Update in lean metabolic dysfunction-associated steatotic liver disease

BACKGROUND A new nomenclature consensus has emerged for liver diseases that were previously known as non-alcoholic fatty liver disease(NAFLD)and metabolic dysfunction-associated fatty liver disease(MAFLD).They are now defined as metabolic dysfunction-associated steatotic liver disease(MASLD),which includes cardiometabolic criteria in adults.This condition,extensively studied in obese or overweight patients,constitutes around 30%of the population,with a steady increase worldwide.Lean patients account for approximately 10%-15%of the MASLD population.However,the pathogenesis is complex and is not well understood.AIM To systematically review the literature on the diagnosis,pathogenesis,characteristics,and prognosis in lean MASLD patients and provide an interpretation of these new criteria.METHODS We conducted a comprehensive database search on PubMed and Google Scholar between January 2012 and September 2023,specifically focusing on lean NAFLD,MAFLD,or MASLD patients.We include original articles with patients aged 18 years or older,with a lean body mass index categorized according to the World Health Organization criteria,using a cutoff of 25 kg/m2 for the general population and 23 kg/m2 for the Asian population.RESULTS We include 85 studies in our analysis.Our findings revealed that,for lean NAFLD patients,the prevalence rate varied widely,ranging from 3.8%to 34.1%.The precise pathogenesis mechanism remained elusive,with associations found in genetic variants,epigenetic modifications,and adaptative metabolic response.Common risk factors included metabolic syndrome,hypertension,and type 2 diabetes mellitus,but their prevalence varied based on the comparison group involving lean patients.Regarding non-invasive tools,Fibrosis-4 index outperformed the NAFLD fibrosis score in lean patients.Lifestyle modifications aided in reducing hepatic steatosis and improving cardiometabolic profiles,with some medications showing efficacy to a lesser extent.However,lean NAFLD patients exhibited a worse prognosis compared to the obese

“Rescue” regimens after Helicobacter pylori treatment failure

Helicobacter pylori (H pylori) infection is the main cause of gastritis, gastroduodenal ulcer disease, and gastric cancer. After more than 20 years of experience in H pylori treatment, in my opinion, the ideal regimen to treat this infection is still to be found. Currently, apart from having to know first-line eradication regimens well, we must also be prepared to face treatment failures. Therefore, in designing a treatment strategy we should not focus on the results of primary therapy alone, but also on the final (overall) eradication rate. The choice of a "rescue" treatment depends on which treatment is used initially. If a clarithromycin- based regimen was used initially, a subsequent metronidazole-based treatment (quadruple therapy) may be used afterwards, and then a levofloxacin- based combination would be a third "rescue" option. Alternatively, it has recently been suggested that levofloxacin-based rescue therapy constitutes an encouraging second-line strategy, representing an alternative to quadruple therapy in patients with previous PPI-clarithromycin-amoxicillin failure, with the advantage of efficacy, simplicity and safety. In this case, a quadruple regimen may be reserved as a third-line rescue option. Finally, rifabutin-based rescue therapy constitutes an encouraging empirical fourth- line strategy after multiple previous eradication failures with key antibiotics such as amoxicillin, clarithromycin, metronidazole, tetracycline, and levofloxacin. Even after two consecutive failures, several studies have demonstrated that H pylori eradication can finally be achieved in almost all patients if several rescue therapies are consecutively given. Therefore, the attitude in H pylori eradication therapy failure, evenafter two or more unsuccessful attempts, should be to fight and not to surrender.

Simulated predation pressure in Pelobates Cultripes tadpoles modulates morphology at the metamorphic stage

Studies on the impacts of variation of biotic interactions at key life cycle stages are crucial to understand the interface between ecological and developmental processes.Predators exert a major impact on prey fitness.Although direct consumption entails the greatest effect,predators can affect prey by means of other mechanisms.For instance,injuries inflicted by failed predation attempts can jeopardize prey fitness,even beyond the short-term.In anuran tadpoles,failed predation typically results in partial tail loss,which is known to reduce swimming speed.However,the potential consequences of tadpole partial tail loss after metamorphosis remain understudied.Because tail materials could be important in conforming metamorph body,we assess the effects of tadpole partial tail loss on metamorph body size in Iberian spadefoot toads Pelobates cultripes.We clipped 55%tail length of pre-tail-resorption stage anesthetized tadpoles,and compared their body size as metamorphs with anesthetized and non-anesthetized non-tail-clipped controls.Also,we tested whether tail length correlated with metamorph body size of individuals of the control groups.Tailclipped tadpoles produced smaller metamorphs than both controls(the bdy size of metamorphs from both controls was similar),which could incur costs in mid-term survival or time to first reproduction.This effect could be particularly important in areas with introduced predators,if autochthonous tadpoles lack defenses against them.Results suggest that materials resorbed from tadpole tail tissues might be reallocated into metamorph body,according to the negative effect of shorter tails in a correlational analysis,and clipped tails in an experimental test,on metamorph body size.

