This paper presents a new three-phase hybrid active power filter configuration that interconnects a passive high-pass filter in parallel with an active power filter and a photovoltaic system. The proposed configuration can improves the filtering performance of the conventional active power filter, as well as simultaneously supply the power from the photovoltaic arrays to the load and utility. This paper will describe the proposed hybrid active power filter control using sliding mode with photovoltaic system. The proposed technique effectively filters harmonics under 1 kHz but also higher frequency to achieve wideband harmonics compensation. The THD of source current is reduced from 30.09% to 1.95%. The result indicates that the sliding mode controller can track the reference signals and have good dynamic characteristics.

The objective of the study was to identify the failure factors of community interventions in terms of HIV activities in the province of Haut-Katanga during the year 2016. Materials and Methods: The study was phenomenological of the factors, carried out in Haut Katanga. All health facilities (FOSAs) having integrated the prevention of transmission of HIV infection from mother to child (PMTCT) were included in the study. The collection was carried out through individual semi-structured interviews with PMTCT focal points, mentor mothers and people living with HIV (PLWHA) cared for in some of these health facilities (FOSAs). The number of participants was determined by the saturation of responses. Nvivo v.11 software was used for the analyses. The data of each woman was handled confidentially. The authorization of the ethics committee of the University of Kinshasa ESP/CE091/2015, the free and written consent, was obtained before collecting the information. Results: It was observed the socio-cultural factors of success and failure of the interventions: the social fear generated by contradictory messages in the sensitization of the community;the lack of an obvious strategy for the involvement of the partner;the weak coordination of community care activities between the central office of the health zone;the FOSA and the community worker: low interest in community care evidenced by weak accountability of FOSAs, community and program providers in this regard;that relates to community activities;coordination of care between care units;overload of the staff in charge of activities within the health structure and their low motivation;the low communication time devoted to people living with HIV in the FOSAs. Conclusion: The study shows that interventions that can improve the quality and outcomes of prevention of mother-to-child transmission of HIV (PMTCT) services can be directly linked to the program itself, as well as come from another or the community, which generally remains

Summary: The aim of this work was to determine the frequency of rectal prolapse and to describe the therapeutic aspects. Material and Methods: A descriptive study with a retrospective collection was carried out between January 2013 and May 2016 in 29 months in the pediatric surgery department at the Center University Hospital of Brazzaville. Children between the ages of 1 and 15 years of age, treated for rectal prolapse and whose records were exploitable were included. The variables studied were: age, gender, parental socio-economic status, consultation time, preference factors, pre-admission treatment, treatment initiated and progression. Results: In 29 months, 22 cases of rectal prolapse were collected, i.e. a hospital frequency of 0.96%. The mean age was 4.5 years extremes (1 and 11 years). The average time of consultation was 5 days extremes (1 and 21 days). Diarrhea 5 cases (23%), constipation diarrhea 3 cases (14%), constipation 10 cases (45%), bronchopneumopathy 4 cases (18%). The treatment was surgical according to the Thiersch technique in all cases. The evolution was favorable. Conclusion: Rectal prolapse, a benign pathology, is relatively uncommon. Constipation remains the main factor favoring the need to take care of upstream. Treatment by the Thiersch method remains the first choice in children.

Background and Aim: Ecthyma gangrenosum (EG) is a rare necrotic skin infection caused by Pseudomonas aeruginosa occurring on healthy skin mainly, but not exclusively, in immunocompromised patients. We report the case of EG of unusual presentation in a 12-month-old infant. Case Presentation: This was a 12-month-old male infant who was referred from a peripheral structure for the treatment of facial swelling associated with fever. In his antenatal and intra nasal history, there was a notion of hospitalization for 15 days for early neonatal infection and a delay in psychomotor development. The admission examination showed the fair general condition, hyperthermia at 39°C, tachycardia at 136 beats/minute, and polypnea at 48 cycles/minute. The weight-for-height index with a z-score of 3DS was also found. The dermatological examination at the entrance found an extensive brownish inflammatory tense infiltrated plaque taking the mandibular, submental, retro-auricular occipital regions rapidly evolving towards necrosis, a significant swelling predominant on the upper right eyelid preventing the opening of the associated eyes to mouth ulceration. Biological abnormalities were predominantly neutrophilic leukocytosis, elevated CRP, and elevated muscle enzymes. Microbiological examination of skin samples had isolated Pseudomonas aeruginosa. He had received antibiotic therapy, and a necrosectomy under general anesthesia. The evolution was favorable and he was discharged from the hospital on D26 of hospitalization. Conclusion: This observation is rare due to its location, its early onset, and non-neutropenic background.

