What is the real practice of exercise echocardiographic testing in asymptomatic patients with severe aortic stenosis？

Background Although exercise testing has been suggested to help predict clinical outcome, limited data are available to guide how exercise Doppler echocardiography （ECG） can be used clinically in asymptomatic patients with aortic stenosis （AS）. The aim of this study was to assess the clinical value of exercise echocardiographic testing in asymptomatic patients with severe AS. Methods Symptom-limited treadmill exercise testing using the modified Bruce protocol was performed in 31 asymptomatic patients （mean age （62＋11） years） with severe AS （aortic valve area 〈1 cm2, peak aortic velocity （AV Vmax）〉4 m/sec, or a mean transaortic pressure gradient （AV mean PG）〉40 mmHg （1 mmHg=0.133 kPa）） with normal left ventricular （LV） systolic function （LV ejection fraction （EF）〉50%）. Clinical symptoms, vital signs, ECG, and Doppler hemodynamics were obtained during and/or immediately after exercise. Results Aortic valve replacement （AVR） was performed in 18 patients during follow-up. The patients who had AVR exhibited higher baseline AV mean PG （51 （35-84） vs. 44 （25.2-57.0） mmHg; P=0.031）. There were no significant differences between the AVR group and non-AVR group including exercise duration （7.47 （2.32-11.59） vs. 7.25 （4.06- 10.52） minutes, P=0.917）, exercise capacity （10.1 （4.6-12.8） vs. 10.1 （7.0-12.8） metabolic equivalents, P=0.675）, and an increment in AV mean PG by exercise （18.5 （3.2-48.0） vs. 12.6 （4.4-32.1） mmHg, P=0.366）. Univariate regression analysis revealed that independent determinant of AVR was the baseline AV mean PG （P=-0.031）. Conclusions Although additional value of exercise ECG was demonstrated, baseline transaortic mean pressure gradient is the major determinant of AVR. Further large-scale prospective studies are required to determine whether surgery should be recommended in the presence of an abnormal exercise ECG in asymptomatic severe AS.

Treatment for gastric ‘indefinite for neoplasm/dysplasia' lesions based on predictive factors

BACKGROUND Gastric ‘indefinite for neoplasm/dysplasia'(IFND) is a borderline lesion that is difficult to diagnose as either regenerative or neoplastic. There is a need for guidance in the identification of a subset of patients, who have an IFND lesion with a higher risk of malignant potential, to enable risk stratification and optimal *** To determine the clinical and pathologic factors for the accurate diagnosis of gastric IFND *** In total, 461 gastric lesions diagnosed via biopsy as IFND lesions were retrospectively evaluated. Endoscopic resection(n = 134), surgery(n = 22), and follow-up endoscopic biopsy(n = 305) were performed to confirm the *** time interval from initial biopsy to cancer diagnosis was measured, and diagnostic delays were categorized as > 2 wk, > 2 mo, > 6 mo, and > 1 year. The IFND lesions presenting as regenerating atypia(60%) or atypical epithelia(40%)at initial biopsy were adenocarcinomas in 22.6%, adenomas in 8.9%, and gastritis in 68.5% of the *** Four clinical factors [age ≥ 60 years(2.445, 95%CI: 1.305-4.580, P = 0.005),endoscopic size ≥ 10 mm(3.519, 95%CI: 1.891-6.548, P < 0.001), single lesion(5.702, 95%CI: 2.212-14.696, P < 0.001), and spontaneous bleeding(4.056, 95%CI:1.792-9.180, P = 0.001)], and two pathologic factors [atypical epithelium(25.575,95%CI: 11.537-56.695, P < 0.001], and repeated IFND diagnosis [6.022, 95%CI:1.822-19.909, P = 0.003)] were independent risk factors for gastric cancer. With two or more clinical factors, the sensitivity and specificity for carcinoma were91.3% and 54.9%, respectively. Ten undifferentiated carcinomas were initially diagnosed as IFND. In the subgroup analysis, fold change(5.594, 95%CI: 1.458-21.462, P = 0.012) predicted undifferentiated or invasive carcinoma in the submucosal layers or deeper. AIM:To determine whether magnified observation of short-segment Barrett’s esophagus(BE)is useful for the detection of specialized intestinal metaplasia(SIM).METHODS:Thirty patients with suspected short-segment BE underwent magnifying endoscopy up to×*** magnified images were analyzed with respect to their pit-patterns,which were simultaneously classified into five epithelial types[Ⅰ(small round),Ⅱ(straight),Ⅲ(long oval),Ⅳ(tubular),Ⅴ(villous)]by Endo’s ***,a 0.5%solution of methylene blue(MB)was sprayed over columnar *** patterns of the magnified image and MB staining were *** were obtained from the regions previously observed by magnifying endoscopy and MB ***:Three of five patients with a typeⅤ(villous)epithelial pattern had SIM,whereas 21 patients with a non-typeⅤepithelial patterns did not have *** sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of pit-patterns in detecting SIM were 100%,91.3%,92.3%,60%and100%,respectively(P=0.004).Three of the 12 patients with positive MB staining had SIM,whereas 14patients with negative MB staining did not have *** sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of MB staining in detecting SIM were 100%,60.9%,65.4%,25%and100%,respectively(P=0.085).The specificity and accuracy of pit-pattern evaluation were significantly superior compared with MB staining for detecting SIM by comparison with the exact McNemar’s test(P=0.0391).CONCLUSION:The magnified observation of a shortsegment BE according to the mucosal pattern and its classification can be predictive of SIM.

