Extramedullary plasmacytoma with the uvula as first affected site:A case report

BACKGROUND Extramedullary plasmacytoma(EMP)represents one of the rarer forms of plasma cell malignancies,capable of impacting a variety of tissues and organs throughout the *** majority of EMP cases are predominantly found in the head and neck region,especially within the laryngopharynx,as well as in the gastrointestinal *** there have been documented instances of oropharyngeal involvement in EMP cases in the academic literature,it is important to note that EMP specifically affecting the uvula is exceedingly ***,it is noteworthy that over 60% of epithelial carcinomas in the upper respiratory tract and oropharynx tend to metastasize to the cervical lymph nodes,indicating a propensity for regional spread in these types of *** this context,we present a rare case of extramedullary plasmacytoma where the uvula served as the initially affected *** case emphasizes the need for heightened awareness among clinicians regarding such unusual comorbidities,as early recognition and diagnosis can significantly influence patient management and treatment *** addition,a review of the relevant literature is included to further educate and inform healthcare professionals about this rare presentation,ultimately aiming to enhance clinical understanding and improve patient care in similar *** SUMMARY A 51-year-old man was admitted to our hospital because of a slowly enlarging neck mass.A physical examination revealed a palpable left lymph node,and magnetic resonance imaging(MRI)of the oropharynx and the neck showed a soft tissue mass in the oropharynx and enlargement of multiple lymph nodes in the *** soft tissue mass was diagnosed as plasmacytoma by immunohistochemical *** immunoglobulins and bone marrow biopsy showed normal ***,we diagnosed that as EMP of the *** four cycles of adjuvant chemotherapy dominated by bortezomib,MRI reexamination showed a significant reduction of the mass

MLP training in a self-organizing state space model using unscented Kalman particle filter

Many Bayesian learning approaches to the multi-layer perceptron （MLP） parameter optimization have been proposed such as the extended Kalman filter （EKF）. This paper uses the unscented Kalman particle filter （UPF） to train the MLP in a self- organizing state space （SOSS） model. This involves forming augmented state vectors consisting of all parameters （the weights of the MLP） and outputs. The UPF is used to sequentially update the true system states and high dimensional parameters that are inherent to the SOSS moder for the MLP simultaneously. Simulation results show that the new method performs better than traditional optimization methods.

Burden of mental disorders and risk factors in the Western Pacific region from 1990 to 2021

BACKGROUND The burden of mental disorders(MD)in the Western Pacific Region(WPR)re-mains a critical public health concern,with substantial variations across demogra-phics and *** To analyze the burden of MD in the WPR from 1990 to 2021,along with associated risk factors,to reveal changing trends and emerging *** We used data from the Global Burden of Disease 2021,analyzing prevalence,incidence,and disability-adjusted life years(DALYs)of MD from 1990 to *** methods included age-standardisation and uncertainty analysis to address variations in population structure and data *** Between 1990 and 2021,the prevalence of MD rose from 174.40 million cases[95%uncertainty interval(UI):160.17-189.84]to 234.90 million cases(95%UI:219.04-252.50),with corresponding DALYs increasing from 22.8 million(95%UI:17.22-28.79)to 32.07 million(95%UI:24.50-40.68).During this period,the burden of MD shifted towards older age *** and anxiety disorders were predominant,with females showing higher DALYs for depressive and anxiety disorders,and males more affected by conduct disorders,attention-deficit hyperactivity disorder,and autism spectrum ***,New Zealand,and Malaysia reported the highest burdens,whereas Vietnam,China,and Brunei Darussalam reported the ***,childhood sexual abuse and bullying,and intimate partner violence emerged as significant risk *** This study highlights the significant burden of MD in the WPR,with variations by age,gender,and *** coronavirus disease 2019 pandemic has exacerbated the situation,emphasizing the need for a coordinated response.

