Survival benefit of concurrent chemoradiotherapy for advanced ampulla of Vater cancer

BACKGROUND Currently,there is no standard adjuvant therapy for patients with resected ampulla of Vater(AoV)cancer.AIM To evaluate the effectiveness of adjuvant concurrent chemoradiotherapy(CCRT)in patients with advanced AoV cancer who underwent curative resection.METHODS This single-centered,retrospective study included 29 patients with advanced AoV cancer who underwent pancreaticoduodenectomy between 2006 and 2018.The impact of CCRT on advanced AoV cancer was analyzed.RESULTS The 1-,3-,and 5-yr recurrence-free survival(RFS)rates for patients with advanced AoV cancer were 82.8%,48.3%,and 40.8%,respectively,and the overall survival(OS)rates were 89.7%,62.1%,and 51.7%,respectively.Lymphovas-cular invasion was found to be a significant risk factor for RFS and OS in patients with advanced AoV cancer in the univariate analysis,whereas T stage and lymph node metastasis were significantly associated with OS in the multivariate analysis.Compared to the patients who did not receive adjuvant CCRT,those who received adjuvant CCRT did not show statistically significant improvements in the RFS and OS,although they had a significantly lower average age and significantly higher platelet-to-lymphocyte ratio.CONCLUSION Adjuvant CCRT did not improve survival outcomes in patients with advanced AoV cancer.These findings contribute to existing knowledge on the effectiveness of CCRT in this patient population and provide important insights for clinical decision-making.

Callus proliferation-induced hypoxic microenvironment decreases shoot regeneration competence in Arabidopsis

Plants are aerobic organisms that rely on molecular oxygen for respiratory energy production.Hypoxic conditions,with oxygen levels ranging between 1%and 5%,usually limit aerobic respiration and affect plant growth and development.Here,we demonstrate that the hypoxic microenvironment induced by active cell proliferation during the two-step plant regeneration process intrinsically represses the regener-ation competence of the callus in Arabidopsis thaliana.We showed that hypoxia-repressed plant regener-ation is mediated by the RELATED TO APETALA2.12(RAP2.12)protein,a memberof the Ethylene Response Factor Vil(ERF-Vll)family.We found that the hypoxia-activated RAP2.12 protein promotes salicylic acid(SA)biosynthesis and defense responses,thereby inhibiting pluripotency acquisition and de novo shoot regeneration in calli.Molecular and genetic analyses revealed that RAP2.12 could bind directly to the SALICYLIC ACID INDUCTION DEFICIENT 2(SID2)gene promoter and activate SA biosynthesis,repressing plant regeneration possibly via a PLETHORA(PLT)-dependent pathway.Consistently,the rap2.12 mutant calli exhibits enhanced shoot regeneration,which is impaired by SA treatment.Taken together,these find-ings uncover that the cell proliferation-dependent hypoxic microenvironment reduces cellular pluripotency and plant regeneration through the RAP2.12-SID2 module.

Complete resection of large-cell neuroendocrine and hepatocellular carcinoma of the liver: A case report

BACKGROUND Combined tumors comprising large-cell neuroendocrine carcinoma and hepatocellular carcinoma have been rarely reported in the literature.CASE SUMMARY We report a case of a 73-year-old woman with chronic hepatitis B suspected to have a malignant hepatic mass(segment 3;size,4.5 cm)and lymph node metastasis based on computed tomography and magnetic resonance imaging.Despite being Child-Pugh class A,esophageal varices were present.She underwent left lateral sectionectomy and lymph node dissection.Pathological examination revealed a collision tumor consisting of large-cell neuroendocrine(90%)and hepatocellular(10%)carcinomas.The combined carcinoma had metastasized to one of the three lymph nodes excised.The patient recovered without any postoperative complications and was discharged in good condition on postoperative day 13.Adjuvant chemotherapy was not performed.No recurrence occurred during a follow-up period of 24 mo.CONCLUSION To improve the therapeutic management of combined tumors in the liver,it is necessary to discuss each clinical experience and consider an appropriate method for the preoperative diagnosis and treatment.

