Objective:This cross-sectional study aimed to determine the epidemiology of olfactory and gustatory dysfunctions related to COVID-19 in ***:This study was conducted by 45 tertiary Grade-A hospitals in *** and offline questionnaire data were obtained from patients infected with COVID-19 between December 28,2022,and February 21,*** collected information included basic demographics,medical history,smoking and drinking history,vaccination history,changes in olfactory and gustatory functions before and after infection,and other postinfection symptoms,as well as the duration and improvement status of olfactory and gustatory ***:Complete questionnaires were obtained from 35,566 *** overall incidence of olfactory and taste dysfunction was 67.75%.Being female or being a cigarette smoker increased the likelihood of developing olfactory and taste *** received four doses of the vaccine or having good oral health or being a alcohol drinker decreased the risk of such *** infection,the average olfactory and taste VAS scores were 8.41 and 8.51,respectively;after infection,they decreased to 3.69 and 4.29 and recovered to 5.83 and 6.55 by the time of the *** median duration of dysosmia and dysgeusia was 15 and 12 days,respectively,with 0.5%of patients having symptoms lasting for more than 28 *** overall self-reported improvement rate was 59.16%.Recovery was higher in males,never smokers,those who received two or three vaccine doses,and those that had never experienced dental health issues,or chronic accompanying ***:The incidence of dysosmia and dysgeusia following infection with the SARS-CoV-2 virus is high in *** and prognosis are influenced by several factors,including sex,SARS-CoV-2 vaccination,history of head-facial trauma,nasal and oral health status,smoking and drinking history,and the persistence of accompanying symptoms.

We report the discovery of a peculiar X-ray transient,EP240408a,by Einstein Probe （EP） and follow-up studies made with EP,Swift,NICER,GROND,ATCA and other ground-based multiwavelength *** new transient was first detected with Wide-field X-ray Telescope （WXT） on board EP on April 8th,2024,manifested in an intense yet brief X-ray flare lasting for *** flare reached a peak flux of 3.9×10-9erg cm-2s-1in 0.5-4 keV,300 times brighter than the underlying X-ray emission detected throughout the *** and more precise follow-up observations by EP/FXT,Swift and NICER confirmed the finding of this new *** X-ray spectrum is non-thermal in 0.5-10 keV,with a power-law photon index varying *** X-ray light curve shows a plateau lasting for4d,followed by a steep decay till becoming undetectable10 d after the initial *** on its temporal property and constraints from previous EP observations,an unusual timescale in the range of 7-23 d is found for EP240408a,which is intermediate between the commonly found fast and long-term *** counterparts have been found in optical and near-infrared,with the earliest observation at 17 h after the initial X-ray detection,suggestive of intrinsically weak emission in these *** demonstrate that the remarkable properties of EP240408a are inconsistent with any of the transient types known so far,by comparison with,in particular,jetted tidal disruption events,gamma-ray bursts,X-ray binaries and fast blue optical *** nature of EP240408a thus remains an *** suggest that EP240408a may represent a new type of transients with intermediate timescales of the order of10 *** detection and follow-ups of more of such objects are essential for revealing their origin.

目的探讨系统免疫炎症指数(systemic immune-inflammation index,SII)与2型糖尿病(type 2 diabetes mellitus,T2DM)患者下肢血管病变的关系。方法采用横断面研究设计,收集2013年1月至2024年1月在我科诊治的390例T2DM患者资料,根据T2DM下肢血管病变诊断标准分为T2DM有下肢血管病变组(n=158)和无下肢血管病变对照组(n=232),分析2组之间的一般资料、实验室检查结果等差异。采用Spearman相关分析研究与T2DM下肢血管病变的相关因素。采用Row Mean Scores和Cochran-Armitage Trend检验研究SII与T2DM下肢血管病变的关系。采用多因素“向前:LR”Logistic回归分析研究T2DM下肢血管病变的危险因素。采用受试者工作特征曲线评估SII对T2DM下肢血管病变的诊断效能。结果与无下肢血管病变的T2DM患者相比,T2DM下肢血管病变患者的年龄较大,总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)和SII水平较高,颈动脉血管病变的比例增加,未服用他汀类药物占比增加。下肢血管病变与SII/100(r=0.429,P<0.001)、年龄(r=0.517,P<0.001)、TC(r=0.161,P=0.001)、LDL-C(r=0.117,P=0.021)、颈动脉病变(r=0.101,P=0.047)、未服用他汀类药物(r=0.266,P<0.001)呈正相关。Logistic回归分析显示,SII、年龄、LDL-C、未服用他汀类药物是T2DM下肢血管病变的危险因素(P<0.01)。SII联合年龄、LDL-C和未服用他汀类药物预测T2DM下肢血管病变的曲线下面积为0.896。结论SII不仅是T2DM下肢血管病变的危险因素,也是反映T2DM下肢血管病变的简便标志物,提示炎症反应对T2DM的下肢血管病变发生发展具有重要作用。

