BACKGROUND： Thyroid hormones modulate proliferation of astrocytes and microglia depending on maturation stage and localization. Studies have demonstrated that triiodothyronine treatment or thyroidectomy during developmental stages results in morphological alterations and changes in the number of astrocytes and microglia. Little is known about the effects of hypothyroidism on astrocytes and microglia in adults. OBJECTIVE： To investigate the effects of hypothyroidism on morphology and number of astrocytes and microglia in the adult rat hippocampus. DESIGN, TIME AND SETTING： A randomized, controlled, neuroendocrinological, animal study was performed at the College of Medicine, Hallym University, South Korea between May 2008 and April 2009. MATERIALS： Methimazole, rabbit anti-glial fibrillary acidic protein （GFAP） antiserum, and rabbit anti-lba-1 antiserum were purchased from Sigma, USA. Rabbit anti-GFAP polyclonal antibody was provided by Chemicon, USA. Rabbit anti-lba-1 polyclonal antibody was purchased from Wako, Japan. Terminal deoxynucleotidyl transferase dUTP-biotin nick-end-labeling （TUNEL） kit was provided by Roche Molecular Biochemicals, Mannheim, Germany. METHODS： Hypothyroidism was induced in Wistar rats via methimazole administration （0.025%） in drinking water for 5 weeks, starting at 6 months of age. MAIN OUTCOME MEASURES： Following methimazole treatment, hippocampai neuronal death was determined using TUNEL staining. The morphology and number of GFAP and lba-1 immunoreactive cells were detected by immunohistochemistry. Hippocampal GFAP and lba-1 protein levels were detected by Western blot analysis. Serum-free triiodothyronine and thyroxine levels were quantified. RESULTS： TUNEL-positive neurons were not observed in the hippocampus of euthyroid and hypothyroid rats. Encoding-based brain-computer interface controlled by non-motor area of rat brain

As the needs of disabled patients are increasingly recognized in society, researchers have begun to use single neuron activity to construct brain-computer interfaces （BCI）, designed to facilitate the daily lives of individuals with physical disabilities. BCI systems typically allow users to control computer programs or external devices via signals produced in the motor or pre-motor areas of the brain, rather than producing actual motor movements. However, impairments in these brain areas can hinder the application of BCI. The current paper demonstrates the feasibility of a one-dimensional （ID） machine controlled by rat prefrontal cortex （PFC） neurons using an encoding method. In this novel system, rats are able to quench thirst by varying neuronal firing rate in the PFC to manipulate a water dish that can rotate in 1 D. The results revealed that control commands generated by an appropriate firing frequency in rat PFC exhibited performance improvements with practice, indicated by increasing water-drinking duration and frequency. These results demonstrated that it is possible for rats to understand an encoding-based BCI system and control a 1D machine using PFC activity to obtain reward.

Electrical Stimulation of Auricular Acupressure for Dry Eye:A Randomized Controlled-Clinical Trial

Objective： To evaluate the clinical efficacy of electrical stimulation（ES） of auricular acupressure on reducing the ocular symptoms and signs before and after treatment for dry eye. Methods： The inclusion criteria were the tear film break-up time（TFBUT） below 5 s and a Schirmer test-Ⅰ below 5 mm in dry eyes with ocular symptoms for at least 6 months. Subjects were randomized into a treatment group（50 cases） with continuous low frequency ES under auricular acupressure at acupoints and a no ES under auricular acupressure（no-ES, control group, 50 cases） on the same acupoints. Auricular acupressure were stimulated with ES at 4 master points of both ears, which were performed twice a week for 4 weeks at each point for 30 s. The ocular symptoms, the TFBUT, and Schirmer test-Ⅰ were evaluated before and after this procedure. Results： There were significantly better scores in TFBUT（P=0.032）, the Schirmer test-Ⅰ（P=0.044） and ocular symptoms（P=0.029） at 3 months post-treatment in the treatment group than in the control group. The total effective rate in the treatment group was accomplished in 41（82%） of the 50 cases of dry eye. Conclusions： Auricular acupressure with ES at auricular acupoint improves ocular symptoms and signs of dry eye for a period of at least 3 months.

