Background:Knee examination guidelines in minors are intended to aid decisionmaking in the management of knee instability.Clinical question:A Delphi study was conducted with a formal consensus process using a validated methodology with sufficient scientific evidence.A group consensus meeting was held to develop recommendations and practical guidelines for use in the assessment of instability injuries in children.Key findings:there is a lack of evidence to analyse anterior cruciate ligament injuries in children and their subsequent surgical management if necessary.Diagnostic guidelines and clinical assessment of the patient based on a thorough examination of the knee are performed and a guide to anterior cruciate ligament exploration in children is developed.Clinical application:In the absence of a strong evidence base,these established guidelines are intended to assist in that decision-making process to help the clinician decide on the most optimal treatment with the aim of benefiting the patient as much as possible.Following this expert consensus,surgical treatment is advised when the patient has a subjective sensation of instability accompanied by a pivot shift test++,and may include an anterior drawer test+and a Lachman test+.If these conditions are not present,the conservative approach should be chosen,as the anatomical and functional development of children,together with a physiotherapy programme,may improve the evolution of the injury.

Pregnancy is a unique immunologic and microbial condition that requires an adequate level of awareness to provide a fast and protective response against pathogens as well as to maintain a state of tolerance to paternal antigens.Dysregulation of inflammatory pathways in the placenta triggered by pathogens is one of the main factors responsible for pregnancy complications.Type I IFNs are key molecules modulating immune responses at the level of the placenta and are crucial for protection of the pregnancy via their antiviral and immune modulatory properties.In this study,we elucidate the mechanisms controlling the basal expression of IFNβand its negative feedback.Using in vitro and in vivo animal models,we found that TLR signaling maintains basal IFNβlevels through the TLR4-MyD88-independent TBK/IRF3 signaling pathway.We describe the role of the taM receptor Axl in the regulation of IFNβfunction in human and mouse trophoblast cells.The absence of taM receptors in vivo is associated with fetal demise due to dysregulation of IFNβexpression and its pro-apoptotic downstream effectors.Collectively,our data describe a feedback signaling pathway controlling the expression and function of IFNβin the trophoblast that is essential for an effective response during viral and microbial infections.

Geographic data and tools are essential in all aspects of emergency management:preparedness,response,recovery,and mitigation.Geographic information created by amateur citizens,often known as volunteered geographic information,has recently provided an interesting alternative to traditional authoritative information from mapping agencies and corporations,and several recent papers have provided the beginnings of a literature on the more fundamental issues raised by this new source.Data quality is a major concern,since volunteered information is asserted and carries none of the assurances that lead to trust in officially created data.During emergencies time is the essence,and the risks associated with volunteered information are often outweighed by the benefits of its use.An example is discussed using the four wildfires that impacted the Santa Barbara area in 20072009,and lessons are drawn.

The disability,mortality and costs due to ionizing radiation(IR)-induced osteoporotic bone fractures are sub-stantial and no effective therapy exists.Ionizing radiation increases cellular oxidative damage,causing an imbalance in bone turnover that is primarily driven via heightened activity of the bone-resorbing osteoclast.We demonstrate that rats exposed to sublethal levels of IR develop fragile,osteoporotic bone.At reactive surface sites,cerium ions have the ability to easily undergo redox cycling:drastically adjusting their electronic con-figurations and versatile catalytic activities.These properties make cerium oxide nanomaterials fascinating.We show that an engineered artificial nanozyme composed of cerium oxide,and designed to possess a higher fraction of trivalent(Ce^(3+))surface sites,mitigates the IR-induced loss in bone area,bone architecture,and strength.These investigations also demonstrate that our nanozyme furnishes several mechanistic avenues of protection and selectively targets highly damaging reactive oxygen species,protecting the rats against IR-induced DNA damage,cellular senescence,and elevated osteoclastic activity in vitro and in vivo.Further,we reveal that our nanozyme is a previously unreported key regulator of osteoclast formation derived from macrophages while also directly targeting bone progenitor cells,favoring new bone formation despite its exposure to harmful levels of IR in vitro.These findings open a new approach for the specific prevention of IR-induced bone loss using synthesis-mediated designer multifunctional nanomaterials.

