Men with a complete absence of normal sperm morphology exhibit high rates of success without assisted reproduction

In couples with infertility, abnormal strict morphology of 0% normal forms （NF） is a criterion to proceed rapidly to in vitro fertilization （IVF）. Since no data currently exist, we investigated the outcomes for men with 0% NF to determine reproductive success without the use of assisted reproductive technologies （ArT）. A cohort of 24 men with 0% NF were identified （2010-2013） with 27 randomly selected men with 〉4% NF as controls. Patient charts were reviewed with men contacted and administered an Institutional review Board （IrB）-approved telephone questionnaire to ascertain outcomes. After a median follow-up time of 2.5 years, 29.2% of men with 0% NF did not require ArT for their first pregnancy （controls = 55.6%, P〈 0.05）. When all pregnancies were analyzed together, men with 0% NF achieved twenty pregnancies of which 75% did not require IVF （controls - thirty pregnancies; 76.7% did not require IVF）. The average age of men and female partners was similar between men with 0% NF and 〉4% NF. All men had normal follicle-stimulating hormone （FSH）, testosterone, prolactin, sex hormone-binding globulin （SHBG）, and estradiol. Although, global semen parameters were worse in men with 0% NF, when a first pregnancy was a natural conception （NC）, 100% of men with 0% NF （n = 7/7） and 37.5% of controls in = 3/8） went on to have a subsequent pregnancy via NC. Men with 0% NF conceived without IVF in 29.2% of cases compared to 55.6% of controls. Strict morphology should not be used to predict fertilization, pregnancy, or live birth potential. In men with 0% NF, alternative modalities should be considered before immediate IVF.

Understanding how alien species assemble is crucial for predicting changes to community structure caused by biological invasions and for directing management strategies for alien species,but patterns and drivers of alien species assemblages remain poorly understood relative to native *** has been suggested as a crucial filter of invasion-driven homogenization of ***,it remains unclear which climatic factors drive the assemblage of alien ***,we compiled global data at both grid scale(2,653 native and 2,806 current grids with a resolution of 2°x 2°)and administrative scale(271 native and 297 current nations and sub-nations)on the distributions of 361 alien amphibians and reptiles(herpetofauna),the most threatened vertebrate group on the *** found that geographical distance,proxy for natural dispersal barriers,was the dominant variable contributing to alien herpetofaunal assemblage in native *** contrast,climatic factors explained more unique variation in alien herpetofaunal assemblage after than before *** pattern was driven by extremely high temperatures and precipitation seasonality,2 hallmarks of global climate change,and bilateral trade which can account for the alien assemblage after *** results indicated that human-assisted species introductions combined with climate change may accelerate the reorganization of global species distributions.

The diagnosis and management of cirrhosis and portal hypertension(PH)with its complications including variceal hemorrhage,ascites,and hepatic encephalopathy continues to *** there are established“standards of care”in liver biopsy and measurement of PH,gastric varices remain an area without a universally accepted therapeutic *** concept of“Endo Hepatology”has been used to describe of the applications of endoscopic ultrasound(EUS)to these ***-liver biopsy(EUS-LB)offers an alternative to percutaneous and transjuglar liver biopsy without compromising safety or efficacy,and with added advantages including the potential to reduce sampling error by allowing biopsies in both hepatic ***,EUS-LB can be performed during the same procedure as EUS-guided portal pressure gradient(PPG)measurements,allowing for the collection of valuable diagnostic and prognostic ***-guided PPG measurements provide an appealing alternative to the transjugular approach,with proposed advantages including the ability to directly measure portal vein *** addition,EUS-guided treatment of gastric varices(GV)offers several possible advantages to current ***-guided treatment of GV allows detailed assessment of the vascular anatomy,similar efficacy and safety to current therapies,and allows the evaluation of treatment effect through doppler ultrasound *** appropriate selection of patients for these procedures is paramount to ensuring generation of useful clinical data and patient safety.

