Background： Metabolic syndrome （MetS） is one of the high cardiovascular （CV） situations. Endothelial dysfunction, which is a common finding in patients with MetS, is related with increased CV risk. In patients with MetS, the effect of the major CV risk factors, not included in the MetS definition, on endothelial dysfunction is not well known. The aim of this study was to determine the effect of major CV risk factors such as gender, smoking, family history, and biochemical parameters on endothelial dysfunction in patients with MetS. Methods： The study was performed between December 2010 and August 2014. A total of 55 patients （ 15 females and 40 males） with MetS and 81 healthy controls （37 females and 44 males） with a body mass index 〈25 kg/m2 were enrolled in the study. Endothelial dysfunction was measured by flow-mediated dilatation （FMD）, oxidative stress parameters; high-sensitivity C-reactive protein （hs-CRP）, oxidized low-density lipoprotein （ox-LDL）, endothelial nitric oxide synthase （e-NOS）, nitric oxide, and cell adhesion markers; yon Willebrand factor, and e-selectin. Platelet aggregation （endothelial adenosine diphosphate）, total platelet count, and mean platelet volume were additionally analyzed and demographic parameters were explored. Student＇s t-test, Mann-Whitney U-test, and Chi-square test were used to analyze the results. Results： The fasting blood glucose （z = 3.52, P = 0.001 ）, hs-CRP （z - 3.23, P = 0.004）, ox-LDL （z = 2.62, P= 0,013）, and e-NOS （z = 2.22, P = 0.026） levels and cardiac risk score （z - 5.23, P 〈 0.001） were significantly higher in patients with MetS compared with the control group. Smoking was correlated with decreased FMD （χ2 = 9.26, P = 0.002） in MetS patients but not in the control group. Conclusions： Increased ox-LDL, hs-CRP, and e-NOS are likely to be a result of oxidative stress, a condition in which an imbalance occurs between the production and inactivation of reactive nitrogen and oxygen species. Retardation of Corrected QT Dispersion Recovery, Heart Rate Recovery and Increase of QT Dispersion in Patients with Pseudoexfoliative Syndrome;Are They Predictors of Malignant Arrythmias?

Background: In pseudoexfoliative syndrome (PEX), extraocular accumulation is associated with cardiovascular morbidity and mortality. The aim of this study was to investigate the effect of exercise test on corrected QT dispersion (QTcd) and QTcd recovery in PEX. Patients and Methods: A total of 37 patients diagnosed as PEX were included in the study. The control group consisted of 38 patients. The Electrocardiographies (ECG) were taken. The patients were subjected to an exercise test. Results: In the study group, baseline, peak and third minute recovery QTcd values were 41.35 ± 7.27 ms, 65.19 ± 7.83 ms, and 53.61 ± 6.45 ms, respectively. In the control group, baseline, peak and third minute recovery QTcd values were 29.92 ± 4.23 ms, 38.31 ± 4.22 ms and 28.76 ± 3.09 ms, respectively. There was statistically significant?difference between the baseline and peak QTcd values of the study group and?the control group (t = 8.34224/p 0.00001, t = 18.55079/p 0.00001, respectively). In addition, while there was a statistically significant?difference between the baseline QTcd and the third minute recovery QTcd in the study group, there was no statistically significant difference between the baseline QTcd value and the third minute recovery QTcd value in the control group as heart rate recovery (t = 8.388558/p 0.00001, t = 1.967315/p = 0.056668, respectively).?Conclusion: All of them were found to have a higher risk in terms of malignant ventricular arrhythmia in the study group, both at rest and with effort, in a statistically significant way compared to the control group.

Application of the Hybrid Differential Transform Method to the Nonlinear Equations

In this paper, a hybrid method is introduced briefly to predict the behavior of the non-linear partial differential equations. The method is hybrid in the sense that different numerical methods, differential transform and finite differences, are used in different subdomains. Our aim of this approach is to combine the flexibility of differential transform and the efficiency of finite differences. An explicit hybrid method for the transient response of inhomogeneous nonlinear partial differential equations is presented;applying finite difference scheme on the fixed grid size is used to approximate the space discretisation, whereas the differential transform method is used for time operator. Comparison of the efficiency of the different approaches is a very important aspect of this study. In our test cases, the hybrid approach is faster than the corresponding highly optimized finite difference method in two dimensional computations. We compared our hybrid approach’s results with the exact and/or numerical solutions of PDE which obtained from Adomian Decomposition Method. Results show that the hybrid approach may be an important tool to reduce the execution time and memory requirements for large scale computations and get remarkable results in predicting the solutions of nonlinear initial value problems.

Highly Nonlinear Bending-Insensitive Birefringent Photonic Crystal Fibres

Highly nonlinear birefringent Photonic Crystal Fibre (PCF) that exhibits low losses and small effective mode area across a wide wavelength range has been presented. The effects of angular orientation on bending losses of the proposed PCFs have been thoroughly investigated by employing a full vectorial finite element method (FEM). It has been demonstrated that it is possible to design a bending-insensitive nonlinear PCF with a birefringence in the order of 10-2 and a nonlinear coefficient of 49 W-1km-1 at the wavelength of 1.55 μm. Also, significant improvements on key propagation characteristics of the proposed PCFs have been demonstrated by carefully altering the desired air hole diameters and the hole-to-hole spacing. It is demonstrated that two zero dispersion wavelengths can be achieved by the proposed design.