Numerical characterization of heat transfer in closed-loop vertical ground heat exchanger

A series of numerical analyses has been performed on the characteristics of heat transfer in a closed-loop vertical ground heat exchanger(U-loop).A 2-D finite element analysis was conducted to evaluate the temperature distribution over the cross section of the U-loop system involving high-density polyethylene(HDPE) pipe/grout/soil to compare the sectional efficiency between the conventional U-loop and a new latticed HDPE pipe system,which is equipped with a thermally insulating lattice in order to reduce thermal interference between the inlet and outlet *** addition,a 3-D finite volume analysis(FLUENT) was adopted to simulate the operation of the closed-loop vertical ground heat exchanger with the consideration of the effect of a distance between the inlet and outlet pipes,grout's thermal properties,the effectiveness of the latticed HDPE pipe system,and the rate of circulation *** was observed that the thermal interference between the two strands of U-loop is of importance in enhancing efficiency of the ground heat ***,it is recommended to modify the configuration of the conventional U-loop system by equipping the thermally insulating lattice between the two pipe strands.

Clinical outcomes of small cell neuroendocrine carcinoma and adenocarcinoma of the gallbladder

AIM: To compare the demographics and survival rates between gallbladder adenocarcinoma(GB-adenocarcinoma) and small cell neuroendocrine carcinoma of the gallbladder(GB-NEC-SCC).METHODS: From March 2007 to September 2012,patients who underwent resection of tumor stage T2/T3 GB cancer were enrolled for this *** patients were included in this study,including 38 diagnosed with GB-adenocarcinoma and four diagnosed with *** the GB-adenocarcinoma group,a radical operation was performed in 28 patients,and ten patients underwent simple *** the GB-NEC-SCC group,a radical operation was performed in three patients,and one patient underwent simple *** analysis of the two groups was performed,including clinicopathologic features and survival ***: The median age of the patients was 68 y(range: 35-83 years) and females comprised 26/42 of the ***-adenocarcinoma patients were significantly older than GB-NEC-SCC patients(67.89 ± 11.15 vs 55.75 ± 10.31 years; P = 0.029).The median tumor size in GB-adenocarcinoma patients was 2.56 ± 1.75 cm and 3.98 ± 3.74 cm in GB-NEC-SCC patients; however,there was no significant difference between the two *** tumors > 2 cm,T stage(T2 vs T3),lymphovascular invasion,perineural invasion,lymph node metastasis and lymph node ratio showed no significant differences between the two *** overall survival rate of the 42 patients at five years was 77.0%.In the GB-adenocarcinoma group,the overall five-year survival rate was 74.8%,and survival in the GB-NEC-SCC group was 100%,which was not significantly different between the two ***: The strategy for treating patients with GB-NEC-SCC should be similar to that used for treating GB-adenocarcinoma,including radical cholecystectomy and liver resection.

Aqueous levels of erythropoietin in acute retinal vein occlusion with macular edema

AIM:To investigate the aqueous erythropoietin(EPO)levels and associated factors in patients with acute retinal vein occlusion(RVO).METHODS:The aqueous EPO level was measured in patients with macular edema(ME)secondary to acute branched retinal vein occlusion(BRVO)or central retinal vein occlusion(CRVO).Aqueous fluid from cataract patients served as the *** also evaluated whether aqueous level of EPO was associated with factors such as serum EPO level,non-perfusion area,central macular thickness(CMT),and arterio-venous(AV)transit timeRESULTS:Twenty-seven RVO patients(16 BRVO,11CRVO)and 9 control subjects were enrolled in the *** aqueous EPO level(mU/mL)was higher in RVO(68.2±54.3)than that in the control subjects(12.9±5.9).More specifically,the aqueous EPO level was higher in CRVO(118.9±52.1)than that in BRVO(33.3±10.8).However,no differences were found in serum EPO levels among three *** in RVO patients had a positive correlation with the aqueous EPO level(r=0.66).Also,in terms of non-perfusion area,the aqueous EPO levels were more elevated in the ischemic subgroup than in the non-ischemic subgroup in both BRVO and ***:Aqueous EPO levels are elevated in patients with macular edema secondary to recent onset *** with CRVO have higher EPO levels than those with *** aqueous EPO level in RVO has a positive correlation with CMT and is associated with non-perfusion *** results suggest that the aqueous EPO level could be associated with retinal ischemia and may be involved in the pathogenesis of macular edema secondary to RVO.

