Here we present the case of a 20-year-old male with a known history of an artificial mechanical aortic valve replacement at ages four and twelve, on chronic warfarin therapy, who presented to our community emergency department for the evaluation of vomiting, diarrhea, generalized malaise and abdominal pain. He was seen at a local urgent care for similar symptoms earlier in the day, subsequently discharged with a diagnosis of mild gastroenteritis; however, according to the patient＇s sister, upon his return home he looked very unwell and she called an ambulance immediately.

We present detailed Raman studies of SrZrO_(3)(SZO)that show three anomalies in Raman modes:One has a small jump in frequencyω,one has its intensity vanish,and a third has a sharp change in temperature derivative dω(T)/dT from flat below T=600 K to a Curie-Weiss dependence above 600 K with extrapolation to zero frequency at the known transition temperature T=970 K,thereby proving the latter to be *** addition,the P4mm ferroelectric phase predicted at high stresses has preliminary support from polarization-voltage *** inference of a new transition in the temperature region 600-650 K is in disagreement with neutron *** are given for family member SrSnO_(3) and SrHfO_(3),and we discuss the different conclusions of kennedy and *** show that a known transition in SrHfO_(3) is also displacive with a well-behaved soft mode.

A 27-year-old male presented to the emergency department with acute exercise induced rhabdomyolysis （EIR） following low intensity, high repetition physical activity. It is paramount for the clinician to consider this diagnosis in the differential of the patient presenting with a complaint of musculoskeletal pain. This case highlights the necessity of staying vigilant for a condition that can develop with seemingly minor, repetitive training of a single muscle group, such as in the exercise of calf raises.

INTRODUCTION The presentation of cardiogenic shock （CS） is usually straightforward, and includes hypotension, absence of hypovolemia, and clinical signs of poor tissue perfusion such as oliguria, cyanosis, cool extremities and altered mentation. The most common etiology of CS is acute myocardial infarction, Here, we report a case of a 42-year-old male who presented with right flank pain, nausea and vomiting initially thought to be nephrolithiasis, which he had a history of but was ultimately diagnosed with CS due to a type II myocardial infarction. This case illustrates the importance of having a broad differential diagnosis especially when a patient＇s vital signs take a drastic turn since this patient initially resembled someone with nephrolithiasis.

Dear editor, We report a case of a 47-year-old female who presented with a toxic bupropion ingestion leading to cardiac arrest. She initially exhibited a loss of brainstem reflexes in conjunction with burst-suppression pattern on EEG. Burst suppression is an EEG waveform pattern of alternating isoelectric suppressions and high voltage bursts, Our patient ultimately made a full neurologic recovery a few days later. While there are two other cases in the literature of bupropion overdose resulting in EEG burst-suppression and loss of brainstem reflexes, we believe this is the only reported adult case complicated by cardiac arrest.

Although methylenetetrahydrofolate reductase, a folate enzyme gene, has been associated with idiopathic male infertility, few studies have examined other folate-related metabolites and genes. We investigated whether idiopathic male infertility is associated with variants in folate, vitamin B12 （B12） and total homocysteine （tHcy）-related genes and measured these metabolites in blood. We conducted a case-control study that included 153 men with idiopathic infertility and 184 fertile male controls recruited at the Fertility Center and Antenatal Care Center, University Hospital, Malmo and Lund, Sweden. Serum folate, red cell folate （RCF）, serum B12, plasma tHcy and semen quality were measured. Subjects were genotyped for 20 common variants in 12 genes related to folate/B12/ homocysteine metabolism. Metabolite concentrations and genotype distributions were compared between cases and controls using linear and logistic regression with adjustment for covariates. The phosphatidylethanolamine N-methyltransferase （PEMT） M 175V and TCblR rs173665 polymorphisms were significantly associated with infertility （P=0.01 and P=0.009, respectively）, but not with semen quality. Among non-users of supplements, infertile men had lower serum folate concentrations than fertile men （12.89 vs. 14.73 nmoll^- 1 P=0.02）, but there were no significant differences in RCF, B 12 or tHcy. Folate, B 12 and tHcy concentrations were not correlated with any semen parameters. This study provides little support for low folate or B12 status in the pathogenesis of idiopathic male infertility. Although additional data are needed to confirm these initial findings, our results suggest that PEMTand TCbIR, genes involved in choline and B12 metabolism, merit further investigation in idiopathic male infertility.

Background Soil-transmitted helminths (STH) affect approximately 1.5 billion people globally. The current STH control strategy is annual or twice-annual preventive chemotherapy, typically school-based deworming targeting children and women of reproductive age. Mathematical modeling suggests that it may be possible to interrupt STH transmission through high-coverage community-wide mass drug administration (cMDA). DeWorm3 is a cluster randomized trial testing cMDA for prevalence reduction and transmission interruption. The purpose of this study is to describe coverage of cMDA in study clusters over time and correlates of coverage at individual and cluster *** From 2018-2020, DeWorm3 delivered six rounds of cMDA with 400 mg albendazole at sites in Benin, India, and Malawi. We report coverage, treatment uptake, and directly observed therapy across all rounds. Factors associated with coverage at the cluster level were identified using binomial generalized estimating equations, while factors associated with non-treatment at the individual level were identified using binomial mixed-effects *** Coverage was high across all clusters and rounds, exceeding the WHO target of 75% in all sites and across all rounds (78% to 95%); cluster-level coverage tended to increase over time. Younger, unmarried, and migratory adults were more likely to be untreated at all sites; adult males were more likely to be untreated in Benin and Malawi. Among children, girls were more likely to be untreated, as were non-school-attending and migratory children. Higher adult education was associated with greater odds of non-treatment among adults, but lower odds among children in the household. Global climate change affects many facets of avian ecology, such as shifts in breeding phenology and migration patterns. Migrating bird species respond to changes in climate by shifting their temporal patterns of spring migration. However, variation in species’ responses exists based on various life history traits, which exposes some species to an increased risk of phenological mismatch. This study examined the spring arrival dates of 115 migrating species over 127 years (1889-2015) using archival sources in West Virginia, USA, making this research unique in the length of study, the high number of species studied, and the historical crowd-sourced observations analyzed. Of the 115 taxa, 45 showed significant negative slopes of spring arrival dates (arriving earlier in the spring) plotted against the year. In contrast, only nine species showed positive slopes (arriving later in the spring), albeit non-significant. The average advance of spring arrival date for all species was 1.7 days per decade, and an advance of 2.6 days per decade in species that showed significance. Arrival dates were associated with increasing spring temperatures—for each 1˚C increase, the arrival date advanced by 0.81 days/decade. Several life history traits were linked to species that advanced their first arrival dates, including a shorter distance migrated to reach wintering grounds, increasing populations, and foraging habitat. Most avian species are advancing their spring arrival dates in response to climate change. However, the implications of earlier spring arrival are unclear. We draw attention to shifts in arrival dates and wintering ranges, leading to a possible increase in overwintering in the mid-latitudes of North America.
