Advanced concepts in knee arthrodesis

The aim is to describe advanced strategies that can beused to diagnose and treat complications after knee arthrodesis and to describe temporary knee arthrodesis to treat infected knee arthroplasty. Potential difficult complications include nonunited knee arthrodesis, limb length discrepancy after knee arthrodesis, and united but infected knee arthrodesis. If a nonunited knee arthrodesis shows evidence of implant loosening or failure, then bone grafting the nonunion site as well as exchange intramedullary nailing and/or supplemental plate fixation are recommended. If symptomatic limb length discrepancy cannot be satisfactorily treated with a shoe lift, then the patient should undergo tibial lengthening over nail with a monolateral fixator or exchange nailing with a femoral internal lengthening device. If a united knee arthrodesis is infected, the nail must be removed. Then the surgeon has the option of replacing it with a long, antibiotic cement-coated nail. The authors also describe temporary knee arthrodesis for infected knee arthroplasty in patients who have the potential to undergo insertion of a new implant. The procedure has two goals: eradication of infection and stabilization of the knee. A temporary knee fusion can be accomplished by inserting both an antibiotic cement-coated knee fusion nail and a static antibiotic cement-coated spacer. These advanced techniques can be helpful when treating difficult complications after knee arthrodesis and treating cases of infected knee arthroplasty.

Background Antimicrobial alternatives are urgently needed,including for poultry production *** this study,we tested the potential broad-range antimicrobial alternative peracetic acid,delivered in feed via the hydrolysis of encapsulated precursors through a 28-day study using 375 Ross 308 broiler *** tested two peracetic acid concentrations,30 and 80 mg/kg on birds housed on re-used litter,and we evaluated the impact of both levels on gut microbial communities,bacterial concentration,antimicrobial resistance genes relative abundance and growth performance when compared to control birds housed on either clean or re-used *** Body weight gain and feed conversion ratio improved in peracetic acid fed *** d 28,birds given 30 mg/kg of peracetic acid had a decreased Firmicutes and an increased Proteobacteria abundance in the jejunum,accompanied by an increase in Bacillus,Flavonifractor and Rombustia in the caeca,and a decreased abundance of tetracycline resistance *** given 80 mg/kg of peracetic acid had greater caecal abundance of macrolides lincosamides and streptogramins resistance *** performance on clean litter was reduced compared to reused litter,which concurred with increased caecal abundance of Blautia,decreased caecal abundance of Escherichia/Shigella,Anaerostipes and Jeotgalicoccus,and greater gene abundance of vancomycin,tetracycline,and macrolides resistance *** Peracetic acid could be used as a safe broad-spectrum antimicrobial alternative in *** precursors were able to reduce the bacterial concentration in the jejunum whilst promoting the proliferation of probiotic genera in the caeca,especially at the low peracetic acid concentrations tested,and improve growth ***,our findings offer further insights on potential benefits of rearing birds on re-used litter,suggesting that the latter could be associated with better performance and reduced antimicrobial resistance

The effects of dietary supplementation with mushroom or selenium enriched mushroom powders on the growth performance and intestinal health of post-weaned pigs

Background There is an urgent need to identify natural bioactive compounds that can enhance gastrointestinal health and promote pig growth performance in the absence of pharmacological levels of zinc oxide(ZnO).The objectives of this study were to:1)compare the effects of mushroom powder supplemented with inorganic selenium(inSeMP)to mushroom powder enriched with organic selenium(orgSeMP)to pharmacological levels of ZnO on growth performance and faecal scores(FS)for the first 21 d post-weaning(Period 1);and 2)compare the molecular and microbial effects of inSeMP and orgSeMP in these pigs on d 39 post-weaning(Period 2).Methods In Period 1,pigs(3 pigs/pen;8 pens/treatment)were assigned to:(1)basal diet(control);(2)basal diet+zinc oxide(ZnO)(3100 mg/kg d 1–14,1550 mg/kg d 15–21);(3)basal diet+mushroom powder supplemented with inorganic selenium(inSeMP)containing selenium(selenite)content of 0.3 mg/kg feed;(4)basal diet+mushroom powder enriched with organic selenium(orgSeMP)containing selenium(selenocysteine)content of 0.3 mg/kg *** powders were included at 6.5 g/kg of *** In Period 1,there was no effect of diets on average daily gain(AdG)and gain:feed(G:F)ratio(P>0.05).The orgSeMP supplemented pigs had a lower average daily feed intake(AdFI)compared to all other groups(Pd pigs had reduced FS compared to the basal and mushroom group,while the orgSeMP supplemented pigs had lower FS compared to the basal group during the 21 d experimental period(Pd 2,there was no effect of diets on AdFI,AdG and G:F ratio(P>0.05).The orgSeMP supplementation increased the caecal abundance of bacterial members of the Firmicutes and Bacteroidetes phylum,including Lactobacillus,Agathobacter,Roseburia,and Prevotella and decreased the abundance of Sporobacter compared to the basal group,while inSeMP increased the caecal abundance of Prevotella and decreased the caecal abundance of Sporobacter compared to the basal group(Pdietary

