This paper presents dynamic-behavior comparisons and related forensic analyses of a submerged floating tunnel(SFT)between numerical simulation and physical experiment under regular and irregular waves.The experiments are conducted in the 3Dwave tank with 1:33.3 scale,and the corresponding coupled time-domain simulation tool is devised for comparison.The entire SFT systemconsists of a long concrete tunnel and 12 tubular aluminummooring lines.Two numerical simulation models,the Cummins equation with 3D potential theory including second-order wave-body interaction effects and the much simpler Morison-equation-based formula with the lumped-massbased line model,are designed and compared.Forensic analyses for mooring-line adjustments in the simulation are carried out in view of the best representation of the physical system.After that,the measured pre-tension distribution and systemstiffness of twelvemooring lines arewell reproduced in the numericalmodel.Subsequently,the dynamic responses and mooring tensions of the SFT are compared under regular and irregular waves.The measured and simulated results coincide reasonably well for both regular-and irregular-wave conditions.

BACKGROUND Fexuprazan,a novel potassium-competitive acid blocker,reversibly suppresses the K+/H+-ATPase enzyme in proton pumps within gastric parietal cells.Fexuprazan’s suppression of gastric acid was maintained in healthy individuals for 24 h in a dose-dependent manner.AIM To compare fexuprazan to esomeprazole and establish its efficacy and safety in patients with erosive esophagitis(EE).METHODS Korean adult patients with endoscopically confirmed EE were randomized 1:1 to receive fexuprazan 40 mg or esomeprazole 40 mg once daily for eight weeks.The primary endpoint was the proportion of patients with healed EE confirmed by endoscopy at week 8.The secondary endpoints included the healing rate of EE at week 4,symptom response,and quality of life assessment.Safety profiles and serum gastrin levels were compared between the groups.RESULTS Of the 263 randomized,218 completed the study per protocol(fexuprazan 40 mg,n=107;esomeprazole 40 mg,n=111).Fexuprazan was non-inferior to esomeprazole regarding the healing rate at week 8[99.1%(106/107)vs 99.1%(110/111)].There were no between-group differences in the EE healing rate at week 4[90.3%(93/103)vs 88.5%(92/104)],symptom responses,and quality of life assessments.Additionally,serum gastrin levels at weeks 4 and 8 and drug-related side effects did not significantly differ between the groups.CONCLUSION Fexuprazan 40 mg is non-inferior to esomeprazole 40 mg in EE healing at week 8.We suggest that fexuprazan is an alternative promising treatment option to PPIs for patients with EE.

Background No study has thoroughly compared the effectiveness of combined antiplatelet treatments(other than clopidogrel–aspirin)versus clopidogrel–aspirin or aspirin alone for early secondary prevention in acute ischaemic stroke.Methods We identified patients with acute,minor,non-cardiogenic ischaemic stroke treated with aspirin alone,clopidogrel–aspirin or other combination treatment.Propensity scores considering the inverse probability of treatment weighting were used to adjust for baseline imbalances.The primary outcome was the composite of all strokes(ischaemic or haemorrhagic),myocardial infarction and all-cause mortality at 3 months.Results Among 12234 patients(male:61.9%;age:65.5±13 years)who met the eligibility criteria,aspirin,clopidogrel–aspirin and other combination treatments were administered in 52.2%,42.9%and 4.9%of patients,respectively.In the crude analysis,the primary outcome event at 3 months occurred in 14.5%of the other combination group,14.4%of the aspirin group and 13.0%of the clopidogrel–aspirin group.In the weighted Cox proportional hazards analysis,the 3-month primary outcome event occurred less frequently in the clopidogrel–aspirin group than in the other combination group(weighted HR:0.82(0.59–1.13)),while no association was found between the aspirin group(weighted HR:1.04(0.76–1.44))or other combination group and the 3-month primary outcome.Conclusion Other combined antiplatelet treatment,compared with aspirin alone or clopidogrel–aspirin,was not associated with reduced risks of primary composite vascular events or recurrent stroke during the first 3 months after stroke.Therefore,the results suggest that other combination treatments,particularly the cilostazol-based combination,may not be effective alternatives for clopidogrel–aspirin to prevent early vascular events in patients with acute minor stroke.Further exploration in clinical trials will be needed.

为了验证，临床无症状前列腺癌在多次前列腺活检后的检出率是不是高一些，研究回顾性分析了2146名经一次以上前列腺活检检出的前列腺癌患者的记录。根据活检次数，将患者分为5组，组1进行了1次活检，组2进行了2次活检，组3进行了3次活检，以此类推。这2146名患者中，1956（91．1％），142（6．6％），38（1．8％），9（0．4％）和1（0．1％）名患者分别被归入组l，组2，组3，组4，和组5中。组4和组5因为患者数量太少，被排除出本研究中。在研究的3组患者中，年龄和PSA的水平无显著差异。三组中的临床无症状前列腺癌的检出率分别为201（10．3％）（组1），28（19．7％）（组2）和9（23．7％）（组3）（P〈0．001）。多因素分析显示，活检次数是检测出临床无症状前列腺癌的一个独立预测因素（OR=2．688，2次活检；OR=4．723，3次活检）。总而言之，做过1次以上活检的患者比仅做1次活检的患者更容易被诊断出临床无症状前列腺癌。然而，患者可能有临床有症状前列腺癌的风险依然存在。所以，在与患者确认是否要进行多次活检时要仔细平衡过度诊断和过度治疗与临床有症状前列腺癌之间的利弊。

