Tumour mutational burden is overestimated by target cancer gene panels

Background:Tumour mutational burden(TMB)has emerged as a predictive marker for responsiveness to immune checkpoint inhibitors(ICI)in multiple tumour *** can be calculated from somatic mutations detected from whole exome or targeted panel sequencing *** mutations are unevenly distributed across the cancer genome,the clinical implications from TMB calculated using different genomic regions are not ***:Pan-cancer data of 10,179 samples were collected from The Cancer Genome Atlas cohort and 6,831 cancer patients with either ICI or non-ICI treatment outcomes were derived from published *** was calculated as the count of non-synonymous mutations and normalised by the size of genomic *** method,linear regression and Poisson calibration models are used to unify TMB from different gene ***:we found that panels based on cancer genes usually overestimate TMB compared to whole exome,potentially leading to misclassification of patients to receive *** overestimation is caused by positive selection for mutations in cancer genes and cannot be completely addressed by the removal of mutational *** compared different approaches to address this discrepancy and developed a generalised statistical model capable of interconverting TMB derived from whole exome and different panel sequencing data,enabling TMB correction for patient stratification for ICI *** show that in a cohort of lung cancer patients treated with ICI,when using a TMB cutoffof 10 mut/Mb,our corrected TMB outperforms the original panel-based ***:Cancer gene-based panels usually overestimate TMB,and these findings will be valuable for unifying TMB calculations across cancer gene panels in clinical practice.

A review on photo-thermal catalytic conversion of carbon dioxide

The conversion of carbon dioxide into value-added products is of great industrial and environmental interest. however, as carbon dioxide is relatively stable, the input energy required for this conversion is a significant limiting factor in the system's performance. By utilising energy from the sun, through a range of key routes, this limitation can be overcome. In this review, we present a comprehensive and critical overview of the potential routes to harvest the sun's energy, primarily through solar-thermal technologies and plasmonic resonance effects. Focusing on the localised heating approach, this review shortlists and compares viable catalysts for the photo-thermal catalytic conversion of carbon ***, the pathways and potential products of different carbon dioxide conversion routes are outlined with the reverse water gas shift,methanation, and methanol synthesis being of key interest. Finally, the challenges in implementing such systems and the outlook to the future are detailed.

Low DhEAS levels are associated with depressive symptoms in elderly Chinese men： results from a large study

This study investigated the association between depressive symptoms in elderly Chinese men and the total testosterone, dehydroepiandrosterone （DhEA）, DhEA sulphate （DhEAS）, oestradiol and sex hormone-binding globulin （ShBG） levels, and the free androgen index. Cross-sectional data from 1147 community-dwelling elderly men, aged 65 and older, were used. Depressive symptoms were measured using the Chinese Geriatric Depression Scale （GDS）. Total testosterone, free testosterone, DhEA, DhEAS, total oestradiol, the free androgen index and ShBG levels were assessed. DhEA was significantly associated with GDS score, and there was a trend towards DhEAS association, but this was not significant （p=-0. 110, P=0.015; p=-0.074, P=0.055）. however, no association was seen between depressive symptoms and total testosterone levels, free testosterone levels, oestradiol levels or ShBG levels. In terms of the presence of clinically relevant depressive symptoms, there were no statistically significant differences between patients in the lowest quartile of sex steroid hormone levels and those in other quartiles of sex steroid hormone levels. Similarly to western studies, our study shows that DhEA and DhEAS levels are associated with depressive symptoms.