世界经济一直面临各种冲击,这些冲击常常从经济周期的变化反映出来。当前,世界经济比以往任何时候更加相互依赖。经济政策、商品价格或消费趋势的细微变化都将对经济产生不同凡响的后果。当前燃油已成为各种交通工具最重要的投入。在海运中,燃油约占总成本的三分之二。本文研究国际石油价格的变化对巴拿马运河通航总量的影响。给定吨位数量,我们可以看到油价的上升(下降)会减少(增加)船只的通行量。巴拿马是一个依靠运河业发展的服务型经济体,其客户希望通过运河以节省时间和成本。任何情况下油价的上涨都会直接影响国际货物的运输成本,这在理论上可能减少通过运河船只的载重吨位,而油价的变化也成为巴拿马经济或好或坏的催化剂。自19世纪来,全球化石燃料投入品价格面临不确定性,且其储备量面临枯竭,由此推动价格上涨,影响全球商品和服务。这种外生依赖性明显使巴拿马经济增长容易受到国际油价的影响,也推动了该国采取措施适应现状、改进运河服务。考虑市场变化,巴拿马已经扩大对运河服务的投资(运河扩建),这将有助于大型船只通过,提高运河通航总量和市场份额。

The NLRP3-IL-1β pathway plays an important role in adipose tissue(AT)-induced inflammation and the development of obesityassociated comorbidities.We aimed to determine the impact of NLRP3 on obesity and its associated metabolic alterations as well as its role in adipocyte inflammation and extracellular matrix(ECM)remodeling.Samples obtained from 98 subjects were used in a case-control study.The expression of different components of the inflammasome as well as their main effectors and inflammation-and ECM remodeling-related genes were analyzed.The impact of blocking NLRP3 using siRNA in lipopolysaccharide(LPS)-mediated inflammation and ECM remodeling signaling pathways was evaluated.We demonstrated that obesity(P<0.01),obesity-associated T2D(P<0.01)and NAFLD(P<0.05)increased the expression of different com ponents of the inflammasome as well as the expression and release of IL-1β and IL-18 in AT.We also found that obese patients with T2D exhibited increased(P<0.05)hepatic gene expression levels of NLRP3,IL1B and IL18.We showed that NLRP3,but not NLRP1,is regulated by inflammation and hypoxia in visceral adipocytes.We revealed that the inhibition of NLRP3 in human visceral adipocytes significantly blocked(P<0.01)LPS-induced inflammation by downregulating the mRNA levels of CCL2,IL1B,IL6,IL8,S100A8,S100A9,TLR4 and TNF as well as inhibiting(P<0.01)the secretion of IL1-β into the culture medium.Furthermore,blocking NLRP3 attenuated(P<0.01)the LPS-induced expression of important molecules involved in AT fibrosis(COL1A1,COL4A3,COL6A3 and MMP2).These novel findings provide evidence that blocking the expression of NLRP3 reduces AT inflammation with significant fibrosis attenuation.

We review the construction of gravitational solutions holographically dual to N=1 quiver gauge theories with dynamical flavor multiplets.We focus on the D3-D7 construction and consider the finite temperature,finite quark chemical potential case where there is a charged black hole in the dual solution.Discussed physical outputs of the model include its thermodynamics (with susceptibilities) and general hydrodynamic properties.

