Impact of tertiary Gleason pattern 5 on prostate cancer aggressiveness:Lessons from a contemporary single institution radical prostatectomy series

Objective:To better evaluate tertiary Gleason pattern reporting and to evaluate the impact of tertiary Gleason pattern 5(TP5)on prostate cancer pathological features and biochemical recurrence at our large single institution.Methods:We retrospectively reviewed 1962 patients who underwent radical prostatectomy(RP)for prostate cancer;TP5 was reported in 159 cases(8.1%).Men with Gleason score(GS)7 and GS 8 disease were divided into subgroups with and without TP5,and histopathological features were compared.Multivariate analyses were conducted to assess the impact on TP5 on biochemical-free survival(BFS).Results:Tumors possessing GS 3+4 with TP5 were more likely to exhibit extraprostatic extension(EPE)and had a larger tumor diameter(TD)than GS 3+4 alone.GS 3+4 with TP5 was also associated with positive surgical margins(SM),seminal vesicle involvement(SVI),and higher pre-operative prostate-specific antigen(PSA)values,but without statistical significance.GS 4+3 with TP5 more commonly presented with EPE,positive SM,SVI,and greater TD and pre-operative PSA level than GS 4+3 alone.In multivariate analysis,Gleason score,EPE,and TP5 were overall independent risk factors for PSA recurrence in this cohort.Additionally,GS 4+3 with TP5 was associated with shorter time to recurrence versus GS 4+3 alone.Conclusion:Our results emphasize the importance of TP5 and suggest that criteria for tertiary pattern reporting in prostate cancer should be standardized.Further studies are needed to evaluate the role of tertiary patterns in prognostic models.

A metabolic syndrome is a cluster of clinical signs that are risk factors for the development of type 2 diabetes mellitus,and cardiovascular disease.This cluster includes obesity,elevated blood sugar,high blood pressure,and high cholesterol.Atypical antipsychotics that are associated with a group of signs and symptoms have resulted in an increased focus on this syndrome.This review included accessible literature,such as review articles and original research articles.It provides useful guidelines for psychiatrists and gives them psycho-education so that they can identify and monitor patients who may have the syndrome and properly manage it.

Background:Substance abuse has become an increasingly serious socio-medical issue in recent decades.Human health is affected by the socio-cultural environment,which has a direct impact on his psychophysical development and well-being.However,sociocultural factors such as bad habits influence lifestyle and behavior,leading to significant health and social problems for substance abusers.Objective:The present study is to evaluate the phenomenology and socio-demographic characteristics of abusers and compare some of the characteristics randomly.The main objectives of this study are a.To evaluate the socio-demographic profile.b.To find out the pattern of substance use.c.To evaluate the association of phenomenology.d.To find out about substance-related disorders among the substance abuse patients.Materials and methods:There was an observational study conducted in the drug de-addiction unit at Taif,Eradah,and Mental Health Hospital in Saudi Arabia.This study included all substance abusers admitted to the hospital who met the inclusion and exclusion criteria.The study included patients of both genders admitted to the drug de-addiction unit between April 2021 and October 2022.Based on a descriptive statistical analysis,categorical variables were presented as frequencies and percentages.Results:An observational study was conducted for nearly one and a half years.We interviewed 358 substance abuse patients following a systematic sampling procedure in a peaceful,non-threatening environment.Among the 358 substance users,353(98.60%)were male and 5(1.39%)were female.According to the survey,117(32.68%)of the participants reported abusing substances during their 31-40 years of age,followed by 102(28.49%)during their 25-30 years of age.The predominant level of substance abuse was among the unemployed 163(45.53%),followed by students 105(29.33%).It was found that 58.66%of all respondents had initiated abuse between the ages of 18-24 years of age.Conclusion:In our study,combined substances like amphetamine and cannabis abusers were

