Alzheimer's disease(AD)is the most serious age-related neurodegenerative disease and causes destructive and irreversible cognitive *** in the development of therapeutics targeting amyloid-β(Aβ)and tau;principal proteins inducing pathology in AD,suggest a paradigm shift towards the development of new therapeutic *** gram-negative bacteria and lipopolysaccharides(LPS)are attractive new targets for AD ***,an altered distribution of gram-negative bacteria and their LPS has been reported in AD ***,gram-negative bacteria and their LPS have been shown to affect a variety of AD-related pathologies,such as Aβ homeostasis,tau pathology,neuroinflammation,and ***,therapeutic approaches targeting gram-negative bacteria or gram-negative bacterial molecules have significantly alleviated AD-related pathology and cognitive *** multiple evidence showing that the gram-negative bacteria and their LPS play a crucial role in AD pathogenesis,the pathogenic mechanisms of gram-negative bacteria and their LPS have not been ***,we summarize the roles and pathomechanisms of gram-negative bacteria and LPS in ***,we discuss the possibility of using gram-negative bacteria and gram-negative bacterial molecules as novel therapeutic targets and new pathological characteristics for AD.

The effect of various metal ions on the DNA mediated energy transfer between simultaneously bound drugs was investigated using spectroscopic *** was found that addition of divalent metal ions(Mg^(2+),Ca^(2+),Mn^(2+),Co^(2+)and Ni^(2+))resulted in further decrease of the ethidium fluorescence intensity,while a small increase was observed in the TMPyP emission band,implying that the energy of excited ethidium was transferred to *** DNA-mediated quenching efficiency between ethidium and TMPyP was significantly enhanced by the presence of all metal *** the divalent metal ions,alkali earth metal ions and Mn^(2+)displayed higher quenching efficiencies than other transition metal *** distances required to permit the energy transfer between the two drugs in DNA were calculated as 68,66,62,48and 38in the presence of100μmol·L^(-1 )of Mg^(2+),Ca^(2+),Mn^(2+),Co^(2+)and Ni ^(2+)ion,*** disturbed binding conformation of TMPyP in DNA by metal ions presumably accounts for the difference.

AIM: To investigate whether, or how, DA-9601, which is a new gastroprotective agent, inhibits TNF-α-induced inflammatory signals in gastric epithelial AGS cells. METHODS: Cell viability was determined by MTT assay. IL-8 and CCL20 promoter activities were determined by a luciferease reporter gene assay. NF-κB-dependent transcriptional activity was determined by I-κBαdegradation, NF-κB p65 nuclear translocation and a luciferase activity assay. IL-8 and CCL20 gene expression and protein secretion were determined by RT-PCR and an enzymelinked immunosorbent assay (ELISA). Total and phos-phorylated forms of mitogen-activated protein kinases (MAPKs) were determined by Western blot. RESULTS: Treatment of AGS cells with DA-9601 reduced TNF-α-induced IL-8 and CCL20 promoter activities, as well as their gene expression and protein release. TNF-αalso induced NF-κB-dependent transcriptional activity in AGS cells. In contrast, in cells treated with DA-9601, TNF-α-induced NF-κB activity was significantly blocked. Although all three MAP kinase family members were phosphorylated in response to TNF-α, a selective inhibitor of p38 kinase SB203580 only could inhibit both NF-κB-dependent transcriptional activity and IL-8 and CCL20 production, suggesting a potential link between p38 kinase and NF-κB-dependent pathways in AGS cells. Interestingly, DA-9601 also selectively inhibited p38 kinase phosphorylation induced by TNF-α. CONCLUSION: DA-9601 blocked TNF-α-mediated inflammatory signals by potentially modulating the p38 kinase pathway and/or a signal leading to NF-κBdependent pathways in gastric epithelial cells.

Immediate balloon deflation method in endoscopic papillary large balloon dilation for extraction of difficult bile duct stones

Background/Aims: Recently, endoscopic papillary large balloon dilation (EPLBD) using a large balloon (12 - 20 mm) for extraction of difficult common bile duct (CBD) stones has been widely accepted with favorable outcomes. However, there is no consensus with regard to the ballooning time. The aim of our study was to evaluate the efficacy and safety of immediate balloon deflation in EPLBD for the treatment of difficult bile duct stone. Methods: This was a retrospective study of 80 consecutive patients with bile duct stones who were treated with an immediate balloon deflation method in EPLBD combined with endoscopic sphincterotomy (EST) between January 2010 and December 2012. Overall success rate, success rate at first ERCP, and the frequency of mechanical lithotripsy for complete stone removal were assessed for efficacy and safety was evaluated by assessing major complications. Results: Overall success rate for complete stone removal was high (78/80, 97.5%) and success rate for complete stone removal at first ERCP was 86.3% (69/80). The use of mechanical lithotripsy was 0% (0/80). The overall complication rate was favorable (5/80, 6.3%). PostERCP pancreatitis was observed in 3 patients (two: mild, one: moderate). In subgroup analysis, the presence of periampullary diverticulum was the only factor affecting the success rate at first ERCP. Conclusion: This study demonstrated the favorable outcome of immediate balloon deflation for treatment of difficult CBD stones and can be considered for clinical application.

