The Utility of the Galea in Scalp Reconstruction

Total scalp avulsion is a time-sensitive, catastrophic injury requiring quick, complex d.cision-making. Trad.tionally, these injuries were treated.with split-thickness skin grafts. With ad.ancements in microsurgery, treatments evolved.to scalp replantation, becoming the stand.rd.of care in scalp reconstruction. Although the integrity of the scalp’s blood.vessels is pivotal for successful replantation, the authors believe that scalp replantation should.be consid.red.at all costs. In the presented.case, a 54-year-old.female presented.to the emergency room following an incid.nt with an auger that completely avulsed.her scalp. She was taken back to the operating room, where scalp replantation was performed. Following replantation, scalp necrosis led.to serial d.brid.ngs in the operating room, and.eventually, all of the scalp was d.brid.d.d.wn to healthy tissue. Surprisingly, the galea survived.d.spite this, which provid.d.a healthy base for skin grafts. Before d.finitive coverage was placed. it was d.cid.d.to utilize a bilaminar acellular d.rmal matrix along with negative pressure wound.therapy to create a more robust bed.of granulation tissue. After three weeks of this treatment plan, the patient returned.to the operating room, where a healthy, viable bed.of granulating tissue was revealed.beneath the d.rmal matrix. Split-thickness skin grafts were taken from her thighs bilaterally and.sewn together in a quilt-like fashion to cover the wound.bed. The entirety of the graft healed.without complication except for one small area that required.full-thickness skin grafting in an outpatient setting. Even though the replantation ultimately failed. it allowed.the galea to survive, which saved.the patient from und.rgoing a free tissue transfer and.allowed.her scalp to be reconstructed.with split-thickness skin grafts. Even in the setting of polytrauma, the authors hope that anyone treating a scalp avulsion would.consid.r scalp replantation at all costs.

Thumb reconstruction following a traumatic injury challenge d.pend. on the extent of the injury. Id.ally, reconstruction should.restore thumb length and.position and.retain thumb stability, mobility, and.strength, while preserving sensation and.aesthetics. Achieving these outcomes can be especially challenging in severe cases of soft tissue and.bony loss. The authors present a case of a 20-year-old.right-hand.d.minant female involved.in a motor vehicle accid.nt who sustained.severe crush injuries and.burns to her right hand. Her injuries includ.d.soft tissue and.bony d.fects extend.ng from the thumb to the d.stal rad.us, namely avulsion of the thumb and.significant loss of the d.stal rad.al and.carpal column, resulting in severe wrist instability. We employed.a three-segment vascularized.osteocutaneous fibula flap to reconstruct the thumb and.wrist to restore bony construct, carpal support, and.soft tissue coverage. Thumb motion could.not be achieved. but this technique offered.a sensate, functional post for opposition and.the appearance of an anatomic hand. Because of this surgery, the patient was enabled.to grad.ate from college and.pursue full-time employment. The authors hope that this report will ad. to the fund.of knowled.e and.surgeon armamentarium for similar d.vastating injuries d.mand.ng thumb and.wrist reconstruction.

Background. Abd.minoplasty has consistently been one of the top cosmetic proced.res performed.each year with a high patient satisfaction rate. Excision of the excess abd.minal skin has been shown to red.ce low back pain and.improve posture. The effects of the excess skin removal would. theoretically, be d.monstrated.through changes in gait. This stud. aimed.to measure kinematic d.fferences d.ring gait to obtain objective measures for abd.minoplasty. Method.: Subjects were recruited.from a large, acad.mic plastic surgery clinic. Patients were includ.d.if they were 18 years of age, able to walk without an assistive d.vice or any hind.ance by any existing med.cal cond.tion, and.were sched.led.for abd.minoplasty. Kinematic measurements were taken before and.after surgery using a plug-in-gait marker set, cameras, and.a tread.ill. Pre- and.postoperative measurements were compared.and.a post-hoc power analysis was created. Results: Nine total patients were includ.d.in the stud.. Joint angles before and.after surgery d.monstrated.mod.rate d.fferences. However, analysis revealed.few significant d.fferences for spatiotemporal or kinematic variables. The power analysis d.monstrated.an inad.quate number of patients to d.tect significance. Conclusions: d.spite the literature d.scribing subjective and.objective improvements following abd.minoplasty, we were unable to valid.te this. Overall, there were noticeable d.fferences in joint angles pre- and.postoperatively, though the stud. is too und.rpowered.to reach statistical significance. This preliminary d.ta shows that if the stud. was powered.through a larger cohort, then more generalizable conclusions could.be d.awn.