The American College of Cardiology/American Heart Association defines resistant hypertension(RH)as a clinical blood pressure(BP)reading of>130/80 mmHg in patients taking three antihypertensive drugs,including a renin–angiotensin system inhibitor,a calcium channel blocker(CCB),and a diuretic at well-tolerated doses.1,2 It is reported from multiple population-based surveys that in the United States,there is an approximately 12%–15%prevalence of RH among adults diagnosed with hypertension.2 A prospective study demonstrates that 20%of people diagnosed with RH had primary hyperaldosteronism.3,4 In addition to the classic three antihypertensive drugs,spironolactone and mineralocorticoid are also administered for RH.However,with respect to the safety profile of spironolactone,it has been reported to have several side effects such as low testosterone production,menstrual irregularities,and excessively raised serum potassium levels,leaving the drug unfit for the longitudinal therapeutic purpose of treating RH.

Flavonoid Myricetin as Potent Anticancer Agent: A Possibility towards Development of Potential Anticancer Nutraceuticals

Good nutrition plays a crucial role in maintaining a balanced lifestyle. The beneficial effects of nutrition have been found to counteract nutritional disturbances with the expanded use of nutraceuticals to treat and manage cardiovascular diseases, cancer, and other developmental defects over the last decade. Flavonoids are found abundantly in plant-derived foods such as fruits, vegetables, tea, cocoa, and wine. Fruits and vegetables contain phytochemicals like flavonoids, phenolics, alkaloids, saponins, and terpenoids. Flavonoids can act as anti-inflammatory, anti-allergic, anti-microbial(antibacterial, antifungal, and antiviral) antioxidant, anti-cancer, and anti-diarrheal agents. Flavonoids are also reported to upregulate apoptotic activity in several cancers such as hepatic, pancreatic, breast, esophageal, and colon. Myricetin is a flavonol which is naturally present in fruits and vegetables and has shown possible nutraceutical value. Myricetin has been portrayed as a potent nutraceutical that may protect against cancer. The focus of the present review is to present an updated account of studies demonstrating the anticancer potential of myricetin and the molecular mechanisms involved therein. A better understanding of the molecular mechanism(s) underlying its anticancer activity would eventually help in its development as a novel anticancer nutraceutical having minimal side effects.

Arachnoid cysts are uncommon benign neurological tumors,and having presentation like schizophrenia,which has been reported in association with this cyst.The presence of psychiatric disturbances of arachnoid cyst has not been clearly mentioned in the literature.Even though,the appearance of some of the references that focuses on a possible link between arachnoid cysts and psychotic symptoms.Here we present a case report of a 35-year-old man,characterized by the insidious onset of psychotic symptoms of varying intensity such as multiple physical assaults on people with stone.Due to organic suspicion one cannot exclude the possibility that the lesion played a significant role in this psychiatric presentation.

Comparative Study of the Effects of Nano ZnO and CuO on the Biodegradation,Biocompatibility,and Antibacterial Properties of Micro-arc Oxidation Coating of Magnesium Alloy

This research systematically examined the degradation,antibacterial effects,and biocompatibility of micro-arc oxidation(MAO)coatings with nano CuO and ZnO on extruded Mg alloys.Both copper(Cu)and Zinc(Zn)possess antibacterial properties.The findings demonstrated that adding ZnO will appreciably reduce the degradation rate of MAO-coating alloy due to the self-sealing micro holes.CuO+MAO coating exhibited excellent antibacterial performance,with an antibacterial rate of over 90%within 6 h co-cultured with Staphylococcus aureus.Similarly,the antibacterial rate of ZnO+MAO coating reached 90%after 12 h co-culture.Cytotoxicity test using MG63 cell indicated that the incorporation of CuO and ZnO did not notably affect the cell viability rate of the coating.Moreover,after 14 days of culture,the CuO+MAO and ZnO+MAO coated samples exhibited higher alkaline phosphatase(ALP)activity than the MAO-coated and uncoated samples,suggesting favorable osteogenic properties.