AIM To investigate the temporal trends in the misdiagnosis rate between Crohn's disease(CD) and intestinal tuberculosis(ITB) in South Korea. METHODS We retrospectively reviewed the medical records of patients managed for CD or ITB at Asan Medical Center, a tertiary referral hospital, Seoul, Korea between 1996 and 2014. The temporal trends in the misdiagnosis rates between the two diseases were analyzed. The demographic and clinical characteristics were compared between CD patients who were initially misdiagnosed as ITB(final CD group) and vice versa(final ITB group). Final diagnostic criteria for ITB and medication for CD before definite diagnosis of TB were also analyzed in final ITB *** In total, 2760 patients were managed for CD and 772 patients for ITB between 1996 and 2014. As well, 494 of the 2760 CD patients(17.9%) were initially misdiagnosed as ITB and 83 of the 772 ITB patients(10.8%) as CD. The temporal trend in misdiagnosing CD as ITB showed a decrease(OR = 0.89, 95%CI: 0.87-0.91, P < 0.001), whereas the temporal trend in misdiagnosing ITB as CD showed an increase(OR = 1.06, 95%CI: 1.01-1.11, P = 0.013). Age at diagnosis, presenting symptoms, and proportion of patients with active/past perianal fistula and active/inactive pulmonary tuberculosis(TB) were significantly different between final CD group and final ITB group. Forty patients(48.2%) in final ITB group were diagnosed by favorable response to empirical anti-TB treatment. Seventeen patients(20.5%) in final ITB group had inappropriately received corticosteroids and/or thiopurines due to misdiagnosis as CD. However, there were no mortalities in both groups. CONCLUSION Cases of CD misdiagnosed as ITB have been decreasing, whereas cases of ITB misdiagnosed as CD have been increasing over the past two decades.

Successful extracorporeal life support in sudden cardiac arrest due to coronary anomaly

Extracorporeal life support(ECLS) has recently been reported to have a survival benefit in patients with cardiac arrest. It is now used widely as a lifesaving modality. Here, we describe a case of sudden cardiac arrest(SCA) in a young athlete with an anomalous origin of the right coronary artery from the left coronary sinus. Resuscitation was successful using ECLS before curative bypass surgery. We highlight the efficacy of ECLS for a patient with SCA caused by a rare, unexpected aetiology. In conclusion, ECLS was a lifesaving modality for SCA due to an anomalous coronary artery in this young patient.

Cimicifuga heracleifolia Komarov(CH) is used traditionally as a herbal remedy for many ***,published literature suggests that large doses of cimicifuga racemosa might be associated with ***,its long-term toxicity was not evaluated *** study was performed to evaluate 14-day repeated dose toxicity(RDT) and genotoxicity of CH extract according to the USNTP protocol and GLP guideline. To determine the appropriate dosage of CH extract administration,14-days RDT study were performed.14-days repeated dose study was performed to dosage levels of 0.125,0.25,0.5,1 and 2g/kg/day in F344/N *** test substance-related effect were observed in some criteria included mortality;clinical signs;body weight changes;urinalysis,organ weight,gross findings and histopathological findings. In reverse mutation assay of CH extract using Salmonella typhimurium TA98,TA100,TA102,TA1535 and TA1537,test substance was diluted to 5 dosage levels of 312.5,625,1,250,2,500 and 5,000μg/plate with D.D.W..Each tester strain was exposed to the test item with and without metabolic activation *** extract might not have reverse mutagenicity under the condition of this study. Chromosome aberration test of CH extract was performed to evaluate its clastogenic potential in Chinese Hamster Lung fibroblast(CHL) cell *** test substance was diluted to 3 dosage levels of 2,500,1,250,625fig/*** were exposed to test items for 6 or 24 hours in the absence of metabolic activation system,or for 6 hours in the presence of metabolic activation *** was no statistically significant increase in the number of structural or numeric aberrant metaphases at any dose both in the presence and absence of metabolic activation.