Introduction Antibiotic use for acute otitis media(AOM)is one of the major sources of antimicrobial resistance.However,the effective minimal antibiotic duration for AOM remains unclear.Moreover,guidelines often recommend broad ranges(5-10 days)of antibiotic use,yet the clinical impact of such a wide window has not been assessed.Methods We systematically searched PubMed/MEDLINE,Embase,Scopus,Web of Science,and Cochrane Library from database inception to 6 October 2021.Network meta-analysis was conducted on randomized controlled trials that assessed antibiotic treatment for AOM in children(PROSPERO CRD42020196107).Results For amoxicillin and amoxicillin-clavulanate,7-day regimens were noninferior to 10-day regimens in clinical responses[amoxicillin:risk ratio(RR)0.919(95%CI 0.820-1.031),amoxicillin-clavulanate:RR 1.108(0.957-1.282)],except for≤2 years.For the third-generation cephalosporins,7-day and 10-day regimens had similar clinical responses compared to placebo[7-day:RR 1.420(1.190-1.694),10-day:RR 1.238(1.125-1.362)compared to placebo].However,5-day regimens of amoxicillin-clavulanate and third-generation cephalosporins were inferior to 10-day regimens.Compared to amoxicillin,a shorter treatment duration was tolerable with amoxicillin-clavulanate.Conclusions Our findings indicated that 1O days of antibiotic use may be unnecessarily long,while the treatment duration should be longer than 5 days.Otherwise,5-day regimens would be sufficient for a modest treatment goal.Our findings revealed that the current wide range of recommended antibiotic durations may have influenced the clinical outcome of AOM,and a narrower antibiotic duration window should be re-established.

Successful resolution of gastric perforation caused by a severe complication of pancreatic walled-off necrosis:A case report

BACKGROUND Pancreatic walled-off necrosis(WON)rarely causes critical gastric necrosis and perforation,which may develop when pancreatic WON squashes against the stomach.The Atlanta 2012 guidelines were introduced for acute pancreatitis and its related clinical entities.However,there are few reported cases describing the clinical course and resolution of pancreatic WON.CASE SUMMARY We report the case of a 45-year-old man who presented to the urgent emergency department with gastric perforation caused by a severe complication of pancreatic WON on computed tomography.The patient underwent an emergency distal pancreatectomy,splenectomy,and gastric wedge resection.Postoperative findings showed re-perforation of the gastric wall at a previously resected margin.Furthermore,endoscopic examination revealed an ulcerative area with a defect in the fundus.After diagnostic endoscopy,endoscopic vacuum-assisted closure was performed,and continuous suction was transferred over all tissues in contact with the sponge surface.The patient recovered without any further complications and was discharged in good condition at postoperative week 8.No recurrence occurred during the 6-mo follow-up period.CONCLUSION When managing a patient with serious gastric perforation complicated by pancreatic WON,a multidisciplinary treatment approach should be considered.

Prostate cancer(PC)biomarker-citrate detection is clinically important to diagnose PC in early stages.Methylquinolinium iodide(Q)conjugated indole-phenylboronic acid(IB)was designed as a red-emissive QIB probe for the detection of citrate through Lewis acid-base reaction and intramolecular charge transfer(ICT)sensing mechanisms.Boronic acid acts as Lewis acid as well as citrate(Lewis base)recognition unit.The probe reacted with citrate,showing enhanced red emissions.Since the probe has excellent water solubility and great biocompatibility,practical application in biological systems is possible.Citrate was monitored precisely in the mitochondria organelle(in vitro)of living cells with a positive charge on QIB.Also,endogenous(in situ)citrate was detected quantitatively to discriminate non-cancerous and PC mice,observed strong and lower(negligible)emission intensity on non-cancerous and cancerous prostate tissues,respectively.Because,the concentration of citrate is higher in healthy prostate compared with PC prostate.Furthermore,the analysis of sliced prostate tissues can give PC-related information for clinical diagnosis to prevent and treat PC in the initial stages.Therefore,we believe that the present probe is a promising biochemical reagent in diagnosing PC.

Right trisegmentectomy with thoracoabdominal approach after transarterial embolization for giant hepatic hemangioma

Hepatic hemangiomas need to be treated surgically in cases where they are accompanied with symptoms, have a risk of rupture, or are hardly distinguishable from malignancy. The present authors conducted embolization of the right hepatic artery one day before an operation for a huge hemangioma accompanied with symptoms and confirmed a decrease in its size. The authors performed a right trisegmentectomy through a J-shape incision, using a thoracoabdominal approach, and safely removed a giant hemangioma of 32.0 cm × 26.5 cm × 8.0 cm in size and 2300 g in weight. Even for inexperienced surgeons, a J-shape incision with a thoracoabdominal approach is considered a safe and useful method when right-side hepatectomy is required for a large mass in the right liver.

