The breast is a rare localization of extra-pulmonary tuberculosis, even in highly endemic countries. It is most often primitive. The clinical and radiological features are sometimes misleading. It poses diagnostic problems especially with cancer. We report a case of breast tuberculosis having clinical and radiological presentations as a breast carcinoma. Through this observation and a review of the literature, we provide an update on the epidemiological characteristics and the means of diagnosis of this lesion. It was a 52-year-old woman with inflammatory lump of the left breast, adherent to the skin. Ultrasonography and mammography revealed a homogeneous, poorly demarcated nodule of approximately 4 cm of diameter, classified ACR4. Surgical excision was performed. The histological diagnosis was breast tuberculosis. The diagnosis of tuberculous mastitis is made on bacteriological and pathological examinations. Histological examination helps for diagnostic confirmation while excluding a malignant lesion of the breast.

Uterine rupture is defined as the occurrence of communication between the abdominal and uterine cavity and may be complete or incomplete depending on the degree of involvement of the different layers of the uterus and surrounding organs. It is a rare complication whose consequences often involve the maternal and fetal prognosis. The majority of uterine rupture occurs on the scarred uterus, its incidence in France is estimated according to the series between 1/1000 and 1/2000 births, it represents 30% of causes of maternal death in the developing countries. The authors report here a case of uterine rupture outside of labor at 33 weeks of age in 32 years old woman, gravida 9 para 8, with no history of uterine surgery discovered during obstetric ultrasound for abdominal pain. During the interrogation, she was alarge multipara and had a child of 15 months. The abdominal ultrasound showed a right lateral corporeal rupture with hemoperitoneum of medium sized and a dead fetus. The emergency laparotomy revealed a right lateral uterine wound approximately 15 cm long with intra-abdominal placenta and a haemoperitoneum of medium sized of about 600 cc and a bladder lesion. After opening the amniotic sac, there was extraction of a dead fetus. The uterine and bladder lesions were repaired followed by bilateral tubal ligation. The patient received 500 cc of whole blood during and 500 cc after the procedure. The postoperative follow-up was simple. This case contributes to the knowledge of this rare and atypical event, and emphasizes the importance of maintaining a suspicion.

The hypoglycaemia paraneoplastic is uncommon in diabetic patients. Our aim was to report a case of paraneoplastic hypoglycaemia discovered after the diagnosis of gastric adenocarcinoma in a diabetic patient. A 65 years old man, was hospitalized for the management of acute digestive haemorrhage. In his medical history, he had diabetes since 20 years. He was alcoholic weaned for 3 years and had an unexplored repetitive epigastralgia. During his hospitalization, the exploration of acute digestive haemorrhage found a gastric adenocarcinoma with hepatic metastasis. At the 4th day of his hospitalization, he presented a hypoglycaemic coma with capillary blood glucose at 0.36 g/L. The patient was conscious after the infusion of Hypertonic Glucose Solution (HGS) 10%. After 3 days, the recurrent hypoglycaemia occurred and we intermittently treated by infusion of glucose solution 10%. The measurements of insulin level and C-peptide weren’t performed because they didn’t exist in the city. So, the notion of cancer and the recurrent hypoglycaemia even if we stopped the oral antidiabetic therapy make us think of a paraneoplastic syndrome. The chemotherapy associated with corticosteroid therapy is the standard treatment of the hypoglycaemia paraneoplastic but the patient refused the chemotherapy and we couldn’t use the corticosteroid therapy because of digestive haemorrhage. At the end of his hospitalization, he still had hypoglycaemia treated by infusion of glucose solution 10%. In the absence of means of diagnosis, we should think of paraneoplastic syndrome in front of recurrent hypoglycaemia and cancer in the diabetic patient.