基于TLR4/NF-κB/NLRP3信号通路探究白芍总苷对自身免疫性甲状腺炎大鼠炎症损伤的影响

目的探讨白芍总苷对自身免疫性甲状腺炎(AIT)大鼠炎症损伤及TLR4/NF-κB/NLRP3通路的影响。方法实验分为对照组(Control)、模型组(Model)、白芍总苷组(TGP)、TLR4抑制剂组(TLR4 inhibitor)和TGP+TLR4激动剂组(TGP+TLR4 agonist),每组各10只。除Control组外,其余各组大鼠采用皮下注射甲状腺球蛋白与弗氏佐剂诱导AIT大鼠模型。给药6周后,苏木精-伊红(HE)染色观察甲状腺组织病理学变化;酶联免疫吸附法(EliSA)测定血清TPOAb、TgAb、TSH、T3、T4、TNF-α、INF-γ、IL-1β、IL-18水平;RT-qPCR和Western blot检测甲状腺组织TLR4/NF-κB/NLRP3信号通路mRNA和蛋白表达。结果与Control组相比,Model组大鼠甲状腺滤泡上皮明显受损,TPOAb、TgAb、TSH、T3、T4、TNF-α、INF-γ、IL-1β、IL-18水平升高(P<0.01),TLR4/NF-κB/NLRP3信号通路mRNA和蛋白表达升高(P<0.01)。与Model组相比,TGP组、TLR4 inhibitor组大鼠甲状腺滤泡上皮损伤减轻,TPOAb、TgAb、TSH、T3、T4、TNF-α、INF-γ、IL-1β、IL-18水平降低(P<0.01),TLR4/NF-κB/NLRP3信号通路mRNA和蛋白表达降低(P<0.01)。与TGP组相比,TGP+TLR4 agonist组大鼠甲状腺滤泡上皮损伤加重,TPOAb、TgAb、TSH、T3、T4、TNF-α、INF-γ、IL-1β、IL-18水平升高(P<0.05或P<0.01),TLR4/NF-κB/NLRP3信号通路mRNA和蛋白表达增加(P<0.05或P<0.01)。结论TGP通过抑制TLR4/NF-κB/NLRP3信号通路,改善甲状腺组织炎症损伤发挥甲状腺保护作用。

Objective:Real-world diagnostic and treatment data for pancreatic cancer in China are *** such,the present study investigated the clinical characteristics,diagnosis,and treatment of advanced pancreatic cancer(including locally advanced and metastatic disease)in the Hospital-based Advanced Pancreatic Cancer Cohort in China of the China Pancreas Data Center ***:A total of 5349 Chinese patients with advanced pancreatic cancer were identified from a *** entire course of real-world pancreatic cancer management was ***:The proportion of patients with advanced pancreatic cancer was higher among males than females(62.4%vs 37.6%,respectively).Patients typically had a history of hypertension(30.8%),diabetes(21.6%),and cholangitis(20.2%).Abdominal pain(51.6%),abdominal distension(27.1%),jaundice(20.1%),and weight loss(16.3%)were the main symptoms observed in patients with advanced pancreatic cancer in this *** carbohydrate antigen(CA)19-9 is one of the most common tumor *** the present study,2562 patients underwent first-line *** median progression-free survival(PFS)for patients undergoing first-line therapy was 4.1 *** major options for first-line therapy included gemcitabine(GEM)plus S-1(GS/X)(23.4%),nab-paclitaxel plus GEM(AG)(18.1%),oxaliplatin,irinotecan,and leucovorin-modulated fluorouracil(FOLFIRINOX;11.9%),nab-paclitaxel plus S-1(AS)(8.9%),and GEM combined with oxaliplatin/cisplatin(GEMOX/GP)(7.6%).The AS and GS/X regimens were associated with the highest PFS ***:This is the first study to report multicenter,real-world data regarding advanced pancreatic cancer in *** revealed that real-world treatment options differed from guideline recommendations,and PFS was shorter than that in previously reported *** intelligent follow-up systems and standardizing diagnosis and treatment of pancreatic cancer is recommended.