AIM:To review international guidelines and to share our infection control experience during the coronavirus disease 2019(COVID-19)pandemic at a tertiary eye centre in hong ***:Infection control guidelines and recommendations from international ophthalmological bodies are reviewed and *** measures at our hospital were drawn up as per international and local health authorities’guidelines and implemented with the collaboration of doctors,nurses and administrative ***:The aims of our infection control measures are to 1)minimize cross-infection within the hospital;2)protect and support hospital staff;3)ensure environmental *** minimize the risk of cross-infection,outpatient attendance and elective surgery have been reduced by 40%,and general anesthesia procedures were reduced by 90%.Patients entering the hospital are screened for fever,travel history,contact and cluster history,and COVID-19 related *** protect and support hospital staff,we ensure provision of adequate personal protective equipment(PPE)and provide clear guidelines on the level of PPE needed,depending on the clinical *** protective measures include provision of work uniforms,easy access to alcohol-based hand rub,opening new lunch areas,implementation of self-monitoring and self-reporting systems,and communication via online education and ***,environmental control is achieved by ensuring regular disinfection of the hospital premise,enhancing ventilation,and usage of disposable ophthalmic ***:Our multi-pronged approach to infection control is,so far,successful in minimizing infection risks,while allowing the maintenance of essential ophthalmic services.

Portal hypertension: Imaging of portosystemic collateral pathways and associated image-guided therapy

Portal hypertension is a common clinical syndrome, defined by a pathologic increase in the portal venous pressure. Increased resistance to portal blood flow, the primary factor in the pathophysiology of portal hypertension, is in part due to morphological changes occurring in chronic liver diseases. This results in rerouting of blood flow away from the liver through collateral pathways to low-pressure systemic veins. Through a variety of computed tomographic, sonographic, magnetic resonance imaging and angiographic examples, this article discusses the appearances and prevalence of both common and less common portosystemic collateral channels in the thorax and abdomen. A brief overview of established interventional radiologic techniques for treatment of portal hypertension will also be provided. Awareness of the various imaging manifestations of portal hypertension can be helpful for assessing overall prognosis and planning proper management.

The aging population is an important issue around the world especially in developed *** med-ical advances have substantially extended life span,the same cannot be said for the duration of health *** are seeing in-creasing numbers of elderly people who are frail and/or have multiple chronic conditions;all of these can affect the quality of life of the elderly population as well as increase the burden on the healthcare *** is mechanistically related to common medical conditions such as diabetes mellitus,ischemic heart disease,cognitive decline,and frailty.A recently accepted concept termed‘Accelerated Biological Aging’can be diagnosed when a person’s biological age—as measured by biomarkers of DNA methylation—is older than their corresponding chronological ***,a conditionally essential amino acid,has received much attention in the past few years.A substantial number of animal studies have provided a strong scientific foundation sug-gesting that this amino acid can improve cellular and metabolic health,including blood glucose control,so much that it has been labelled one of the‘longevity amino acids’.In this review article,we propose the rationale that an adequately powered random-ized-controlled-trial(RCT)is needed to confirm whether taurine can meaningfully improve metabolic and microbiome health,and biological *** trial should incorporate certain elements in order to provide the much-needed evidence to guide doctors,and also the community at large,to determine whether this promising and inexpensive amino acid is useful in improving human metabolic health.

目的:机器人手术目前已在头颈部多个区域得到了应用,本文探讨香港中文大学头颈外科机器人手术的经验。方法:回顾性分析2015年1月以来在香港中文大学耳鼻咽喉头颈外科开展的所有机器人头颈外科手术病例资料。结果:24例患者采用达芬奇S或达芬奇Xi系统进行了手术,21例患者使用新型柔性机器人达芬奇SP系统行了手术。均无二次手术等严重不良事件。已经发表的达芬奇SP系统手术的7例患者早期结果表明,通过达芬奇SP系统可以到达鼻咽、口咽部、下咽部、喉部和咽后淋巴结完成手术,不需中途改变手术方式,没有发生与使用该系统有关的严重不良事件或不良事件。结论:通过我们在香港中文大学的经验,我们认为采用达芬奇机器人系统进行经口手术或经耳后入路颈部手术是安全有效的。

A cross-sectional study on compliance with topical glaucoma medication and its associated socioeconomic burden for a Chinese population
