Early plasmapheresis and rituximab for acute humoral rejection after ABO-compatible liver transplantation

Acute humoral rejection （AHR） is uncommon after ABO- compatible liver transplantation. Herein, we report two cases of AHR treated with plasmapheresis and rituximab in two ABO-compatible liver-transplant patients with preformed anti-human leukocyte antigen donor-specific antibodies. Patient 1 experienced a biopsy-proven AHR at day 10 post-transplant. She was treated by steroid pulses, and OKT3. Because of persisting signs of biopsy-proven AHR at day 26, she was treated by plasmapheresis and rituximab. Uver enzyme levels did not improve, and she died on day 41. Patient 2 experienced a biopsy-proven AHR on day 10 post-transplant. She was treated by steroid pulses, plasmapheresis, and rituximab. Liver enzymes returned to within normal range 18 d after diagnosis. Uver biopsies, at 3 and 9 mo post-transplant, showed complete resolution of AHR. We conclude that plasmapheresis should be started as soon as AHR is diagnosed, and be associated with a B-cell depleting agent. Rituximab may be considered as a first-line therapy.

Localized magnetization reversal processes in cobalt nanorods with different aspect ratios

We present results of the synthesis of cobalt nanorods using the polyol process and the mechanism of magnetization reversal. We show that the nucleation step is significantly dependent on the nature of the ruthenium chloride used as the nucleating agent. This allows varying the diameter and aspect ratio of the cobalt nanorods independently. Co nanorods with aspect ratio, mean diameter, and mean length in the ranges ARm =3-16, Din= 7-25 nm, and Lm=30-300 nm, respectively, were produced using this method. X-ray diffraction and electron microscopy showed that a strong discrepancy between the structural coherence and morphological aspect ratio can exist because of stacking faults. The coercivity of assemblies of different nanorods was systematically measured, and the highest values were obtained for the smallest diameter and the largest structural coherence length. Micromagnetic simulations were performed to account for the dependence of the coercive field on the diameter. An important observation is that simple coherent magnetization rotation models do not apply to these magnetic nano-objects. Even for very small diameters （Dm = 5-10 nm） well below the theoretical coherent diameter Dcoh（CO）= 24 nm, we observed inhomogeneous reversal modes dominated by nucleation at the rod edges or at structural defects such as stacking faults. We conclude that, in order to produce high-coercivity materials based on nanowires, moderate aspect ratios of 5-10 are sufficient for providing a structural coherence similar to the morphological aspect ratio. Thus, the first priority should be to avoid the formation of stacking faults within the Co nanowires.

Our goal is to decipher which DNA sequences are required for tissue-specific expression of epididymal genes. At least 6 epididymis-specific lipocalin genes are known. These are differently regulated and regionalized in the epididymis. Lipocalin 5 （Lcn5 or mE-RABP） and Lipocalin 8 （Lcn8 or mEP17） are homologous genes belonging to the epididymis-specific lipocalin gene cluster. Both the 5 kb promoter fragment of the Lcn5 gene and the 5.3 kb promoter fragment of the Lcn8 gene can direct transgene expression in the epididymis （Lcn5 to the distal caput and Lcn8 to the initial segment）, indicating that these promoter fragments contain important cis-regulatory element（s） for epididymisspecific gene expression. To define further the fragments regulating gene expression, the Lcn5 promoter was examined in transgenic mice and immortalized epididymal cell lines. After serial deletion, the 1.8 kb promoter fragment of the Lcn5 gene was sufficient for tissue-specific and region-specific gene expression in transgenic mice. Transient transfection analysis revealed that a transcription factor forkhead box A2 （Foxa2） interacts with androgen receptor and binds to the 100 bp fragment of the Lcn5 promoter between 1.2 kb and 1.3 kb and that Foxa2 expression inhibits androgen-dependent induction of the Lcn5 promoter activity. Immunohistochemistry indicated a restricted expression of Foxa2 in the epididymis where endogenous Lcn5 gene expression is suppressed and that the Foxa2 inhibition of the Lcn5 promoter is consistent with the lack of expression of Lcn5 in the corpus and cauda. Our approach provides a basic strategy for further analysis of the epididymal lipocalin gene regulation and flexible control of epididymal function. （Asian J Androl 2007 July; 9： 515-521）

Help received from relatives and services to satisfy needs of adults with severe mental disorders

Background: Few studies have considered the impact of demographic and clinical variables on help received respectively from services and relatives to satisfy needs of adults with severe mental disorders (SMD). Purposes: To identify major needs receiving help and, using the Andersen’s Behavioural Model of Health Service Use, to identify and compare predisposing, enabling and need factors associated with help given respectively by services and relatives. Methods: 352 adults with SMD from Montreal (Canada) were interviewed using six standardized instruments. Clinical records were consulted. Multiple linear regression analyses were processed to measure level of help received from relatives and services. Results: Factors significantly associated with help from relatives were: higher number of perceived needs;fewer diagnoses;better community functioning;being younger, and in a conjugal relationship;living autonomously;having social support and better quality of life;and, marginally, being an immigrant. Factors significantly associated with help from services were: higher number of perceived needs, better quality of life, supervised housing, adjustment disorder and schizophrenia. Conclusions: Help overall is insufficient to meet users’ needs. Services are more helpful than relatives, in particular regarding health needs. Comparatively to help from services, help from relatives is associated with predisposing factors like age, marital status and nationality.

