Endoscopic retrograde cholangiopancreatography with ampullary biopsy vs ERCP alone:a matched-pairs controlled evaluation of outcomes and complications

Mechanical strength and corrosion resistance of Al-additive friction stir welded AZ31B joints

Compared to other structural alloys,magnesium alloys have a relatively poor corrosion resistance and low mechanical strength,which can be further deteriorated when these alloys are subjected to joining processes using the existing joining ***,we propose for the first time an additive friction stir-welding(AFSW)using fine Al powder as an additive to improve the mechanical strength as well as corrosion resistance of AZ31B weld *** is a solid-state welding method of forming a high-Al AZ31B joint via an in-situ reaction between pure Al powders filled in a machined groove and the AZ31B *** optimize the process parameters,AFSW was performed under different rotational and transverse speeds,and number of passes,using tools with a square or screw *** particular,to fabricate a weld zone,where the Al was homogenously dispersed,the effects of the groove shape were investigated using three types of grooves:surface one-line groove,surface-symmetric grooves,and inserted symmetric *** homogenous and defect-less AFS-welded AZ31B joint was successfully fabricated with the following optimal parameters:1400 rpm,25 mm/min,four passes,inserted symmetric grooves,and the tool with a square *** AFSW fully dissolved the additive Al intoα-Mg and in-situ precipitated Mg_(17)Al_(12)particles,which was confirmed via scanning electron microscopy,transmission electron microscope,and X-ray diffraction *** microhardness,joint efficiency,and elongation at the fracture point of the AFS-welded AZ31B joint were 80 HV,101%,and 8.9%,*** values are higher than those obtained for the FS-welded AZ31 joint in previous *** corrosion resistance of the AFS-welded AZ31B joint,evaluated via hydrogen evolution measurements and potentiodynamic polarization tests,was enhanced to 55%relative to the FS-welded AZ31B joint.

Calculation of the intake retention fraction and dose coefficients in ^99mTc-labelled compound for internal exposure for medical workers

For recent decades, a considerable amount of99mTc has been stimulated for diagnosis radiopharmaceuticals because of its physical advantages. The increase in the use of99mTc makes naturally more chances of internal exposure for not only patients but also medical workers. The patient internal exposure by an intravenous injec- tion has been assessed relatively well with the reports of International Commission on Radiological Protection (ICRP) or Medical Internal Radiation Dose(MIRD). However, there are few studies which can support its accu- rate assessment for medical worker who treats99mTc. In spite of the absence of information, the physiological information of each99mTc-labelled compound for patient, provided by ICRP, can be used optionally for worker exposure because the behaviors after uptake to blood are similar for patient and worker. Using the data, in this study, the data for bioassay were given as the intake whole-body retention and urinary excretion function. We selected the two most frequently used99mTc-labelled compounds based on statistical data;99mTc-phosphonate, pertechnetate. The data of the Human Alimentary Tract Model (HATM, publication 100) and the revised Human Respiratory Tract Model (revised HRTM, OIR) were used for compartment models which depict the physio- logical behavior in body after intake. In case of99mTc-phosphonate, we adopted the systemic model for patient intake described in ICRP publication 53 based on the assumption that the behaviors after uptake to blood are similar for patient and worker. On the other hand, recent updated systemic model in OIR report could be di- rectly adopted for pertechnetate. We used Birchall’s algorithm for calculation and developed the module which could calculate the retention amounts in each compartment at given time by MATLAB. Multiphoton microscopy:An introduction to gastroenterologists

Multiphoton microscopy, relying on the simultaneous absorption of two or more photons by a fluorophore, has come to occupy a prominent place in modern biomedical research with its ability to allow real-time observation of a single cell and molecules in intact tissues. Multiphoton microscopy exhibits nonlinear optical contrast properties, which can make it possible to provide an exceptionally large depth penetration with less phototoxicity. This system becomes more and more an inspiring tool for a non-invasive imaging system to realize "optical biopsy" and to examine the functions of living cells. In this review, we briefly present the physical principles and properties of multiphoton microscopy as well as the current applications in biological fields. In addition, we address what we see as the future potential of multiphoton microscopy for gastroenterologic research.

