Don’t forget emergency surgery! Lessons to learn from elective indocyanine green-guided gastrointestinal interventions

Fluorescence-based imaging has found application in several fields of elective surgery,but there is still a lack of evidence in the literature about its use in the emergency setting.Clinical trials have consistently shown that indocyanine green(ICG)-guided surgery can dramatically reduce the risk of postoperative complic-ations,length of in-hospital stay and total healthcare costs in the elective setting.It is well-known that emergency surgery has a higher complication rate than its elective counterpart,therefore an impelling need for research studies to explore,validate and develop this issue has been highlighted.The present editorial aims to provide a critical overview of currently available applications and pitfalls of ICG fluorescence in abdominal emergencies.Furthermore,we evidenced how the experience of ICG-fluorescence in elective surgery might be of great help in implementing its use in acute situations.In the first paragraph we analyzed the tips and tricks of ICG-guided cancer surgery that might be exploited in acute cases.We then deepened the two most described topics in ICG-guided emergency surgery:Acute cholecystitis and intestinal ischemia,focusing on both the advantages and limitations of green-fluorescence application in these two fields.In emergency situations,ICG fluorescence demonstrates a promising role in preventing undue intestinal resections or their entity,facilitating the detection of intestinal ischemic zones,identifying biliary tree anatomy,reducing post-operative complications,and mitigating high mortality rates.The need to improve its application still exists,therefore we strongly believe that the elective and routinary use of the dye is the best way to acquire the necessary skills for emer-gency procedures.

Preclinical and clinical studies indicate that synthetic psychoactive substances,in addition to having abuse potential,may elicit toxic effects of varying severity at the peripheral and central levels.Nowadays,toxicity induced by synthetic psychoactive substances poses a serious harm for health,since recreational use of these substances is on the rise among young and adult people.The present review summarizes recent findings on the peripheral and central toxicity elicited by “old” and “new” synthetic psychoactive substances in humans and experimental animals,focusing on amphetamine derivatives,hallucinogen and dissociative drugs and synthetic cannabinoids.

Intraoperative ultrasonography(IOUS)in hepatobiliary surgery is well known as an essential tool for radical and safe hepatectomies,allowing to perform parenchymal sparing surgery and,as consequence,to expand the surgical indications for patients otherwise considered unresectable.Nevertheless,since many years,despite its effectiveness in the study of vascular anatomy of the liver,the gold standard for the assessment of biliary anatomy during surgery is intraoperative cholangiography(IOC),which is used for the validation of preoperative imaging as well as for guiding reconstruction in case of bile duct injury or resection.

Respiratory protein-driven selectivity during the Permian-Triassic mass extinction

Extinction selectivity determines the direction of macroevolution,especially during mass extinction;however,its driving mechanisms remain poorly understood.By investigating the physiological selectivity of marine animals during the Permian-Triassic mass extinction,we found that marine clades with lower O2-carrying capacity hemerythrin proteins and those relying on O2 diffusion experienced significantly greater extinction intensity and body-size reduction than those with higher O2-carrying capacity hemoglobin or hemocyanin proteins.Our findings suggest that animals with high O2-carrying capacity obtained the necessary O2 even under hypoxia and compensated for the increased energy requirements caused by ocean acidification,which enabled their survival during the Permian-Triassic mass extinction.Thus,high O2-carrying capacity may have been crucial for the transition from the Paleozoic to the Modern Evolutionary Fauna.

BACKGROUND As one of the most common aesthetic surgical procedures carried out today,blepharoplasty should be in the repertoire of every plastic surgeon.The term blepharoplasty encompasses a wide range of techniques and options that must be tailored to the specific defect and patient one has to treat.A sound knowledge of the upper and lower eyelids’anatomy is essential for proper surgical execution.Trends have shifted towards more conservative methods(especially of the fat compartment)and sometimes in combination with augmentation techniques,helping to reach a rejuvenated appearance.AIM To present an overview of the surgical techniques considered for upper lid blepharoplasty and fat pad management,in addition to information on how a surgeon may approach the best treatment for his patient based on current publications in literature.METHODS We searched the literature published between 2013,to 2023 using Medline and Reference Citation Analysis.The database was searched using the keywords“upper blepharoplasty”AND“fat”.Papers without full text/abstracts and reviews were excluded.The search strategy followed the PRISMA.The American Society of Plastic Surgeons guidelines for Therapeutic Studies checklist was used to assess all articles.Two authors individually reviewed each article and rated them for importance and relevance to the topic.A consensus was sought and the most relevant studies.RESULTS After the application of the selection criteria used in our review,13 publications were found to address upper lid blepharoplasty specifically.Three of these studies were reviews and three were retrospective studies.Five publications were comparative studies and a further two were clinical trials.CONCLUSION The tendency of modern surgery is to be conservative,by removing adipose tissue only if strictly necessary and restoring the volume of the upper eyelid in a concept of beauty that espouses a"full"sight.There is no gold standard technique to achieve younger and enhanced eyelids.Long-term prospective comparative studies are f