The objective of this study is to report clinical, electrocardiographic, and echocardiographic data on heart failure for the evaluation of its cardiovascular risk factors and causes at the Institute of Cardiology of Abidjan and the National Police Hospital. Patients and Methods: The 12-month prospective study included 989 patients who were diagnosed with heart failure at the Institute of Cardiology of Abidjan and the Heart Department of the National Police Hospital in Cote d’Ivoire. Results: The mean age of the patients was 55 ± 12 years. Patients ≤ 50 years, aged 50 and 70 years, and ≥ 70 years were 42%, 60%, and 18%, respectively. The major cardiovascular risk factors were high blood pression (60%), smoking (19%), type 2 diabetes (11%), and hypercholesterolemia (8%). The status of 11% of HIV positive patients were ignored upon admission to the Heart Hospitals. Severe kidney failure (25%) was found in all hypertensive patients. Atrial fibrillation patients and sinus rhythm subjects were 20% and 80%, respectively. The average duration of QRS was 102 ± 24 ms. 20% of patients had a complete left bundle branch block. The mean of the left ventricular ejection fraction (LVEF) was 35.8% ± 13%. The LVEF was? ≤ 45% in 57% of cases and ≤ 30% in 30% of cases. The identified causes of heart failure were ischemic heart disease (60%), hypertensive heart disease (20%), and rheumatic valvulopathy (12%). Among the patients with ischemic heart disease, 80% were infarction. The coronarography and the myocardial revascularization were performed in 50% of cases. The primitive cardiomyopathy was diagnosed in 15% of cases. The average follow-up was 12.6 ± 8 months. 18% of patients died, 14% of whom deceased within 1 year after the initial diagnosis and 5% of death occurred in 19 months following the first symptoms of heart failure. The end stage of heart failure (13%) was the most frequent cause of death followed by the sudden death (5%) and the terminal kidney failure (2%). The patients

Knee dislocation is a serious and rare injury. Its rarity and the variety of injuries that result from it mean that there is no adequate management that is universally accepted. The aim of this study was to evaluate our emergency care strategy for these injuries. Materials and methodology: This was a retrospective study conducted from January 1992 to December 2004 on nine cases of knee dislocation. It consisted of six men and three women. The average age of these subjects was 35, ranging from 15 to 50 years. The causes for these injuries included: public road accidents (n = 4), household accidents (n = 3), a sporting accident and a dislocation that occurred following a fight. The dislocations were anteromedial (n = 4), posterolateral (n = 2), posterior (n = 2) and anterior (n = 1). Associated injuries were sore joint (n = 2), contusion of the common peroneal nerve (n = 1) and vascular injury (n = 1). An angiography was performed on one patient for a vascular injury and an Elecmyography (EMG) for a common peroneal nerve injury. Closed reduction was used for eight patients and open reduction for one patient. Results: Five knees were stable with normal range of motion and some residual pain. Four patients had knee instability;two anterior, one anteroposterior and one lateral. In three of these patients, the CT arthrography/arthro-scan confirmed central pivot damage, with meniscal damage in two of them. The four patients were referred to a knee surgeon in a private practice. The post-operational effects were minimal in vascular injury. The EMG performed for the common peroneal nerve detected a nerve contusion. Conclusion: Our emergency care strategy remains closed reduction. This therapeutic management is consistent, at least in the first 15 days, with the literature.
