Background:Differential diagnosis of idiopathic Parkinson disease (IPD) and multiple system atrophy-Parkinson type (MSA-P) is challenging since they share clinical features with parkinsonism and autonomic *** distinguish MSA-P from IPD when the symptoms are relatively mild,we investigated the usefulness of the quantitative fractionalized autonomic indexes and evaluated the correlations of autonomic test indexes and functional ***:Thirty-six patients with parkinsonism (22 with IPD and 14 with MSA-P) in Soonchunhyang University Bucheon Hospital from February 2014 to June 2015 were prospectively enrolled in the *** compared fractionalized autonomic indexes and composite autonomic scoring scale between patients with IPD and MSA-P with Hoehn and Yahr (H&Y) score ≤*** indexes included expiratory/inspiratory ratio during deep breathing,Valsalva ratio (VR),and regression slope of systolic blood pressure (BP) in early phase Ⅱ (vagal baroreflex sensitivity) during Valsalva *** adrenergic indexes were pressure recovery time (PRT) and adrenergic baroreflex sensitivity (BRSa) (BP decrement associated with phase 3 divided by the PRT),sympathetic index 1,sympathetic index 3,early phase Ⅱ mean BP drop,and pulse pressure reduction ***,we compared the unified multiple system atrophy rating scale (UMSARS) and H&Y scores and the autonomic indexes in all ***:PRT was significantly different between the IPD and MSA-P groups (P =0.004) despite the similar BP drop during *** value of PRT was 5.5 s (sensitivity,71.4%;specificity,72.7%).VR (r =-0.455,P =0.009) and BRSa (r =-0.356,P =0.036) demonstrated a significant correlation with UMSARS and H&Y ***:Among the cardiovascular autonomic indexes,PRT can be a useful parameter in differentiating the early stage of MSA-P from that of ***,VR,and BRSa may be the optimal indexes in determining functional symptom severity.

目的利用乳腺癌患者连续PET/CT显像和MRI所得的参数评价肿瘤的分化程度。方法回顾性研究78例乳腺癌患者,患者均行连续的PET/CT显像和MRI[注射;F-脱氧葡萄糖（FDG）后1 h和2 h分别行PET/CT显像,随后立刻行MRI检查]。PET/CT显像后计算1 h和2 h FDG的最大标准摄取值(SUV;1和SUV;2)、代谢肿瘤体积（MTV）和总病灶糖酵解（TLG）。动态对比增强MRI参数用动态增强曲线进行分析。对有关表观弥散系数（ADC）值的弥散加权成像（DWI）进行评价。组织学分级为1级和2级的乳腺癌归为低等级肿瘤,3级的归为高等级肿瘤。结果 78例乳腺癌患者的病灶组织学分级属3级,2级和1级的分别为45、26和7个病灶。高等级肿瘤患者的平均ADC值（0.99±0.19）低于低等级肿瘤患者的平均ADC值（1.12±0.32;P=0.007）。高等级肿瘤患者的SUV;1、MTV2.5[平均标准摄取值(SUV;)取2.5时的MTV]和TLG2.5(SUV;取2.5时的TLG)值要高于低等级肿瘤患者[SUV;1:7.92±4.5和6.19±3.05, P=0. 09, MTV2.5:7.90±9.32和4.38±5.10,P=0.095;TLG2.5:40.83±59.17和19.66±26.08, P=0.082]。但高、低等级肿瘤患者间的其他参数差异无统计学意义。受试者工作特征（ROC）曲线分析发现高等级肿瘤的平均ADC值曲线下面积最大,达到0.681（95%CI:0.566～0.782）。结论在乳腺癌PET显像和MRI的多种功能性参数中,较低的平均ADC值也许能预测肿瘤较差的分化程度。

本文《血流动力学相关直立性头晕/眩晕(hemodynamic orthostaticdizziness/vertigo,HOD/V)诊断标准》被纳入国际前庭疾病分类(International ClassificationofVestibularDisorders,ICVD)。定义HOD/V诊断标准的目的是帮助临床医生能够更好地理解直立性头晕/眩晕相关的术语以鉴别直立性头晕/眩晕是由脑低灌注因素还是由其他病因所致。HOD/V诊断标准:(A)至少5次由起身引发或直立体位时出现的头晕、不稳或眩晕,坐下或躺下后缓解;(B)站立或直立倾斜试验时记录到直立性低血压、体位性心动过速综合征或晕厥;(C)不能归因于其他疾病。很可能的HOD/V诊断标准:(A)至少5次由起身引发或直立体位时出现的头晕、不稳或眩晕,坐下或躺下后缓解;(B)至少有一项伴随症状:全身乏力或疲劳感、思维迟缓或注意力难以集中、视物模糊、心动过速或心悸;(C)不能归因于其他疾病。这些诊断标准是基于广泛回顾近九十年来关于HOD/V、体位性低血压或心动过速及自主神经性头晕的相关研究达成的专家共识。进一步测量直立位血压和心率对于筛查和记录直立性低血压、体位性心动过速综合征以确立HOD/V的诊断具有重要意义。

