Objective. To determine the efficacy of adjuvant platinum-based chemotherapy in Stage I uterine papillary serous carcinoma (UPSC). Methods. A retrospective multi-institu-tional investigation was performed to identify surgically staged patients with Stage I UPSC who were (1) treated after surgery with 3-6 courses of platinum-based chemotherapy without radiation from 1990-2003, and (2) followed for a minimum of 12 months, or until recurrence. results. Six patients (IA-2, IB-3, IC-1) were treated with carboplatin (AUC 6) or cisplatin (50 mg/m2) alone. One patient recurred to the vagina, was treated with chemo-radiation, and is alive and well at 122 months. One patient recurred to the lung, liver, and brain, and died of disease at 24 months. The remaining 4 patients are alive with no evidence of disease 15-124 months (mean 62 months) after treatment. Two patients (IB-1, IC-1) were treated with cisplatin (50 mg/m2) and cyclophosphamide (1000 mg/m2), and both are alive and well with no evidence of disease 75 and 168 months after treatment. Twenty-one patients (IA-5, IB-13, IC-3) were treated with a combination of carboplatin (AUC 6)-and paclitaxel (135 mg/m2-175 mg/m2). One patient recurred to the vagina after 3 cycles of carboplatin/paclitaxel, and was treated with chemo-radiation. She is now without evidence of disease 10 months after treatment. At present, all 21 patients with Stage I UPSC treated following surgical staging with carboplatin/ paclitaxel chemotherapy are alive and well with no evidence of disease 10-138 months (mean 41 months) after treatment. Conclusion. Combination carboplatin/paclitaxel chemotherapy following surgery is effective in the treatment of Stage I UPSC.

New geochronologic data from midcontinental Laurentia demonstrate that emplacement of the 1476-1470 Ma Wolf river granitic batholith was not an isolated igneous event,but was accompanied by regional metamorphism,deformation,and *** for such metamorphism and deformation is best seen in siliciclastic sedimentary rocks of the Baraboo Interval,which were deposited closely following the 1.65-1.63 Ga Mazatzal *** Baraboo Interval strata,muscovite parallel to slatey cleavage,in hydrothermal veins,in quartzite breccia,and in metamorphosed paleosol yielded ^(40)Ar/^(39)Ar plateau ages of 1493-1465 *** addition,U-Th-total Pb dating of neoblastic overgrowths on detrital monazite gave an age of 1488±20 Ma,and recrystallized hematite in folded metapelite gave a mean U/Th-He age of 1411±39 ***-Baraboo,arkosic polymictic conglomerate,which contains detrital zircon with a minimum peak age of 1493 Ma,was intruded by a 1470 Ma granite porphyry at the northeastern margin of the Wolf river *** episode of magmatism,regional deformation and metamorphism,and sedimentation,which is designated herein as the Baraboo orogeny,provides a midcontinental link between the Picuris orogeny to the southwest and the Pinware orogeny to the northeast,completing the extent of early Mesoproterozoic(Calymmian)orogenesis for 5000 km along the southern margin of *** transcontinental orogen is unique among Precambrian orogenies for its great width(-1600 km),the predominance of ferroan granites derived from partial melting of lower continental crust,and the prevalence of regional high T-P metamorphism related to advective heating by granitic magmas emplaced in the middle to upper crust.

Comparison of the extrusion rate of Crawford tubes

· AIM: To compare the outcomes of dacryocystorhinostomy(DCr) using traditional Crawford tubes(TCT) and Crawford tubes with suture(CTS) in the lumen.· METHODS: retrospective case series consisting of patients who underwent DCr between 2008 and 2013.·rESULTS: A total of 61 DCrs were performed on 50 patients. Patients who underwent DCr using CTS had higher rates of prolapse compared to the TCT group(50% vs 9.4%; P =0.003). Stent removal occurred earlier in patients who received CTS(3.3mo vs 5.1mo; P =0.004). Success rates were equivalent between the two groups(75% vs 81.1%; P =0.684).·CONCLUSION: CTS in the lumen increases the risk of prolapse, prompting earlier tube removal in patients following DCr for nasolacrimal duct obstruction(NLDO).Earlier removal of tubes does not appear to significantly decrease success rates.

We describe a new karst-dwelling Cyrtodactylus from Ban Thathom, Xiangkhoang Province, northeastern Laos. The new species can be distinguished from other congeners by having four dark dorsal bands between limb insertions, a discontinuous nucha~ loop, 10 precloacal pores in males or 10-12 precloacal pits （females） separated by a diastema from a series of enlarged femoral scales bearing 18 or 19 pores （male） or 8-10 pits （females） along each femur, 14-18 dorsal tubercle rows at midbody, no precloacal groove, 30-36 midbody scale rows across belly between ventrolateral skin folds, transversely enlarged subcaudal plates, and a maximal known snout-vent length of 75.5 mm. Our description brings to 22 the number of Cyrtodactylus species recorded from Laos.

DEAr EDITOr,Species of Xenophrys are conserved morphologically and live primarily in forests. In Thailand, the genus harbors many cryptic species. Herein we report the collection of specimens from Doi Inthanon, Chiang Mai Province, northern Thailand, which were identified previously as X. minor. Molecular and morphological analyses find that these specimens differ significantly from other known congeners, and therefore we describe a new species. Further, our phylogenetic analyses indicate that X. latidactyla is a junior synonym of X. palpebralespinosa.

