类风湿关节炎合并心血管病高危风险患者预后不良的影响因素的预测模型

目的探讨类风湿关节炎(RA)合并心血管病高危风险患者预后不良的影响因素,构建并验证风险预测模型。方法回顾性分析2021年1月至2023年1月于河北医科大学第二医院接受治疗的195例RA合并心血管病高危风险患者的病历资料。依据80/20定律随机分为训练集(156例)和验证集(39例)。根据RA患者心血管疾病预后结局分为预后良好组和预后不良组,筛查RA患者发生心血管疾病的影响因素,构建并验证RA患者发生心血管疾病的风险预测模型。结果156例RA合并心血管病高危风险患者中,发生心血管疾病44例,发生率为21.21%(44例/156例)。预后不良组胰岛素抵抗占比、血脂异常占比、C反应蛋白(CRP)、红细胞沉降率(ESR)、颈动脉内膜-中层厚度(CIMT)高于预后良好组,托珠单抗使用占比低于预后良好组(P<0.05)。胰岛素抵抗(OR=4.043,95%CI:1.243~13.151)、血脂异常(OR=3.329,95%CI:1.398~7.934)、CRP(OR=3.059,95%CI:1.284~7.288)、ESR(OR=3.340,95%CI:1.402~7.959)、CIMT(OR=3.819,95%CI:1.603~9.099)是RA合并心血管病高危风险患者发生心血管疾病的危险因素(P<0.05),使用托珠单抗(OR=0.281,95%CI:0.118~0.668)是RA合并心血管病高危风险患者发生心血管疾病的保护因素(P<0.05)。列线图模型预测训练集RA合并心血管病高危风险患者发生心血管疾病的灵敏度为0.799(95%CI:0.726~0.921),特异度为0.895(95%CI:0.803~0.972),曲线下面积为0.906(95%CI:0.813~0.991)。列线图模型预测验证集RA合并心血管病高危风险患者发生心血管疾病的灵敏度为0.781(95%CI:0.725~0.919),特异度为0.863(95%CI:0.784~0.935),曲线下面积为0.885(95%CI:0.816~0.947)。结论胰岛素抵抗、血脂异常、CRP、ESR、CIMT、使用托珠单抗与RA合并心血管病高危风险患者发生心血管疾病有关,构建风险预测模型有助于早期筛查RA心血管疾病风险。

Metal additive manufacturing(AM),as a disruptive technology in the feld of fabricating metallic parts,has shown its ability to design component with macrostructural ***,some of these functionally complex structures typically contain a wide range of feature sizes,namely,the characteristic length of elements in AM-produced components can vary from millimeter to *** requisite for controlling performance covers nearly six orders of magnitude,from the microstructure to macro scale *** the mechanical variation with the feature size is of critical importance for topology optimization engineers to make required design *** this work,laser metal deposition(LMD)is adopted to manufacture 316L stainless steel(SS)*** evaluate the efect of defects and specimen size on mechanical properties of LMD-produced samples,fve rectangular sample sizes which ranged from non-standard miniature size to ASTM standard sub-sized samples were machined from the *** test reveals that the mechanical properties including yield strength(YS),ultimate tensile strength(UTS),and elongation to failure(εf)are almost the identical for samples with ASTM standard ***,relatively lower YS and UTS values,except forεf,are observed for samples with a miniature size compared with that of ASTM standard ***εf values of LMD-produced 316L SS samples show a more complex trend with sample size,and are afected by three key infuencing factors,namely,slimness ratio,cluster of pores,and occupancy location of lack of fusion *** general,theεf values exhibit a decreasing trend with the increase of slimness *** characterization reveals that the LMD-produced 316L samples exhibited a high stress status at low angle grain boundaries,whilst its location changed to high angle grain boundaries after plastic *** grain size refnement and austenite-to-martensite phase transformation occurred during plastic deformatio

目的探讨替格瑞洛对不稳定型心绞痛患者经皮冠状动脉介入术(PCI)围手术期血小板反应性及短期预后的影响。方法入选不稳定型心绞痛患者424例,随机分为2组:(1)替格瑞洛组(n=212):给予替格瑞洛治疗(负荷剂量180 mg,维持剂量90 mg,每天2次,口服);(2)氯吡格雷组(n=212):给予氯吡格雷治疗(负荷剂量300 mg,维持剂量75 mg,每天1次,口服)。2组均成功接受PCI术,支架均选择国产Firebird雷帕霉素药物洗脱支架。观察2组患者PCI术围手术期血小板反应性及心肌损伤标志物心肌肌钙蛋白I(cTnI)及PCI术后90天不良事件发生情况。结果2组患者PCI术后cTnI水平比较差异无统计学意义(P>0.05)。替格瑞洛组患者花生四烯酸及二磷酸腺苷诱导的血小板聚集率均低于氯吡格雷组(P0.05)。结论替格瑞洛并不降低PCI术后心肌损伤的发生,但较氯吡格雷能发挥更强的抗血小板作用,减少术后再发心肌缺血事件发生率,并不增加出血风险。

A new implementation of on-the-fly trajectory surface hopping(TSH)approach based on the improved Zhu-Nakamura formula is present in this paper[1].Employing this method photo-induced ring opening reaction of 1,3-cyclohexadiene(CHD)to 1,3,5-hexatriene(HT)has been investigated,involving the energy and gradient calculations of S02 at the level of three state average complete active space self consistent field(SA3-CASSCF)with the 6-31G*basis *** calculated results show that there are six reaction mechanisms for the opening reaction,corresponding to different S0photoproducts of CHD,all-cis-HT and cis-trans-cis-HT and so *** excited state lifetime before the crossing between S1 and S2 is about 80 fs and the S1 lifetime after this crossing is more than 220fs,which is in general consistent with the previously experimental and computational *** quantum yield 94%of HT in our calculations is in good agreement with the experimentally complete reaction in gas phase[2]and different from 41%in condensed phaseand 50%of the previous dynamics[3].With the photoproduct distributions,the large molecular deformations are proved to be significant into this *** new ZN method will be a simple and feasible tool to study nonadiabatic molecular dynamics of different size molecules.

联合国教科文组织(UNESCO)长期利用线上和线下多个渠道,联合成员国政府、专业机构及其他相关力量致力于保护语言多样性。本文系统地梳理了UNESCO保护语言多样性的行动与举措,发现该组织不但扩展了语言多样性的保护范围和传播渠道,还逐渐将工作重心转移到保护项目的本地化和少数语言的可持续发展上,但同时也在政策落实及成效等方面存在问题,这表明国际组织职能与国家功能之间的协同与张力始终同时存在。这些发现为相关组织、国家、地区乃至全世界的语言多样性保护工作提供了重要的参考与借鉴。
