Most existing studies assign a polyynic and cumulenic character of chemical bonding in carbon-based chains relying on values of the bond *** on our recent work,in this paper we add further evidence on the limitations of such an analysis and demonstrate the significant insight gained via natural bond *** reported results include atomic charges,natural bond order and valence indices obtained from ab initio computations for representative members of the astrophysically relevant neutral and charged HC_(2k/2k+1)H chain *** unravel a series of counter-intuitive aspects and/or help naive intuition in properly understanding microscopic processes,e.g.,electron removal from or electron attachment to a neutral *** that the Wiberg indices adequately quantify the chemical bonding structure of the HC_(2k/2k+1)H chains—while the often heavily advertised Mayer indices do not—represents an important message conveyed by the present study.

This study performed a bridge pier collar comparison for the purpose of reducing scour,while introducing a new collar *** new collar,referred to as Collar Prototype Number 3,was designed based on an equilibrium scour hole and provides a method of controlling the horseshoe *** modelling was utilized to show the flow field and bed shear stress as a result of using Collar Prototype Number 3.A prototype model of Collar Prototype Number 3 and a flat plate collar were constructed for the purpose of comparing both experimentally under different flow *** Prototype Number 3 proved to be an effective device as it reduced scour in terms of the maximum depth downstream of the pier and overall volume,in some instances,by up to 69.7%and 75.7%,***,Collar Prototype Number 3 outperformed the flat plate collar,especially because,unlike the flat plate collar,Collar Prototype Number 3 did not experience undermining.

The preparation of two highly luminescent europium（Ill） and terbium（Ill） complexes with succinimide （SI） and N-hydroxysuccinimide （NHSI） were reported, which were further investigated through elemental chemical analysis, thermal analysis, FT-IR, powder X-ray diffraction, SEM and fluorescence spectroscopy. data collected through these analysis revealed the formation of the Eu（III） and Th（III） complexes with the above mentioned ligands at a metal to ligand （M：L） molar ratio of 1：3. Interesting conclusions regarding the effi- cient sensitization through the coordination site of the lanthanide luminescent centers were retained from the photoluminescent spectra inves- tigation. Strong luminescence emission was observed in case of Eu（III）-SI and Tb（III）-NHSI complexes while the corresponding Eu（III）-NHSI and Tb（III）-SI complexes exhibited no photo luminescent properties. Newly obtained luminescent lanthanide complexes may be of particular interest for various applications in optoelectronics.

Severe adverse events during antiviral therapy in hepatitis C virus cirrhotic patients: A systematic review

AIM: To identify severe adverse events (SAEs) leading to treatment discontinuation that occur during antiviral therapy in hepatitis C virus (HCV)-infected cirrhotic patients. METHOdS: We identified all the articles published prior to december 2011 in the PubMed, Medline, Lilacs, Scopus, Ovid, EMBASE, Cochrane and Medscape databases that presented these data in cirrhotic patients. These studies evaluated the rate of SAEs leading to discontinuation of standard care treatment: Pegylated interferon (PegIFN) alpha 2a (135-180 μg/wk) or PegIFN alpha 2b (1 or 1.5 μg/kg per week) and ribavirin (800-1200 mg/d). Patients with genotype 1 + 4 underwent treatment for 48 wk, whereas those with genotypes 2 + 3 were treated for 24 ***: We included 17 papers in this review, comprising of 1133 patients. Treatment was discontinued due to SAEs in 14.5% of the patients. The most common SAEs were: severe thrombocytopenia and/or neutropenia (23.2%), psychiatric disorders (15.5%), decompensation of liver cirrhosis (12.1%) and severe anemia (11.2%). The proportion of patients who needed to discontinue their therapy due to SAEs was significantly higher in patients with Child-Pugh class B and Cvs those with Child-Pugh class A: 22%vs 11.4% (P = 0.003). A similar discontinuation rate was found in cirrhotic patients treated with PegIFN alpha 2a and those treated with PegIFN alpha 2b, in combination with ribavirin: 14.2%vs 13.7% (P = 0.96). The overall sustained virological response rate in cirrhotic patients was 37% (95%CI: 33.5-43.1) but was significantly lower in patients with genotype 1 + 4 than in those with genotype 2 + 3: 20.5% (95%CI: 17.9-24.8) vs 56.5% (95%CI: 51.5-63.2), (P d with PegIFN and ribavirin needed early discontinuation of therapy due to SAEs, the most common cause being hematological disorders.

AIM: To analyze contrast-enhanced ultrasound(CEUS) features of histologically proven hepatic epithelioid hemangioendothelioma(HEHE) in comparison to other multilocular benign focal liver lesions(FLL).METHOdS: Twenty-five patients with histologically proven HEHE and 45 patients with histologically proven multilocular benign FLL were retrospectively reviewed. Four radiologists assessed the CEUS enhancement pattern in ***: HEHE manifested as a single(n = 3) or multinodular(n = 22) FLL. On CEUS, HEHE showed rim-like(18/25, 72%) or heterogeneous hyperenhancement(7/25, 28%) in the arterial phase and hypoenhancement(25/25, 100%) in the portal venous and late phases(PVLP), a sign of malignancy. Eighteen patients showed central unenhanced areas(18/25, 72%); in seven patients(7/25, 28%), more lesions were detected in the PVLP. In contrast, all patients with hemangioma and focal nodular hyperplasia showed hyperenhancement as the most distinctive feature(P determine the malignant nature of HEHE.