AIM:To investigate the relationship between the function of vagus nerve and peptide YY 3-36 and ghrelin levels after subtotal gastrectomy.METHODS:We enrolled a total of 16 patients who underwent subtotal gastrectomy due to gastric cancer.All surgeries were performed by a single skilled surgeon.We measured peptide YY 3-36,ghrelin,leptin,insulin,growth hormone levels,and body weight immediately before and one month after surgery.RESULTS:Vagus nerve preservation group showed less body weight loss and less increase of peptide YY 3-36 compared with vagotomy group(-5.56 ± 2.24 kg vs-7.85 ± 1.57 kg,P = 0.037 and 0.06 ± 0.08 ng/mL vs 0.19 ± 0.12 ng/mL,P = 0.021,respectively).Moreover,patients with body weight loss of less than 10% exhibited reduced elevation of peptide YY 3-36 level,typically less than 20% [6(66.7%) vs 0(0.0%),P = 0.011,odd ratio = 3.333,95% confidence interval(1.293,8.591)].CONCLUSION:Vagus nerve preservation contributes to the maintenance of body weight after gastrectomy,and this phenomenon may be related to the suppressed activity of peptide YY 3-36.

Aim:To investigate the long-term effect of soybean isoflavones on changes inparvalbumin (PV) immunoreactivity in the hippocampus in normal female,ovariec-tomized (OVX) female and normal male rats.Methods:Ten-month-old rats wereassigned to one of 9 groups (n=7 in each group) based on body weight using arandomized complete-block design.The groups were:control diet-treated females,OVX females,and males;0.3g/kg isoflavone-treated females,OVX females,andmales;and 1.2g/kg isoflavone-treated females,OVX females,and males.The PVimmunostaining was conducted by using the standard avidin-biotin complexmethod.Results:PV immunoreactivity and the number of PV-immunoreactiveneurons in all the groups after isoflavone treatment were significantly changed inthe hippocampal CA1 region and in the dentate gyrus,but not in the hippocampalCA2/3 region.PV immunoreactivity and the number of PV-immunoreactive neu-rons in the control diet OVX females were similar to those in the control diet,andwere greater than those in the control diet normal females.PV immunoreactivityand the number of PV-immunoreactive neurons in all the isoflavone-treated groupsdecreased dose-dependently after isoflavone treatment.Conclusion:Long-termadministration of isoflavones may induce a reduction of PV in interneurons in thehippocampal CA1 region and in the dentate gyrus.The reduction of PV in theseregions suggests that the long-term administration of isoflavones may cause achange in calcium homeostasis in the hippocampal CA1 region and in the dentategyrus.

BACKGROUND： Thyroid hormones modulate proliferation of astrocytes and microglia depending on maturation stage and localization. Studies have demonstrated that triiodothyronine treatment or thyroidectomy during developmental stages results in morphological alterations and changes in the number of astrocytes and microglia. Little is known about the effects of hypothyroidism on astrocytes and microglia in adults. OBJECTIVE： To investigate the effects of hypothyroidism on morphology and number of astrocytes and microglia in the adult rat hippocampus. DESIGN, TIME AND SETTING： A randomized, controlled, neuroendocrinological, animal study was performed at the College of Medicine, Hallym University, South Korea between May 2008 and April 2009. MATERIALS： Methimazole, rabbit anti-glial fibrillary acidic protein （GFAP） antiserum, and rabbit anti-lba-1 antiserum were purchased from Sigma, USA. Rabbit anti-GFAP polyclonal antibody was provided by Chemicon, USA. Rabbit anti-lba-1 polyclonal antibody was purchased from Wako, Japan. Terminal deoxynucleotidyl transferase dUTP-biotin nick-end-labeling （TUNEL） kit was provided by Roche Molecular Biochemicals, Mannheim, Germany. METHODS： Hypothyroidism was induced in Wistar rats via methimazole administration （0.025%） in drinking water for 5 weeks, starting at 6 months of age. MAIN OUTCOME MEASURES： Following methimazole treatment, hippocampai neuronal death was determined using TUNEL staining. The morphology and number of GFAP and lba-1 immunoreactive cells were detected by immunohistochemistry. Hippocampal GFAP and lba-1 protein levels were detected by Western blot analysis. Serum-free triiodothyronine and thyroxine levels were quantified. RESULTS： TUNEL-positive neurons were not observed in the hippocampus of euthyroid and hypothyroid rats. Compared with the euthyroid rats, the number of GFAP immunoreactive astrocytes was decreased, and serum triiodothyronine and thyroxine levels were significantly decreased. AIM To investigate Helicobacter pylori (H. pylori) eradication rates using second-line bismuth-containing quadruple therapy and to identify predictors of eradication failure.METHODS This study included 636 patients who failed firstline triple therapy and received 7d of bismuthcontaining quadruple therapy between January 2005 and December 2015. We retrospectively demonstrated H. pylori eradication rates with respect to the year of therapy as well as demographic and clinical factors. H. pylori eradication was confirmed by a 13C-urea breath test or a rapid urease test at least 4 wk after the completion of bismuth-based quadruple therapy: proton pump inhibitor, metronidazole, bismuth, and tetracycline.RESULTS The overall eradication rates by intention-to-treat analysis and per-protocol analysis were 73.9%( 95% CI : 70.1 %- 77.4%) and 94.5% (95% CI : 92.4%-96.5%), respectively. Annual eradication rates from 2005 to 2015 were 100.0%, 92.9%, 100.0%, 100.0%, 100.0%, 97.4%, 100.0%, 93.8%, 84.4%, 98.9%, and 92.5%, respectively, by per-protocol analysis. A multivariate analysis showed that diabetes mellitus (OR=3.99, 95%CI: 1.56-10.20, P=0.004)was associated with H. pylori eradication therapy failure.CONCLUSION The second-line bismuth-containing quadruple therapy for H. pylori infection is still effective in Korea, and diabetes mellitus is suggested to be a risk factor for eradication failure.
