BACKGROUND An ectopic hepatocellular carcinoma(EHCC)arises from the ectopic liver which is defined as a hepatic organ or tissue not connected to surrounding *** is a rare disease and it is difficult to diagnose ***,the clinical features are not fully *** SUMMARY A retroperitoneal tumor(6 cm)was located at the dorsal side of the pancreas head on abdominal ultrasonography in an 81-year old woman positive for hepatitis C virus *** enhanced-computed tomography and gadolinium-ethoxybenzyl-diethylenetriamine pentaacetic acid-enhanced magnetic resonance imaging showed viable HCC patterns with early enhancement and delayed *** tumor markers-serum alphafetoprotein and alpha-fetoprotein-L3%-were increased to 30.1 ng/m L and83.1%,*** induced by vitamin K absence or antagonist-Ⅱwas within normal levels(17 m AU/m L).Positron emission tomography-computed tomography showed strong accumulation into the tumor(Standardized Uptake Value max:13.8),and the tumor cytology following endoscopic ultrasoundguided fine needle aspiration showed poorly differentiated *** extirpation was performed,and operative findings showed that the retroperitoneal tumor was disconnected from the pancreas and the *** lymph nodes near the tumor were histologically *** histological examination,the tumor was finally diagnosed as EHCC with Arginase-1 positive *** We report our experience of a rare EHCC which was difficult to diagnose,and we present a review of the literature.

Surgical resection for colorectal liver metastases(CRLM)may offer the best opportunity to improve ***,only about 20% of CRLM cases are indicated for resection at the time of diagnosis(initially resectable),and the remaining cases are treated as unresectable(initially unresectable).Thanks to recent remarkable developments in chemotherapy,interventional radiology,and surgical techniques,the resectability of CRLM is ***,some metastases are technically resectable but oncologically questionable for upfront *** pancreatic cancer,such cases are categorized as"borderline resectable",and their definition and treatment strategies are ***,in CRLM,although various poor prognosis factors have been identified in previous reports,no clear definition or treatment strategy for borderline resectable has yet been *** the efficacy of hepatectomy for CRLM was reported in the 1970 s,multidisciplinary treatment for unresectable cases has improved resectability and prognosis,and clarifying the definition and treatment strategy of borderline resectable CRLM should yield further improvement in *** review outlines the present status and the future perspective for borderline resectable CRLM,based on previous studies.

Assessment for the minimal invasiveness of laparoscopic liver resection by interleukin-6 and thrombospondin-1

BACKGROUND Laparoscopic surgery has been introduced as a minimally invasive technique for the treatment of various ***,there are few reports that have scientifically investigated the minimally invasive nature of laparoscopic liver resection(LLR).AIM To investigate whether LLR is scientifically less invasive than open liver *** During December 2011 to April 2015,blood samples were obtained from 30 patients who treated with laparoscopic(n=10,33%)or open(n=20,67%)partial liver resection for liver *** levels of serum interleukin-6(IL-6)and plasma thrombospondin-1(TSP-1)were measured using ELISA kit at four time points including preoperative,immediate after operation,postoperative day 1(POD1)and ***,we investigated the impact of the operative approaches during partial hepatectomy on the clinical time course including IL-6 and *** Serum level of IL-6 on POD1 in laparoscopic hepatectomy was significantly lower than those in open hepatectomy(8.7 vs 30.3 pg/mL,respectively)(P=0.003).Plasma level of TSP-1 on POD3 in laparoscopic hepatectomy was significantly higher than those in open hepatectomy(1704.0 vs 548.3 ng/mL,respectively)(P=0.009),and have already recovered to preoperative level in laparoscopic *** patients with higher IL-6 Levels on POD1,plasma level of TSP-1 on POD3 was significantly lower than those in patients with lower IL-6 Levels on *** analysis showed that open approach was the only independent factor related to higher level of IL-6 on POD1[odds ratio(OR),7.48;95%confidence interval(CI):1.28-63.3;P=0.02].Furthermore,the higher level of serum IL-6 on POD1 was significantly associated with lower level of plasm TSP-1 on POD3(OR,5.32;95%CI:1.08-32.2;P=0.04)in multivariate *** In partial hepatectomy,laparoscopic approach might be minimally invasive surgery with less IL-6 production compared to open approach.