Macrophage PD-L1 strikes back: PD-1/PD-L1 interaction drives macrophages toward regulatory subsets

Activated macrophages have been simply de?ned as cells that secrete in?ammatory mediators and kill intracellular pathogens until few years ago. Recent studies have proposed a new classification system to separate activated macrophages based on their functional phenotypes: host defense, wound healing, and immune regulation. Regulatory macrophages can arise following innate or adaptive immune responses and hinder macrophage-mediated host defense and inflammatory functions by inhibiting the production of pro-inflammatory mediators. In this study, we investigated whether PD-1 and PD-L1 interaction between macrophages and T cells alters macrophage activities. Our data provide evidence for PD-1/PD-L1 engagement inducing a regulatory profile in macrophages. Regulatory macrophages derived from PD-L1 signaling lost their host defense activity, which consists of the production of pro-inflammatory cytokine IL-6 and the exhibition of increased IL-10, SPHK1 and LIGHT gene levels in early phases of LPS stimulation. This differentiation seems to occur through excessive activation of TLR4 downstream MAPK signaling pathways. Regulatory macrophages induced from PD-1/PD-L1 interaction decrease inflammatory mediators and produce anti-inflammatory cytokines, so this macrophage subset has been under considerable attention as a possible immune regulation mechanism. Understanding and modulating regulatory macrophages may lead to new approches to treat or prevent auto-immune diseases such as type I diabetes, rheumatic syndrome and hypersensitivity-related diseases, which are concerned with the overproduction of inflammatory cytokines in macroages.

Infectious agents causing aborted fetus problems in domestic pigs were investigated in this study. More than 10 different infectious agents were known to cause abortion in swine and the major eight viruses among them were inspected. One hundred twelve samples of aborted fetuses from nine provinces in South Korea were collected during April to November, 2013 in this study for the diagnosis of infectious agents causing abortions in pigs. Eight major infection viruses were examined in this study mainly using various diagnostic kits and reverse transcriptase polymerase chain reaction （RT-PCR）. Positive rate of the detection differed from each viruses. In this study, the main focus was the porcine reproductive and respiratory syndrome virus （PRRSV）, which took the second large portion in the positive rate of detection, and then its ORF5 gene was compared with modified live virus （MLV） vaccine strain to figure out the influence of vaccine on disease. Between four positive samples＇ sequence, two of them were 99.9%-100% similar to MLV vaccine strain and two other samples were 88.6%-92.7% similar. Similarity rate of the sequences between the vaccine and virus from aborted fetuses are very crucial, because it implies that abortion in swine can be made due to the usage of vaccine not only by the infection of field virus, and if MLV vaccine actually do have an impact on the infection, usage of the vaccine should be reconsidered.