Multifunctional carbon materials from rugose rose for energy storage and water purification

With the advancement of social process,the resource problem is becoming more prominent,biomass materials come into being,and it is becoming more and more important to explore and prepare efficient and multifunctional biomass materials to alleviate the problems of energy storage and water *** this paper,nitrogen-doped hierarchical porous carbon materials(NRRC)were produced by one-step carbonization of withered rose as raw material and melamine as nitrogen source with KOH-activated *** resulting nitrogen-doped porous carbon material had the most abundant pores and the best microspherical graded pore structure,with a specific surface area of up to 1393 m2·g^(-1),a pore volume of 0.68 cm3·g^(-1),and a nitrogen-doped content of 5.52%.Electrochemical tests showed that the maximum specific capacitance of NRRC in the three-electrode system was 346.4 F·g^(-1)(0.5 A·g^(-1)),which was combined with favorable capacitance retention performance and cycling *** NRRC//NRRC symmetric supercapacitors were further assembled,and the maximum energy density of a single device was 23.88 Wh·kg^(-1),which still maintains excellent capacitance retention and cyclic charging/discharging *** example,the capacitance retention rate was always close to 96.27%with almost negligible capacitance loss after 10000 consecutive charge/discharge cycles(current density:10 A·g^(-1)).Regardless of the three-electrode or two-electrode system,the super capacitive performance of NRRC porous carbon materials was comparable to the electrochemical performance of many reported biomass porous carbon materials,which showed better energy storage advantages and practical application *** addition,NRRC porous carbon materials had excellent water purification *** dye adsorption test confirmed that NRRC had a high adsorption capacity(491.47 mg·g^(-1))for methylene *** undoubtedly also showed a potential and promising avenue for high value-added utilization

NK cells in liver homeostasis and viral hepatitis

As an important member of the innate immune system, natural killer(NK) cells are well known for their rapid and efficient immune responses against infectious agents and tumors. NK cells are widely distributed throughout the body and are particularly enriched within the liver, where they display unique phenotypic and functional properties, playing important roles in various liver diseases. Herein, we present an overview of liver NK cell properties with regard to phenotype, function, and subset composition at steady state, and we also summarize the complex reciprocal interactions between liver NK cells and other cell types within the local environment of the liver. We also provide an overview of recent advances demonstrating the roles of NK cells in viral hepatitis, including a discussion of NK cell altered states and their beneficial versus harmful effects during hepatitis B virus and hepatitis C virus infection.

Multi-Level Parallel Network for Brain Tumor Segmentation

Accurate automatic segmentation of gliomas in various sub-regions,including peritumoral edema,necrotic core,and enhancing and non-enhancing tumor core from 3D multimodal MRI images,is challenging because of its highly heterogeneous appearance and *** convolution neural networks(CNNs)have recently improved glioma segmentation ***,extensive down-sampling such as pooling or stridden convolution in CNNs significantly decreases the initial image resolution,resulting in the loss of accurate spatial and object parts information,especially information on the small sub-region tumors,affecting segmentation ***,this paper proposes a novel multi-level parallel network comprising three different level parallel subnetworks to fully use low-level,mid-level,and high-level information and improve the performance of brain tumor *** also introduce the Combo loss function to address input class imbalance and false positives and negatives imbalance in deep *** proposed method is trained and validated on the BraTS 2020 training and validation *** the validation dataset,ourmethod achieved a mean Dice score of 0.907,0.830,and 0.787 for the whole tumor,tumor core,and enhancing tumor core,*** with state-of-the-art methods,the multi-level parallel network has achieved competitive results on the validation dataset.

Nature killer （NK） cells are important lymphocytes of the innate immune system, well known for their pivotal roles in immune surveillance and defense against tumor and virus-infected cells. Current studies have revealed that NK cells are not a homogeneous population, but instead consist of distinct subsets with diverse characteristics. As an organ with predominant innate immunity, the liver is enriched with NK cells, among which two distinct NK cell subsets have recently been identified： conventional NK （cNK） ceils and tissue-resident NK （trNK） ceils. Liver trNK cells are markedly different from cNK cells in many aspects, representing a new lineage of innate lymphoid cell （ILC） family. Here, we summarize the phenotypic and functional features of liver trNK cells, and review current knowledge regarding developmental pathway of liver trNK cells. We also overview recent advances in human liver trNK cells and discuss the striking shared hallmarks of trNK cells in different tissues.