Bone and Soft Tissue Blood Flow during Normobaric and Hyperbaric Oxygen Breathing in Healthy Divers

Purpose: The study aimed to investigate, using a photoplethysmographic (PPG) technique, how pulsatile blood flow within the patellar bone and skin over the patella reacts to normobaric (NBO) and hyperbaric oxygen breathing (HBO). Methods: Eleven healthy volunteers, breathed air or oxygen. Subjects were blinded to breathing gas. A range of partial pressures of oxygen were administered in 10 minute intervals: 21 kPa, 101 kPa (NBO), 21 kPa, (compression to 280 kPa), 59 kPa, 280 kPa (HBO), 59 kPa, (decompression), and 21 kPa. Changes were measured continuously for each individual. results: Hyperoxia decreased pulsatile patellar blood flow ~32 resp. 38% and skin blood flow ~36 resp. 42% during the first 2 - 3 minutes of NBO resp. HBO. This decrease was normalized within 5 minutes after exposure. The results were similar when switching from air to NBO (101 kPa) and from air at pressure (59 kPa) to HBO (280 kPa). Conclusions: The study shows that pulsatile patellar skin and bone blood flow, decreases significantly as a reaction to oxygen breathing in healthy subjects. The results suggest that a non-invasive PPG technique could be used to monitor blood flow changes in bone during oxygen treatment.

Background: Trauma is the main reason for consultation in the emergency unit. They deserve particular attention in the Congo where road accidents and violence of all kinds are increasingly observed. Methods: This study who carried out in the surgical emergencies at the University Hospital of Brazzaville, aims to analyze the epidemiological and etiological aspects, the anatomical lesions encountered;to describe their management, as well as to evaluate the induced mortality while emphasizing their place in all surgical emergencies. The prospective and analytical study included 2127 patients admitted to a traumatic emergency unit from January 1 to June 30, 2018. Medical records served as data sources. The measure of trauma severity was determined using Champion’s Trauma Score. The epidemiological, clinical and therapeutic parameters were studied. results: The population most affected was adolescents and young adults (mean age, 27.3 years) with a male predominance: 1318 men (i.e. 61.9%) for 809 women (i.e. 38.1%). road accidents (n = 819, or 38.5%) were the main cause of injuries, followed by violence (n = 702, or 33%) and falls (27.3%). The skull, limbs, thorax and abdomen are the main targets. The most observed lesions were limb fractures (n = 344, i.e. 16.1%), intracranial hematomas and contusions (n = 315, i.e. 14.8%), thoracic and abdominal wounds (n = 28, i.e. 1.3%). Mortality was evaluated at 0.6%, and was correlated with the severity of the lesions and the type of trauma. Conclusion: Trauma is very common in emergency consultations in Congo, with a hospital frequency of 75.1% of all emergencies. Preventive actions are essential to reduce their frequency.

The purpose of this study is to quantify the polyphenolic content and evaluated anti-inflammatory and antioxidant activity of four plants extracts of Annona senegalensis, Ipomoea batatas, Terminalia superba and Psidium guajava. Phenolic and flavonoid contents were determined by Folin-Ciocalteu and aluminum trichloride methods, respectively with ethanolic extract. The antioxidant activity was accessed by DPPH and FrAP methods. The results of the analysis of this study indicate that the ethanol extracts of the test plants (A. senegalensis, I. batatas, P. guajava and T. superba) are potential source of natural antioxidant and anti-inflammatory.

Schistosomiasis is a chronic disease, and remains a major public health problem. It holds second place among parasitic endemics in the world. Schistosoma can infect various organs through the blood vessels. The genital form affects at least 16 million women in endemic areas, and the uterine cervix is the most common site. Two cases of cervical cancer associated with schistosomiasis of the cervix are presented. The aim of this study is to discuss the epidemiological, clinical and histopathological features. Both of the patients lived in Mahanjaga, aged respectively 57 and 43 years old. They complained of uterine bleeding disorder and presented cauliflower lesions with ulceration of the uterine cervix that extended to the vagina in one case. They were undergone cervical biopsies. The diagnosis was, in both cases, invasive, well-differentiated squamous cell carcinoma with cervical schistosomiasis.

Alloimmunization to platelet antigens exposes to a serious immunological incident. We report a probable case of a post-transfusion purpura from homozygous sickle cell child alloimmunized against platelet antigen. We detail the challenges of alloantibody identification and transfusion management. These challenges are due to the lack of laboratory techniques for typing HLA and HPA system and the use of leukocyte-depleted (filtered) blood products.