Portopulmonary hypertension in liver transplant candidates

Pulmonary vascular disorders including portopulmonary hypertension(PoPHT) are among the common complications of liver disease and are prognostically significant. Survival is very low without medical treatment and liver transplantation. With advances in medical therapy for elevated pulmonary artery pressure(PAP) and liver transplant surgery, survival of patients with Po PHT and advanced liver disease is significantly improved. Because of the prognostic significance of Po PHT and the limited donor pool, a comprehensive preoperative cardio-pulmonary assessment is of great importance in cirrhotic patients prior to transplant surgery. Therefore, a detailed transthoracic Doppler echocardiographic examination must be an essential component of this evaluation. Patients with mild Po PHT can safely undergo liver transplant surgery. In cases of moderate to severe Po PHT, right heart catheterization(RHC) should be performed. In patients with moderate to severe Po PHT on RHC(mean PAP 35-45 mm Hg), vasodilator therapy should be attempted. Liver transplantation should be encouraged in cases that demonstrate a positive response. Bridging therapy with specific pulmonary arterial hypertension treatment agents should be considered until the transplant surgery and should be continued during the peri- and post-operative periods as needed.

AIM: To evaluate the effects of dexamethasone on liver damage in rats with bile duct ligation. METHODS: A total of 40 male Sprague-Dawley rats, weighing 165-205 g, were used in this study. Group 1 (sham-control, n = 10) rats underwent laparotomy alone and the bile duct was just dissected from the surrounding tissue. Group 2 rats (untreated, n = 10) were subjected to bile duct ligation (BDL) and no drug was applied. Group 3 rats (low-dose dexa, n = 10) received a daily dose of dexamethasone by orogastric tube for 14 d after BDL. Group 4 rats (high-dose dexa, n = 10) received a daily dose of dexamethasone by orogastric tube for 14 d after BDL. At the end of the two-week period, biochemical and histological evaluations were processed. RESULTS: The mean serum bilirubin and liver enzyme levels signifi cantly decreased, and superoxide dismutase (SOD), catalase (CAT) and glutathione peroxidase (GSH-Px) values were significantly increased in low-dose dexa and high-dose dexa groups when compared to the untreated group. The histopathological score was significantly less in the low-dose and high-dose dexa groups compared to the untreated rats. In the low-dose dexa group, moderate liver damage was seen, while mild liver damage was observed in the high-dose dexa group. CONCLUSION: Corticosteroids reduced liver damage produced by bile duct obstruction. However, the histopathological score was not signifi cantly lower in the high-dose corticosteroid group as compared to the low-dose group. Thus, low-dose corticosteroid provides asignifi cant reduction of liver damage without increased side effects, while high dose is associated not with lower fi brosis but with increased side effects.

Agrotechnical Assessment of Struvite Application

Agrotechnical assessment of struvite produced by a lab-scale sedimentation unit is performed. As a source of phosphates and ammonia, liquor obtained through dewatering of Мunicipal wastewater treatment plant sludge was in use. The range of phosphates and ammonia was in the range of 130 - 250 and 380 - 560 mg/L, respectively. Seawater brine with a magnesium concentration of around 60 g Mg2+/L was applied as a source of magnesium. The agrotechnical characteristics of struvite obtained were compared to ammonium nitrate and carbamide in regards to the productivity effects on maize hybrid P9241. The results show that the effectiveness of struvite and some commercial fertilizers is quite close. The highest yield in the experiment was achieved with the application of carbamide plus ammonium nitrate (56.64 kg/ha), while in applying struvite solely it is 54.60 kg/ha. The highest protein content of maize (9.7%) was observed in the case of struvite/ammonium nitrate application.

Interaction of L-Arginine-methyl ester and Sonic hedgehog in liver ischemia-reperfusion injury in the rats

AIM: To investigate the role of Sonic hedgehog (Shh) on the course of liver ischemia and reperfusion (I/R) in rats, and the interaction between treatment with nitric oxide donor L-Arginine-methyl ester (L-Arg) and up-regulation of Shh expression. METHODS: A total of 30 male Sprague-Dawley rats weighing 220-240 g were used in this study. Sham-control group (G1, n = 10): a sham operation was performed (except for liver I/R). I/R-untreated group (G2, n = 10): rats underwent liver ischemia for 1 h followed by reperfusion for 45 min. I/R-L-Arg group (G3, n = 10): after performing the same surgical procedure as in group 2, animals were treated with L-Arg. Liver tissues were taken for determination of malondialdehyde (MDA) levels, and biochemical and histological evaluations were made. RESULTS: Plasma alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase (LDH) and γ-glutamyltranspeptidase (GGT) activities were higher in group 2 than in group 3. MDA values and the hepatic injury score decreased in the L-Arg treated group compared to the I/R-untreated group. In group 2, the hepatocytes were swollen with marked vacuolization. Group 3 rats showed well-preserved liver parenchyma, with hepatocytes extending from the central vein. The morphology of the hepatocytes and the sinusoidal structures was normal, without any signs of congestion. Mild Shh positive immunostaining was detected in group 2 animals. The expression of immunoreactive cells was increased markedly in liver tissue from I/R-L-Arg ***: Our findings suggest that Shh molecules are critical factors in the pathophysiology of inflammatory liver injury induced by I/R. In addition, NO plays an important role in the immunohistochemical expression of these molecules.
