Cytomegalovirus colitis induced segmental colonic hypoganglionosis in an immunocompetent patient: A case report

BACKGROUND Cytomegalovirus(CMV)colitis is usually seen in immunocompromised patients with risk factors such as human immunodeficiency virus infection,solid organ transplant,inflammatory bowel disease,or malignancy.Therefore,many clinicians usually do not consider the possibility of CMV colitis in immunocompetent patients.We reported a rare case of segmental colonic hypoganglionosis associated with CMV colitis in an immunocompetent patient.CASE SUMMARY A 48-year-old woman with no underlying disease was admitted to our hospital for severe abdominal pain and constipation.Computed tomography of the abdomen showed diffuse dilatation of the small intestine and the entire colon.Initial sigmoidoscopic findings and result of polymerase chain reaction(PCR)for CMV revealed the compatible findings of CMV colitis,the patient was treated with intravenous ganciclovir.After treatment,sigmoidoscopic findings and CMV PCR results improved.However the patient continued to suffered from constipation.Eight months after the initial admission,patient visited the emergency department with severe abdominal pain and imaging revealed aggravation of fecal impaction and bowel dilatation.We performed subtotal colectomy to control patient’s symptom.Histological examination of the resected specimen showed significantly reduced number of mature ganglion cells in the sigmoid colon compared to that in the proximal colon.CONCLUSION Our case demonstrates that CMV colitis can develop even in patients with no other underlying disease,and that CMV colitis can be one of the causes for developing colonic hypoganglionosis.

Effects of Chairside Polishing and Brushing on SurfaceRoughness of Acrylic Denture Base Resins

The effects of 3 chairside polishing kits and mechanical brushing on the surface roughness of 3 different acrylic denture base resins were compared. Acrylic denture base resins （auto-polymerizing, heat-polymerizing, injected heat-polymerizing resins） were examined after a tungsten carbide bur, and after chairside polishing using 3 polishing kits and pumice. The specimens were subjected to mechanical brushing using a wear tester to simulate 30 000 strokes of brushing. The surface roughness of the acrylic denture base resin specimens was measured using a contact pro-filometer. After the test, the random polished acrylic resins were evaluated by scanning electron mi-croscopy （SEM） and atomic force microscopy （AFM）. Acrylic denture base resins polished using the 3 types of polishing kits had a smoother surface than those finished with the tungsten carbide bur （p〈0.05）. The surface of the resin polished by a TC cutter exceeded the Ra of 0.2 μm （p〈0.05）. The auto-polymerizing resin showed a significantly higher surface roughness than the heat-polymerizing resin and injected heat-polymerizing resin （p〉0.05）. In the case of polishing step wise, there was almost no change in surface roughness after brushing （p〉0.05）.

Endoscopic features suggesting gastric cancer in biopsy-proven gastric adenoma with high-grade neoplasia

AIM:To elucidate the endoscopic features that predict the cancer following endoscopic submucosal dissection(ESD)in patients with high-grade neoplasia(HGN).METHODS:We retrospectively analyzed the medical records of patients who underwent ESD of gastric neoplasms from January 2007 to September 2010.ESD was performed in 555 cases involving 550 patients.A total of 112 lesions from 110 consecutive patients were initially diagnosed as HGN without cancer by forceps biopsy,and later underwent ESD.We classified lesions into two groups according to histologic discrepancies between the biopsy and ESD diagnosis.Gastric adenoma in the final diagnosis by ESD specimens were defined as adenoma group.Lesions with coexisting cancer after ESD were defined as cancer group.RESULTS:The mean age was 65.3 years,and 81 patients were male.There was no significant difference in the age or gender distribution between the adenoma(n=52)and cancer(n=60)groups.Thirty-six of these lesions(32.1%)showed histologic concordance between the forceps biopsy and ESD specimens,16(14.3%)showed a downgraded histology(low-grade neoplasia),and 60(53.6%)showed an upgraded histology(cancer).A red color change of the mucosal surface on endoscopy was found in 27/52(51.9%)of cases in the adenoma group and in 46/60(76.7%)of cases in the cancer group(P=0.006).Ulceration of the mucosal surface on endoscopy was found in 5(9.6%)of 52 lesions in the adenoma group and in 17(28.3%)of 60 lesions in the cancer group(P=0.013).In the multivariate analysis,a reddish surface color change and mucosal ulceration were significant predictive factors correlated with cancer after ESD of the HGN by forceps biopsy.CONCLUSION:HGN with a red color change or mucosal ulceration correlated with the presence of gastric cancer.These finding may help to guide the diagnosis and treatment.

