Non-cirrhotic portal hypertension consists of a group of diseases characterized by signs and complications of portal hypertension,which differ from cirrhosis through histological alterations,hemodynamic characterization and,clinical *** of the similarities in clinical presentation and imaging signs,frequently these patients,and particularly those with porto-sinusoidal vascular disease(PSVD),are misdiagnosed as having liver cirrhosis and thus raising difficulties in their *** most challenging differentiation to be considered is between PSVD and cirrhosis and,although not pathognomonic,liver biopsy is still the standard of *** they still require extended validation before being broadly used,new non-invasive methods for the diagnosis of porto-sinusoidal vascular disease,like transient elastography,contrast-enhanced ultrasound or metabolomic profiling,have shown promising *** issue is the differentiation between PSVD and chronic extrahepatic portal vein obstruction,especially now when it is known that 40%of patients suffering from PSVD develop portal vein *** this particular case,once the portal vein thrombosis occurred,the diagnosis of PSVD is impossible according to the current ***,so far,the differentiation between PSVD and sinusoidal obstruction syndrome has not been clear so far in particular *** this review we highlighted the diagnostic challenges regarding the PSVD,as well as the current techniques used in the evaluation of these patients.

Primary Small Bowel Melanoma or Small Bowel Metastasis with Vanishing Primary Cutaneous Lesion

The small bowel represents one of the main sites for cutaneous melanoma metastasis;however, numerous cases of primary intestinal melanoma have recently been described. In view of this, we present the case of a 39-year-old woman admitted for nausea, heartburn, abdominal pain, change in bowel habits and weight loss. Contrast-enhanced CT revealed a small bowel mass. Surgical resection of a 6 cm ileal tumour with regional mesenteric lymphadenectomy and end-to-end anastomosis was performed. Histopathological findings indicated the presence of an ileal melanoma metastasis. Subsequent dermatological examination identified a cutaneous lesion on the right forearm, however no malignant cells were found at the histopathological exam. Whole body PET CT with FDG identified multiple frontal and parietal lesions. Genetic testing was positive for BRAF gene V600 E mutation. The patient underwent multiple neurosurgical procedures for the resection of cerebral metastases. Palliative external radiation and chemotherapy was also attempted. After approximately 2 years after the diagnosis, the patient died following multiple episodes of intracranial hypertension.

Zoonotic sources and the spread of antimicrobial resistance from the perspective of low and middle-income countries

Background Antimicrobial resistance is an increasing challenge in low and middle-income countries as it is wide-spread in these countries and is linked to an increased *** from human and environmental factors,animal-related drivers of antimicrobial resistance in low-and middle-income countries have special features that differ from high-income *** aim of this narrative review is to address the zoonotic sources and the spread of antimicrobial resistance from the perspective of low-and middle-income *** body Contamination with extended-spectrum beta-lactamase(ESBL)-producingEscherichia coli is highest in poultry(Africa:8.9–60%,Asia:53–93%)and there is a risk to import *** through poultry meat in *** aquacultures,the proportion of ESBL-producers *** can be high(27%)but the overall low quality of published studies limit the general conclusion on the impact of aquacultures on human *** colonization of wildlife is 1–9%in bats or 2.5–63%*** most of them are migratory animals,they can disperse antimicrobial resistant bacteria over large ***-called‘filth flies’are a relevant vector not only of enteric pathogens but also of antimicrobial resistant bacteria in settings where sanitary systems are *** Africa,up to 72.5%of‘filth flies’are colonized with ***,mostly conferred by CTX-M(24.4–100%).While methicillin-resistantStaphylococcus aureus plays a minor role in livestock in Africa,it is frequently found in South America in poultry(27%)or pork(37.5–56.5%)but less common in Asia(poultry:3%,pork:1–16%).Conclusions Interventions to contain the spread of AMR should be tailored to the needs of low-and middle-income *** comprise capacity building of diagnostic facilities,surveillance,infection prevention and control in small-scale farming.

Industrial woven meshes are composed of metal materials and are often used in construction,industrial and residential activities or *** objective of this work is defect detection in industrial fabrics in the quality control *** order to overcome the limitations of manual methods,which are often tedious and time-consuming,we propose a strategy that can automatically detect defects in micrometric steel meshes by means of a Convolutional Neural *** database used for such a purpose comes from real problem data for anomaly detection in micrometric woven *** detection is performed through supervised classification with a Convolutional Neural Network using a VGG19 *** define a pipeline and a strategy to tackle the related small amount of *** includes i)augmenting the database with translation,rotation and symmetry,ii)using pre-trained weights and iii)checking the learning curve behaviour through *** obtained results show that,despite the small size of our databases,detection accuracy of 96%was reached.

Left main coronary compression syndrome(LMCS)may complicate pulmonary artery aneurysms(PAA),usually developed in the context of pulmonary arterial hypertension(PAH).We report the case of a 51-year-old female patient with an atrial septal defect(unsuitable for device closure)complicated by a PAA generating a 90%left main *** significant PAH held us back from immediate *** specific dual PAH-targeted therapy(sildenafil and bosentan),the atrial septal defect could be closed with a unidirectional valved patch;the PAAinduced LMCS was treated by reductive *** postoperative course was ***-up showed clinical improvement,but PAH treatment was still *** three months,coronary angiography showed only an insignificant residual left main stenosis,proving that reductive pulmonary arterioplasty was effective in treating *** PAA requires further evaluation for LMCS,a dangerous but treatable ***“treat-repair-treat”strategy and shunt-closure with a unidirectional valved patch can both improve surgical prospects of LMCS with shunt-related PAH.

