Usefulness of a novel slim type Flush Knife-BT over conventional Flush Knife-BT in esophageal endoscopic submucosal dissection

To investigated the usefulness of a novel slim type ball-tipped FlushKnife (FlushKnife-BTS) over ball-tipped FlushKnife (FlushKnife-BT) in functional experiments and clinical *** order to evaluate the functionality of FlushKnife-BTS, water aspiration speed, resistance to knife insertion through the scope, and waterjet flushing speed were compared between FlushKnife-BTS and BT. In clinical practice, esophageal endoscopic submucosal dissection (ESD) performed using FlushKnife-BTS or BT by an experienced endoscopist between October 2015 and January 2016 were retrospectively reviewed. The treatment speed and frequency of removing and reinserting the knife to aspirate fluid and air during ESD sessions were *** experiments revealed that water aspiration speed by the endoscope equipped with a 2.8-mm working channel with FlushKnife-BTS was 7.7-fold faster than that with conventional FlushKnife-BT. Resistance to knife insertion inside the scope with a 2.8-mm working channel was reduced by 40% with FlushKnife-BTS. The waterjet flushing speed was faster with the use of FlushKnife-BT. In clinical practice, a comparison of 6 and 7 ESD using FlushKnife-BT and BTS, respectively, revealed that the median treatment speed was 25.5 mm2/min (range 19.6-30.3) in the BT group and 44.2 mm2/min (range 15.5-55.4) in the BTS group (P = 0.0633). However, the median treatment speed was significantly faster with FlushKnife-BTS when the resection size was larger than 1000 m2 (n = 4, median 24.2 mm2/min, range 19.6-27.7 vs n = 4, median 47.4 mm2/min, range 44.2-55.4, P = 0.0209). The frequency of knife replacement was less in the BTS group (median 1.76 times in one hour, range 0-5.45) than in the BT group (7.02 times in one hour, range 4.23-15) (P = 0.0065).CONCLUSIONOur results indicate that FlushKnife-BTS enhances the performance of ESD, particularly for large lesions, by improving air and fluid aspirat

AIM: To investigate the expressions of omithine decarboxylase (ODC), MMP-2, and Erk, and their relationship in human colon ***: ODC activity, MMP-2 expression, and mitogenactivated protein (MAP) kinase activity (Erk phosphorylation) were determined in 58 surgically removed human colon tumors and their adjacent normal tissues, using [1-14C]-ornithine as a substrate, ELISA assay, and Western blotting, ***: ODC activity, MMP-2 expression, and Erk phosphorylation were significantly elevated in colon tumors, compared to those in adjacent normal tissues. A significant correlation was observed between ODC activities and MMP-2 ***: This is the first report showing a significant correlation between ODC activities and MMP-2 levels in human colon tumors. As MMP-2 is involved in cancer invasion and metastasis, and colon cancer overexpresses ODC, suppression of ODC expression may be a rational approach to treat colon cancer which overexpresses ODC.

Primary cutaneous anaplastic large cell lymphoma(CALCL)is regarded as an indolent type of cutaneous T-cell ***,a few recent publications revealed that C-ALCL patients with initial leg involvement had significantly worse survival than those without initial leg ***,we report a case of C-ALCL with subsequent leg involvement,which led to death after chemoradiation therapy.A 75 years old Japanese man presented with multiple erythematous nodules in his left arm and the side of his left *** and immunohistochemical studies led to the diagnosis of primary *** the initial diagnosis,no leg lesion was *** year after the initial diagnosis,C-ALCL appeared in his right lower thigh and left *** therapy,low-dose etoposide and CHOP therapy were performed;however,the patient died of malignant lymphoma 4 years after the initial *** speculated that the occurrence of subsequent leg involvement may also be indicative of a worse prognosis,as in the case with initial leg involvement in ***,we propose that C-ALCL patients with initial or subsequentleg involvement should be classified as a distinct clinicopathological variant of C-ALCL("leg-type"involvement)and that they may require intense therapy.