Surgical failure after colonic stenting as a bridge to surgery

AIM: To identify risk factors for surgical failure after colonic stenting as a bridge to surgery in left-sided malignant colonic obstruction.METHODS: The medical records of patients who underwent stent insertion for malignant colonic obstruction between February 2004 and August 2012 were retrospectively reviewed. Patients with malignant colonic obstruction had overt clinical symptoms and signs of obstruction. Malignant colonic obstruction was diagnosed by computed tomography and colonoscopy. A total of 181 patients underwent stent insertion during the study period; of these, 68 consecutive patientswere included in our study when they had undergone stent placement as a bridge to surgery in acute leftsided malignant colonic obstruction due to primary colon cancer.RESULTS: Out of 68 patients, forty-eight(70.6%) were male, and the mean age was 64.9(range, 38-89) years. The technical and clinical success rates were 97.1%(66/68) and 88.2%(60/68), respectively. Overall, 85.3%(58/68) of patients underwent primary tumor resection and primary anastomosis. Surgically successful preoperative colonic stenting was achieved in 77.9%(53/68). The mean duration, defined as the time between the SEMS attempt and surgery, was 11.3 d(range, 0-26 d). The mean hospital stay after surgery was 12.5 d(range, 6-55 d). On multivariate analysis, the use of multiple self-expanding metal stents(OR = 28.872; 95%CI: 1.939-429.956, P = 0.015) was a significant independent risk factor for surgical failure of preoperative stenting as a bridge to surgery. Morbidity and mortality rates in surgery after stent insertion were 4.4%(3/68) and 1.5%(1/68), respectively.CONCLUSION: The use of multiple self-expanding metal stents appears to be a risk factor for surgical failure.

Biodegradable metals as electrodes, interconnectors, and device conductors are essential components in the emergence of transient electronics, either for passive implants or active electronic devices, especially in the fields of biomedical electronics. Magnesium and its alloys are strong candidates for biodegradable and implantable conducting materials because of their high conductivity and biocompatibility, in addition to their well-understood dissolution behavior. One critical drawback of Mg and its alloys is their considerably high dissolution rates originating from their low anodic potential, which disturbs the compatibility to biomedical applications. Herein, we introduce a single-phase thin film of a Mg-Zn binary alloy formed by sputtering, which enhances the corrosion resistance of the device electrode, and verify its applicability in biodegradable electronics. The formation of a homogeneous solid solution of single-phase Mg-3Zn was confirmed through X-ray diffraction and transmission electron microscopy. In addition, the dissolution behavior and chemistry was also investigated in various biological fluids by considering the effect of different ion species. Micro-tensile tests showed that the Mg-3Zn alloy electrode exhibited an enhanced yield strain and elongation in relation to a pure Mg electrode. Cell viability test revealed the high biocompatibility rate of the Mg-3Zn binary alloy thin film. Finally, the fabrication of a wireless heater demonstrated the integrability of biodegradable electrodes and highlighted the ability to prolong the lifecycle of thermotherapy-relevant electronics by enhancing the dissolution resistance of the Mg alloy.

Endoscopic stenting for recurrence-related colorectal anastomotic site obstruction:Preliminary experience

AIM:To evaluate the efficacy of stents in treating patients with anastomotic site obstructions due to cancer recurrence following colorectal surgery.METHODS:The medical records of patients who underwent endoscopic self-expanding metal stents(SEMS)insertion for colorectal obstructions between February2004 and January 2014 were retrospectively reviewed.During the study period,a total of 218 patients underwent endoscopic stenting for colorectal obstructions.We identified and examined the patients who underwent endoscopic stenting for obstructions caused by cancer recurrence at the anastomotic site following colorectal surgeries for primary colorectal cancer.RESULTS:Five consecutive patients[mean age,56.4years(range:39-82 years);4 women,1 man]underwent endoscopic stenting for obstructions caused by cancer recurrence at the anastomotic site following colorectal surgeries for primary colorectal cancer.Technical and clinical success was achieved in all 5 patients,without any early complications.During follow-up,3patients did not need further intervention,prior to their death,after the first stent insertion;thus,the overall success rate was 3/5(60%).Perforations occurred in 2patients who required a second SEMS insertion due to re-obstruction;none of the patients experienced stent migration.CONCLUSION:SEMS placement is a promising treatment option for patients who develop obstructions of their colonic anastomosis sites due to cancer recurrence.