Retinal and Choroidal Alterations in Migraine Patients Compared to Normal Healthy Controls

Purpose: Migraine is an incapacitating neurovascular disorder that primarily affects the working-age population. Researchers have postulated that the transient vascular alterations during each migraine attack lead to ischemic damage in the eye which can be measured via optical coherence tomography. Methods: We recruited 29 volunteers: 13 migraineurs (mean age 28 ± 8.8 years;12 female and 1 male) and 16 age-matched controls (mean age 26.6 ± 6.9;9 female and 7 male). All individuals underwent a detailed ophthalmic examination by a qualified optometrist and a Migraine disability Assessment. The investigators were blind to the migraine diagnosis. Retinal Nerve Fiber Layer (RNFL) thickness, Retinal Thickness (RT), Ganglion Cell Complex (GCL), ranging from the inner-limiting membrane to the inner plexiform layer, and Choroidal Thickness (CT) were measured using the 3d OCT-1Maestro, Topcon, a Spectral domain OCT (Sd-OCT) device. Results: In the migraine population average RNFL was lower for several parameters. However, results did not reach statistical significance. A significant decrease in the right eye inferior parafoveal ganglion cell layer in the migraine group of patients (mean = 25.15, Sd = 4.08) compared to normal healthy controls (mean = 28.81, Sd = 4.85;t = (27) = 2.17, p = 0.039) was documented. No other ganglion cell layer or choroidal thickness reached significance. No significant relationship between ocular thickness parameters and MIdAS score, parameters and either MIdAS score or frequency of headaches was found. Conclusion: A significant decrease in the right inferior parafoveal ganglion cell layer for migraine patients was reported. All other parameters did not reach significance. .

Three-dimensional impact kinetics with foot-strike manipulations during running

Background: Lack of an observable vertical impact peak in fore/mid-foot running has been suggested as a means of reducing lower extremity impact forces, although it is unclear if impact characteristics exist in other axes. The purpose of the investigation was to compare three-dimensional(3 d) impact kinetics among foot-strike conditions in over-ground running using instantaneous loading rate–time ***: Impact characteristics were assessed by identifying peak loading rates in each direction(medial–lateral(ML), anterior–posterior(AP),vertical, and 3 d resultant) following foot-strike instructions(fore-foot, mid-foot, subtle heel, and obvious heel strike). Kinematic and kinetic data were analyzed among 9 male participants in each foot-strike ***: Loading rate peaks were observed in each direction and foot-strike condition, differing in magnitude by direction(3 d resultant and vertical > AP > ML, p ≤ 0.031) and foot-strike: ML(fore-foot and mid-foot strike > obvious heel strike, p ≤ 0.032), AP(fore-foot and mid-foot strikes > subtle-heel and obvious heel strikes, p ≤ 0.023). In each direction, the first loading rate peak occurred later during heel strike running relative to fore-foot(p ≤ 0.019), with vertical and 3 d resultant impact durations exceeding shear(ML and AP, p ≤ 0.007) in each ***: Loading rate–time assessment identified contrasting impact characteristics in each direction and the 3 d resultant following foot-strike manipulations, with potential implications for lower extremity structures in running.
