AIm:To evaluates the effectiveness and safety of the first generation,NS3/4A protease inhibitors(PIs) in clinical practice against chronic C virus,especially in patients with advanced fibrosis. mETHODS:Prospective study and non-experimental analysis of a multicentre cohort of 38 Spanish hospitals that includes patients with chronic hepatitis C genotype 1,treatment-na?ve(TN) or treatment-experienced(TE),who underwent triple therapy with the first generation NS3/4A protease inhibitors,boceprevir(BOC) and telaprevir(TVR),in combination with pegylated interferon and ribavirin. The patients were treatment in routine practice settings. Data on the study population and on adverse clinical and virologic effects were compiled during the treatment period and during follow ***:One thousand and fifty seven patients were included,405(38%) were treated with BOC and 652(62%) with TVR. Of this total,30%(n = 319) were TN and the remaining were TE:28%(n = 298) relapsers,12%(n = 123) partial responders(PR),25%(n = 260) null-responders(NR) and for 5%(n = 57) with prior response unknown. The rate of sustained virologic response(SVR) by intention-to-treatment(ITT) was greater in those treated with TVR(65%) than in those treated with BOC(52%)(P modified intention-to-treatment(m ITT) no were found significant differences. By degree of fibrosis,56% of patients were F4 and the highest SVR rates were recorded in the non-F4 patients,both TN and TE. In the analysis by groups,the TN patients treated with TVR by ITT showed a higher SVR(P = 0.005). However,by m ITT there were no significant differences between BOC and TVR. In the multivariate analysis by m ITT,the significant SVR factors were relapsers,IL28 B CC and non-F4; the type of treatment(BOC or TVR) was not significant. Integrated physiological and molecular approaches to improvement of abiotic stress tolerance in two pulse crops of the semi‐arid tropics

Chickpea(Cicer arietinum L.) and pigeonpea [Cajanus cajan L.(millsp.)] play an important role in mitigating protein malnutrition for millions of poor vegetarians living in regions of the semi-arid tropics. Abiotic stresses such as excess and limited soil moisture(water-logging and drought), heat and chilling(high and low temperature stresses), soil salinity, and acidity are major yield constraints, as these two crops are grown mostly under rainfed conditions in risk-prone marginal and degraded lands with few or no inputs. Losses due to such stresses vary from 30% to 100% depending on their severity. The literature abounds in basic information concerning screening techniques, physiological mechanisms, and genetics of traits associated with resistance/tolerance to abiotic stresses in these two crops. However, the final outcome in terms of resistant/tolerant varieties has been far from satisfactory. This situation calls for improving selection efficiency through precise phenotyping and genotyping under high-throughput controlled conditions using modern tools of genomics. In this review, we suggest that an integrated approach combining advances from genetics, physiology, and biotechnology needs to be used for higher precision and efficiency of breeding programs aimed at improving abiotic stress tolerance in both chickpea and pigeonpea.

Lymphomas are neoplastic transformations that affect lymphoid cells. Diffuse large B-cell non-Hodgkin’s lymphoma has a high degree of cell proliferation, accounting for 30% of all lymphomas. Lung cancer is the leading cause of death worldwide and the recommended treatment is chemotherapy. Among the main complications resulting from non-Hodgkin’s lymphoma, lung cancer and chemotherapy used in their treatment, we can mention sepsis, acute kidney injury and febrile neutropenia. Febrile neutropenia can occur by suppressing the production of neutrophils. Sepsis, a widespread infection, is the main cause of acute kidney injury, which can also be caused by hydroelectrolytic complications or by nephrotoxicity. This is a report of a smoking patient with metastatic lung cancer who sought care due to progressive dysphagia, cough with chest pain, fever, and lower airways critical obstruction due to mediastinal lymphadenopathy, being diagnosed with diffuse large B-cell non-Hodgkin’s lymphoma. The patient evolved to death because of a significant worsening of the ventilatory pattern of multifactorial cause, mainly due to sepsis, acute kidney injury, and febrile neutropenia. The patient had mostly classic characteristics of her comorbidities, however, the overlapping of interrelated comorbidities led to the outcome of death. What is unusual about the present case report is that the patient’s characteristics, such as age, sex, and ethnicity, are opposite to those described as risk factors for diffuse large B-cell non-Hodgkin’s lymphoma.

Patients with decompensated hepatic cirrhosis may present systemic alterations and dysfunction of multiple organs. Ascites, portal hypertension, esophageal varices, and hepatic encephalopathy are common complications arising from cirrhosis. The aim of this paper is to report a case of a patient with liver cirrhosis and the complications of a transjugular intrahepatic portosystemic shunt. Male, elderly, and ex-alcoholic, diagnosed with liver cirrhosis, ascites, and esophageal varices. He underwent transjugular intrahepatic portosystemic shunt due to portal hypertension and returned to the hospital after 24 hours with agitation and mental confusion. He had a bowel movement stop, neurological worsening, loss of renal function, hepatic hydrothorax, hepatic encephalopathy, hypernatremia, hypocalcemia, and hypophosphatemia. He underwent a new procedure to occlude the transjugular intrahepatic portosystemic shunt, showing improvement of the mental status and ascites. However, continued with decompensation and hydro-electrolytic disorders. He evolved with worsening of the ventilatory pattern, and neurological and renal function, with a fatal outcome. Esophageal varices due to portal hypertension can be corrected with the transjugular intrahepatic portosystemic shunt. However, complications such as hypernatremia, hepatic hydrothorax, and hepatic encephalopathy may occur. Therefore, there is a need for reintervention to shunt or reduce its caliber. Thus, for patients with advanced age and decompensated cirrhosis, the potential risks and benefits of this procedure should be carefully evaluated due to the risk of complications and death.