Intestinal tuberculosis(TB) is an uncommon lesion for which differential diagnosis can be difficult. We present a case of a 53-year-old male and a systematic review of the literature, from clinical symptoms to differential diagnosis, unusual complications and therapy. The patient was admitted to the hospital with signs of acute abdomen as a result of a perforated terminal ileitis. Based on the skip lesions of the terminal ileum and cecum, Crohn's disease(Cd) was clinically suspected. An emergency laparotomy and right colectomy with terminal ileum resection was performed and systematic antibiotherapy was prescribed. The patient's status deteriorated and he died 4 d after the surgical intervention. At the autopsy, TB ileotyphlitis was discovered. The clinical criteria of the differential diagnosis between intestinal TB and Cd are not very well established. despite the large amount of published articles on this subject, only 50 papers present new data regarding intestinal TB. Based on these studies and our experience, we present an update focused on the differential diagnosis and therapy of intestinal TB. We highlight the importance of considering intestinal TB as a differential diagnosis for inflammatory bowel disease. despite the modern techniques of diagnosis and therapy, the fulminant evolution of TB can still lead to a patient's death.

Lymph node status is considered a key prognostic and predictive factor in patients with gastric cancer(GC).Although there is a practical approach to the intraoperative detection of sentinel lymph nodes(SLNs),such a procedure is not included in the European surgical *** this report,we present a practical approach to SLN mapping in a representative case with early gastric cancer(EGC).A 74-year-old female was hospitalized with an endoscopically observed,superficially ulcerated tumor located in the antral *** gastrectomy with d2 lymphadenectomy and SLN mapping was performed by injecting methylene blue dye into the peritumoral submucosal *** incidentally detected blue-stained lymph node located along the middle colic artery was also *** was detected 40 min after injection of the methylene *** examination showed a p T1b-staged well-differentiated HER-2-negative *** of the 41 LNs located at the first,third,and fifth station of the regional LN compartments were found to be free of tumor *** only lymph node with metastasis was located along the middle colicartery and was considered a non-regional lymph *** incidentally identified skip metastasis indicated stage Ⅳ GC.A classic chemotherapy regimen was given,and no recurrences were observed six months after *** this representative case,low-cost SLN mapping,with a longer intraoperative waiting time,totally changed the stage of the tumor in a patient with EGC.

AIM： To evaluate the response to pegylated-interferon alpha 2a in chronic hepatitis C patients on chronic haemodialysis. METHOdS： Ten patients with chronic C hepatitis were enrolled in this study. All had increased aminotransferases for more than 6 too, positive antiHCV antibodies and positive PCR HCV-RNA. We administrated Peg-Interferon alpha 2a 180 μg/wk for 48 wk. After 12 wk of treatment we evaluated the biochemical and early virological response （EVR）. At the end of the treatment we evaluated the biochemical response and 24 wk after the end of the treatment we evaluated the sustained virological response （SVR）. We monitored the sideeffects during the treatment. RESULTS： Two patients dropped out in the first 12 wk of treatment and 2 after the first 12 wk of treatment. After 12 wk of treatment, 7 out of 8 patients had biochemical response and EVR and 1 had biochemical response but persistent viremia. We had to reduce the dose of pegylated-interferon to 135 μg/wk in 2 cases. Three out of 6 （50%） patients had SVR 24 wk after the end of the treatment. Intention-to-treat analysis showed that 3 out of 10 patients （30%） had SVR. Side-effects occurred in most of the patients （flu-like syndrome, thrombocytopenia or leucopoenia）, but they did not impose the discontinuation of treatment. CONCLUSION： After 12 wk of treatment with Peg-Interferon alpha 2a （40 ku） in patients on chronic haemodialysis with chronic C hepatitis, EVR was obtained in 87.5% （7/8） of the cases. SVR was achieved in 50% of the cases （3/6 patients） that finished the 48 wk of treatment.

AIM： To explore portal hypertension and portosystemic shunts and to stage chronic liver disease （CLd） based on the pathophysiology of portal hemodynamics. METHOdS： Per-rectal portal scintigraphy （PRPS） was performed on 312 patients with CLd and liver angioscintigraphy （LAS） on 231 of them. The control group included 25 healthy subjects. We developed a new model of PRPS interpretation by introducing two new parameters, the liver transit time （LTT） and the circu-lation time between right heart and liver （RHLT）. LTT for each lobe was used to evaluate the early portal hypertension. RHLT is useful in cirrhosis to detect liver areas missing portal inflow. We calculated the classical per-rectal portal shunt index （PRSI） at PRPS and the hepatic perfusion index （HPI） at LAS. RESULTS： The normal LTT value was 24 ± 1 s. Abnormal LTT had PPV = 100% for CLd. Twenty-seven noncirrhotic patients had LTT increased up to 35 s （median 27 s）. RHLT （42 ± 1 s） was not related to liver disease. Cirrhosis could be excluded in all patients with PRSI 〈 5% （P 〈 0.01）. PRSI 〉 30% had PPV = 100% for cirrhosis. Based on PRPS and LAS we propose the classification of CLd in 5 hemodynamic stages. Stage 0 is normal （LTT = 24 s, PRSI 〈 5%）. In stage 1, LTT is increased, while PRSI remains normal. In stage 2, LTT is decreased between 16 s and 23 s, whereas PRSI is increased between 5% and 10%. In stage 3, PRSI is increased to 10%-30%, and LTT becomes undetectable by PRPS due to the portosystemic shunts. Stage 4 includes the patients with PRSI 〉 30%. RHLT and HPI were used to subtype stage 4. In our study stage 0 had NPV = 100% for CLd, stage 1 had PPV = 100% for non-cirrhotic CLd, stages 2 and 3 represented the transition from chronic hepatitis to cirrhosis, stage 4 had PPV = 100% for cirrhosis. CONCLUSION： LTT allows the detection of early portal hypertension and of opening of transhepatic shunts. PRSI is useful in CLd with extrahepatic portosystemic shunts.