Melatonin modulates adiponectin expression on murine colitis with sleep deprivation

AIM To determine adiponectin expression in colonic tissue of murine colitis and systemic cytokine expression after melatonin treatments and sleep deprivation.METHODS The following five groups of C57BL/6 mice were used in this study:(1) group Ⅰ,control;(2) group Ⅱ,2% DSS induced colitis for 7 d;(3) group Ⅲ,2% DSS induced colitis and melatonin treatment;(4) group Ⅳ,2% DSS induced colitis with sleep deprivation(SD) using specially designed and modified multiple platform water baths; and(5) group V,2% DSS induced colitis with SD and melatonin treatment. Melatonin(10 mg/kg) or saline was intraperitoneally injected daily to mice for 4 d. The body weight was monitored daily. The degree of colitis was evaluated histologically after sacrificing the mice. Immunohistochemical staining and Western blot analysis was performed using anti-adiponectin antibody. After sampling by intracardiac punctures,levels of serum cytokines were measured by ELISA. RESULTS Sleep deprivation in water bath exacerbated DSS induced colitis and worsened weight loss. Melatonin injection not only alleviated the severity of mucosal injury,but also helped survival during stressful condition. The expression level of adiponectin in mucosa was decreased in colitis,with the lowest level observed in colitis combined with sleep deprivation. Melatonin injection significantly(P < 0.05) recovered the expression of adiponectin. The expression levels of IL-6 and IL-17 were increased in the serum of mice with DSS colitis but decreased after melatonin injection. CONCLUSION This study suggested that melatonin modulated adiponectin expression in colonic tissue and melatonin and adiponectin synergistically potentiated antiinflammatory effects on colitis with sleep deprivation.

Background: The Korea National Health and Nutrition Examination Survey nonalcoholic fatty liver disease (K-NAFLD) score was recently developed with the intent to operationally define nonalcoholic fatty liver disease (NAFLD). However, there remained an external validation that confirmed its diagnostic performance, especially in patients with alcohol consumption or hepatitis virus infection. Methods: Diagnostic accuracy of the K-NAFLD score was evaluated in a hospital-based cohort consisting of 1388 participants who received Fibroscan (R). Multivariate-adjusted logistic regression models and the contrast estimation of receiver operating characteristic curves were used for validation of the K-NAFLD score, fatty liver index (FLI), and hepatic steatosis index (HSI). Results: K-NAFLD-moderate [adjusted odds ratio (aOR) = 2.53, 95% confidence interval (CI): 1.13-5.65] and K-NAFLD-high (aOR = 4.14, 95% CI: 1.69-10.13) groups showed higher risks of fatty liver compared to the K-NAFLD-low group after adjustments for demographic and clinical characteristics, and FLI-moderate and FLI-high groups revealed aORs of 2.05 (95% CI: 1.22-3.43) and 1.51 (95% CI: 0.78-2.90), respectively. In addition, the HSI was less predictive for Fibroscan (R)-defined fatty liver. Both K-NAFLD and FLI also demonstrated high accuracy in the prediction of fatty liver in patients with alcohol consumption and chronic hepatitis virus infection, and the adjusted area under curve values were comparable between K-NAFLD and FLI. Conclusions: Externally validation of the K-NAFLD and FLI showed that these scores may be a useful, noninvasive, and non-imaging modality for the identification of fatty liver. In addition, these scores also predicted fatty liver in patients with alcohol consumption and chronic hepatitis virus infection. Incidence and Related Risk Factors of junctional Ectopic Tachycardia in Infants after Cardiac Surgery for Congenital Heart Disease

Objective:junctional ectopic tachycardia is common after cardiac surgery for congenital heart disease.However,its incidence and related risk factors in infants after cardiac surgery are not well known.The objective of this study was to determine the overall incidence and related risk factors for junctional ectopic tachycardia in neonates and infants.Methods:We enrolled a total of 271 patients agedjunctional ectopic tachycardia was 12.9%.The logistic regression analysis revealed that longer cardiopulmonary bypass time,surgery involving atrioventricular node stretching,and the presence of early repolarization on preoperative electrocardiography increased the risk of junctional ectopic tachycardia.Patients with junctional ectopic tachycardia had longer intubation time and intensive care unit stay.Conclusion:junctional ectopic tachycardia is a common arrhythmia after cardiac surgery for congenital heart disease in infants.Occasionally,infants developing junctional ectopic tachycardia after cardiac surgery have specific preoperative electrocardiography findings.The risk factors for junctional ectopic tachycardia were associated not only with surgical procedural factors but also with preoperative electrocardiographic parameters.

miR-215 overexpression distinguishes ampullary carcinomas from pancreatic carcinomas

Distinguishing ampullary carcinoma from pancreatic carcinoma is important because of their different prognoses. microRNAs are differentially expressed according to the tissue of origin. However, there is rare research on the differential diagnosis between the two types of cancers by microRNA in periampullary cancers. The present study was undertaken to compare microRNA profiles between ampullary and pan- creatic carcinomas using microarrays, miR-215 was most significantly overexpressed in ampullary carcinomas; whereas the expressions of miR-134 and miR-214 were significantly lower in ampuUary carcinomas than in pancreatic carcino- mas. When these discriminatory microRNAs were applied to liver metastases, they were correctly predicted for the tissue of origin. Although this study is limited by small sample size, striking difference in microRNA expression and concordant expression of discriminating microRNAs in primary tumors and metastases suggest that these novel discriminatory mi- croRNAs warrant future validation.