补充维生素是提高猪饲料营养价值的方法。某些维生素如维生素A和维生素E都与肉质有关，已有相关报道其可以作为颉颃剂来使用。因此，养猪生产者和消费者对这些维生素在组织中的含量有强烈的兴趣。为研究日粮补充或停止使用维生素的持续时间和时机对育肥猪组织中维生素A的沉积和a-生育酚的积累和基因表达的影响，选取体重16．3±2．5kg的伊比利亚断奶仔猪80头，对照组饲喂富含维生素A的日粮（维生素A10000U·kg-1），早期限饲组（ER组）从试验开始时（猪重16．3kg）饲喂未添加维生素A的日粮，后期限饲组（LR组）从仔猪平均体重达到35．8kg时开始饲喂未添加维生素A的日粮。体重达到101．4kg时，ER、LR组与对照组相比平均日增重和饲料转化率均较低（P=0．001，P=0．034）。而不论是试验初期（〈35．8kg）、肥育期（101．4~157．9kg）还是整个试验期间（16.3-157．9kg）猪的终重、平均日增重、平均日采食量和饲料转化率差异均无显著影响。组织中视黄醇浓度反映了饲料中维生素A的水平。对照组动物肝脏和脂肪库中视黄醇和视黄醇棕榈酸酯的积累在生长期比育肥期更为显著。维生素A限制组的脂肪视黄醇浓度下降，各组织间表现出不同的转移灵敏度，肝脏视黄醇转移更快。ER组肝脏a-生育酚的增加比LR组更显著（P〈0．0001）。而LR组比ER组在a-生育酚水平和脂肪含量的增加更为显着（P〈0．0001）；活体重（LW）101kg时，对照组ADHIC基因表达较ER组更高（P=0．0237）；LW101、158kg时，ER组LRAT基因的表达呈剂量依赖性下降（P〈0．0001）。RBP4、ALDHIA1、mTTP和TTP基因的表达没有受到日粮处理影响。生长时间影响基因的表达，ADH1C和RBP4基因的表达主要表现在猪101kg时（P〈0．05）。mTTP和TTP基因相对表达量也受时间影响，并且显示出和维生素A相关的基因表达相反的模式。结果表明，日粮长期或短期消除维生素A具有降低饲料的成本、增加组织a-生育酚的水平的潜力，且不影响屠宰重和饲料转化率。

【目的】明确环斑猛猎蝽(Sphedanolestes impressicollis Stal.)对烟田烟蚜[myzus persicae(Sulzer)]若虫的捕食潜力,为田间应用环斑猛猎蝽防控烟蚜提供参考。【方法】实验室条件下,在培养皿中设置1头饥饿24 h的环斑猛猎蝽4龄若虫与不同密度梯度的1~4龄烟蚜若虫,统计环斑猛猎蝽的捕食量并通过GrapPad Prism 8.0、DPS等软件进行生物学统计分析,明确环斑猛猎蝽对烟蚜若虫的功能反应和搜寻效应;以不同密度的环斑猛猎蝽4龄若虫与烟蚜4龄若虫在不同温度梯度及空间格局条件下,通过控制单一变量法并构建线性回归方程,明确环斑猛猎蝽4龄若虫种内干扰对捕食率的影响以及自身密度、温度、空间格局对捕食量的影响,评估环斑猛猎蝽4龄若虫对烟蚜4龄若虫的捕食功能反应模型。【结果】环斑猛猎蝽4龄若虫对烟蚜若虫的捕食功能反应符合Holling-II模型,其种内干扰及自身密度对捕食量的影响分别符合Hassell模型和Watt模型。环斑猛猎蝽4龄若虫对1~4龄烟蚜若虫中的3龄和4龄若虫捕食具有偏好性,对4龄若虫的捕食量最大,最大日捕食量为55.7头,其次为3龄烟蚜(55.3头)。在空间一定和烟蚜若虫定量条件下,环斑猛猎蝽4龄若虫个体间存在种内干扰,干扰系数为1.31,并随着自身密度的增加干扰增强,平均捕食量下降。在不同空间且烟蚜若虫密度一定的条件下,随着空间的增大,环斑猛猎蝽4龄若虫对猎物的捕食量减少。在16~32℃的温度梯度下,环斑猛猎蝽4龄若虫对烟蚜若虫的捕食量随着温度的升高而逐渐增强,32℃下的日捕食量最大,达23头。【结论】环斑猛猎蝽在烟田中具有防治烟蚜的潜力。

研究的目的是探讨日粮维生素A水平是否与伊比利亚猪生长前期(活重35.8 kg)和育成期(活重158 kg)脂肪细胞分化或脂肪积累差异有关。选取活重16.3 kg的伊比利亚猪分配到两个日粮组,一组饲喂添加维生素A 10 000 U·kg-1的日粮(对照组);另一组不添加维生素A(VAR)。试验结果表明,维生素A的水平对猪生长性能和胴体品质没有影响。维生素A的早期限饲在生长早期极显著增加了背最长肌(LT)前脂肪细胞数量(P<0.001);并且增加育成期中性脂质含量,改善了其组成(较高的单不饱和脂肪酸和较低的饱和脂肪酸含量,P<0.05)。维生素A的限饲增加仔猪脂肪生成潜力,且不影响胴体性状。
