Among all head and neck (H&N) cancers,nasopharyngeal carcinoma (NPC) represents a distinct entity regarding epidemiology,clinical presentation,biological markers,carcinogenic risk factors,and prognostic *** is endemic in certain regions of the world,especially in Southeast Asia,and has a poor *** Indonesia,the recorded mean prevalence is 6.2/100 000,with 13 000 yearly new NPC cases,but otherwise little is documented on NPC in ***,we report on a group of 1121 NPC patients diagnosed and treated at *** mangunkusumo Hospital,Jakarta,Indonesia between 1996 and *** studied NPC incidence among all H&N cancer cases (n=6000) observed in that period,focusing on age and gender distribution,the ethnic background of patients,and the disease *** also analyzed most prevalent signs and symptoms and staging of NPC patients at first *** this study population,NPC was the most frequent H&N cancer (28.4%),with a male-to-female ratio of 2.4,and was endemic in the Javanese ***,NPC appeared to affect patients at a relatively young age (20% juvenile cases) without a bimodal age ***,NPC initiated in the fossa of Rosenmuller and spreaded intracranially or locally as a mass in the ***,NPC developed at the submucosal level spreading outside the anatomic limits of the *** presentation,NPC associated with hearing problems,serous otitis media,tinnitus,nasal obstruction,anosmia,bleeding,difficulty in swallowing and dysphonia,and even eye symptoms with diplopia and *** initial diagnosis is difficult to make because early signs and symptoms of NPC are not specific to the ***-age Epstein-Barr virus (EBV) infection combined with frequent exposure to environmental carcinogenic co-factors is suggested to cause NPC *** NPC is the most frequent histological type and is closely associated with *** of the EBV-encoded latent membrane protein

目的研究EBNA1和VCA-P18双多肽包被(EBNA1+VCA-P18)的ELISA检测在鼻咽癌(NPC)血清学诊断中的价值。方法用合成肽EBNA1和VCA-P18建立ELISA法,检测37例病理确诊的鼻咽癌患者血清和33例健康人血清,并与国产EBVVCA/IgAELISA试剂盒比较。结果双多肽EBNA1+VCA-P18/IgA灵敏度为91.9%,明显高于国产试剂盒的83.8%;二者特异度相同;EBNA1+VCA-P18/IgA的阳性预测值、阴性预测值、阳性似然比及优势比均高于国产试剂盒。结论双多肽EBNA1+VCA-P18/IgA相比商品化试剂盒在鼻咽癌血清学检测中有更高的灵敏度,适用于鼻咽癌早期诊断和人群筛查。

Analysis of the meiosis in the F_1 hybrids of Longiflorum × Asiatic(LA) of lilies(Lilium) using genomic in situ hybridization

Longiflorum and Asiatic lilies of the genus Lilium of the family Liliaceae are two important groups of modem lily cultivars. One of the main trends of lily breeding is to realize introgression between these groups. With cut style pollination and embryo rescue, distant hybrids between the two groups have been obtained. However, the FI hybrids are highly sterile or some of them could produce a small number of 2n gametes, and their BC1 progenies are usually triploids. Dutch lily breeders have selected many cultivars from these BC1 progenies based on their variation. It is presumably suggested that such variation could be caused by intergenomic recombination and abnormal meiosis during gamete formation in F1 hybrids of Longiflorum × Asiatic （LA） hybrids in Lilium. Therefore, the meiotic process of ten F1 LA hybrids was cytologically investigated using genomic in situ hybridization and traditional cytological methods in the present research. The results showed that： at metaphase I, the homoeologous chromosome pairing among different F1 hybrids ranged from 2.0 to 11.4 bivalents formed by homoeologous chromosomes per pollen mother cell （PmC）, and very few multivalents, and even very few bivalents were formed by two chromosomes within one genome rather than homoeologous chromosomes in some PmCs; at anaphase I, all biva- lents were disjoined and most univalents were divided. Both the disjoined bivalents （half-bivalents） and the divided univalents （sister chromatids） moved to the opposite poles, and then formed two groups of chromosomes; because the two resulting half-bivalents retained their axes in the cell undisturbed, many crossover types, including single crossovers, three strand double crossovers, four strand double crossovers, four strand triple crossovers, and four strand multiple crossovers between the non-sister chromatids in the tetrads of bivalents, were clearly inferred by analyzing the breakpoints on the disjoined bivalents. Twin Anemia Polycythemia Sequence: Knowledge and Insights After 15 Years of Research

