Retardation of Corrected QT Dispersion Recovery, Heart Rate Recovery and Increase of QT Dispersion in Patients with Pseudoexfoliative Syndrome;Are They Predictors of Malignant Arrythmias?

Background: In pseudoexfoliative syndrome (PEX), extraocular accumulation is associated with cardiovascular morbidity and mortality. The aim of this study was to investigate the effect of exercise test on corrected QT dispersion (QTcd) and QTcd recovery in PEX. Patients and Methods: A total of 37 patients diagnosed as PEX were included in the study. The control group consisted of 38 patients. The Electrocardiographies (ECG) were taken. The patients were subjected to an exercise test. Results: In the study group, baseline, peak and third minute recovery QTcd values were 41.35 ± 7.27 ms, 65.19 ± 7.83 ms, and 53.61 ± 6.45 ms, respectively. In the control group, baseline, peak and third minute recovery QTcd values were 29.92 ± 4.23 ms, 38.31 ± 4.22 ms and 28.76 ± 3.09 ms, respectively. There was statistically significant?difference between the baseline and peak QTcd values of the study group and?the control group (t = 8.34224/p 0.00001, t = 18.55079/p 0.00001, respectively). In addition, while there was a statistically significant?difference between the baseline QTcd and the third minute recovery QTcd in the study group, there was no statistically significant difference between the baseline QTcd value and the third minute recovery QTcd value in the control group as heart rate recovery (t = 8.388558/p 0.00001, t = 1.967315/p = 0.056668, respectively).?Conclusion: All of them were found to have a higher risk in terms of malignant ventricular arrhythmia in the study group, both at rest and with effort, in a statistically significant way compared to the control group.

Application of the Hybrid Differential Transform Method to the Nonlinear Equations

In this paper, a hybrid method is introduced briefly to predict the behavior of the non-linear partial differential equations. The method is hybrid in the sense that different numerical methods, differential transform and finite differences, are used in different subdomains. Our aim of this approach is to combine the flexibility of differential transform and the efficiency of finite differences. An explicit hybrid method for the transient response of inhomogeneous nonlinear partial differential equations is presented;applying finite difference scheme on the fixed grid size is used to approximate the space discretisation, whereas the differential transform method is used for time operator. Comparison of the efficiency of the different approaches is a very important aspect of this study. In our test cases, the hybrid approach is faster than the corresponding highly optimized finite difference method in two dimensional computations. We compared our hybrid approach’s results with the exact and/or numerical solutions of PDE which obtained from Adomian Decomposition Method. Results show that the hybrid approach may be an important tool to reduce the execution time and memory requirements for large scale computations and get remarkable results in predicting the solutions of nonlinear initial value problems.

Highly nonlinear birefringent Photonic Crystal Fibre (PCF) that exhibits low losses and small effective mode area across a wide wavelength range has been presented. The effects of angular orientation on bending losses of the proposed PCFs have been thoroughly investigated by employing a full vectorial finite element method (FEM). It has been demonstrated that it is possible to design a bending-insensitive nonlinear PCF with a birefringence in the order of 10-2 and a nonlinear coefficient of 49 W-1km-1 at the wavelength of 1.55 μm. Also, significant improvements on key propagation characteristics of the proposed PCFs have been demonstrated by carefully altering the desired air hole diameters and the hole-to-hole spacing. It is demonstrated that two zero dispersion wavelengths can be achieved by the proposed design.

Pulmonary vascular disorders including portopulmonary hypertension(PoPHT) are among the common complications of liver disease and are prognostically significant. Survival is very low without medical treatment and liver transplantation. With advances in medical therapy for elevated pulmonary artery pressure(PAP) and liver transplant surgery, survival of patients with Po PHT and advanced liver disease is significantly improved. Because of the prognostic significance of Po PHT and the limited donor pool, a comprehensive preoperative cardio-pulmonary assessment is of great importance in cirrhotic patients prior to transplant surgery. Therefore, a detailed transthoracic Doppler echocardiographic examination must be an essential component of this evaluation. Patients with mild Po PHT can safely undergo liver transplant surgery. In cases of moderate to severe Po PHT, right heart catheterization(RHC) should be performed. In patients with moderate to severe Po PHT on RHC(mean PAP 35-45 mm Hg), vasodilator therapy should be attempted. Liver transplantation should be encouraged in cases that demonstrate a positive response. Bridging therapy with specific pulmonary arterial hypertension treatment agents should be considered until the transplant surgery and should be continued during the peri- and post-operative periods as needed.

Staining Effect of Pomegranate Flower Extract on Human Blood Cells： First Results

Pomegranate flower plant, popularly known using for the treatment of various diseases, was not investigated as a source of dye for cytological studies using human blood cells. The importance of this study is to appear dyeing result of pomegranate flower extract on human blood cells. The natural dye source was pomegranate flower known as roselle and potassium aluminum sulfate （alum = KAISO4＇ 12H20） was used as mordant or metal salt. Distilled water was used as solvent. Fresh, clean and air-dried flowers were extracted with distilled water at 100℃ for 30 minutes and then filtered. One drop blood from a healthy 20-year woman was spread as a peripheral on to ten plates and dried at nearly 25℃. These slides were stained by soaking in pomegranate flower extract with/without alum （KA1SO4·12H2O） at 100℃ for 60 minutes. Slides were washed with distilled water, dried and done microscobic examination. The different blood cells dyed dark orange in alum mordant media at 100 ℃. As a result, pomegranate flower has the capacity to use dyeing human blood cells such as eosinophil, basophil and neutrophil.