Insight into Metabolomic Profiling of Tinospora cordifolia: Recent Advances and Future Perspectives of Quality Control

The objective of this study was to offer a review of previous works on Tinospora cordifolia(TC) metabolomics with a highlight on the use of metabolomics in comparative evaluations between varieties and selection of optimum cultivation, collection, and extraction procedures, providing insights into pharmaceutical and food product quality monitoring. Scholarly databases were searched to gather relevant information on TC's therapeutic usefulness, phytoconstituent studies, metabolomics, and applications. Pub Chem was used to learn the structures of several of the metabolites. The various medicinal properties of TC were extensively researched. The most essential portions of the plant in Ayurvedic medicines are the stems. Bioactive phytochemicals such as steroidal lactones, alkaloids, diterpenoids, and phenolic compounds are present in the roots and leaves. Metabolomic analysis revealed that, in addition to the season and geographical region of the sample collected, the metabolite profiles of TC also depend on the parts of the plant taken as a sample and also the plant's stage of growth. The amounts of secondary metabolites differed between morpho/chemotypes within the TC species. Despite the fact that research on TC began several decades ago, the diversity of phytoconstituents was not appreciated because of a lack of reliable techniques for phytochemical fingerprinting. The uncovering of several novel metabolites was aided by advances in chromatography combined with mass spectrometry. To produce a full metabolomic profile, researchers used mutually complementary techniques. Following that, data analysis and searches against spectrum databases allowed for signal annotation and interpretation of metabolites in large quantities without separating them separately. The current review includes a critical assessment of metabolomic data and an overview of the technique's various applications. Sleep paralysis (SP) is a benign,transient episode of immobility and it lasts a few seconds to a few minutes.It can occur while falling asleep or on awakening.It is a condition of unknown etiology and all the skeletal muscles are almost ‘paralyzed’.It leads to an inability to speak or move but individual remains conscious.This review summarizes the existential clinical literature on sleep paralysis most relevant to practitioners;also summarizes the many historical and artistic manifestations of SP in different cultures.It also throws light on the available Arabic literature and others as per the aim of the review.For this review,literature search using engines was carried out,and review papers and original research articles were analyzed.We start with a review which summarizes the collection of symptoms,prevalence rate,risk factors and etiological theories,characteristics and classification of the SP over the past years up to the present time,also the management in the form of psychotherapy and pharmacotherapy as well as the cultural significances in different countries.SP plays an important role in the genesis and maintenance of many supernatural beliefs such as nocturnal alien abductions,demonic and ghost attacks in individuals with intact reality testing.

Congophilic fibrils in the glomeruli with polyclonal immunoglobulin gamma staining-another cause for diagnostic overlap:A case report

BACKGROUND Glomerulopathy with fibrillary deposits is not uncommon in routine nephropathology practice,with amyloidosis and fibrillary glomerulonephritis being the two most frequently encountered entities.Renal amyloid heavy and light chain(AHL)is relatively uncommon and its biopsy diagnosis is usually limited to cases that show strong equivalent staining for a single immunoglobulin(Ig)heavy chain and a single light chain,further supported by mass spectrometry(MS)and serum studies for monoclonal protein.But polyclonal light chain staining can pose a challenge.CASE SUMMARY Herein we present a challenging case of renal AHL with polyclonal and polytypic Ig gamma(IgG)staining pattern by immunofluorescence.The patient is a 62-yearold Caucasian male who presented to an outside institution with a serum creatinine of up to 8.1 mg/dL and nephrotic range proteinuria.Despite the finding of a polyclonal and polytypic staining pattern on immunofluorescence,ultrastructural study of the renal biopsy demonstrated the presence of fibrils with a mean diameter of 10 nm.Congo red was positive while DNAJB9 was negative.MS suggested a diagnosis of amyloid AHL type with IgG and lambda,but kappa light chains were also present supporting the immunofluorescence staining results.Serum immunofixation studies demonstrated IgG lambda monoclonal spike.The patient was started on chemotherapy.The chronic renal injury however was quite advanced and he ended up needing dialysis shortly after.CONCLUSION Tissue diagnosis of AHL amyloid can be tricky.Thorough confirmation using other available diagnostic techniques is recommended in such cases.
