Safety and efficacy of tenofovir in chronic hepatitis B-related decompensated cirrhosis

AIM To evaluate the safety and efficacy of tenofovir disoproxil fumarate(TDF) as a first-line therapy in decompensated liver disease. METHODS We enrolled 174 chronic hepatitis B-related liver cirrhosis patients treated with 300 mg/d TDF at six Korean centers. Of the 174 cirrhosis patients, 57 were assigned to the decompensated cirrhosis group and 117 were assigned to the compensated cirrhosis group. We followed the patients for 12 mo and evaluated clinical outcomes, including biochemical, virological, and serological responses. We also evaluated changes in hepatic and renal function and compared the decompensated and compensated cirrhosis groups. RESULTS The 1-year complete virological response(CVR) and Hepatitis B e antigen(HBe Ag) seroconversion were seen in 70.2% and 14.2% in the decompensated cirrhosis group, respectively. The rates of HBe Ag seroconversion/loss and ALT normalization at month 12 were similar in both groups. TDF treatment was also effective for decreasing the level of hepatitis B virus(HBV) DNA in both groups, but CVR was higher in the compensated group(88.9% vs 70.2%, P = 0.005). Tenofovir treatment for 12 mo resulted in improved Child-Turcotte-Pugh(CTP) and model for end-stage liver disease(MELD) scores in decompensated group(P < 0.001). Of the 57 decompensated patients, 39(68.4%) achieved CTP class A and 27(49.1%) showed improvement in the CTP score of 2 points after 12 mo of TDF. The observed rate of confirmed 0.5 mg/d L increases in serum levels of creatinine in the decompensated and compensated cirrhosis group were 7.0% and 2.5%, respectively(P < 1.000).CONCLUSION TDF therapy in decompensated cirrhosis patients was effective for decreasing HBV DNA levels and improving hepatic function with relatively lower CVR than in compensated cirrhosis. Thus, physicians should carefully monitor not only renal function but also treatment responses when using TDF in decompensated cirrhosis patients.

High-purity Nd metal was recovered from waste Nd–Fe–B magnet by the molten salt electrowinning process with chemical pretreatment. X-ray diffraction（XRD）, scanning electron microscopy（SEM）, energy dispersive spectroscopy（EDS）, cyclic voltammetry（CV）,chronopotentiometry（CP）, and inductively coupled plasma-atomic emission spectrometer（ICP/AES） were used to characterize the deposit and electrochemical behaviors. The results show that NdF_3 is effectively synthesized from the Nd–Fe–B magnet using HCl solution and NH4F. During the chemical treatment of the waste magnet,iron impurity is eliminated as a soluble [NH4]3[FeF6]complex. Electrowinning using NdF_3 in LiF molten salt shows that Nd metal is deposited from the electrolyte onthe cathode at the reduction potentials ranging from-1.48to-1.35 V（vs. W） with the concentration change of NdF_*** final purity of Nd metal deposit is higher than99.78 %.

Recently, the National Typhoon Center （NTC） at the Korea Meteorological Administration launched a track-pattern-based model that predicts the horizontal distribution of tropical cyclone （TC） track density from June to October. This model is the first approach to target seasonal TC track clusters covering the entire western North Pacific （WNP） basin, and may represent a milestone for seasonal TC forecasting, using a simple statistical method that can be applied at weather operation centers. In this note, we describe the procedure of the track-pattern-based model with brief technical background to provide practical information on the use and operation of the model. The model comprises three major steps. First, long-term data of WNP TC tracks reveal seven climatological track clusters. Second, the TC counts for each cluster are predicted using a hybrid statistical-dynamical method, using the seasonal prediction of large-scale environments. Third, the final forecast map of track density is constructed by merging the spatial probabilities of the seven clusters and applying necessary bias corrections. Although the model is developed to issue the seasonal forecast in mid-May, it can be applied to alternative dates and target seasons following the procedure described in this note. Work continues on establishing an automatic system for this model at the NTC.

Reactive oxygen species(ROS)are byproducts of cellular metabolism;they play a significant role as secondary messengers in cell *** cells,high concentrations of ROS induce apoptosis,senescence,and contact inhibition,while low concentrations of ROS result in angiogenesis,proliferation,and cytoskeleton ***,controlling ROS generation is an important factor in cell *** designed a chlorin e6(Ce6)-immobilized polyethylene terephthalate(PET)film(Ce6-PET)to produce extracellular ROS under red-light *** application of Ce6-PET films can regulate the generation of ROS by altering the intensity of light-emitting diode *** confirmed that the Ce6-PET film could effectively promote cell growth under irradiation at 500 μW/cm^(2) for 30 min in human umbilical vein endothelial *** also found that the Ce6-PET film is more efficient in generating ROS than a Ce6-incorporated polyurethane film under the same ***6-PET fabrication shows promise for improving the localized delivery of extracellular ROS and regulating ROS formation through the optimization of irradiation intensity.

Two-dimensional metal dichalcogenides have been evidenced as potential electrocatalysts for hydrogen evolution reaction(HER);however,their application is limited by a poor oxygen evolution reaction(OER)activity due to insufficient number/types of multi-integrated active *** this study,we report a novel bifunctional catalyst developed by simultaneous engineering of single nickel atoms(Ni_(SA)) and nickel phosphate clusters(Ni_(Pi)) to synergistically trigger surface-functionalized MoS_(2) nanosheets(NSs)resulting in high reactivities for both HER and *** Ni_(SA)-Ni_(Pi)/MoS_(2)NSs material exhibits a fairly Pt-like HER behavior with an overpotential of 94.0 mV and a small OER overpotential of 314.0 mV to reach 10 mA cm^(-2) in freshwater containing 1.0 M *** results of the catalyst are well supported by theoretical study,which reveals the significant modulation of electronic structure and enrichment of electroactive site number/types with their reasonably adjusted free adsorption *** evaluating practicability,the Ni_(SA)-Ni_(Pi)/MoS_(2)NSs-based electrolyzer delivers effective operation voltage of 1.62,1.52,and 1.66 V at 10 mA cm^(-2) and superior long-term stability as compared to Pt/C//RuO_(2) system in freshwater,mimic seawater,and natural seawater,*** present study indicates that the catalyst is a promising candidate for the practical production of green hydrogen via water electrolysis.

Topology optimization of reactive material structures for penetrative projectiles

Recently,reactive materials have been developed for penetrative projectiles to improve impact resistance and energy ***,the design of a reactive material structure,involving shape and size,is challenging because of difficulties such as high non-linearity of impact resistance,manufacturing limitations of reactive materials and high expenses of penetration *** this study,a design optimization methodology for the reactive material structure is developed based on the finite element analysis.A finite element model for penetration analysis is introduced to save the expenses of the *** resistance is assessed through the analysis,and result is calibrated by comparing with experimental *** on the model,topology optimization is introduced to determine shape of the *** design variables and constraints of the optimization are proposed considering the manufacturing limitations,and the optimal shape that can be manufactured by cold spraying is *** on the optimal shape,size optimization is introduced to determine the geometric dimensions of the *** a result,optimal design of the reactive material structure and steel case of the penetrative projectile,which maximizes the impact resistance,is *** the design process proposed in this study,reactive material structures can be designed considering not only mechanical performances but also manufacturing limitations,with reasonable time and cost.