中-晚三叠世是现代型生态系统构建的关键时期,系统研究该时期生物与环境的协同演变过程对揭示生物复苏机制和生态系统重建规律具有重要意义.琥珀是探讨中-晚三叠世植物演化和古气候演变的重要载体.对青岩剖面雷打坡段的琥珀和伴生的植物化石开展研究,并将全球的中-晚三叠世琥珀的分布与湿润事件进行对比分析.结果发现青岩琥珀是由松柏类植物伏脂杉生产,琥珀的出现与松柏类植物的辐射演化有关.菊石生物地层表明含有琥珀的地层属于中三叠世安尼期Pelsonian亚期.认为琥珀的产出与湿润事件具有对应性,说明湿润事件可能是中-晚三叠世琥珀频繁出现的重要原因.

Costs and benefits of brain lateralization may depend on environmental conditions.Growing evidence indicates that the development of brain functional asymmetries is adaptively shaped by the environmental conditions experienced during early life.Food availability early in life could act as a proxy of the environmental conditions encountered during adulthood,but its potential modulatory effect on lateralization has received little attention.We increased food supply from egg laying to early nestling rearing in a wild population of lesser kestrels Falco naumanni,a sexually dimorphic raptor,and quantified the lateralization of preening behavior(head turning direction).As more lateralized individuals may perform better in highly competitive contexts,we expected that extra food provisioning,by reducing the level of intra-brood competition for food,would reduce the strength of lateralization.We found that extra food provisioning improved nestling growth,but it did not significantly affect the strength or direction of nestling lateralization.In addition,maternal body condition did not explain variation in nestling lateralization.Independently of extra food provisioning,the direction of lateralization differed between the sexes,with female nestlings turning more often toward their right.Our findings indicate that early food availability does not modulate behavioral lateralization in a motor task,suggesting limited phenotypic plasticity in this trait.

The modern view of the immune system as a sensitizing and modulating machinery of the central nervous system is now well recognized.However,the specific mechanisms underlying this fine crosstalk have yet to be fully disentangled.To control cognitive function and behavior,the two systems are engaged in a subtle interacting act.In this scenario,a dual action of pro-inflammatory cytokines in the modulation of brain network connections is emerging.Pro-inflammatory cytokines are indeed required to express physiological plasticity in the hippocampal network while being detrimental when over-expressed during uncontrolled inflammatory processes.In this dynamic equilibrium,synaptic functioning and the performance of neural networks are ensured by maintaining an appropriate balance between pro-and anti-inflammatory molecules in the central nervous system microenvironment.

The United Nations(UN)’s call for a decade of“ecosystem restoration”was prompted by the need to address the extensive impact of anthropogenic activities on natural ecosystems.Marine ecosystem restoration is increasingly necessary due to increasing habitat degredation in deep waters(>200 m depth).At these depths,which are far beyond those accessible by divers,only established and emerging robotic platforms such as remotely operated vehicles(ROVs),autonomous underwater vehicles(AUVs),landers,and crawlers can operate through manipulators and multiparametric sensor arrays(e.g.,optoacoustic imaging,omics,and environmental probes).The use of advanced technologies for deep-sea ecosystem restoration can provide:①high-resolution three-dimensional(3D)imaging and acoustic mapping of substrates and key taxa,②physical manipulation of substrates and key taxa,③real-time supervision of remote operations and long-term ecological monitoring,and④the potential to work autonomously.Here,we describe how robotic platforms with in situ manipulation capabilities and payloads of innovative sensors could autonomously conduct active restoration and monitoring across large spatial scales.We expect that these devices will be particularly useful in deep-sea habitats,such as①reef-building cold-water corals,②soft-bottom bamboo corals,and③soft-bottom fishery resources that have already been damaged by offshore industries(i.e.,fishing and oil/gas).