MXenes: Versatile 2D materials with tailored surface chemistry and diverse applications

MXenes,the most recent addition to the 2D material family,have attracted significant attention owing to their distinctive characteristics,including high surface area,conductivity,surface characteristics,mechanical strength,etc.This review begins by presenting MXenes,providing insights into their structural characteristics,synthesis methods,and surface functional groups.The review covers a thorough analysis of MXene surface properties,including surface chemistry and termination group impacts.The properties of MXenes are influenced by their synthesis,which can be fluorine-based or fluorinedependent.Fluorine-based synthesis techniques involve etching with fluorine-based reagents,mainly including HF or LiF/HCl,while fluorine-free methods include electrochemical etching,chemical vapor deposition(CVD),alkaline etching,Lewis acid-based etching,etc.These techniques result in the emergence of functional groups such as-F,-O,-OH,-Cl,etc.on the MXenes surface,depending on the synthesis method used.Properties of MXenes,such as electrical conductivity,electronic properties,catalytic activity,magnetic properties,mechanical strength,and chemical and thermal stability,are examined,and the role of functional groups in determining these properties is explored.The review delves into the diverse applications of MXenes,encompassing supercapacitors,battery materials,hydrogen storage,fuel cells,electromagnetic interference(EMI) shielding,pollutant removal,water purification,flexible electronics,sensors,additive manufacturing,catalysis,biomedical and healthcare fields,etc.Finally,this article outlines the challenges and opportunities in the current and future development of MXenes research,addressing various aspects such as synthesis scalability,etching challenges,and multifunctionality,and exploring novel applications.The review concludes with future prospects and conclusions envisioning the impact of MXenes on future technologies and innovation.

Generating synthetic as-built additive manufacturing surface topography using progressive growing generative adversarial networks

Numerically generating synthetic surface topography that closely resembles the features and characteristics of experimental surface topography measurements reduces the need to perform these intricate and costly measurements.However,existing algorithms to numerically generated surface topography are not well-suited to create the specific characteristics and geometric features of as-built surfaces that result from laser powder bed fusion(LPBF),such as partially melted metal particles,porosity,laser scan lines,and balling.Thus,we present a method to generate synthetic as-built LPBF surface topography maps using a progressively growing generative adversarial network.We qualitatively and quantitatively demonstrate good agreement between synthetic and experimental as-built LPBF surface topography maps using areal and deterministic surface topography parameters,radially averaged power spectral density,and material ratio curves.The ability to accurately generate synthetic as-built LPBF surface topography maps reduces the experimental burden of performing a large number of surface topography measurements.Furthermore,it facilitates combining experimental measurements with synthetic surface topography maps to create large data-sets that facilitate,e.***.relating as-built surface topography to LPBF process parameters,or implementing digital surface twins to monitor complex end-use LPBF parts,amongst other applications.

Few-Shot Graph Classification with Structural-Enhanced Contrastive Learning for Graph Data Copyright Protection

Open-source licenses can promote the development of machine learning by allowing others to access,modify,and redistribute the training dataset.However,not all open-source licenses may be appropriate for data sharing,as some may not provide adequate protections for sensitive or personal information such as social network data.Additionally,some data may be subject to legal or regulatory restrictions that limit its sharing,regardless of the licensing model used.Hence,obtaining large amounts of labeled data can be difficult,time-consuming,or expensive in many real-world scenarios.Few-shot graph classification,as one application of meta-learning in supervised graph learning,aims to classify unseen graph types by only using a small amount of labeled data.However,the current graph neural network methods lack full usage of graph structures on molecular graphs and social network datasets.Since structural features are known to correlate with molecular properties in chemistry,structure information tends to be ignored with sufficient property information provided.Nevertheless,the common binary classification task of chemical compounds is unsuitable in the few-shot setting requiring novel labels.Hence,this paper focuses on the graph classification tasks of a social network,whose complex topology has an uncertain relationship with its nodes'attributes.With two multi-class graph datasets with large node-attribute dimensions constructed to facilitate the research,we propose a novel learning framework that integrates both meta-learning and contrastive learning to enhance the utilization of graph topological information.Extensive experiments demonstrate the competitive performance of our framework respective to other state-of-the-art methods.
