The growing diffusion of nonalcoholic fatty liver disease(NAFLD) is a consequence of the worldwide increase in the prevalence of *** stress is widely recognized to play a pivotal role in NAFLD evolution to nonalcoholic steatohepatitis(NASH).Here we review recent evidence suggesting that oxidative stress-derived antigens originating within fatty livers stimulate both humoral and cellular adaptive immune responses and the possible mechanisms involved in sustaining hepatic inflammation in NASH.

Aim:Recurrence of hepatocellular carcinoma (HCC) after liver transplantation (LT) for chronic hepatitis B (CHB) can be associated with reappearance of hepatitis B surface antigen (HBsAg). The current study determined the significance of HBsAg qualitatively and quantitatively using a highly sensitive assay in recurrent HCC after transplantation. Methods:Consecutive patients with HBV-related HCC with LT were included. Oral nucleos(t)ide analogues without hepatitis B immune globulin were used as hepatitis B virus (HBV) prophylaxis. Quantitative HBsAg levels were performed at time of transplant, at 1 month, 3 and 6 months post transplant using a highly sensitive (hs)-HBsAg assay. Results:One hundred and fourteen patients were included, with a median follow-up of 80 months, with 24 cases of HCC recurrence, and a cumulative rate of 20.7% at 5 years. There was significant correlation between time of tumor recurrence and time of HBsAg reappearance (r = 0.551,P = 0.027). Early HCC recurrence was associated with higher median level of hs-HBsAg at the time of transplant (72.85vs. 69.70 IU/mL,P = 0.018). Using a hs-HBsAg cut-off level of 0.0005 IU/mL, patients with levels above this threshold at 3 and 6 months were associated with higher rate of early HCC recurrence (28.6%vs. 3.0% and 26.9%vs. 2.9% respectively, bothP =0.0006). There was no significant difference in HCC recurrence between positive and negative HBsAg using the conventional qualitative HBsAg assay. Conclusion:Serum hs-HBsAg levels of≥ 0.0005 IU/mL at 3 to 6 months after LT is associated with higher rates of early HCC recurrence, and may be useful as an early tumor marker.

Adaptive models are based on the observation that there are some actions that people can and actually do take to achieve thermal comfort. Studies regarding thermal comfort conditions in economical dwellings were carried out simultaneously in seven Mexican cities, corresponding to warm dry and warm humid climates. In this article, case studies of low-cost dwellings in the city of Hermosillo (in northwest Mexico), are presented and analyzed. Field surveys were carried out to obtain information about the physical characteristics of the dwellings and their occupants, as well as the indoor thermal environment. Neutral temperature was obtained from the applied survey. The high neutral temperature reveals the effect of inhabitants" adaptation mechanism to extreme climates. Occupant comfort votes as a function of indoor air temperatures were analyzed, and different characteristics such as age, size and gender were evaluated separately. The results show the variability of the neutral temperature and the tolerance to temperature changes, depending on the population"s specific characteristics. In many cases where the population does not have access to artificial acclimatization devices, the neutral temperature values for specific climates and people can inform architects when choosing the most suitable thermal strategies for building design.

Lactobacillus plantarum HL-15 is able to inhibit the growth of mycotoxin producing *** support the application of that culture,the research aimed to produce dried *** HL-15 and observe its stability during storage was *** of dried culture was started by fermentation of *** HL-15 then centrifuged to get the *** pellet was mixed with filler(rice flour or tapioca)with a ratio 1:1(pellet:filler(10%),v/v)then *** temperature of the inlet and outlet of the spray dryer used were 105°C and 65°C,*** culture was packaged in aluminum foil and sealed then stored at 4°*** showed that viable cells of dried inoculum with rice flour filler was 11.99±0.01 log CFU/g and dried inoculum with tapioca filler was 11.90±0.04 log CFU/*** dried *** HL-15 was proved being able to inhibit the growth of Aspergillus *** six months storage,there is a decrease in the viability of spray dried *** HL-15 either with rice flour or tapioca *** four months of storage,there was a decrease in the activity of inhibition of spray dried culture of *** HL-15 both with fillers of rice flour and tapioca to the growth of *** HL-15 spray dried cultures could be stored at 4°C for four months for tapioca and five months for rice flour as *** HL-15 spray dried cultures could inhibit the growth of *** so it could be used as a culture for inhibiting the growth of mycotoxin producing fungi in food.

The poliomyelitis is an acute infection produced by the polio virus that affects the human central nervous system. It is transmitted by fecal-oral and respiratory contact. There are two types of vaccine, OPV (live attenuated virus) and IPV (inactivated polio virus). Currently, there is a plan of vaccination until the age of 5 with OPV. The children vaccinated expel a virus (derived from the vaccine) to the environment, and some of the people that have oral contact with them, get vaccinated by the herd behavior. Nevertheless, taking into account the lately observed facts about the reversion to virulence of the oral polio vaccine during its circulation in the environment, a change in the current vaccination schedule is being contemplated, where the oral polio vaccine can be replaced by the inactivated vaccine. Nowadays, In Colombia the inactivated oral polio vaccine is recommended for children presenting immune deficiency who are vaccinated with IPV. These children do not expel poliovirus to the environment. This work presents a mathematical model that describes the dynamics of the infection in a population where the two types of vaccination are carried out. The population is divided into two groups of age and Michaelis-Menten interactions. Different strategies of vaccination are simulated and analyzed.

Cardiopulmonary bypass with brain perfusion for renal cell carcinoma with caval thrombosis

AIM: To compare a modified technique preserving brain circulation during cardiopulmonary by-pass(CPB) for radical nephrectomy and caval thrombectomy, to the standard ***: Retrospective evaluation of an institutional database that collects the data of patients submitted to nephrectomy and removal of caval thrombosis with CPB since 1998. In period between 1998 and 2007, CPB followed a standard technique(group s CPB); then, since 2008, a variation in the perfusional technique was introduced, allowing the anterograde perfusion of brain circulation during circulatory arrest(group CPB + BP) with the aim to reduce the risk of ischemic damage to the brain and also the need of deeper hypothermia. Patients(age, gender, comorbidity) and tumor characteristics(side, histology, staging, level of thrombosis), as well as parameters of CPB(times of CPB, aortic clamping andcirculatory arrest, minimum temperature reached during hypothermia), intra- and perioperative morbidity(complications in general, bleeding, renal and hepatic failure) and mortality were analyzed and compared between 2 groups(s CPB vs CPB + BP)RESULTS: The data of 24 patients, respectively 9 in s CPB group and 15 in CPB + BP group, have been reviewed. No differences in the characteristics of patients and tumors were observed. Only 1(11.1%) and 4(26.0%) of s CPB and CPB + BP patients, respectively, didn't experience any event of complication. In s CPB group were observed 15 events of complication(5 of which Clavien ≥ 3, 33% of the events), for a mean of 1.66 events/patient; 29 events(10 Clavien ≥ 3, 30.3%), in the CPB + BP group, for a mean of 2.1 events/patient. 1(11.1%) and 2(14.2%) deaths occurred, respectively. For patients submitted to CPB + BP, the minimum temperature reached was significantly higher(29.9 ℃ vs 26.4 ℃, P = 0.001), the time of circulatory arrest was longer(17.4 min vs 13.7 min, NS), but the overall time of CPB shorter(76.1 min vs 92.5 min, NS), albeit these latter differences were n