Several studies on earthquake occurrence and associated faulting have demonstrated that both phenomena have a scale-invariant behavior which can be analyzed by means of a set of non-integer dimensions （Dq） describing their fractal properties and the calculation ofmulti-fractal spectra. It is the case that the behavior of these spectra is asymptotic at the ends of the variation interval of q, which is a real number that enters into the definition of the partition function of the dataset. The difference between the extreme values, called multi-fractal spectrum slope, is used to investigate the heterogeneity of the spatial distribution of earthquakes and fault systems. In this paper we focus on the Betic Cordillera, southeastern Spain, which is commonly considered the contact between the Eurasian and African plates and has an important seismic activity in the context of the Iberian Peninsula. Some of the most conspicuous Iberian earthquakes, such as the 1829 mb6.3 Torrevieja and the 1884 mb6.1 Alhama de Granada earthquakes occurred in this mountain range and both reached intensity X. The pre- sent work implies a new analysis based on the slope of multi-fractal spectra and referred to the historical seismicity of the re- gion, specifically b-value （frequency distribution of earthquakes respect to magnitude）, epicentral location, seismic energy and faulting. On this basis we propose a seismotectonic zonation that is contrasted with the stress state and the geodynamical evolution of the Betic Cordillera.

藏族文学史上传记和证道记虽词汇上大不相同,二者在体裁上混淆不清,学界对此置之不理,迄今为止未划分其界限。不仅如此,藏学界未出现较为全面的传记文学理论或者传记文学史,进而藏族传记文学处于滞后状态。为此,本论文认为产生此现象的直接原因不出二。一是没有理论的指导;二是没有明了文学概念。因而,本论文借用久负盛名的《米拉日巴传》为中心,通过解析其文学特征对这一问题做出思考。本文包括五章的内容,第一章对传记的概念、本质、结构等进行了分析;第二章详细介绍了《米拉日巴传》及作者、国内外各种不同版本等。第三、四章分别为米拉日巴传记的思想色彩与故事性、趣味性、形象性等文学特征作了阐述;附加一节考证了印度本生记在思想和结构两个方面对藏族传记文学的深渊影响。第五章研究了此传记在藏族文学领域的影响、历史地位、区别文学传记与历史传记的意义,尾段总结了此传记的弊端。

The purpose of this study was to investigate the correlation between idiopathic pulmonary fibrosis(IPF)and tumor markers to provide evidence for early screening of precancerous lesions.In our hospital from July 2017 to May 2019,40 patients with IPF treatment were selected as the IPF group,and 40 patients with idiopathic pulmonary fibrosis with lung cancer(IPF-LC)were selected as the IPF-LC group.In the same period,40 healthy physical examinees were used as control group.Different types of patients in the IPF-LC group were divided into lung adenocarcinoma group,small cell lung cancer group and l squamous carcinoma group.The expression levels of tumor markers were detected in the three groups,the positive rates of tumor markers in IPF group,IPF-LC group and their subgroups were compared.The results showed that the levels of neuron specific enolase(NSE),antigen CYFRA211,carcinogenic antigen(CEA)and cancer antigen 125(ca125)in IPF and IPF-LC groups were significantly higher than those in control group(Pcant difference in CEA and CYFRA211 between IPF-LC group and IPF group.The level of NSE in IPF-LC group was significantly higher than that in IPF group,while the level of ca125 was significantly lower than that in IPF group(Pca125 in IPF-LC group and IFP group was statistically significant(Ptatistically significant difference in the positive rate of other indicators(P>0.05)The NSE positive rate of IPF group was significantly lower than that of IPF-LC group(Pcantly lower than those of each subgroup of IPF-LC group(Pcal attention.Among them,NSE can be used as an early screening indicator for IPF precancerous lesions.

We report on the application of Laser Induced Breakdown Spectroscopy(LIBS)technique to the study of ca,P,Zn and Sr evolutions for adult caries-affected teeth using nanosecond laser pulses.The aim of this work is to better understand the behaviors of Zn and Sr as trace-elements in the caries eruption with respect to the behaviors of one of the main compounds of the hydroxyapatite crystal which is the calcium.The study was focused on the investigation of these elements’evolutions from three parts of the enamel surfaces of twenty two adult caries-affected teeth;the healthy part,the dental plaque part and the caries-affected part.The decrease rates of Zn,Sr and P was,also,compared to the ca one.Comparison concerned normalized emission line intensities.For every species,normalization was done relatively to the emission line intensities of the healthy part.Results showed that abundances of these elements decrease similarly from the healthy parts to the affected-caries parts.The higher decrease rate was noticed for the calcium.The evolutions of Zn and Sr for the three parts of the teeth surfaces cannot inform about the substitution of the calcium by these trace-elements,however the comparison of their decrease rates to the calcium one can be considered as a valuable index of this substitution.
