Donor risk factors in pancreas transplantation

The aim of the work was to analyze and expose the donor and recipient riskfactors in pancreas transplantation. In the following paper, we exposed the 2018spanish Consensus Document on Donor and Recipient selection Criteria forPancreas Transplantation. An assessment of the previous selection Criteria forDonors and Recipients of Pancreas Transplantation, published in 2005 by thespanish Pancreas Transplant Group (GETP) and the National TransplantOrganization (ONT) was performed. A literature review was performed usingCochrane library, PubMed and Google scholar databases. some of the followingterms were used for the literature search: “Diabetes Mellitus,” “PancreasTransplantation,” “Insulin-secreting Cells,” “Pancreas Allograft Thrombosis,”“Allograft Pancreatitis,” “Donors’ Risk Factors,” “Recipients’ Risk Factors,”“Pancreas Allograft Rejection” and “Pancreas Allograft survival.” After anextended search, different inclusion criteria were established. Articles anddocuments with abstracts of full text and in English or spanish language wereselected. subsequently, different scientific meetings took place during 2015 and2016 by the GETP. Finally, the updated criteria were published by the GETP andONT in 2018. several risk factors have been described in pancreas transplantationthat can be divided into donor risk factors: Advanced age (> 50 years);high bodymass index (BMI) (> 30 kg/m2);cause of death (e.g., stroke);previoushyperglycemia;hyperamylasemia;cold ischemia time (greater than 8 or 12 h,depending on the type of donation);the use of vasopressors in the intensive careunit or cardiac arrest;and the macroscopic aspect of the pancreas allograft. Thefollowing are recipient risk factors: Advanced age (> 50 years);active smoking;high BMI (> 30 kg/m2);and peripheral artery disease or sensorimotorpolyneuropathy. Based on the aforementioned parameters, different selectioncriteria have been established for the recipients depending on the type of pancreastransplantation. Knowledge

The number of people with food allergy(FA)has increased in recent decades,as has the number of foods to which they are *** of FA prevalence vary widely,likely because of different definitions,evaluated foods and geo-graphic *** developed countries,overall preva-lence estimates range from 1.9%to 7.2%[1,2].In latin America,the prevalence of FA in children is between 1.4%and 5.3%[3-6],with important age-based variations(from 1.9%in infants to 0.4%in preschoolers)[7].FA is a complex trait in which the interplay of genetic and environmental fac-tors results in disease ***-known factors that increase atopic susceptibility include sex[8,9],age[10],and socioeconomic status[11-13].Furthermore,the asso-ciation between family history of allergies and childhood FA has been consistently reported in recent ***,a maternal history of allergic disease appears to be more strongly associated with FA than paternal history of atopy[14].

The relationship between DNA fragmentation and the intensity of morphologically abnormal human spermatozoa

Objective:To determine the relationship between teratozoospermia and sperm DNA fragmentation(sDF)in the human ***:This retrospective study included 100 normozoospermic men as a control cohort(abnormal forms>14%),210 patients with a high level of abnormal forms(≤4%)and 65 patients presenting with a moderate level of abnormal forms(>4%to≤14%)based on the World Health Organization *** morphology was assessed using bright field *** DNA fragmentation was assessed using the sperm chromatin dispersion ***-parametric analyses were conducted to examine the relationship between abnormal sperm morphology and sperm DNA fragmentation;receiver operating characteristic(ROC)analyses were conducted to assess sensitivity and specificity of this ***:A correlation analysis revealed that the higher the proportion of abnormal spermatozoa in the ejaculate,the higher the level of sDF(spearman's Rho=-0.230;Psignificant differences in the proportion of sDF were found when all cohorts were compared(Pse significant differences were also retained when the different cohorts were compared *** analysis showed a moderate but significant predictive value for sDF to differentiate patients with different levels of ***:Although analysis of a more continuous range of values for teratozoospermia would help further clarify any causal relationship with sDF,there is clearly a synergistic or coincident affiliation between these variables that needs to be acknowledged by the clinician when interpreting the spermiogram.

Fibrosing cholestatic hepatitis(FCH) is a variant of viral hepatitis reported in hepatitis B virus or hepatitis C virus infected liver,renal or bone transplantation recipients and in leukemia and lymphoma patients after conventional cytotoxic *** constitutes a well-described form of fulminant hepatitis having extensive fibrosis and severe cholestasis as its most characteristic pathological ***,we report a case of a 49-year-old patient diagnosed with small-cell lung cancer who developed this condition following conventional chemotherapy-induced *** is the first reported case in the literature of FCH after conventional chemotherapy for a solid *** addition to a detailed report of the case,a physiopathological examination of this potentially life-threatening condition and its treatment options are discussed.