Hepatic resection (HR) and radiofrequency ablation (RFA) are popular local therapies for early-stage hepatocellular carcinoma (HCC).Alpha-fetoprotein,Lens culinaris agglutininreactive fraction of alpha-fetoprotein,and des-c-carboxy prothrombin are well-known and useful tumor markers for *** positive number status of these tumor markers has recently been demonstrated as beneficial for predicting outcome for HCC patients treated with local *** the normal ranges reported have differed by institution,the positivity of tumor markers is consistent and can easily be *** and Wakayama's group clearly demonstrated the following:1) Regardless of the degree of tumor stage,a triple-positive tumor marker profile can predict poor outcome in HCC patients undergoing HR;2) For RFA alone,HCC patients with double-and triple-positive status,having less than three lesions and lesions ≤3 cm in diameter show comparably insufficient outcomes;3) For HCC patients with lesions ≤5 cm in Child-Pugh grade A,HR is preferred over RFA;4) Microvascular invasion rates increased even in the double-positive patients,while poorly differentiated HCC was frequently observed only in the triple-positive patients;and 5) RFA with chemoembolization,anatomical liver resection,and postoperative adjuvant chemoembolization or hepatic arterial chemotherapy might improve the outcome for patients with highly malignant HCC with multiple positive tumor ***,the impacts of these therapies still need to be evaluated in prospective comparative studies.

Cytosolic chaperonin CCT (also known as TRiC) is a hetero-oligomeric cage-like molecular chaperone that assists in protein folding by ATPase cycle-dependent conformational changes. However, role of the nucleo-tide binding and hydrolysis in CCT-assisted protein folding is still poorly understood. We purified CCT by using ATP-Sepharose and other columns, and found that CCT possesses ability to hydrolyze GTP, with an activity level very similar to the ATPase activity. CCT was more resistant to proteinase K treatment in the presence of GTP or ATP. These results suggest that the GTPase activity of CCT may play a role in chaperone-assisted protein folding.

Oxygen uptake(V O_(2))was measured during a non-exhaustive high-intensity intermittent cross-exercise(HIICE)protocol consisting of four alternating bouts of 20 s running(R)and three bouts of bicycle exercise(BE)at~160%and~170%maximal oxygen uptake(V O_(2)max),respectively,with 10 s between-bout rests(sequence R-BE-R-BER-BE-R).The V O_(2) during the last BE([52.2±5.0]mL⋅kg^(-1)⋅min^(-1))was significantly higher than the V O_(2)max of the BE([48.0±5.4]mL⋅kg^(-1)⋅min^(-1),n=30)and similar to that of *** clarifying the underlying mechanisms,a corresponding HIICE-protocol with BE and arm cranking ergometer exercise(AC)was used(sequence AC-AC-BE-AC-BE-AC-AC-BE).In some experiments,thigh blood flow was occluded by a cuff around the upper *** occlusion,the V O_(2) during the AC([39.2±7.1]mL⋅kg^(-1)⋅min^(-1)[6th bout])was significantly higher than the V O_(2)max of AC([30.2±4.4]mL⋅kg^(-1)⋅min^(-1),n=7).With occlusion,the corresponding V O_(2)([29.8±3.9]mL⋅kg^(-1)⋅min^(-1))was reduced to that of the V O_(2)max of AC and significantly less than the V O_(2) without *** findings suggest that during the last bouts of HIICE may exceed the of the specific exercise,probably because it is a summation of the V O_(2) for the ongoing exercise plus excess post-oxygen consumption(EPOC)produced by the previous exercise with a higher V O_(2)max.
