Advanced concepts in knee arthrodesis

The aim is to describe advanced strategies that can beused to diagnose and treat complications after knee arthrodesis and to describe temporary knee arthrodesis to treat infected knee arthroplasty. Potential difficult complications include nonunited knee arthrodesis, limb length discrepancy after knee arthrodesis, and united but infected knee arthrodesis. If a nonunited knee arthrodesis shows evidence of implant loosening or failure, then bone grafting the nonunion site as well as exchange intramedullary nailing and/or supplemental plate fixation are recommended. If symptomatic limb length discrepancy cannot be satisfactorily treated with a shoe lift, then the patient should undergo tibial lengthening over nail with a monolateral fixator or exchange nailing with a femoral internal lengthening device. If a united knee arthrodesis is infected, the nail must be removed. Then the surgeon has the option of replacing it with a long, antibiotic cement-coated nail. The authors also describe temporary knee arthrodesis for infected knee arthroplasty in patients who have the potential to undergo insertion of a new implant. The procedure has two goals: eradication of infection and stabilization of the knee. A temporary knee fusion can be accomplished by inserting both an antibiotic cement-coated knee fusion nail and a static antibiotic cement-coated spacer. These advanced techniques can be helpful when treating difficult complications after knee arthrodesis and treating cases of infected knee arthroplasty.

The diagnosis and management of cirrhosis and portal hypertension(PH)with its complications including variceal hemorrhage,ascites,and hepatic encephalopathy continues to *** there are established“standards of care”in liver biopsy and measurement of PH,gastric varices remain an area without a universally accepted therapeutic *** concept of“Endo Hepatology”has been used to describe of the applications of endoscopic ultrasound(EUS)to these ***-liver biopsy(EUS-LB)offers an alternative to percutaneous and transjuglar liver biopsy without compromising safety or efficacy,and with added advantages including the potential to reduce sampling error by allowing biopsies in both hepatic ***,EUS-LB can be performed during the same procedure as EUS-guided portal pressure gradient(PPG)measurements,allowing for the collection of valuable diagnostic and prognostic ***-guided PPG measurements provide an appealing alternative to the transjugular approach,with proposed advantages including the ability to directly measure portal vein *** addition,EUS-guided treatment of gastric varices(GV)offers several possible advantages to current ***-guided treatment of GV allows detailed assessment of the vascular anatomy,similar efficacy and safety to current therapies,and allows the evaluation of treatment effect through doppler ultrasound *** appropriate selection of patients for these procedures is paramount to ensuring generation of useful clinical data and patient safety.

Three-dimensional impact kinetics with foot-strike manipulations during running

Background: Lack of an observable vertical impact peak in fore/mid-foot running has been suggested as a means of reducing lower extremity impact forces, although it is unclear if impact characteristics exist in other axes. The purpose of the investigation was to compare three-dimensional(3 d) impact kinetics among foot-strike conditions in over-ground running using instantaneous loading rate–time ***: Impact characteristics were assessed by identifying peak loading rates in each direction(medial–lateral(ML), anterior–posterior(AP),vertical, and 3 d resultant) following foot-strike instructions(fore-foot, mid-foot, subtle heel, and obvious heel strike). Kinematic and kinetic data were analyzed among 9 male participants in each foot-strike ***: Loading rate peaks were observed in each direction and foot-strike condition, differing in magnitude by direction(3 d resultant and vertical > AP > ML, p ≤ 0.031) and foot-strike: ML(fore-foot and mid-foot strike > obvious heel strike, p ≤ 0.032), AP(fore-foot and mid-foot strikes > subtle-heel and obvious heel strikes, p ≤ 0.023). In each direction, the first loading rate peak occurred later during heel strike running relative to fore-foot(p ≤ 0.019), with vertical and 3 d resultant impact durations exceeding shear(ML and AP, p ≤ 0.007) in each ***: Loading rate–time assessment identified contrasting impact characteristics in each direction and the 3 d resultant following foot-strike manipulations, with potential implications for lower extremity structures in running.

The dimensional and temporal distribution of Antarctic Intermediate Water(AAIW)and North Pacifi c Intermediate Water(NPIW)in the Philippine Sea were explored using Argo profi les and gridded Argo *** the salinity minimum of intermediate water from mid-high latitude of the southern and northern hemisphere of the Pacifi c Ocean,the properties of AAIW and NPIW merge at about 10°N with diff erent properties in the Philippine *** core of AAIW is located below 600 dbar with potential density of 27≤σθ≤27.3 kg/m 3 and salinity of 34.5≤S≤*** core of NPIW is located between 300–700 dbar with potential density of 26.2≤σθ≤27 kg/m 3 and salinity of 34≤S≤*** volume of AAIW and NPIW during January 2004 to december 2017 is negatively *** time series of AAIW and NPIW is dominated by signifi cant periods of 6 and 8 months,*** variations of AAIW and NPIW are mainly aff ected by volume transport through a 130°E section by the North Equatorial Current(NEC)and North Equatorial Undercurrent(NEUC).

