Objective: Metabolic syndrome results from a complex interaction between lifestyle and genetic factors. Among this population, adhesion to healthy recommendations is a cornerstone of cardiovascular disease prevention. We examined the association between depression and multiple unhealthy behaviours in hypertensive patients with the metabolic syndrome. Research design and methods: Eight hundred and forty consecutive hypertensive subjects with the metabolic syndrome were studied in our secondary-care centre. Separated scores reflecting unhealthy behaviours(physical inactivity, smok.ng and unhealthy diet) were combined to produce a global unhealthy lifestyle score. The Hospital Anxiety and Depression scale was used to assess and quantify depression. We performed a separate analysis for each sex. Results: The prevalence of depression(13.0 versus 7.3%, Pk.) but also with decreased physical activity in men and with smok.ng habits in women. In both sexes, the global unhealthy lifestyle score, reflecting a cluster of unhealthy behaviours, was positively correlated with the depression score. In multivariate analysis, the depression score appeared in both sexes as an independent determinant of unhealthy lifestyle. Conclusions: Among hypertensive subjects with the metabolic syndrome, depressive symptoms along a continuum of severity are independently associated with multiple unhealthy lifestyles. This suggests that even minor forms of depression may impact on adhesion to health behaviours beyond major depressive symptoms and/or psychiatric condition.

Multispectral light scattering endoscopic imaging of esophageal precancer

Esophageal adenocarcinoma is the most rapidly growing cancer in *** the prognosis after diagnosis is unfavorable,the chance of a successful outcome increases tremendously if detected early while the lesion is still ***,the present standard-of-care,endoscopic surveillance,has major limitations,since dysplasia is invisible,often focal,and systematic biopsies typically sample less than one percent of the esophageal lining and therefore easily miss *** solve this problem we developed a multispectral light scattering endoscopic imaging *** surveys the entire esophageal lining and accurately detects subcellular dysplastic *** system combines light scattering spectroscopy,which detects and identifies invisible dysplastic sites by analyzing light scattered from epithelial cells,with rapid scanning of the entire esophageal lining using a collimated broadband light beam delivered by an endoscopically compatible fiber optic *** we report the results of the first comprehensive multispectral imaging study,conducted as part of routine endoscopic procedures performed on patients with suspected *** a double-blind study that characterized the system’s ability to serve as a screening tool,55 out of 57 patients were diagnosed *** addition,a smaller double-blind comparison of the multispectral data in 24 patients with subsequent pathology at locations where 411 biopsies were collected yielded an accuracy of 90%in detecting individual locations of dysplasia,demonstrating the capability of this method to serve as a guide for biopsy.

在美国华盛顿州北喀斯喀特地区斯卡吉特混合岩杂岩体的交质沉积岩中已鉴别出两类层状淡色体。一类淡色体REE相对富集,其中含有小的(k.英云闪长质-奥长花岗岩)淡色体可能是通过水饱和局部熔融或通过亚固相线作用形成。两种淡色体形成机制都可以在上角闪岩相区域交质作用过程中进行。在中地壳造山作用过程中,如果正在结晶的深成岩体充当为超变质作用所必需的富水流体的外部来源,则局部熔融可由同变质的岩浆活动触发。大规模的混合岩杂岩体例如斯卡吉特混合岩的形成可能至少部分地与高级变质过程中深成作用的接触效应有关。因此,尽管混合岩杂岩体在体积上是许多造山带的一部分,但它们本身并不代表大规模火成岩体原来的岩浆来源。

Objective:Endoscopic approaches for sinus and sk.ll base surgery are increasing in *** objective of this narrative review is to characterize risk.factors for internal carotid artery injury in endoscopic endonasal surgery(EES),highlight preventative measures,and illustrate k.y management *** Sources:Comprehensive literature ***:Relevant literature was reviewed using PubMed/***:Carotid artery injury in EES is rare,with most studies reporting an incidence below 0.1%.Anatomic aberrancies,wide dissection margins,as well as specific provider and hospital factors,may increase the risk.of *** teams,comprehensive preoperative imaging,patient risk.assessment,and formal training in vascular emergencies may reduce the *** protocols should emphasize proper visualization of the injury site,fluid replacement,rapid pack.ng,angiography,and endovascular techniques to achieve ***:While EES is a relatively safe procedure,carotid artery injury is a devastating complication that warrants full consideration in surgical *** preventative measures include identifying patients with notable risk.factors and obtaining preoperative *** teams and management protocols are ultimately necessary to reduce morbidity and mortality.

Hypercalciuria switches Ca^(2+) signaling in proximal tubular cells,induces oxidative damage to promote calcium nephrolithiasis

Proximal tubule(PT)transports most of the renal Ca^(2+),which was usually described as paracellular(passive).We found a regulated Ca^(2+)entry pathway in PT cells via the apical transient receptor potential canonical 3(TRPC3)channel,which initiates transcellular Ca^(2+)*** TRPC3 k.ock.ut(−/−)mice were mildly hypercalciuric and displayed luminal calcium phosphate(CaP)crystals at Loop of Henle(LOH),no CaP+calcium oxalate(CaOx)mixed urine crystals were spotted,which are mostly found in calcium nephrolithiasis(CaNL).Thus,we used oral calcium gluconate(CaG;2%)to raise the PT luminal[Ca^(2+)]o further in TRPC3−/−mice for developing such mixed stones to understand the mechanistic role of PT-Ca^(2+)signaling in ***,CaG-treated mice urine samples presented with numerous mixed crystals with remains of PT cells,which were pronounced in TRPC3−/−mice,indicating PT cell ***,PT cells from CaG-treated groups switched their mode of Ca^(2+)entry from receptor-operated to store-operated pathway with a sustained rise in intracellular[Ca^(2+)]([Ca^(2+)]i),indicating the stagnation in PT Ca^(2+)***,those PT cells from CaG-treated groups demonstrated an upregulation of calcification,inflammation,fibrotic,oxidative stress,and apoptotic genes;effects of which were more robust in TRPC3 ablated ***,k.dneys from CaG-treated groups exhibited fibrosis,tubular injury and calcifications with significant reactive oxygen species generation in the urine,thus,indicating in vivo *** together,excess PT luminal Ca^(2+)due to escalation of hypercalciuria in TRPC3 ablated mice induced surplus CaP crystal formation and caused stagnation of PT[Ca^(2+)]i,invok.ng PT cell injury,hence mixed stone formation.