Ethanol Extract of Lycopodium serratum Thunb. Attenuates Lipopolysaccharide-Induced C6 Glioma Cells Migration via Matrix Metalloproteinase-9 Expression

Objectives： To elucidate how ethanol extract of L. serratum（ELS） could exert anti-migratory effects on glioma with the suppression of nuclear factor kappa B（NF-κB） downstream pathway. Methods： Cell viability of ELS on C6 glioma was detected by 3-（4,5-dimethylthiazol-2-yl）-2,5-diphenyltetrazolium bromide（MTT） assay. Nitric oxide（NO） assay and 2＇,7＇-dichlorofluorescin diacetate（DCFH-DA） assay were applied to measure NO production and reactive oxygen species（ROS） generation on lipopolysaccharide（LPS）-induced C6 glioma cells. NF-κB, mitogen-activated protein kinase（MAPK）, inducible nictric oxide synthase（i NOS） and cyclooxygenase-2（COX-2） protein were determined by Western blot. Wound healing assay was used to investigate the inhibitory effect of ELS on fetal bovine serum（FBS）-induced migration and matrix metalloproteinase（MMP）-9 and-2 activity was examined by zymography. Results： ELS suppressed LPS-induced phosphorylation of extracellular signal-regulated kinase（ERK）, c-Jun N-terminal kinase（JNK）, and p38 through inhibiting the expression of chemokine CCL2（or monocyte chemoattractant protein-1, MCP-1）. In addition, ELS inhibited the expression of i NOS, COX-2, and the production of NO by LPS in C6 glioma cells. ELS also significantly decreased serum-induced migration of C6 glioma cells in scratch wound healing in a dose-dependent manner（P〈0.01）. The activity of MMP-9 and-2 were also significantly attenuated by ELS with LPS treatment（P〈0.01）. Conclusion： Our results suggest that downregulation of MMP-9 gene expression might be involved in the anti-migration effect of ELS against LPS-induced C6 glioma cells.

Endoscopic en bloc resection of an exophytic gastrointestinal stromal tumor with suction excavation technique

Here, we report the first successful endoscopic resection of an exophytic gastrointestinal stromal tumor (GIST) using a novel perforation-free suction excavation technique. A 49-year-old woman presented for further management of a gastric subepithelial tumor on the lesser curvature of the lower body, originally detected via routine upper gastrointestinal endoscopy. Abdominal computed tomography and endoscopic ultrasound showed a 4-cm extraluminally protruding mass originating from the muscularis propria layer. The patient firmly refused surgical resection owing to potential cardiac problems, and informed consent was obtained for endoscopic removal. Careful dissection and suction of the tumor was repeated until successful extraction was achieved without serosal injury. We named this procedure the suction excavation technique. The tumor’s dimensions were 3.5 cm × 2.8 cm × 2.5 cm. The tumor was positive for C-KIT and CD34 by immunohistochemical staining. The mitotic count was 6/50 high-power fields. The patient was followed for 5 years without tumor recurrence. This case demonstrated the use of endoscopic resection of an exophytic GIST using the suction excavation technique as a potential therapy without surgical resection.

We report our experience with a synchronous case of gastrointestinal stromal tumor(GIST) and intraductal papillary neoplasm of the bile duct(IPNB) in anelderly woman with neurofibromatosis type 1(NF-1). A 72-year-old woman presented with a 2-mo history of right upper abdominal pain unrelated to diet and indigestion. Fourteen years earlier, she had been diagnosed with NF-1, which manifested as café au lait spots and multiple nodules on the skin. Computed tomography(CT) revealed a multilocular low-density mass with septation, and mural nodules in the right hepatic lobe, as well as a 1.7-cm-sized well-demarcated enhancing mass in the third portion of the duodenum. The patient subsequently underwent right hepatectomy and duodenal wedge resection. We present here the first report of a case involving a synchronous IPNB and GIST in a patient with NF-1. Self-expandable metal stents in patients with postoperative delayed gastric emptying after distal gastrectomy

AIM To investigate the efficacy and safety of endoscopic stent insertion in patients with delayed gastric emptying after *** In this study, we prospectively collected data from patients who underwent stent placement for delayed gastric emptying(DGE) after distal gastrectomy between June 2010 and April 2017, at a tertiary referral academic center. Clinical improvement, complications, and consequences after stent insertion were *** Technical success was achieved in all patients(100%). Early symptom improvement was observed in 15 of 20 patients(75%) and clinical success was achieved in all patients. Mean follow-up period was 1178.3 ± 844.1 d and median stent maintenance period was 51 d(range 6-2114 d). During the follow-up period, inserted stents were passed spontaneously per rectum without any complications in 14 of 20 patients(70%). Symptom improvement was maintained after stent placement without the requirement of any additional intervention in 19 of 20 patients(95%).CONCLUSION Endoscopic stent placement provides prompt relief of obstructive symptoms. Thus, it can be considered an effective and safe salvage technique for post-operative DGE.
