Incremental value of preprocedural coronary computed tomographic angiography to classical coronary angiography for prediction of PCI complexity in left main stenosis

Introduction: the aim of our study was to assess the incremental value of Coronary Computed Tomography Angiography (CCTA) added to classical coronary angiography, for complex characterization of coronary lesions and prediction of procedural complexity in patients with significant left main (LM) stenoses. Material and Methods: thirty-six patients with LM disease were enrolled in the study, and each subject underwent CCTA followed by coronary angiography and percutaneous revascularization. Results: Logistic regression analysis indicated a good correlation between the angiographic-calculated and the CCTA-derived Syntax scores for the whole group (r = 0.87, p the high risk subgroup (r = 0.86, p the low and intermediate risk (r = 0.38, p = 0.21 and r = 0.62, p = 0.07 respectively). In cases which required complex PCI procedures, both angiographic and CCTA Syntax score were significantly higher than those who did not require complex revascularization procedures (24.5 +/-11.5 vs 32.2 +/-14.6, p = 0.09 for Angio Syntax, 35.3 +/-11.5 vs 25.2 +/-11.3, p = 0.01 for CCTA). In the same time, Ca scoring was significantly higher and plaque volumes were significantly larger in cases requiring complex revascularization procedures (299.5 +/-359.6 vs 917.3 +/-495.4, p = 0.04 for calcium score, 79.7 +/-28.5 vs 108.7 +/-25.3 mm3, p = 0.002 for plaque volumes). Multivariate analysis identified the following CCTA parameters as significant predictors of increased risk for complex intervention in LM lesions: plaque volume (OR 8.00, p = 0.008), Ca scoring (OR 6.37, p = 0.02) and CCTA Syntax score (OR 6.87, p = 0.01). Conclusions: CCTA derived parameters provide incremental information to classical coronary angiography for preoperative assessment of lesion severity in complex left main stenosis. 目的通过在倾斜床和 Valsalva 试验(VM)中连续监测双侧大脑中动脉(MCA)的平均血流速度、平均动脉血压、潮气末 CO_2等多种指标,分析帕金森病(Parkinson's disease,PD)的静态和动态脑自动调节功能的临床特征。方法同时连续监测17例 PD 患者和8名年龄、性别相匹配的健康对照者两侧 MCA 平均血流速度、平均动脉血压、校正到脑 Willis 环水平的脑平均动脉压、心率、潮气末 CO_2。首先进行倾斜床试验(基线平卧位,向上倾斜10°、30°、70°和恢复平卧位),然后进行 VM。计算所有相位动脉血压和左、右 MCA 的搏动指数(PI_(BP)、PI_L、PI_R)以及 PI_L/PI_(BP),PI_R/PI_(BP)。在倾斜床试验过程中计算脑血管阻力和静态脑自动调节指数,VM 则计算系列脑自动调节指数和相关指标的相位及波幅变化。结果倾斜床试验显示向上倾斜70°时,对照和 PD 组仅脑平均动脉压(mm Hg,1 mm Hg=0.133kPa)在明显比基线状态降低(59.9±10.2、77.1±12.3,P=0.03;61.8±15.1、79.2±11.8,P=0.02),但两组间差异无统计学意义(P>0.05),PD 组平均血流速度(cm/s)在所有相位[(34.1±7.5)～(44.1±13.8)]均明显低于对照组(61.4±15.9～65.4±19.2,p0.05);PI_(BP)在相位Ⅰ～Ⅲ[(0.6±0.1)～(0.5±0.2)]明显低于基线值[(0.7±0.1),P<0.05],但 PI_L/PI_(BP)和 PI_R/PI_(BP)从相位Ⅱa～Ⅲ[(2.1±1.0)～(2.9±1.5)]明显高于基线[(1.5±0.5),P<0.05]。结论 PD 患者处于高脑血管阻力和低平均血流速度的基础状态,倾斜床试验提示他们因脑远端阻力血管的舒张功能障碍,而显示一种特殊类型的静态脑自动调节障碍。VM 过程中动态脑自动调节反应显示特征性的时相延迟,PD 可使时相进一步延迟,而且脑自动调节反应的临床阈值升高,强度明显减弱,PI_L/PI_(BP)和 PI_R/PI_(BP)是反映 PD 动态脑自动调节功能的较好指标。

