amylose-free (amf) potato mutant. It appeared that SBD was superior to GBSSI as a targeting sequence, mainly because the luciferase retained higher activity in the SBD-containing fusion proteins than in the GBSSI-containing ones.

AIM: This study was designed to characterize the en-dothelin pathway in an immortalized human adenocarcino-ma-derived alveolar epithelial cell line (A549) and human umbilical vein endothelial cell line (HUVEC). METHODS: The release of BT-1 and big-ET-1 was measured in the incubation medium of both cell lines. The expression of mRNAs coding for the endothelin iso-forms (hppET-1, -2, -3), the endothelin converting enzymes (hECE-la, b, c, and d) and the hETA and hETB receptors was investigated using RT-PCR. The expression of ECE-1 mRNA in various human tissues and in A549 cells was investigated by Northern blot analysis and the subcellular localization of ECE-1 in A549 cells was investigated by immunoblotting using a polyclonal antibody. RESULTS: Under control conditions, HUVEC release both ET-1 and big-ET-1 (ratio 5 to 1) while in A549 cells the big-ET-1 levels were below the threshold of detection. Plasma concentration of Natriuretic Peptide (NP) is a valuable diagnostic tool for heart failure (HF). It can help rule out or confirm a diagnosis of HF based on symptoms, but its use is not clearly defined. NPs should be used in conjunction with physical examination and other diagnostic tests. However, it is important to note that several conditions besides the diagnosis of HF may cause NPs levels to be elevated. Additionally, there are situations when NP concentrations may be below diagnostic thresholds in [1]. This consensus statement aims to provide a straightforward diagnostic flowchart for clinicians in both the emergency department and outpatient settings to aid in diagnosing both acute and chronic HF. The diagnosis of acute HF can be ruled out with a BNP level of 100 pg/mL or NTproBNP level of 300 pg/mL, regardless of the patient’s age. To identify HF, a 3-level cut-off point based on the patient’s age is recommended. Chronic heart failure can be ruled out with a BNP level of 35 pg/mL or NTproBNP level of 125 pg/mL, regardless of thepatient’s age [1].