Depositional Model for Peat Swamp and Coal Facies Evolution Using Sedimentology, Coal Macerals, Geochemistry and Sequence Stratigraphy

As one of the most important source rocks and reservoirs of unconventional natural gas, the sedimentary environment and mode of peat swamp（the predecessor of coal seam） is important to the coal seam＇s spatial distribution, material composition, hydrocarbon generation potential, reservoir physical properties, etc. To reveal the depositional characteristics and history of environmental change in a terrestrial basin during a period of peat accumulation, the Middle Jurassic aged #7 coal from Gaoquan in the Qaidam Basin（NW China） was investigated using sedimentology, maceral composition, geochemistry and sequence stratigraphy. Based on identification of the sedimentary shoreline break belt, wave energy depletion point and position of wave base, the peat swamp system can be subdivided into（1） lakeside plain,（2） low energy lakeshore,（3） high energy lakeshore, and（4） shallow lake subfacies. A new method for determining coal facies is proposed based on the combination of environmental parameters including oxidation-reduction levels, energy conditions and the influence of terrigneous sediments. The evolution of the coal seam shows that peat was deposited mainly in the low energy lakeshore and lakeside plain subfacies. Five types of sequence stratigrpahic surface and two types of parasequence were identified. Forced lake regressions and normal lake regressions are attributed as the causes of sequence boundaries. The sequence stratigraphic framework comprises six sequences and corresponding system tracts, and the curve of base-level for each demonstrates a characteristic initial period of slow rising followed by fast rising and then returning to slow rising. A model indicating the relationship among base-level changes, coal facies evolution, and the environmental features in the swamp is proposed that shows the environmental features of the swamp were controlled by both base-level changes and coal facies. Modern societal pressures and expectations over the past several decades have resulted in the tendency for couples to delay conception. While women experience a notable decrease in oocyte production in their late thirties, the effect of age on spermatogenesis is less well described. While there are no known limits to the age at which men can father children, the effects of advanced paternal age are incompletely understood. This review summarizes the current state of knowledge regarding advanced paternal age and its implications on semen quality, reproductive success and offspring health. This review will serve as a guide to physicians in counseling men about the decision to delay paternity and the risks involved with conception later in life.

Precision medicine（PM） proposes customized medical care based on a patient＇s unique genome, biomarkers, environment and behaviors. Hearing loss（HL） is the most common sensorineural disorder worldwide and is frequently caused by a single genetic mutation. With recent advances in PM tools such as genetic sequencing and data analysis, the field of HL is ideally positioned to adopt the strategies of ***, we review current and future applications of PM in HL as they relate to the four core qualities of PM（P4）： predictive, personalized, patient-centered, and participatory. We then introduce a strategy for effective incorporation of HL PM into the design of future research studies, electronic medical records,and clinical practice to improve diagnostics, prognostics, and, ultimately, individualized patient treatment. Finally, specific anticipated ethical and economic concerns in this growing era of genomics-based HL treatment are discussed. By integrating PM principles into translational HL research and clinical practice, hearing specialists are uniquely positioned to effectively treat the heterogeneous causes and manifestations of HL on an individualized basis.

Soft tissue swelling incidence using demineralized bone matrix in the outpatient setting

AIM To assess use of demineralized bone matrix(DBM) use in anterior cervical discectomy and fusion(ACDF) in outpatient *** One hundred and forty-five patients with prospectively collected data undergoing single and two level ACDF with DBM packed within and anterior to polyetheretherketone(PEEK) cages. Two groups created, Group 1(75) outpatients and control Group 2(70) hospital patients. Prevertebral soft tissue swelling(PVSTS) was measured anterior to C2 and C6 on plain lateral cervical radiographs preoperatively and one week postoperatively and fusion assessed at two years. rESULTS There was no intergroup significance between preoperative and postoperative visual analogue scales(VAS)and neck disability index(NDI) scores between Group 1 and 2. Mean preoperative PVSTS in Group 1 was 4.7 ± 0.2 mm at C2 level and 11.1 ± 0.5 at C6 level compared to Group 2 mean PVSTS of 4.5 ± 0.5 mm and 12.8 ± 0.5, P = 0.172 and 0.127 respectively. There was no radiographic or clinical evidence of adverse reaction noted. In Group 1 mean postoperative PVSTS was 5.5 ± 0.4 mm at C2 and 14.9 ± 0.6 mm at C6 compared Group 2 mean PVSTS was 4.9 ± 0.3 mm at C2 and 14.8 ± 0.5 mm at C6, P = 0.212 and 0.946 respectively. No significant increase in prevertebral soft tissue space at C2 and C6 level *** ACDF with adjunct DBM packed PEEK cages showed a statistical significant intragroup improvement in VAS neck pain scores and NDI scores(P = 0.001). There were no reported serious patient complications; post-operative radiographs demonstrated no significant difference in prevertebral space. We conclude that ACDF with DBMpacked PEEK cages can be safely done in an ASC with satisfactory outcomes.
