Prognostic factors and predictive model for in-hospital mortality following hip fractures in the elderly

Purpose： The incidence of hip fractures is increasing within the aging population. Our objective was to identify and quantify the risk factors and develop a predictive model for the in-hospital mortality among hip fracture patients older than 65 years. Methods： This is a prospective study conducted on 331 hip fracture patients older than 65 years admitted to our hospital from 2011 to 2014. Patients＇ demographics, prehospitalization residential status, pre- fracture comorbidity data, anti-aggregant and anticoagulant medication, preoperative hemoglobin value, type of fractures, type of treatments, time to surgery, and complications were recorded. Results： The average age was 83 years, 73% female, and 57% of them sustained a femoral neck fracture. In 62.8% of patients, the number of pre-fracture baseline comorbidities was 〉2. The in-hospital mortality rate was 11.4%. In multivariate analysis, age over 90 years, congestive heart failure, asthma, rheumatologic disease, lung cancer, and not taking antiaggregant medication were independently associated with in-hospital mortality. A formula and risk stratification scoring for predicting the risk for in-hospital mortality was developed, Risk-adjustment model based on these variables had acceptable accuracy for predicting in-hospital mortality （c-statistic 0.77）. Conclusion： Advanced age, and five prefracture comorbidities have a strong association with in-hospital mortality in a hip fracture patient older than 65 years old. Our predictive model was specifically designed for the old hip fracture population. It has an accuracy similar to other risk models. The specificity, positive predictive value, and negative predictive value are high. In addition, it could discriminate a high risk patient from a low risk patient for in-hospital mortality.

A detailed vibrational Raman-lR spectroscopic and diffractional analyses have been performed on basalts from two locations from Tenerife Island：（1） the Arenas Negras volcano which belongs to the historical eruption not showing visible alteration and （2） Pillow Lavas zone from Anaga Massif which shows a clearly fluid-rock interaction caused by submarine alteration. These places have been extensively studied due to its similarity with the surface of Mars. The analysis is based on the mineral detection of selected samples by a Micro-Raman study of the materials. The complementary techniques have confirmed the mineralogy detected by the Raman measurement. The results show a volcanic environment behavior with primary phases like olivine, pyroxene, and feldsparjplagioclase. Moreover, the presence of accessory minerals or secondary mineralization like phosphate, iron oxides, zeolite or carbonates shows the alteration processes on each outcrop. The variation in the crystallinity and amorphous phases is related to fluid-rock interaction caused by hydrothermal episodes and external weathering processes, which shows several analogies with the ancient volcanic activity from Mars.

Adult stem cells,especially from bone marrow,have a tremendous therapeutic potential,and its application in regenerative medicine has been increasingly studied [1,2].It is well known that three-dimensional (3D)cell culture matrix promotes many biological relevant functions not observed in two-dimensional (2D) monolayer cell culture [3]and therefore,engineering 3D cell culture platforms to mimic the native in vivo biological systems represents a realistic approach for studies of bone marrow stem cells. High-throughput microfluidics constitutes a promising technology to generate hydrogel three dimensional constructs [4,5],however, its application to host adult stem cells remains still a challenge.

CD36 genetic variation, fat intake and liver fibrosis in chronic hepatitis C virus infection

AIM To analyze the association of the CD36 polymorphism(rs1761667) with dietary intake and liver fibrosis(Lf) in chronic hepatitis C(CHC) patients. METHODS In this study, 73 patients with CHC were recruited. The CD36 genotype(G > A) was determined by a TaqM an real-time PCR system. Dietary assessment was carried out using a three-day food record to register the daily intake of macronutrients. Serum lipids and liver enzymes were measured by a dry chemistry assay. Lf evaluated by transient elastography(fibroscan~)and APRI score was classified as mild Lf(f1-f2) and advanced Lf(f3-f4).RESULTS Overall, the CD36 genotypic frequencies were AA(30.1%), AG(54.8%), and GG(15.1%), whereas the allelic A and G frequencies were 57.5% and 42.5%, respectively. CHC patients who were carriers of the CD36 AA genotype had a higher intake of calories attributable to total fat and saturated fatty acids than those with the non-AA genotypes. Additionally, aspartate aminotransferase(AST) serum values were higher in AA genotype carriers compared to non-AA carriers(91.7 IU/L vs 69.8 IU/L, P = 0.02). Moreover, the AA genotype was associated with an increase of 30.23 IU/L of AST(β = 30.23, 95%CI: 9.0-51.46, P = 0.006). Likewise, the AA genotype was associated with advanced Lf compared to the AG(OR = 3.60, 95%CI: 1.16-11.15, P = 0.02) or AG + GG genotypes(OR = 3.52, 95%CI: 1.18-10.45, P = 0.02).CONCLUSION This study suggests that the CD36(rs1761667) AA genotype is associated with higher fat intake and more instances of advanced Lf in CHC patients.

Effects of Silicon Nanoparticles on the Transient Liquid Phase Bonding of 304 Stainless Steel

Transient liquid phase （TLP） bonding of 304 stainless steel with nickel based filler metal, BNi-9, was performed to study the influence of silicon nanoparticles （NPs） on the mechanical and structural properties of the bonding area. It was found that silicon NPs act as a melting point depressant in the brazing process; the formation of silicon TLP induces the dissolution of elements of the metal filler and promotes a uniform distribution in the bonding area. Silicon NPs induce the development of smaller eutectic structures in the melting zone; it has been related to microhardness measurements, which are lower when the silicon NPs are used in the brazing process.