Age-stratified cutpoints for aminoterminal pro-brain natriuretic peptide(NT-pro-BNP) concentrations are diagnostic in 83%of all subjects with acute dyspnea. This study analyzed subjects with NT-pro-BNP concentrations between the “rule-out”and “rule-in”cutpoints, the so-called natriuretic peptide gray zone. NT-pro-BNP concentrations, clinical characteristics, and 60-day mortality were studied in 1,256 acutely dyspneic patients from an international multicenter study. Of all subjects, 215 had gray-zone NT-pro-BNP concentrations, 116 of whom(54%) were diagnosed with heart failure(HF). Among these subjects, patients with HF were more likely to be older, to have a history of HF, to be in atrial fibrillation, and to have elevated troponin T concentrations compared with those without HF. In multivariate analysis, the use of loop diuretics on presentation(odds ratio[Or] 3.99, 95%confidence interval[CI] 1.58 to 10.1, p=0.003), paroxysmal nocturnal dyspnea(Or 4.50, 95%CI 1.31 to 15.4, p=0.02), jugular venous distention(Or 3.05, 95%CI 1.06 to 8.79, p=0.04), and the absence of cough(Or 0.18, 95%CI 0.06 to 0.52, p=0.001) were associated with a diagnosis of acute HF in gray-zone patients. Subjects with HF and diagnostically elevated NT-pro-BNP concentrations had the highest mortality rates, subjects without HF and NT-pro-BNP concentrationsrtality rates, and subjects with gray-zone NT-pro-BNP had intermediate outcomes, irrespective of their final diagnoses. Adding specific clinical information to NT-pro-BNP improves diagnostic accuracy in subjects with intermediate NT-pro-BNP concentrations. Mortality rates in subjects with intermediate NT-pro-BNP concentrations are lower than in those with NT-pro-BNP concentrations diagnostic for HF but are higher than in subjects with NT-pro-BNP concentrations less than the gray zone.

Productivity and nutritive quality of dallisgrass (<i>Paspalum dilatatum</i>) as influenced by cutting height and rate of fertilization with poultry litter or commercial fertilizer

Dallisgrass (Paspalum dilatatum) is well adapted to the Black Belt region of the southeastern US, and information on its productivity and nutritive quality as influenced by fertility is needed. In each yr of a 2-yr study, an existing dallisgrass pasture that had been subdivided into 48 plots of 9.3 m2 each was fertilized with the equivalent of 34 (34N), 67 (67N), 101 (101N) or 134 (134N) kg N/ha from poultry litter (PL) or commercial fertilizer (CF;NH4NO3). In both years, primary-growth and vegetative regrowth forage was harvested in mid-August and late September, respectively, and forage from each harvest was clipped to either a 5- or 10-cm stubble height. Forage cut to a 5-cm height yielded 71% more (P rage cut to a 10-cm height, but forage dry matter (DM) yields were not different between CF and PL treatments across years and fertilization rates. Concentration of crude protein (CP) was greater (P = 0.002) for CF than PL forage and increased for both fertilizer sources with increasing rates of N application. Forage concentrations of cell-wall constituents were not different between CF and PL treatments. Forage amended with CF had a higher concentration of Ca, Mg and Mn than PL-amended forage;however, forage amended with PL had a higher concentration of P and K than CF-amended forage. There was no effect of fertilizer source on forage concentration of Al, Cu or Zn. results indicate that PL and CF were comparable for supporting productivity and nutritive quality of dallisgrass on Black Belt soils.

Background &Aims: Secretory diarrhea is caused by inhibition of intestinal active sodium absorption and stimulation of active chloride secretion. The resulting increase in fecal sodium salts causes an isotonic increase in fecal water output. Abnormalities in potassium transport are not known to be a cause of secretory diarrhea. The aim of our report is to describe a patient with secretory diarrhea that was mediated by excess intestinal secretion of potassium. Methods: A 78-yearold woman developed colonic pseudo-obstruction, complicated by severe dia rrhea and hypokalemia. Her stools were collected quantitatively on 11 occasions and analyzed for electrolyte concentrations. rectosigmoid potential difference w as measured. results: The diarrheal fluid had a very high potassium concentratio n (130-170 mEq/L) and a very low sodium concentration (4-15 mEq/L). Stool pota ssium losses were as high as 256 mEq/day (normal, 9 mEq/day), and fecal sodium l osses were never higher than 13 mEq/day. Potential difference between colonic lu men and a peripheral reference electrode was -14 mV (lumen side negative). Conc lusions: Fecal potassium salts were the exclusive driving force for severe secre tory diarrhea in a patient with colonic pseudo-obstruction. The high fecal outp ut of potassium was due to stimulation of active colonic potassium secretion, po ssibly because of changes in autonomic nervous system activity and distention of the colon in association with colonic pseudo-obstruction. The extremely low fe cal excretion of sodium indicates that active sodium absorption was not inhibite d. This case study reveals an ion transport mechanism of secretory diarrhea that has not been previously appreciated.
