Classical and Non-Classical Celiac Disease Comparison: Ten Years of Study

Objective: Celiac disease (CD) is an immune-mediated systemic disorder triggered by gluten. It has a variable combination of clinical manifestations and changes that have been occurring in recent decades however they are not known in detail. The purpose of the article is to compare Classical and Non-Classical CD cases in terms of demographic characteristics, duodenal biopsy, extraintestinal manifestations, and associated comorbidities. Materials and Methods: A comparative retrospective cohort study from January 2008 to December 2018. Results: A total of 128 cases were included: 84 Classical (66%) and 44 Non-Classical CD (34%). The family history of CD was identified in 14% of cases without differences between groups. The age at diagnosis was distinct for Classical and Non-Classical CD (4.9 ± 4 and 8.3 ± 4 years old;p 0.001), respectively. Important changes were found within the classical presentation, including mono symptoms and a significantly higher rate of intestinal atrophy;p = 0.04. The main Non-Classical CD symptom was recurrent abdominal pain. The extraintestinal manifestations (EIM) were identified in 42% and occurred in both groups. The comparison between groups showed differences in rates of migraine and vitamin D deficiency and was higher for Non-Classical CD (p 0.05). Associated diseases occurred in 10.9%, and type 1 diabetes was significant for the Non-Classical CD group (p = 0.04). Conclusion: The classical CD was the most prevalent profile and presented a decrease in the severity of symptoms however remain a higher rate of intestinal atrophy. Recurrent abdominal pain was the main symptom of Non-Classical CD. Extraintestinal manifestations and associated diseases presented an increasing trend of occurrence among cases of Non-Classical CD.

Depth of closure:New calculation method based on sediment data

Obtaining depth of closure(DoC) in an accurate manner is a fundamental issue for coastal engineering,since good results for coastal structures and beach nourishment depend mainly on DoC. Currently, there are two methods for obtaining the DoC, mathematical formulations and profile surveys. However, these methods can incur important errors if one does not take into account the characteristics and morphology of the area, or if one does not have a sufficiently long time series. In this work the DoC is obtained from the break in the trend of the sediment with the depth, that is, in general with the increase of the depth a decrease in the size of the sediment takes place. However, at one point this tendency changes and the size increases, and then decreases again. When comparing the point where the minimum sediment size occurs before the increase, it is observed that the error incurred is small compared to other methods. If the Standard Deviation of Depth Change(SDDC) method is considered as the most accurate method, the error incurred by the proposed method is less than 7%. In addition, it can be seen that the dispersion of the sediment method always occurs outside the zone of bar movement. Whereas in the methods of profiles survey(using 2 cm precision profiles), sometimes the DoC is obtained within the active zone of bar movement. In addition, where the relative minimum of the median sediment size is found, and the sizes of 0.063 and 0.125 mm predominate in the composition of the sample. Therefore, this new method allows the precise location of the DoC to be obtained in a fast and simple way. Furthermore, this method has the advantage that it is not affected by the modifications that may be experienced by both the study area and the cross-shore beach profile.

AIM:To evaluates the effectiveness and safety of the first generation,NS3/4A protease inhibitors(PIs) in clinical practice against chronic C virus,especially in patients with advanced fibrosis. METHODS:Prospective study and non-experimental analysis of a multicentre cohort of 38 Spanish hospitals that includes patients with chronic hepatitis C genotype 1,treatment-na?ve(TN) or treatment-experienced(TE),who underwent triple therapy with the first generation NS3/4A protease inhibitors,boceprevir(BOC) and telaprevir(TVR),in combination with pegylated interferon and ribavirin. The patients were treatment in routine practice settings. Data on the study population and on adverse clinical and virologic effects were compiled during the treatment period and during follow ***:One thousand and fifty seven patients were included,405(38%) were treated with BOC and 652(62%) with TVR. Of this total,30%(n = 319) were TN and the remaining were TE:28%(n = 298) relapsers,12%(n = 123) partial responders(PR),25%(n = 260) null-responders(NR) and for 5%(n = 57) with prior response unknown. The rate of sustained virologic response(SVR) by intention-to-treatment(ITT) was greater in those treated with TVR(65%) than in those treated with BOC(52%)(P < 0.0001),whereas by modified intention-to-treatment(m ITT) no were found significant differences. By degree of fibrosis,56% of patients were F4 and the highest SVR rates were recorded in the non-F4 patients,both TN and TE. In the analysis by groups,the TN patients treated with TVR by ITT showed a higher SVR(P = 0.005). However,by m ITT there were no significant differences between BOC and TVR. In the multivariate analysis by m ITT,the significant SVR factors were relapsers,IL28 B CC and non-F4; the type of treatment(BOC or TVR) was not significant. Yield models for predicting aboveground ectomycorrhizal fungal productivity in Pinus sylvestris and Pinus pinaster stands of northern Spain

Background: Predictive models shed light on aboveground fungal yield dynamics and can assist decision-making in forestry by integrating this valuable non-wood forest product into forest management planning. However, the currently existing models are based on rather local data and, thus, there is a lack of predictive tools to monitor mushroom yields on larger ***: This work presents the first empirical models for predicting the annual yields of ectomycorrhizal mushrooms and related ecosystem services in Pinus sylvestris and Pinus pinaster stands in northern Spain, using a long-term dataset suitable to account for the combined effect of meteorological conditions and stand *** were fitted for the following groups of fungi separately: all ectomycorrhizal mushrooms, edible mushrooms and marketed mushrooms. Our results show the influence of the weather variables(mainly precipitation) on mushroom yields as well as the relevance of the basal area of the forest stand that follows a right-skewed unimodal curve with maximum predicted yields at stand basal areas of 30–40 m2·***: These models are the first empirical models for predicting the annual yields of ectomycorrhizal mushrooms in Pinus sylvestris and Pinus pinaster stands in northern Spain, being of the highest resolution developed to date and enable predictions of mushrooms productivity by taking into account weather conditions and forests’ location, composition and structure.

As hydropower development expands across lowland tropical forests,flooding and concomitant insular fragmentation have become important threats to *** created insular landscapes serve as natural laboratories to investigate biodiversity responses to *** of these most iconic landscapes is the Balbina Hydroelectric Reservoir in Brazilian Amazonia,occupying>400000 ha and comprising>3500 forest ***,we synthesise the current knowledge on responses of a wide range of biological groups to insular fragmentation at *** has largely concentrated on a set of 22 islands and three mainland *** total,39 studies were conducted over nearly two decades,covering 17 vertebrate,invertebrate,and plant *** species responses varied according to taxonomic group,island area was consistently included and played a pivotal role in 66.7%of all studies examining patterns of species *** persistence was further affected by species traits,mostly related to species capacity to use/traverse the aquatic matrix or tolerate habitat degradation,as noted for species of vertebrates and orchid *** research is needed to improve our understanding of such effects on wider ecosystem *** Impact Assessments must account for changes in both the remaining habitat amount and configuration,and subsequent long-term species losses.
