Experience on the Management of Priapism at the Yaounde central Hospital

Introduction: Priapism is a urological emergency defined as a prolonged penile erection for more than 6 hours, painful and sustained, occurring without any sexual stimulation. The diagnosis is essentially clinical. However, the management is often late because of the delay in consultation, which is characteristic in our context. The purpose of our study is to describe the clinical and therapeutic aspects of patients who presented with priapism at the Yaoundé central Hospital as well as to propose a present synthesis concerning the management of priapism. Methodology: We conducted a descriptive cross-sectional study with a retrospective recruitment method in the urology department of the YcH. It extended over a period of 6 months from January 2022 to June 2022. We reviewed the files of patients seen within a period spanning 10 years from January 1st, 2011 to January 31st, December 2021. The study population consisted of patients who had been hospitalized and treated for priapism. We carried out consecutive and non-exhaustive sampling. Results: During the study, we collected data from the medical files of 13 patients. The median age was 25 years, with extremes of 12 and 82 years. Most of the patients were between 20 and 30 years old (38.5%). Most of the patients were single (69.2%). Sickle cell disease was the main comorbidity encountered during the study (76.9%). The median time from onset of symptoms to consultation was 10 [6 - 20.5] hours, with extremes of 1 and 264 hours. All patients presented with painful penile erection (100%). There was concomitant multifocal pain in 38.5% of cases suggestive of vaso-occlusive crises (VOc). The symptoms occurred in a non-traumatic context in the majority of cases (92.3%). The T-shunt was the most used surgical technique (84.6%). The median duration of surgery was 36 [35 - 40] minutes with extremes of 25 and 60 minutes, the majority being operated in less than 45 minutes (84.6%). The frequency of postopera

The Interest of Endoscopy in the Diagnosis of Fallopian Tubes Diseases at the Yaoundé Gyneco-Obstetric and Pediatric Hospital (Ygoph)

Introduction: Fallopian diseases are often implicated in female infertility. Several radiological and surgical explorations have been proposed in order to evaluate the severity of lesions found in utero-adnexal pathology. Among tools that are used to investigate such pathologies, we have ultrasound, hysterosalpingography and endoscopy. But, in many developing countries like cameroon, the usage of endoscopy in gynecology is not yet known by many practicians. The objective of our study was to show the interest of endoscopy in the diagnosis of fallopian tube pathologies. Methodology: We conducted a cross-sectional and descriptive study from March 1st, 2017 to May 31st, 2017 at the Gyneco-Obstetrics and Pediatric department of our Hospital. We included all women who presented infertility and underwent ultrasound, hysterosalpingography (HSG) and endoscopy at the Yaoundé Gyneco-obstetrics Hospital. We analyzed epidemiological parameters, clinics, ultrasound, hysterosalpingography and endoscopic finding. We used cohen’s Kappa test to determine the correlation between HSG/endoscopy and ultrasound/endoscopy in the diagnosis of fallopian tube pathologies. The threshold was significant for a K-value > 0.20. Results: We got a sample of 156 women;the mean age was 32.6 ± 4.5 years. The Secondary infertility dominated in 66.7% of cases;31.1% of women presented a past medical history of sexually transmitted infections and 41.7% did abortions before. Endoscopic lesions were dominated by a fallopian obstruction in 54.5% of cases, 8.3% of adhesions, 33.9% of women presented uterine myomas, 37.8% of ovarian cysts and 1.3% of pelvic endometriosis. The K-values between HSG and endoscopy for distal and proximal tube obstructions were significant with respective thresholds of 0.25 and 0.30. Neural Model-Based Self-Tuning PID Strategy Applied to PEMFc

This paper illustrates the benefits of a self-tuning PID strategy applied to a proton exchange membrane fuel cell system. controller parameters are updated on-line, at each sampling time, based on an instantaneous linearization of an artificial neural network model of the process and a General Minimum Variance control law. The self-tuning PID scheme allows managing nonlinear behaviors of the system while avoiding heavy computations. The applicability, efficiency and robustness of the proposed control strategy are experimentally confirmed using varying control scenarios. In this aim, the original built-in controller is overridden and the self-tuning PID controller is implemented externally and executed on-line. Experimental results show good performance in setpoint tracking accuracy and robustness against plant/model mismatch. The proposed strategy appears to be a promising alternative to heavy computation nonlinear control strategies and not optimal linear control strategies.

Energy Efficiency Improvement of an Industrial crystallization Process Using Linearizing control

This paper illustrates the benefits of a multivariable linearizing control approach applied to an industrial crystallization process. This relevant approach is declined according to two different strategies: first, a setpoint tracking is proposed for the couple crystal mass/concentration, whereas a second way consists in tracking of crystal content and concentration. The controlled variables, unavailable online, are issued from an observer developed in previous works. The performance of these strategies, which application to cane sugar crystallization constitutes a real novelty, are compared with experimental data issued from a PID-controlled industrial plant. The results reveal a significant improvement of energy efficiency, leading to an economy of more than 10% of energy.

Experimental Evaluation of Parameterized Nonlinear MPc Applied to PEM Fuel cell

This paper proposes a parameterized nonlinear model-based predictive control (NMPc) strategy to tackle the oxygen excess ratio regulation challenge of a proton exchange membrane fuel cell. In practice, the most challenging part regarding NMPc strategies remains the on-line implementation. In fact, NMPc strategies, at least in their basic form, involve heavy computation to solve the optimization problem. In this work, a specific parameterization of control actions has been designed to address this limitation and achieve on-line implementation. To assess the effectiveness and relevance of the proposed strategy, the controller has been implemented on-line, experimentally validated on a real fuel cell and compared to the built-in controller. Performance of the parameterized NMPc controller in terms of setpoint tracking accuracy, disturbances rejection and computational cost, have tested under several control scenarios. Experimental results have shown the excellent tracking capability, disturbances rejection ability and low computational cost of the NMPc controller, regardless of the operating conditions. Moreover, compared to the built-in controller the proposed strategy has demonstrated better disturbances rejection capability. Overall, the proposed parameterized NMPc controller appears as an excellent candidate to address the oxygen excess ratio regulation issue.

In most eusocial insects,the division of labor results in relatively few individuals foraging for the entire ***,the survival of the colony depends on its efficiency in meeting the nutritional needs of all its ***,we characterize the network topology of a eusocial insect to understand the role and centrality of each caste in this network during the process of food *** constructed trophallaxis networks from 34 food-exchange experiments in black garden ants(Lasius niger).We tested the influence of brood and colony size on(i)global indices at the network level(i.e.,efficiency,resilience,centralization,and modularity)and(ii)individual values(i.e.,degree,strength,betweenness,and the clustering coefficient).Network resilience,the ratio between global efficiency and centralization,was stable with colony size but increased in the presence of broods,presumably in response to the nutritional needs of *** metrics highlighted the major role of foragers in food *** addition,a hierarchical clustering analysis suggested that some domestics acted as intermediaries between foragers and other *** appeared to be hierarchical rather than random or centralized exclusively around ***,our results suggested that networks emerging from social insect interactions can improve group performance and thus colony fitness.