The Proterozoic metasedimentary rocks of the Yaounde Group on the northern edge of the Congo Shield in Central Africa were investigated to understand their provenance and depositional ***,geochemistry,and field evidence helped to subdivide the metasediments into paragneiss,mica schist,chlorite schist,and quartzite which were derived from greywacke,shale,quartz arenite,litharenite *** are immature with some mature samples,moderately weathered and reworked Neo-and Post-Archean *** earth element signatures(Chondrite Eu/Eu^(*)≤1),enrichment of light rare earth elements over the heavy ones,and the La/Sc ratio(>0.7)are compatible with those of the intermediate and felsic sources from the upper continental *** metasediments were deposited in the continental arc setting and have evolved during Proterozoic times according to the Wilson cycle to form the West Gondwana including NE Brazil.

网络流量分布特性是众多网络应用的基础。提出一种网络流量特性的综合分析方法。首先对获取的流量样本进行时间尺度化处理,之后引入半马尔可夫模型描述网络流量特性,通过忙阈值和闲阈值的设定将网络流量划分为三种状态:忙、中间、空闲,研究各状态下的网络流量特性及各状态间的相互转换关系,然后建立了基于参数估计和非参数估计分布模型库,最后进行分布拟合。该方法适用于对所有协议的网络流量分析,比使用单一分布模型分析更合理。实例分析证明该方法是正确、有效的。

Chinese Patent, A Tentative Explanation of Various Strategies of Patenting

The recent increase in patent granted in China prompts for an analysis of the various strategies which are certainly developed by Chinese applicants in that field. An analysis （APA Automatic Patent Analysis） has been done in several fields which have different impacts on the Chinese economy. We chose four different subjects： solar panels; photovoltaic panels; fireworks; and honeysuckle. These areas go from deep international concern to only Chinese interest. We analyzed in the different ratios between Chinese priority patents, Chinese patents extended to other countries, Patent extended from other countries to China and Chinese utility models. The results show clearly that various directions are followed depending on the economic importance of the field and also the pressure of the competition inside and outside China. In this context the strategic dependence （number of patents extended to China by other countries） was examined for each of the fields. We also show that in the domain where the man power is important such as fireworks the number of utility models is important. This suggests that utility models are used to spread in that field minor innovations. In the fields such as honeysuckle many applicants are the same that the inventors and the involvement of universities larger than in other fields. These two fields more or less centered on Chinese domestic aspects only a few patents （not for Honeysuckle） are extended to other countries. The impact of foreign patents in these two fields is quasi nil. In other domains such as photovoltaic panel the triadic patents （extension to US JP EP） are examined its show that if some Chinese patents are extended to other countries there are also an important numbers of foreign patents extended to China. This also suggests that in these most critical subjects, China develops a sort of technological protection barrier from domestic patents. This underlines a strategy on attack and defense in that field.

Development of a standardized in vitro approach to evaluate microphysical,chemical,and toxicological properties of combustion-derived fine and ultrafine particles

Ultrafine particles represent a growing concern in the public health community but their precise role in many illnesses is still unknown. This lack of knowledge is related to the experimental difficulty in linking their biological effects to their multiple properties, which are important determinants of toxicity. Our aim is to propose an interdisciplinary approach to study fine(FP) and ultrafine(UFP) particles, generated in a controlled manner using a mini CAST(Combustion Aerosol Standard) soot generator used with two different operating conditions(CAST1 and CAST3). The chemical characterization was performed by an untargeted analysis using ultra-high resolution mass spectrometry. In conjunction with this approach, subsequent analysis by gas chromatography–mass spectrometry(GC–MS) was performed to identify polycyclic aromatic hydrocarbons(PAH). CAST1 enabled the generation of FP with a predominance of small PAH molecules, and CAST3 enabled the generation of UFP, which presented higher numbers of carbon atoms corresponding to larger PAH molecules. Healthy normal human bronchial epithelial(NHBE) cells differentiated at the air-liquid interface(ALI) were directly exposed to these freshly emitted FP and UFP. Expression of MUC5AC, FOXJ1, OCLN and ZOI as well as microscopic observation confirmed the ciliated pseudostratified epithelial phenotype. Study of the mass deposition efficiency revealed a difference between the two operating conditions, probably due to the morphological differences between the two categories of particles. We demonstrated that only NHBE cells exposed to CAST3 particles induced upregulation in the gene expression of IL-8 and NQO1. This approach offers new perspectives to study FP and UFP with stable and controlled properties.

The author first studies the Lipschitz properties of the monotone and relative rearrangement mappings in variable exponent Lebesgue spaces completing the result given in [9]. This paper is ended by establishing the Lipschitz properties for quasilinear problems with variable exponent when the right-hand side is in some dual spaces of a suitable Sobolev space associated to variable exponent.