Detection of KRAs G12D in colorectal cancer stool by droplet digital PCR

AIMTo assess KRAs G12D mutation detection by droplet digital PCR (ddPCR) in stool-derived DNA from colorectal cancer (CRC) *** this study, tumor tissue and stool samples were collected from 70 patients with stage I-IV CRC diagnosed by preoperative biopsy. KRAs mutational status was determined by pyrosequencing analysis of DNA obtained from formalin-fixed paraffin-embedded (FFPE) tumor tissues. The KRAs G12D mutation was then analyzed by ddPCR in FFPE tumors and stool-derived DNA from patients with this point mutation. Wild-type (WT) tumors, as determined by pyrosequencing, were included as controls; analysis of FFPE tissue and stool-derived DNA by ddPCR was performed for these patients as *** the total 70 patients included, KRAs mutations were detected by pyrosequencing in 32 (45.71%), whereas 38 (54.29%) had WT tumors. The frequency of KRAs mutations was higher in left-sided tumors (11 located in the right colon, 15 in the left, and 6 in the rectum). The predominant point mutation was KRAs G12D (14.29%, n = 10), which was more frequent in early-stage tumors (I-IIA, n = 7). In agreement with pyrosequencing results, the KRAs G12D mutation was detected by ddPCR in FFPE tumor-derived DNA, and only a residual number of mutated copies was found in WT controls. The KRAs G12D mutation was also detected in stool-derived DNA in 80% of all fecal samples from CRC patients with this point *** is a reliable and sensitive method to analyze KRAs G12D mutation in stool-derived DNA from CRC patients, especially at early stages. This non-invasive approach is potentially applicable to other relevant biomarkers for CRC management.

Can DNA fragmentation of neat or swim-up spermatozoa be used to predict pregnancy following ICsI of fertile oocyte donors？

This study compared the potential of assessing sperm DNA fragmentation （sDF） from neat semen and the subsequent swim-up （sU） procedure to predict pregnancy when conducting ICsI of fertile donor oocytes. Infertile females （n=81） were transferred embryos resulting from intracytoplasmic sperm injection （ICsI） of their partner＇s spermatozoa and proven donor oocytes. This model normalized the impact of female factor in putative sperm DNA repair. semen was blindly assessed for sDF using Halosperm immediately following ejaculation （Ns） and after swim-up at the time of ICsI fertilisation. There was a decrease in sDF values of the ejaculated semen sample following the swim-up protocol （P=0.000）. Interestingly, pregnancy could be equally predicted from sDF values derived from either neat or swim-up semen samples. Receiver operator curves and the derived Youden＇s indices determined sDF cutoff values for Ns and sU of 24.8% and 17.5%, respectively. Prediction of pregnancy from Ns sDF had a sensitivity of 75% and a specificity of 69%, whereas for sU sDF was 78% and 73%, respectively. While increased levels of sDF negatively impact reproductive outcome, we have shown that a reduction in sDF following sperm selection using ICsI with proven donor oocytes is not mandatory for achieving pregnancy. This suggests that a certain level of DNA damage that is not detectable using current technologies could be impacting on the relative success of assisted reproductive technology （ART） procedures. Consequently, we propose a modification of the so called ＇iceberg model＇ as a possible rationale for understanding the role of sDF in reproductive outcome.

The reliability optimization process in earthquake engineering requires that the expected present value of the total cost is minimized, including the initial cost as well as the damage costs caused by earthquakes, which include indirect costs. One of these costs is concerned with how much society is willing to invest for preserving human life. Ethical principles, on which to base the assessment of this cost, are presented and discussed in this work. Individual and social values are analyzed. Finally, an optimal seismic design coefficient with the results obtained is calculated for a site with low seismicity.

Assessment of Intangible losses in Earthquake Engineering

In order to find optimum design parameters in earthquake engineering, an objective function is optimized. This function comprises the initial cost of a structure and the cost due to the damage of earthquakes. Intangible losses may be included in the latter, such as how much society is willing to invest to preserve a human life. In this paper, the expression of the objective function is developed in terms of the seismic design coefficient, and the aforementioned intangible loss is calculated from both the individual point of view and that of society. The calculation of the intangible is based on utility curves. Finally, optimum seismic design coefficients are calculated for a firm ground site.