Background:This study investigates the effects of exercise training on exerkines in patients with type 2 diabetes mellitus to determine the optimal exercise prescription.Methods:A systematic search for relevant studies was performed in 3 databases.Randomized controlled trials investigating the effects of exercise training on at least one of the following exerkines were included:adiponectin,apelin,brain-derived neurotrophic factor,fetuin-A,fibroblast growth factor-21,follistatin,ghrelin,interleukin(IL)-6,IL-8,IL-10,IL-15,IL-18,leptin,myostatin,omentin,resistin,retinol-binding protein 4,tumor necrosis factor-α,and visfatin.Results:Forty randomized controlled trials were selected for data extraction(n=2160).Exercise training induces changes in adiponectin,fetuin-A,fibroblast growth factor-21,IL-6,IL-10,leptin,resistin,and tumor necrosis factor-a levels but has no significant effects on apelin,IL-18,and ghrelin compared to controls.Physical exercise training favored large and positive changes in pooled exerkines(i.e.,an overall effect size calculated from several exerkine s)(Hedge’s g=1.02,95%confidence interval(95%CI):0.76-1.28),which in turn were related to changes in glycated hemoglobin(mean difference(MD)=-0.81%,95%CI:-0.95%to-0.67%),fasting glucose(MD=-23.43 mg/dL,95%CI:-30.07 mg/dL to-16.80 mg/dL),waist circumference(MD=-3.04 cm,95%CI:-4.02 cm to-2.07 cm),and body mass(MD=-1.93 kg,95%CI:-2.00 kg to-1.86 kg).Slightly stronger effects were observed with aerobic,resistance,or high-intensity interval protocols at moderate-to vigorous-intensity and with programs longer than 24 weeks that comprise at least 3 sessions per week and more than 60 min per session.Conclusion:Exercise training represents an anti-inflammatory therapy and metabolism-improving strategy with minimal side effects for patients with type 2 diabetes mellitus.

Cyber-applications as gateway to data-rich Digital Earth systems

The underlying vision of the Digital Earth(DE)calls for applications that can embed vast quantities of geo-referenced data and allow users to study and analyse of our planet.Since the declaration of this vision in the late 90s,a significant number of DE data-sets have been created by the industry,governments,non-governmental organisations and individuals.An overwhelming majority of the successful applications that use DE data-sets has its end-user applications running on the desktop.While these applications are great tools,they remain inaccessible to the community as a whole.In this paper,we present a framework for the development of cyber-applications.We define an abstract architecture for cyber-applications based on the model-view-controller paradigm,which allows the dynamic inclusion of functional and data components into its execution engine at run-time.We define the operational characteristics of cyber-applica-tions.We also specify the interface of pluggable components to the architecture.Finally,we demonstrate the appropriateness of the abstract architecture by means of a case study.

The Multifaceted Roles of HY5 in Plant Growth anc Development

ELONGATED HYPOCOTYL5 （HY5）, a member of the bZlP transcription factor family, inhibits hypocotyl growth and lateral root development, and promotes pigment accumulation in a light-dependent manner in Arabidopsis. Recent research on its role in different processes such as hormone, nutrient, abiotlc stress （abscisic acid, salt, cold）, and reactive oxygen species signaling pathways clearly places HY5 at the center of a transcriptional network hub. HY5 regulates the transcription of a large number of genes by directly binding to cis-regulatory elements. Recently, HY5 has also been shown to activate its own expression under beth visible and UV-B light. Moreover, HY5 acts as a signal that moves from shoot to root to promote nitrate uptake and root growth. Here, we review recent advances on HY5 research in diverse aspects of plant development and highlight still open questions that need to be addressed in the near future for a complete understanding of its function in plant signaling and beyond.

The short form of the SUR1 and its functional implications in the damaged brain

Sulfonylurea receptor(SUR) belongs to the adenosine 5′-triphosphate(ATP)-binding cassette(ABC) transporter family;however,SUR is associated with ion channels and acts as a regulatory subunit determining the opening or closing of the pore.Abcc8 and Abcc9 genes code for the proteins SUR1 and SUR2,respectively.The SUR1 transcript encodes a protein of 1582 amino acids with a mass around 140–177 k Da expressed in the pancreas,brain,heart,and other tissues.It is well known that SUR1 assembles with Kir6.2 and TRPM4 to establish K_(ATP) channels and non-selective cation channels,respectively.Abbc8 and 9 are alternatively spliced,and the resulting transcripts encode different isoforms of SUR1 and SUR2,which have been detected by different experimental strategies.Interestingly,the use of binding assays to sulfonylureas and Western blotting has allowed the detection of shorter forms of SUR(-65 k Da).Identity of the SUR1 variants has not been clarified,and some authors have suggested that the shorter forms are unspecific.However,immunoprecipitation assays have shown that SUR2 short forms are part of a functional channel even coexisting with the typical forms of the receptor in the heart.This evidence confirms that the structure of the short forms of the SURs is fully functional and does not lose the ability to interact with the channels.Since structural changes in short forms of SUR modify its affinity to ATP,regulation of its expression might represent an advantage in pathologies where ATP concentrations decrease and a therapeutic target to induce neuroprotection.Remarkably,the expression of SUR1 variants might be induced by conditions associated to the decrease of energetic substrates in the brain(e.***.during stroke and epilepsy).In this review,we want to contribute to the knowledge of SUR1 complexity by analyzing evidence that shows the existence of short SUR1 variants and its possible implications in brain function.