Twin anemia polycythemia sequence (TAPS) is a chronic form of unbalanced feto-fetal transfusion through minuscule placental anastomoses in monochorionic twin pregnancies, leading to anemia in the donor twin and polycythemia in the recipient twin. TAPS can occur spontaneously in up to 5% of monochorionic twins or can arise in 2%-16% of cases after incomplete laser surgery for twin-twin transfusion syndrome. TAPS can develop across the entire second and third trimester. Antenatal diagnosis for TAPS is reached via Doppler measurement of the fetal middle cerebral artery peak systolic velocity, showing an increased velocity in the donor, combined with a decreased velocity in the recipient. Treatment options for TAPS include expectant management, preterm delivery, intrauterine blood transfusion with or without a partial exchange transfusion, fetoscopic laser surgery and selective feticide. The best treatment option is unclear and is currently being investigated in an international multicenter randomized trial (the TAPS trial). Spontaneous fetal demise occurs in 5%-11% of TAPS twins, more often in donors (8%-18%) than in recipients (2%-5%). Severe long-term neurodevelopmental impairment is seen in 9% of TAPS twins, with donors having an increased risk for cognitive impairment and hearing problems (15%).

目的：探讨中国南方鼻咽癌高发区鼻咽鳞状细胞癌或称角化性鳞状细胞癌（Keratinizingsquamouscellcarcinoma，KSCC）是否也如非角化性癌那样与EB病毒感染有密切的关系。方法：应用PCRSouthernblotting、原位分子杂交和免疫组化法检测38例鼻咽鳞状细胞癌组织中EB病毒DNA及其产物EBNA－1，EBNA－2，EBERs，LmP-1，ZEBRA，EA－D，VCA和mA的表达。结果：（1）37例鳞状细胞癌组织中有EBERs阳性癌细胞出现，阳性率达97．4％（37／38）；（2）在37例鳞状细胞癌中的大部分未分化和低分化癌细胞表达EBNA－1和EBERS，21例（56．8％，21／37）组织内有LmP－1表达；（3）其中23例（62．2％，23／37）组织中只有少数癌细胞表达ZEBRA或／和EA－D；（4）22例（59．5％，22／37）组织中分化较好的或角化性癌细胞表达VCA或／和mA。结论：（1）中国南方地区鼻咽鳞状细胞癌与非角化性癌一样与EB病毒的感染有着较恒定而非偶然的关系；（2）鳞状细胞癌组织中大多数未分化和低分化癌细胞均以11型潜伏状态感染EB病毒，主要表达EBNA－1、EBERS和LmP－1，其中一小部分癌细胞也表达溶解期流产型蛋白ZEBRA和EA－D；（3）EB病毒在大部分分化好的和角化性癌细胞中则能够完成其复制周期并表达溶解晚期蛋白VCA或／和mA。

The key risk factor for glaucoma is increased intraocular pressure (IOP). Glaucoma drainage devices implanted in theeye can reduce IOP and thus stop disease progression. However, most devices currently used in clinical practice arepassive and do not allow for postsurgical IOP control, which may result in serious complications such as hypotony (i.e.,excessively low IOP). To enable noninvasive IOP control, we demonstrate a novel, miniature glaucoma implant that willenable the repeated adjustment of the hydrodynamic resistance after implantation. This is achieved by integrating amagnetic microvalve containing a micropencil-shaped plug that is moved using an external magnet, thereby openingor closing fluidic channels. The microplug is made from biocompatible poly(styrene-block-isobutylene-block-styrene)(SIBS) containing iron microparticles. The complete implant consists of an SIBS drainage tube and a housing elementcontaining the microvalve and fabricated with hot embossing using femtosecond laser-machined glass molds. Usingin vitro and ex vivo microfluidic experiments, we demonstrate that when the microvalve is closed, it can providesufficient hydrodynamic resistance to overcome hypotony. Valve function is repeatable and stable over time. Due to itssmall size, our implant is a promising, safe, easy-to-implant, minimally invasive glaucoma surgery device.