Technical advances in external radiotherapy for hepatocellular carcinoma

Radiotherapy techniques have substantially improved in the last two decades. After the introduction of 3-dimensional conformal radiotherapy, radiotherapy has been increasingly used for the treatment of hepatocellular carcinoma(HCC). Currently, more advanced techniques, including intensity-modulated radiotherapy(IMRT), stereotactic ablative body radiotherapy(SABR), and charged particle therapy, are used for the treatment of HCC. IMRT can escalate the tumor dose while sparing the normal tissue even though the tumor is large or located near critical organs. SABR can deliver a very high radiation dose to small HCCs in a few fractions, leading to high local control rates of 84%-100%. Various advanced imaging modalities are used for radiotherapy planning and delivery to improve the precision of radiotherapy. These advanced techniques enable the delivery of high dose radiotherapy for early to advanced HCCs without increasing the radiation-induced toxicities. However, as there have been no effective tools for the prediction of the response to radiotherapy or recurrences within or outside the radiation field, future studies should focus on selecting the patients who will benefit from radiotherapy.

Numerical simulation for thermal response test performance in closed-loop vertical ground heat exchanger

In this study, a series of numerical analyses was performed in order to evaluate the performance of full-scale closed-loop vertical ground heat exchangers constructed in Wonju, South Korea. The circulating HDPE pipe, borehole and surrounding ground formation were modeled using FLUENT, a finite-volume method (FVM) program, for analyzing the heat transfer process of the ground heat exchanger system. Two user-defined functions (UDFs) accounting for the difference in the temperature of the circulating inflow and outflow fluid and the variation of ground temperature with depth were adopted in the FLUENT modeling. The thermal conductivities of grouts (cement vs. bentonite) measured in laboratory were used as input values in the numerical analyses to compare the thermal efficiency of the cement and bentonite grouts used for installing the closed-loop vertical ground heat exchanger. A series of numerical analyses was carried out to simulate in-situ thermal response tests performed in the construction site. From the comparison between the in-situ thermal response test results and numerical simulations, the average thermal conductivity of the ground formation in the construction site is back-calculated as approximately 4 W/mK. This value can be used in evaluating the long-term performance of the closed-loop vertical ground heat ex changer.

Quality of survival in patients treated for malignant biliary obstruction caused by unresectable pancreatic head cancer: surgical versus non-surgical palliation

BACKGROUND:Appropriate palliation for unresectable pancreatic head cancer is most *** study was undertaken to compare the survival of patients with biliary obstruction caused by unresectable pancreatic head cancer after surgical and non-surgical palliation. METHODS:We retrospectively reviewed 69 patients who underwent palliative treatment for unresectable pancreatic head ***-two patients with locally advanced disease (local vascular invasion)and 17 with distant metastatic disease were *** patients were divided into two groups,surgical and non-surgical palliation. RESULTS:Thirty-eight patients underwent biliary bypass surgery and 31 had percutaneous transhepatic biliary drainage(PTBD).There was no significant difference in the early complications,successful biliary drainage, recurrent jaundice,and 30-day mortality between surgical palliation and ***,in 52 patients whose tumor was unresectable secondary to local vascular invasion,the rate of recurrent jaundice after successful surgical biliary palliation was lower than that in patients who had non- surgical palliation(P<0.05).The patients who underwent surgical palliation had a longer hospital-free survival rate(P<0.001),although they had a longer postoperative hospital stay(P=0.004)during the first admission ***:In patients with preoperative evaluations showing potentially resectable tumors and/or no metastatic lesions,surgical exploration should be ***,in patients who have unresectable cancer or limited metastatic disease on exploration,surgical palliation should be performed for longer survival and better quality of survival.