BACKGROUND The cellular prion protein(PrPC),traditionally associated with neurodegenerative disorders,plays an important role in cancer progression and metastasis by inhibiting *** To investigate the influence of PrPC expression in cholangiocarcinoma(CCA)on patient outcomes following surgical *** Patients who underwent curative surgical resection for either intrahepatic or hilar CCA were enrolled in this retrospective *** on the immunohistochemical staining results of the surgical specimens,patients were categorized into two groups:The low PrPC group(negative or 1+)and the high PrPC group(2+or 3+).Survival analyses,including overall survival and recurrence-free survival,were conducted using the Kaplan-Meier method and compared using the log-rank *** In total,seventy-six patients diagnosed with CCA(39 with intrahepatic and 37 with hilar CCA)underwent curative hepatectomy from January 2011 to November *** these patients,38(50%)demonstrated high PrPC expression,whereas the remaining 38(50%)showed low expression of *** a median follow-up period of 31.2 months(range:1 to 137 months),the high PrPC group had a significantly shorter median overall survival than the low PrPC group(40.4 months vs 137.9 months,respectively;P=0.041).Moreover,the high PrPC group had a significantly shorter median recurrence-free survival than the low PrPC group(13.3 months vs 23.8 months,respectively;P=0.026).CONCLUSION PrPC expression is significantly associated with early recurrence and decreased survival period in CCA patients following surgical ***,PrPC may be used as a prognostic factor in treatment planning.

Objective:SC-E3 is a polyherbal formula that contains five medicinal herbs used frequently in traditional herbal *** our previous study,we demonstrated the antioxidant and anti-inflammatory effects of *** present study examined the effects of SC-E3 in a mouse model of type-II collagen-induced arthritis(CIA).Methods:In vivo,male DBA/1 J mice were immunized by intradermal injection of bovine type-II collagen and complete or incomplete Freund’s adjuvant,to induce ***-E3 was orally administered daily for 23 *** vitro,bone marrow-derived macrophages(BMMs)were treated with macrophage colony-stimulating factor(M-CSF)and receptor activator of nuclear factor-j B ligand(RANKL)in the absence or presence of ***:Administrations of SC-E3 were found to have anti-arthritic effects in the joints of CIA mice,as evidenced by reduced paw swelling,bone erosion and deformation,inflammatory cell infiltration,and inflammation in synovial ***-E3 also reduced serum levels of tumor necrosis factor-a,interleukin-1 b,aspartate aminotransferase and alanine ***,tartrateresistant acid phosphatase-positive osteoclast numbers in the joints were significantly lower in SC-E3-treated CIA mice than in CIA *** addition,the differentiations of BMMs to multinucleated osteoclasts induced by M-CSF and RANKL stimulation were dose-dependently reduced by ***:These results suggest that SC-E3 possesses substantial anti-arthritic activity because it inhibits pro-inflammatory cytokines and osteoclastogenesis,and that SC-E3 has potential therapeutic use for the treatment of rheumatoid arthritis.

Selecting and applying quantification models for ecosystem services to forest ecosystems in South Korea

There is growing interest in using ecosystem services to aid development of management strategies that target sustainability and enhance ecosystem support to humans. Challenges remain in the search for methods and indicators that can quantify ecosystem services using metrics that are meaningful in light of their high priorities. We developed a framework to link ecosystems to human wellbeing based on a stepwise approach. We evaluated prospective models in terms of their capacity to quantify national ecosystem services of forests. The most applicable models were subsequently used to quantify ecosystem services. The Korea Forest Research Institute model sat- isfied all criteria in its first practical use. A total of 12 key ecosystem services were identified. For our case study, we quantified four ecosystem functions, viz. water storage capacity in forest soil for water storage service, reduced suspended sediment for water purification service, reduced soil erosion for landslide prevention service, and reduced sediment yield for sediment regulation service. Water storage capacity in forest soil was estimated at 2142 t/ha, and reduced suspended sediment was estimated at 608 kg/ ha. Reduced soil erosion was estimated at 77 m^3/ha, and reduced sediment yield was estimated at 285 m^3/ha. These results were similar to those reported by previous studies. Mapped results revealed hotspots of ecosystem services around protected areas that were particularly rich in bio- diversity. In addition, the proposed framework illustrated that quantification of ecosystem services could be sup- ported by the spatial flow of ecosystem services. However, our approach did not address challenges faced when quantifying connections between ecosystem indicators and actual benefits of services described.

Despite the success of antiretroviral therapy(ART),efforts to develop new classes of antiviral agents have been hampered by the emergence of drug ***-indole-bearing aristolactams are compounds that have been isolated from various plants species and which show several clinically relevant effects,including anti-inflammatory,antiplatelet,and antimycobacterial ***,the effect of these compounds on human immunodeficiency virus type 1(HIV-1)infection has not yet been *** this study,we discovered an aristolactam derivative bearing dibenzo[cd,f]indol-4(5 H)-one that had a potent anti-HIV-1 effect.A structure-activity relationship(SAR)study using nine synthetic derivatives of aristolactam identified the differing effects of residue substitutions on the inhibition of HIV-1 infection and cell *** the compounds tested,1,2,8,9-tetramethoxy-5-(2-(piperidin-1-yl)ethyl)-dibenzo[cd,f]indol-4(5 H)-one(Compound 2)exhibited the most potent activity by inhibiting HIV-1 infection with a half-maximal inhibitory concentration(IC50)of 1.03 lmol/L and a half-maximal cytotoxic concentration(CC50)of 16.91 lmol/L(selectivity index,16.45).The inhibitory effect of the compounds on HIV-1 infection was linked to inhibition of the viral replication ***-of-action studies showed that the aristolactam derivatives did not affect reverse transcription or integration;instead,they specifically inhibited Tat-mediated viral *** together,these findings show that several aristolactam derivatives impaired HIV-1 infection by inhibiting the activity of Tat-mediated viral transcription,and suggest that these derivatives could be antiviral drug candidates.