＜正＞ A phenomenological kinetic model has been developed for the prediction of non-isothermal reversible incomplete transformations in low-carbon hypoeutectoid steels. the theoretical basis of the proposed method has its origin in a possible extension of the traditional Austin-Rickett kinetic differential equation. To critically assess the applicability of the model, a number of experiments based on computer simulations have been performed to predict the austenite/ferrite proeutectoid transformation in the temperature range of Ae1to Ae3 on plain carbon hypoeutectoid steels. A comparison with published experimental data has verified that the model developed is reasonable both quantitatively and with respect to well-established trends. Extension of the method of prediction appears promising when the non-isothermal reactions occurring during heat treatment （such as carbide precipitation and dissolution） can be assumed to proceed in a reversible manner.

A nuclear import inhibitory peptide ameliorates the severity of cholecystokinin-induced acute pancreatitis

AIM: To assess the effect of our novel cell-permeable nuclear factor-kappaB (NF-κB) inhibitor peptide PN50 in an experimental model of acute pancreatitis. PN50 was produced by conjugating the cell-penetrating penetratin peptide with the nuclear localization signal of the NF-κB p50 ***: Pancreatitis was induced in male Wistar rats by administering 2×100 μg/kg body weight of cholecystokininoctapeptide (CCK) intraperitoneally (IP) at an interval of 1 h. PN50-treated animals received 1 mg/kg of PN50 IP 30 min before or after the CCK injections. the animals were sacrificed 4 h after the first injection of ***: All the examined laboratory (the pancreatic weight/body weight ratio, serum amylase activity,pancreatic levels of TNF-α and IL-6, degree of lipid peroxidation, reduced glutathione levels, NF-κB binding activity, pancreatic and lung myeloperoxidase activity) and morphological parameters of the disease were improved before and after treatment with the PN50 *** to the histological findings, PN50 protected the animals against acute pancreatitis by favoring the induction of apoptotic, as opposed to necrotic acinar cell death associated with severe acute ***: Our study implies that reversible inhibitors of stress-responsive transcription factors like NF-κB might be clinically useful for the suppression of the severity of acute pancreatitis.

Meticulous mapping of the lymph node status is a general principle in present-day head and neck surgery. the removal of a certain number of lymphatic levels during neck dissection may well be therapeutic in intent, but it is also mandatory for correct tumour staging. We pre- sent a precise lymph node mapping during dif- ferent types of neck dissection in the course of major head and neck surgery by a sterile plastic tray moulded in the shape of the neck. this de- vice makes lymph node mapping simpler, safer, quicker and methodically more structured than any of the present methods. It facilitates the work of the pathologist and the flow of reliable information along the surgeon-pathologist- oncologist chain. With this device, a more stru- ctured, methodical means of lymph node removal has become possible.

全球海捕对虾产量从1985年开始停滞不前,一直徘徊在190万t左右,然而这一期间的对虾养殖却取得了前所未有的成功,续续扩大着世界对虾市场的供给。养殖对虾产量从1982年的10万t上升到1992年的70万t,在水产养殖历史上写下了最成功的一页。80年代中期对虾养殖产量的指数增长主要得益于几项养殖技术的突破性进展(主要是苗种和饵料),但是经济和生态方面的限制接踵而至,使得增长速度在80年代末开始变慢。由于加工业和市场方面的成就使养殖对虾产量仍以稍慢的速度在增长,然而现在对虾养殖已发展到了一个关键时期。尽管对虾养殖业有捕捞业所不可企及的几项优点,但它的进一步发展还有赖于养殖技术的改进,管理水平的提高以及产品和市场的多元化发展。过去10年的经验累积已提供了足够的依据来纠正前一时期因盲目快速发展而造成的失误。
