Precore/basal core promoter mutants and hepatitis B viral DNA levels as predictors for liver deaths and hepatocellular carcinoma

AIM： To conduct a retrospective study in 400 chronic hepatitis B patients in order to identify hepatitis B viral factors associated with complications of liver disease or development of hepatocellular carcinoma. METHODS： The mean follow-up time was 83.6 ± 39.6 mo. Alpha-fetoprotein test and abdominal ultrasound were used for cancer surveillance. Hepatitis B basal core promoter mutants, precore mutants, genotypes, hepatitis B viral DNA （HBV DNA） level and hepatitis B e antigen （HBeAg） were measured. Univariate analysis and logistic regression were used to assess odds ratios for viral factors related to liver deaths and hepatocellular carcinoma development. RESULTS： During follow-up, 38 patients had liver deaths not related to hepatocellular carcinoma. On multivariate analysis, older age [odds ratio： 95.74 （12.13-891.31）, P 〈 0.0001], male sex [odds ratio： 7.61 （2.20-47.95）; P = 0.006], and higher Iogzo HBV DNA [odds ratio： 4.69 （1.16-20.43）; P 〈 0.0001] were independently predictive for these liver related deaths. Also, 31 patients developed hepatocellular carcinoma. Multivariate analysis showed that older age [odds ratio： 26.51 （2.36-381.47）; P = 0.007], presence of precore mutants [odds ratio： 4.23 （1.53-19.58）, P = 0.02] and presence of basal core promoter mutants [odds ratio： 2.93 （1.24-7.57）; P = 0.02] were independent predictors for progression to hepatocellular carcinoma. cONcLUSION： Our results show that high levels of baseline serum HBV DNA are associated with non- hepatocellular carcinoma-related deaths of liver failure, while genetic mutations in the basal core promoter and precore regions are predictive for development of hepatocellular carcinoma.

Background： clinical studies and genetic analyses have revealed that juvenile myelomonocytic leukemia （JMML） is caused by somatic and/or germline mutations of genes involved in the RAS/MAPK signalling pathway. Given the vastly different clinical prognosis among individual patients that have had this disease, mutations in genes of other pathways may be involved. Methods： In this study, we conducted whole-exome and cancer-panel sequencing analyses on a bone marrow sample from a 2-year old juvenile myelomonocytic leukemia patient. We also measured the microRNA profile of the same patient＇s bone marrow sample and the results were compared with the normal mature monocytic cells from the pooled peripheral blood. Results： We identified additional novel mutations in the Pi3K/AKT pathway and verified with a cancer panel targeted sequencing. We have confirmed the previously tested PTPNII gene mutation （exon 3 181G〉T） in the same sample and identified new nonsynonymous mutations in NTRKI, HMGA2, MLH3, MYH9 and AKT1 genes. Many of the microRNAs found to be differentially expressed are known to act as oncogenic MicroRNAs （onco^MicroRNAs or oncomiRs）, whose target genes are enriched in the PI3K/AKT signalling pathway. conclusions： Our study suggests an alternative mechanism for JMML pathogenesis in addition to RAS/MAPK pathway. This discovery may provide new genetic markers for diagnosis and new therapeutic targets for JMML patients in the future.

Adjuvant chemotherapy for intrahepatic cholangiocarcinoma: approaching clinical practice consensus?

Background:Intrahepatic cholangiocarcinoma(Icc)is rare with limited evidence-based *** retrospective study evaluates the use of chemotherapy in patients with resected ***:The Surveillance Epidemiology and End Results(SEER)program database was used to identify patients with resected *** were stratified by date of diagnosis(2000-2004,2005-2009,2010-2014),T,and N *** logistic regression models identified predictors of chemotherapy ***-Meier and cox proportional hazard models were used to identify survival ***:One thousand and two hundred twenty-three patients met inclusion *** utilization increased over time(33%to 41%,P≤0.05).chemotherapy use increased in lymph node(LN)positive patients[32%to 60%in 2010-2014;(P≤0.05)and T3/T4 disease(40%to 60%in 2010-2014;P≤0.01)],but not in patients with LN negative or T1/T2 *** positivity was associated with utilization of chemotherapy in 2005-2009 and *** survival increased from 32 to 41 months(P≤0.05).In LN positive patients,chemotherapy was associated with a decreased hazard ratio of death(P≤0.05)and T3/T4 disease was associated with an increased hazard ratio of death(P≤0.05).conclusions:Adjuvant chemotherapy use in Icc has *** LN positive or patients with T3/T4 tumors are receiving chemotherapy,which may explain the improvement in overall survival.

