Automatically Updated Soundmaps are maps that convey the sound rather than the visual information content of an area of interest, at a certain time instant or period. Sound features encapsulate information that can be combined with the visual features of the landscape, thus leading to useful environmental conclusions. This work aims to construct an Automatically Updated Soundmap of an area of environmental interest. A hierarchical pattern recognition approach method is proposed here, that can exploit sound recordings collected by a network of microphones. Hence, after appropriate signal processing, the large amounts of information, originally in the raw form of sound recordings, can be presented in the concise yet meaningful form of a periodically updated soundmap.

AIM: To evaluate the diagnostic sensitivity and accuracy and the cost-effectiveness of this technique in the detection of gastroenteropancreatic cardnoid tumors and their metastases in comparison with conventional imaging ***: Somatostatin receptor scintigraphy (SRS) was performed in 24 patients with confirmed carcinoids and 7 under investigation. The results were compared with those of conventional imaging methods (chest X-ray, upper abdominal ultrasound, chest CT, upper and lower abdominal CT). Also a cost-effectiveness analysis was performed comparing the cost in Euro of several combinations of SRS with conventional imaging ***: SRS visualized primary or metastatic sites in 71.0% of cases and 61.3% of conventional imagings. The diagnostic sensitivity of the method was higher in patients with suspected lesions (85.7% vs57.1%). SRS was less sensitive in the detection of metastatic sites (78.9% vs 84.2%). The undetectable lesions by SRS metastatic sites were all in the liver. Between several imaging combinations, the combinations of chest X-ray/upper abdominal CT/SRS and chest CT/upper abdominal CT/SRS showed the highest sensitivity (88.75%) in terms of the number of detected lesions. The combinations of chest X-ray/upper abdominal US/SRS and chest CT/upper abdominal ultrasound/SRS yielded also a quite similar sensitivity (82%). Compared to the cost of the four sensitive combinations the combination of chest X-ray/upper abdominal ultrasound/SRS presented the lower cost,1183.99 Euro vs 1251.75 Euro for chest CT/upper abdominal ultrasound/SRS, 1294.93 Euro for chest X/ray/upper abdominal CT/SRS and 1362.75 Euro for chest CT/upper abdominal CT/***: SRS imaging is a very sensitive method for the detection of gastroenteropancreatic carcinoids but is less sensitive than ultrasound and CT in the detection of liver metastases. Based on twinned calcite in carbonate samples collected from the External Hellenides, we calculated paleostress magnitude and its across-strike variation into the Pindos Zone (PZ) and the Gavrovo Zone (GZ). These two zones are located west of the suture zone between the External and the Internal Hellenides. Paleostress magnitude is in the order of 110 MPa close to the suture, dropping toward the foreland and reaching a value of 50 MPa 80 km from the suture. The obtained distribution of paleostress levels in the External Hellenides resembles that recorded in other orogens, such as the Appalachian and Northern Pyrenean. The paleodepths of sampled carbonates during the initial stage of thrusting, which were estimated using the differential stress values, are structurally sensible. Comparing our results to available information on the evolution of paleostress magnitude with depth in compressional settings at the upper crust, we suggest that the mechanical behaviour of the cover thrust sheets of the External Hellenides is governed by the law of friction under nearly hydrostatic conditions. Based on this finding, as well as on published rheological data from rocks exposed at the deeper structural levels of the External Hellenides, a rheological profile was proposed. On this profile, brittle-ductile transition occurs at a depth of around 9-10 km, slightly deeper than the basal decollement of the External Hellenides.

Despite the effectiveness of endoscopic retrograde cholangiopancreatography（ERCP） for the treatment of choledocholithiasis, various complications have been described. We herein report the first case of prolonged postERCP jaundice due to toxicity of the contrast agent Iobitridol（~174;XENETIX, Guerbet, Roissy Cd G Cedex, France） in a patient who underwent ERCP with sphincterectomy and common bile duct stone removal. While clinical improvement and normalization of aminotransferases and cholestatic enzymes after the procedure, an unexplained increase of direct bilirubin was noticed. A second ERCP was performed one week later, excluding possible remaining choledocholithiasis. Nevertheless, serum direct bilirubin increased further up to 15 mg/d L. Other potential causes of direct hyperbilirubinemia were ruled out and patient＇s liver biopsy was compatible with drug-induced liver toxicity. Additionally, the cause-result time connection between the use of Iobitridol and bilirubin increase indicated the possibility of a toxic effect related to the repeated use of the particular contrast agent. Iobitridol, a contrast agent, can induce prolonged direct hyperbilirubinemia.

Although colonoscopy has been proven effective in reducing the incidence of colorectal cancer through the detection and removal of precancerous lesions, it remains an imperfect examination, as it can fail in detecting up to almost one fourth of existing adenomas. Among reasons accounting for such failures, is the inability to meticulously visualize the colonic mucosa located either proximal to haustral folds or anatomic curves, including the hepatic and splenic flexures. In order to overcome these limitations, various colonoscope attachments aiming to improve mucosal visualization have been developed. All of them-transparent cap, Endocuff, Endocuff Vision and Endorings-are simply mounted onto the distal tip of the scope. In this review article, we introduce the rationale of their development, present their mode of action and discuss in detail the effect of their implementation in the detection of lesions during colonoscopy.

BACKGROUND Endocuff-a plastic device with flexible projections-mounted on the distal tip of the colonoscope, promises improved colonic mucosa *** To elucidate the effect of Endocuff on adenoma detection rate(ADR), advanced ADR(AADR) and mean number of adenomas per colonoscopy(MAC).METHODS Literature searches identified randomized-controlled trials evaluating Endocuffassisted colonoscopy(EAC) vs conventional colonoscopy(CC) in terms of ADR,AADR and MAC. The effect size on study outcomes was calculated using fixed or random effect model, as appropriate, and it is shown as relative risk(RR) [95%confidence interval(CI)] and mean difference(MD)(95%CI). The rate of device removal in EAC arms was also *** We identified nine studies enrolling 6038 patients. All studies included mixed population(screening, surveillance and diagnostic examinations). Seven and two studies evaluated the first and the second-generation device, respectively. EAC was associated with increased ADR compared to CC [RR(95%CI): 1.18(1.05-1.32); Ι~2 = 71%]; EAC benefits more endoscopists with ADR ≤ 35% compared to those with ADR > 35% [RR(95%CI): 1.37(1.08-1.74); Ι~2 = 49% vs 1.10(0.99-1.24); Ι~2= 71%]. In terms of AADR and MAC, no difference was detected between EAC and CC [RR(95%CI): 1.03(0.85-1.25); Ι~2 = 15% and MD(95%CI): 0.30(-0.17-0.78);Ι~2 = 99%].