Influence of Doping Ions on the Antibacterial Activity of Biomimetic Coating on CoCrMo Alloy

In this study biomimetic fluoridated phosphate doped hydrophilic coatings with various ions on CoCrMo alloy were pre- pared by electrodeposition. Cu and Zn ions were chosen for doping because of their well known antibacterial activity. The struc^xes of the coatings were identified using Fourier-transform Infrared （FTIR） analysis. X-ray Diffraction （XRD） analysis was performed to evaluate crystallite dimensions of the specimen surface. The contact angle was measured in order to establish the hydrophilic/hydrophobic balance and to compute surface energy. All studied samples have a hydrophilic character which is weaken after doping. The time evolution of ions releasing from the coatings was evaluated with an inductively plasma mass spectrometer after immersion in saline phosphate. The hemolytic experiments indicate that except the fluoridated coatings doped with Zn which is slightly hemolytic, all other samples are non hemolytic. The test for antibacterial activity for Staphy- lococcus aureus and Pseudomonas aeruginosa indicated that the fluoridated biomimetic coating doped with various positive ions increases bacterial growth inhibition level significantly. Fluoridated phosphate coating doped with Cu has best antibacterial activity

Neurological complications of hematopoietic cell transplantation in children and adults

Hematopoietic cell transplantation（HCT） is widely performed for neoplastic and non-neoplastic diseases. HCT involves intravenous infusion of hematopoietic progenitor cells from human leukocyte antigen（HLA）-matched donor（allogeneic） or from the patient（autologous）. Before HCT, the patient is prepared with high dose chemotherapy and/or radiotherapy to destroy residual malignant cells and to reduce immunologic resistance. After HCT, chemotherapy is used to prevent graft rejection and graft versus host disease（Gv HD）. Neurological complications are related to the type of HCT, underlying disease, toxicity of the conditioning regimens, immunosuppression caused by conditioning regimens, vascular complications generated by thrombocytopenia and/or coagulopathy, Gv HD and inappropriate immune response. In this review, neurological complications are presented according to time of onset after HCT：（1） early complications（in the first month）-related to harvesting of stem cells, during conditioning（drug toxicity, posterior reversible encephalopathy syndrome）, related to pancytopenia,（2） intermediate phase complications（second to sixth month）-central nervous system infections caused by prolonged neutropenia and progressive multifocal leukoencephalopathy due to JC virus,（3） late phase complications（after sixth month）-neurological complications of Gv HD, second neoplasms and relapses of the original disease.

乡村旅游是一个在全球范围内广泛关注的领域。最近,随着新冠疫情的爆发,乡村旅游业受到了更多关注。本文的研究,以罗马尼亚拉普斯地区为典型案例,分析罗马尼亚乡村旅游的主要特征,思考改善乡村旅游业的方法,并对中国乡村旅游提出罗马尼亚的经验以资借鉴。促使笔者分析罗马尼亚乡村旅游发展的原因是因为罗马尼亚仍然是一个高度传统的国家,它以自然的方式保留其民俗习惯,并且在文化遗产方面具有巨大的潜力。事实上,也只有少数研究论文从乡村旅游的角度对该地区进行分析,这使笔者将理论定义和个人经验结合起来,以便对这一特定主题有一个更切实的分析。本文一共分为五章,前两章是介绍选题的背景、意义、方法、思路和研究现状。正文共有两部分,第一部分以Tara Lǎpusului地区为例,从多个角度分析了罗马尼亚乡村旅游的现状和未来发展状况。第二部分介绍了这些方法对中国的有益影响和借鉴,并讨论了这些方法在中国环境下的可能整合。

Mapping flood and flooding potential indices： a methodological approach to identifying areas susceptible to flood and flooding risk. Case study： the Prahova catchment （Romania）

Given that floods continue to cause yearly significant worldwide human and material damages, flood risk mitigation is a key issue and a permanent challenge in developing policies and strategies at various spatial scales. Therefore, a basic phase is elaborating hazard and flood risk maps, documents which are an essential support for flood risk management. The aim of this paper is to develop an approach that allows for the identification of flash-flood and flood-prone susceptible areas based on computing and mapping of two indices： FFPI （Flash-Flood Potential Index） and FPI （Flooding Potential Index）. These indices are obtained by integrating in a GIS environment several geographical variables which control runoff（in the case of the FFPI） and favour flooding （in the case of the FPI）. The methodology was applied in the upper （mountainous） and middle （hilly） catchment of the Prahova River, a densely populated and socioeconomically well-developed area which has been affected repeatedly by water-related hazards over the past decades. The resulting maps showing the spatialization of the FFPI and FPI allow for the identification of areas with high susceptibility to flash- floods and flooding. This approach can provide useful mapped information, especially for areas （generally large） where there are no flood/hazard risk maps. Moreover, the FFPI and FPI maps can constitute a preliminary step for flood risk and vulnerability assessment.