Design of DC-DC Converter for Flash Memory IPs

A DC-DC converter for flash memory IPs performing erasing by the FN (Fowler-Nordheim) tunneling and programming by the CHEI (channel hot electron injection) is designed in this paper. For the DC-DC converter for flash memory IPs using a dual voltage of VDD (=1.5V±0.15V)/VRD (=3.1V±0.1V), a scheme of using VRD (Read Voltage) instead of VDD is proposed to reduce the pumping stages and pumping capacitances of its charge pump circuit. VRD (=3.1V±0.1V) is a regulated voltage by a voltage regulator with an external voltage of 5V, which is used as the WL activation voltage in the read mode and an input voltage of the charge pump. The designed DC-DC converter outputs positive voltages of VP6V (=6V), VP8V (=8V) and VP9V(=9V);and a negative voltage of? VM8V (=-8V) in the write mode.

A Study on Water Repellent Effectiveness of Natural Oil-Applied Soil as a Building Material

This study was performed to investigate the water repellent effectiveness of natural oil-applied soil when it is used as a building material. Natural oil types such as olive oil, bean oil, perilla oil and linseed oil, which are being used for producing water repellent timber, are selected for the experiments. It is expected that perilla oil and in seed oil, which are drying oil types will have better water repellent effectiveness than the other types. For the evaluation of water repellence of natural oil-applied soil, a contact angle test was performed. A contact angle of water drop on various surface conditions were tested, and large differences were seen between the natural oil-applied soil and untreated soil. As a result, it is showed that all natural oil types have water repellent effectiveness. However, linseed oil, which is a drying oil type, shows an outstanding water repellent effectiveness value, while perila oil, which is also a drying oil type, shows the lowest value. Additionally, results show that there is no link between water repellent effectiveness and the number of applications of natural oil. Nevertheless, existing commercial water repellents show better performance than natural oil, and it is anticipated that the results of this study will provide essential information for further research to enhance the water repellent effectiveness of soil as a building material.

＜正＞EMBRYONIC FL0WER1（EMF1） together with EMF2 has very important role in the maintenance of vegetative development by repressing directly the flower organ identity genes in Arabidopsis. To investigate EMF1-mediated vegetative development, we have analyzed molecular function of EMF1-Interacting protein （EMF1IP） 1, 6, and 9 that had been isolated from yeast two-hybrid screen using EMF1 as bait, and are predicted to encode a MAP kinase （EMF1IP1）, zinc-finger protein （EMF1IP6） and a DnaJ-domain protein （EMF1IP9）, respectively. Using yeast two-hybrid system, it was revealed that middle region of EMF1 shows strong interaction only with the EMF1IP1, whereas C-terminal region of EMF1 shows the interaction with the EMF1IP6 and 9. We also found that C-terminal region of EMF1IP1 and N-terminal region of EMF1IP6 are involved in the interaction with EMF1, and entire EMF1IP9 is necessary for interaction with EMFl. Interestingly, the N-terminal region of EMF1IP6 includes B-box zinc-finger domain, indicating that the B-box zinc-finger might play an important role in the protein-protein interaction. As results of subcellular localization assay of EMF1IPs, it was suggested that EMF1IP1 is localized in nucleus, EMF1IP6 in both cytoplasm and nucleus, and EMF1IP9 in cytoplasm. In addition, experiments using EMF1IPs::GUS and semi-quantitative RT-PCR analysis revealed that EMF1 was expressed during most of vegetative developmental stages after germination, but not in floral clusters including inflorescence meristem. Especially EMF1 was strongly expressed during early growth stage of vegetative development. EMF1IPs were detected in all of developmental stages investigated except at 4 days after germination （DAG）, but differential expressions were observed during the developmental stages after 4 DAG These expression patterns suggest that the interaction between EMF1 and EMF1IPs might strongly occur during early growth stage.