Observables and Dynamics Quantum to classical from a Relativity Symmetry and Noncommutative-Geometric Perspective

With approaching quantum/noncommutative models for the deep microscopic spacetime in mind, and inspired by our recent picture of the (projective) Hilbert space as the model of physical space behind basic quantum mechanics, we reformulate here the WWGM formalism starting from the canonical coherent states and taking wavefunctions as expansion coefficients in terms of this basis. This provides us with a transparent and coherent story of simple quantum dynamics where both the wavefunctions for the pure states and operators acting on them arise from the single space/algebra, which exactly includes the WWGM observable algebra. Altogether, putting the emphasis on building our theory out of the underlying relativity symmetry—the centrally extended Galilean symmetry in the case at hand—allows one to naturally derive both a kinematical and a dynamical description of a quantum particle, which moreover recovers the corresponding classical picture (understood in terms of the Koopman-von Neumann formalism) in the appropriate (relativity symmetry contraction) limit. Our formulation here is the most natural framework directly connecting all of the relevant mathematical notions and we hope it may help a general physicist better visualize and appreciate the noncommutative-geometric perspective behind quantum physics. It also helps to inspire and illustrate our perspective on looking at quantum mechanics and quantum physics in general in direct connection to the notion of quantum (deformed) relativity symmetries and the corresponding quantum/noncommutative models of spacetime as various levels of approximations all the way down to the Newtonian.

Predictors of nodal metastasis in sinonasal squamous cell carcinoma:A national cancer database analysis

印度板块和亚洲大陆的初始碰撞时间是所有相关的喜马拉雅-西藏造山体系演化模式的主控条件,并严重影响到对众多与青藏高原隆升和东亚大陆挤出相关的地质过程速率的解释,以及对新生代全球气候变化的理解。尽管印度板块和亚洲大陆汇聚的速率在55Ma突然减缓被广泛地认为是初始碰撞的标志,但这次碰撞所造成的主要构造效应直到20多个百万年以后才显现出来。对印度板块和亚洲大陆相对位置的重新估算,表明它们在55Ma时并没有达到可以彼此发生碰撞的距离。基于来自西藏新的野外证据和对已有数据的重新评估,认为初始碰撞发生在始新世—渐新世之交(约34Ma),并对55Ma时发生的地质事件提出了另一种解释。

SemipolarⅢ-nitrides have attracted increasing attention in applications of optoelectronic devices due to the much reduced polarization field.A high-quality semipolar AlN template is the building block of semipolar AlGaN-based deep-ultraviolet light emitting diodes(DUV LEDs),and thus deserves special *** this work,a multi-step in situ interface modification technique is developed for the first time,to our knowledge,to achieve high-quality semipolar AlN *** stacking faults were efficiently blocked due to the modification of atomic configurations at the related *** regrown AlGaN layers were obtained on the in situ treated AlN template,and stacking faults were eliminated in the post-grown AlGaN *** strains between AlGaN layers were relaxed through a dislocation glide in the basal plane and misfit dislocations at the *** contrast,high-temperature ex situ annealing shows great improvement in defect annihilation,yet suffers from severe lattice distortion with strong compressive strain in the AlN template,which is unfavorable to the post-grown AlGaN *** strong enhancement of luminous intensity is achieved in in situ treated AlGaN DUV *** in situ interface modification technique proposed in this work is proven to be an efficient method for the preparation of high-quality semipolar Al N,showing great potential towards the realization of high-efficiency optoelectronic devices.