Encapsulated peracetic acid as a valid broad‑spectrum antimicrobial alternative,leading to beneficial microbiota compositional changes and enhanced performance in broiler chickens

Background Antimicrobial alternatives are urgently needed,including for poultry production *** this study,we tested the potential broad-range antimicrobial alternative peracetic acid,delivered in feed via the hydrolysis of encapsulated precursors through a 28-day study using 375 Ross 308 broiler *** tested two peracetic acid concentrations,30 and 80 mg/kg on birds housed on re-used litter,and we evaluated the impact of both levels on gut microbial communities,bacterial concentration,antimicrobial resistance genes relative abundance and growth performance when compared to control birds housed on either clean or re-used *** Body weight gain and feed conversion ratio improved in peracetic acid fed *** d 28,birds given 30 mg/kg of peracetic acid had a decreased Firmicutes and an increased Proteobacteria abundance in the jejunum,accompanied by an increase in Bacillus,Flavonifractor and Rombustia in the caeca,and a decreased abundance of tetracycline resistance *** given 80 mg/kg of peracetic acid had greater caecal abundance of macrolides lincosamides and streptogramins resistance *** performance on clean litter was reduced compared to reused litter,which concurred with increased caecal abundance of Blautia,decreased caecal abundance of Escherichia/Shigella,Anaerostipes and Jeotgalicoccus,and greater gene abundance of vancomycin,tetracycline,and macrolides resistance *** Peracetic acid could be used as a safe broad-spectrum antimicrobial alternative in *** precursors were able to reduce the bacterial concentration in the jejunum whilst promoting the proliferation of probiotic genera in the caeca,especially at the low peracetic acid concentrations tested,and improve growth ***,our findings offer further insights on potential benefits of rearing birds on re-used litter,suggesting that the latter could be associated with better performance and reduced antimicrobial resistance

Fecal microbiome of growing pigs fed a cereal based diet including chicory(Cichorium intybus L.) or ribwort(Plantago lanceolata L.) forage

Background： The purpose of this study was to investigate how inclusion of chicory forage or ribwort forage in a cereal-based diet influenced the fecal microbial community（microbiome） in newly weaned（35 days of age） *** piglets were fed a cereal-based diet without（B） and with inclusion（80 and 160 g/kg air-dry forage） of vegetative shoots of chicory（C） and leaves of ribwort（R） forage in a 35-day growth trial. Fecal samples were collected at the start（d0）, 17（d17） and 35（d35） days after weaning and profiles of the microbial consortia were generated using terminal restriction fragment length polymorphism（T-RFLP）. 454-FLX pyrosequencing of 16 S r RNA gene amplicons was used to analyze the microbial composition in a subset of the samples already analyzed with ***： The microbial clustering pattern was primarily dependent on age of the pigs, but diet effects could also be observed. Lactobacilli and enterobacteria were more abundant at d0, whereas the genera Streptococcus, Treponema,Clostridium, Clostridiaceae1 and Coprococcus were present in higher abundances at d35. Pigs fed ribwort had an increased abundance of sequences classified as Treponema and a reduction in lactobacilli. However, the abundance of Prevotellaceae increased with age in on both the chicory and the ribwort diet. Moreover, there were significant correlations between the abundance of Bacteroides and the digested amount of galactose, uronic acids and total non-starch polysaccharides, and between the abundance of Bacteroidales and the digested amount of ***： This study demonstrated that both chicory and ribwort inclusion in the diet of newly weaned pigs influenced the composition of the fecal microbiota and that digestion of specific dietary components was correlated with species composition of the microbiota. Moreover, this study showed that the gut will be exposed to a dramatic shift in the microbial community structure several weeks after weaning.

Tight focusing properties of an azimuthally polarized Gaussian beam with a pair of vortices through a dielectric interface is theoretically investigated by vector diffraction theory. For the incident beam with a pair of vortices of opposite topological charges, the vortices move toward each other, annihilate and revive in the vicinity of focal plane, which results in the generation of many novel focal patterns. The usable focal structures generated through the tight focusing of the double-vortex beams may find applications in micro-particle trapping, manipulation, and material processing, etc.

Tuberculosis(TB)is one of the deadliest infectious diseases in the *** meta-bolic disease type 2 diabetes(T2d)significantly increases the risk of developing ac-tive *** new TB vaccine candidates and novel therapeutic interventions are required to meet the challenges of global TB *** evidence suggests that the microbiota plays a significant role in how the host responds to infection,in-jury and neoplastic *** models that closely reflect human physiology are crucial in assessing new treatments and to decipher the underlying immunological defects responsible for increased TB susceptibility in comorbid *** this study,using a diet-induced murine T2d model that reflects the etiopathogenesis of clinical T2d and increased TB susceptibility,we investigated how the intestinal microbiota may impact the development of T2d,and how the gut microbial composition changes following a very low-dose aerosol infection with Mycobacterium tuberculosis(Mtb).Our data revealed a substantial intestinal microbiota dysbiosis in T2d mice compared to non-diabetic *** observed differences were comparable to previous clini-cal reports in TB patients,in which it was shown that Mtb infection causes rapid loss of microbial ***,diversity index and principle component analyses demonstrated distinct clustering of Mtb-infected non-diabetic mice ***-infected T2d *** findings support a broad applicability of T2d mice as a tractable small animal model for studying distinct immune parameters,microbiota and the immune-metabolome of TB/T2d *** model may also enable answers to be found to critical outstanding questions about targeted interventions of the gut mi-crobiota and the gut-lung axis.