Role of echo Doppler ultrasonography in the evaluation of postprandial hyperemia in cirrhotic patients

AIM: To assess the role of echo-Doppler ultrasonography in postprandial hyperemia in cirrhotic patients by comparing the results with the hepatic vein catheterization ***: Patients with cirrhosis, admitted to the portal hemodynamic laboratory were included into the study. After an overnight fast, echo-Doppler ultrasonography (basal and 30 min after a standard meal) and hemodynamic studies by hepatic vein catheterization (basal, 15 min and 30 min after a standard meal) were performed. Ensure Plus (Abbot Laboratories, North Chicago, IL) was used as the standard liquid meal. Correlation analysis of the echo-Doppler and hepatic vein catheterization measurements were done for the basal and postprandial ***: Eleven patients with cirrhosis (5 Child A, 4 Child B, 2 Child C) were enrolled into the study. After the standard meal, 8 of the 11 patients showed postprandial hyperemia with increase in portal blood flow, portal blood velocity and hepatic venous pressure gradient. Hepatic venous pressure gradient in the postprandial period correlated positively with postprandial portal blood velocity (r = 0.8, P < 0.05) and correlated inversely with postprandial superior mesenteric artery pulsatility index (r = -1, P < 0.01).CONCLUSION: Postprandial hyperemia can be efficiently measured by echo-Doppler ultrasonography and the results are comparable to those obtained with the hemodynamic studies.

Background Transcatheter aortic valve implantation （TAVI） is a minimally invasive, emerging therapy in surgically high risk, or in- operable patients. Parameters used for risk classification have some deficiencies in the selection of patients. The objective of this study is to evaluate the impact of TAVI on carbohydrate antigen 125 （CA125） and N-terminal pro brain-type natriuretic peptide （NT-proBNP） as biomarkers that have been used frequently in recent years, and also the relationship of these biomarkers to prognosis. Methods ＆ Results Transcatheter aortic valve implantation was practiced on 31 patients in this study. Then, CA125 and NT-proBNP levels studied in patients prior to and after the TAVI were evaluated. The patients were also grouped in accordance with their left ventrieular ejection fraction （LVEF） and CA125 levels （LVEF 〉 40% and 〈 40%; CA125 ≤ 35 U/L and 〉 35 U/L）. The TAVI operation was successfully performed in all patients. There was no in-hospital mortality and substantial improvement in functional capacity was detected at follow ups. In addition, a statistically significant decrease was detected in post-TAVI CA 125 and NT-proBNP levels of all patients （CA 125 83.8 ± 18.1 U/L vs. 64.3 ±14.2 U/L, P = 0.008; NT-proBNP： 4633.6± 627.6 pg/mL vs. 2866.3±536.8 pg/mL, P 〈 0.001）. In groups divided according to the CA125 levels, there was also statistically significant post-TAVI decline in CA125 levels. Within CA125 〉 35 U/L and LVEF 〈 40% groups, the permanent need for a pacemaker was required in one （3.2%） patient and mortality was observed in two （6.4%） patients after TAVI at follow up. Conclusions The results show that TAVI can be performed effectively and reliably in patients with high baseline levels of CA125 and NT-proBNP. These biomarkers are reduced substantially with TAVI, while high biomarker levels are associated with undesired events, and certainly, these biomarkers can be used for risk classifications in patient selection for TAVI.

Agrotechnical assessment of struvite produced by a lab-scale sedimentation unit is performed. As a source of phosphates and ammonia, liquor obtained through dewatering of Мunicipal wastewater treatment plant sludge was in use. The range of phosphates and ammonia was in the range of 130 - 250 and 380 - 560 mg/L, respectively. Seawater brine with a magnesium concentration of around 60 g Mg2+/L was applied as a source of magnesium. The agrotechnical characteristics of struvite obtained were compared to ammonium nitrate and carbamide in regards to the productivity effects on maize hybrid P9241. The results show that the effectiveness of struvite and some commercial fertilizers is quite close. The highest yield in the experiment was achieved with the application of carbamide plus ammonium nitrate (56.64 kg/ha), while in applying struvite solely it is 54.60 kg/ha. The highest protein content of maize (9.7%) was observed in the case of struvite/ammonium nitrate application.

Performing the functions of the bank＇s own funds lack of funds, those living in them to pass on more of those who use and quite a large amount of capital to determine the risks and managing them are facing. Chain of crises in financial markets spread to other sectors see that starting. This phenomenon depending on the potential risks of financial market actors, and especially banks, system identification, measurement, and control needs to be increased. This is referred to as the Basel Accords, depending on the needs the agreement of risk management has emerged. At first glance, the Basel Ⅱ Accord in terms of risk management in financial institutions risk management recommendations to the perception of the principles of the Basel Ⅱ Accord, but with a specific timetable, gradually emerges as a set of rules that must be passed as risk management. Finally, the financial crisis spread across the world from the United States, how much it reveals that risk management in the financial markets. In this study, capital adequacy ratio （CAR） banks operating in Turkey＇s correlation analysis investigated the effect of the banks.