Advanced therapies for patients with mild-to-severe ulcerative colitis(UC)may result in treatment *** examined whether the lymphocyte-to-monocyte ratio(L/M ratio)could predict the failure of advanced *** retrospective,observational,cohort study included 73 patients who were treated with advanced therapies at the hamamatsu University School of Medicine(Shizuoka,Japan)between February 2011 and November *** patients were divided into the nonfailure and failure groups,and their leukocyte counts and ratios before induction were *** and multivariate analyses were performed to identify the prognostic *** therapies failed within 3 months in 15(20.5%)*** the L/M ratio was significantly lower in the failure group than in the non-failure group(P=0.004).Receiveroperating characteristic(ROC)curve analysis revealed that an L/M ratio of ≤3.417 was predictive of treatment failure;the area under the curve(AUC)was 0.747(95%CI,0.620–0.874).Kaplan–Meier analysis revealed that the failure-free rate was significantly lower in the group with an L/M ratio of≤3.417 than in the group with an L/M ratio of>3.417(log-rank test P=0.002).Cox proportional hazard regression analysis identified an L/M ratio of≤3.417 as an independent risk factor for failure within 3 months after the induction of advanced ***,ROC analysis of patients who did not receive immunomodulators also revealed that the cut-off L/M ratio was 3.417 and the AUC was 0.796(95%CI,0.666–0.925).In patients receiving advanced therapies for active UC,the L/M ratio can predict treatment failure within 3 months.L/M ratios could facilitate the transition from advanced therapies to subsequent treatments.

The mass spectrometry of gold clusters stabilized by poly（N‐vinyl‐2‐pyrrolidone）（Au：PVP） re‐vealed the presence of Cl adsorbates derived from synthetic precursors, mainly on the Au34 and Au43 clusters. Changes in the amount of Cl adsorbates on the Au clusters did not affect the catalytic prop‐erties for the aerobic oxidation of benzyl alcohol, suggesting that the Cl atoms were only weakly bound to the Au clusters. In contrast, the replacement of Cl with Br on the Au34 and Au43 clusters significantly suppressed activity, without any influence on the electronic structure. This result indi‐cated that the Br atoms were strongly bound to the Au clusters and sterically blocked their active sites. The substantial reduction of the catalytic activity by the Br adsorbates suggested that the Au34 and Au43 clusters made a major contribution to the catalytic activity of the Au：PVP.

We encountered a 63-year-old male with a sarcomatoid carcinoma of the penis accompanied by metastasis to the bilateral inguinal lymph nodes and lungs. He noticed a penile mass, but neglected it. The mass rapidly increased in size, forming an ulcer, and began to disintegrate. He visited our hospital 4 months after noticing the mass. After cystostomy and blood transfusion, surgical resection was performed. A diagnosis of a squamous cell carcinoma with a sarcomatoid carcinoma of the penis was made. After the operation, best supportive care (BSC) was selected. We made efforts to maintain his quality of life (QOL), and he died 3 months after the operation.

The present study was performed to clarify the influence of having a firm purpose in life (PIL) on psychological and physical stresses. Participants were 59 subjects (19 males and 40 fe- males) with a mean age of 30.9 years (standard deviation, 12.1;range, 18 - 68 years). We used a mental arithmetic (MA) test as a goaloriented stressor;the test was supposed to be finished before a time limit expired without using pencil and paper. We used a PIL test to assess the intensity of personal meaning in one’s life, an interest in mental arithmetic test (IMAT) to assess the intensity of interest in the MA test, and an anxiety inventory (AI) to assess the anxiety level of before and during the MA test. Heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP) and nose tip temperature (NTT) were recorded before and after MA. We used logistic regression modeling to estimate the odds ratio (OR) of independent variables of age, sex, PIL test score, AI score, HR, SBP, DBP, and NTT before the MA test. Multivariate-adjusted models showed that PIL test scores prior to the MA test significantly predicted the observed changes in AI score and HR. Other variables were not significant predictors. In conclusion, we found that a firm PIL decreased anxiety under psychological stress as measured with the MA test, even with objective evaluation by others. having a firm PIL also lowered the response of the sympathetic nervous system to psych0logical stress.

A 59-year-old man, who was treated for schizophrenia and psoriasis vulgaris with risperidone and cyclosporine, presented with high fever and myalgia. Those did not respond to treatment for neuroleptic malignant syndrome for two weeks, and multiple organ dysfunction developed, so he was admitted to our hospital, but died two days later. Autopsy detected the hemophagocytosis, Epstein Barr Virus (EBV)-reactivated cells, and the absence of glomerulonephritis and interstitial tubulitis. We considered that hemophagocytic syndrome (HPS) and myalgia were caused by reactivated EBV and the viremia under immunosuppression, and renal failure was caused by sepsis-like state by cytokine storm of HPS.