Multicolor photometry of the galaxy cluster A98: substructures and star formation properties

An optical photometric observation with the Beijing-Arizona-Taiwan- Connecticut （BATC） multicolor system is carried out for A98 （z =0.104）, a galaxy cluster with two large enhancements in X-ray surface brightness. Spectral energy distributions （SEDs） covering 15 intermediate bands are obtained for all sources detected down to V - 20 mag in a field of 58′× 58′. After star-galaxy separation with colorcolor diagrams, a photometric redshift technique is applied to the galaxy sample for further membership determination. The color-magnitude relation is taken as a further restriction of the early-type cluster galaxies. As a result, a list of 198 faint member galaxies is achieved. Based on the newly generated sample of member galaxies, the dynamical substructures, A98N, A98S, and A98W, are investigated in detail. A separate galaxy group, A98X, is also found to the south of the main concentration of A98, which is gravitationally unbound to A98. For 74 spectroscopically confirmed member galaxies, the environmental effect on the star formation history is investigated. The bright galaxies in the core region are found to have shorter time scales of star formation, longer mean stellar ages, and higher interstellar medium metallicities, which can be interpreted in the context of the hierarchical cosmological scenario.

【目的】探究氯化两面针碱(Nitidine chloride,NitC)对热应激水牛乳腺上皮细胞β-酪蛋白含量降低的影响,为实际生产中缓解热应激对水牛乳品质的负面影响提供理论依据。【方法】记录非热应激季节和热应激季节牛舍温湿度指数(Temperature-humidity index,THI),检测水牛呼吸频率和直肠温度以判断水牛是否处于热应激状态;采集奶样进行乳成分分析以探究热应激对水牛奶β-酪蛋白含量的影响;以水牛乳腺上皮细胞为研究对象,经40.5℃处理24 h建立细胞热应激模型,使用liCl和LGK974分别激活和抑制Wnt通路,检测水牛乳腺上皮细胞β-酪蛋白含量及β-酪蛋白合成相关基因和蛋白的表达水平,探究氯化两面针碱缓解热应激水牛乳腺上皮细胞β-酪蛋白含量降低的作用机制。【结果】与非热应激季节相比,热应激季节牛舍THI升高并超过水牛热应激阈值,水牛呼吸频率和直肠温度显著升高(P<0.05,下同),水牛奶β-酪蛋白含量显著降低;细胞试验结果表明,40.5℃处理24 h能显著降低水牛乳腺上皮细胞β-酪蛋白含量、细胞活力及Wnt2、mTOR、β-casein基因和蛋白的相对表达量;而氯化两面针碱处理能有效缓解热应激对水牛乳腺上皮细胞的影响,显著提高水牛乳腺上皮细胞β-酪蛋白含量、细胞活力及Wnt2、mTOR、β-casein基因和蛋白的相对表达量。【结论】热应激降低了水牛乳腺上皮细胞β-酪蛋白含量及β-酪蛋白合成相关基因和蛋白的相对表达量,氯化两面针碱则通过Wnt/mTOR信号通路提高水牛乳腺上皮细胞β-酪蛋白含量、细胞活力及Wnt2、mTOR、β-casein基因和蛋白的相对表达量,以缓解热应激造成的水牛乳腺上皮细胞β-酪蛋白含量降低。

Laparoscopic finding of a hepatic subcapsular spider-like telangiectasis sign in biliary atresia

AIM To assess the diagnostic value of a laparoscopic finding of a hepatic subcapsular spider-like telangiectasis (HSST) sign in biliary atresia. METHODS A retrospective study was conducted first and then a validation set was used to investigate the value of an HSST sign in predicting biliary atresia (BA). In the retrospective study, laparoscopic images of the liver surface were reviewed in 126 patients with infantile cholestasis (72 BA patients and 54 non-BA cholestasis patients) and a control group of 38 patients with nonhepatic conditions. Analysis was first made by two observers separately and finally, a consensus conclusion was achieved. Then, the diagnostic value of the HSST sign was validated in an independent cohort including 45 BA and 45 non-BA patients. RESULTS In the retrospective investigation, an *** HSST sign was found in all BA patients, while we were unable to detect the HSST sign in 98.1% of the 54 non-BA patients. There was no HSST sign in any of the control subjects. In the first review, the sensitivity and specificity from one reviewer were 100% and 98.1%, respectively, and the results from the other reviewer were both 100%. The consensus sensitivity and specificity were 100% and 98.1%, respectively. The HSST sign was defined as being composed of several enlarged tortuous spider-like vascular plexuses with two to eight branches distributed on all over the liver surface, which presented as either a concentrated type or a dispersed type. In the independent validation group, the sensitivity, specificity, positive predictive value and negative predictive value of the HSST sign were 100%, 97.8%, 97.8% and 100%, respectively. CONCLUSION The HSST sign is characteristic in BA, and laparoscopic exploration for the HSST sign is valuable in the diagnosis of BA.

【目的】探究相思子碱(Abrine)通过调节P53/mTOR通路缓解脂多糖(lipopolysaccharide,LPS)对水牛乳腺上皮细胞酪蛋白合成的影响,为缓解乳房炎引起的水牛乳品质下降提供理论依据。【方法】使用脂多糖构建水牛乳腺上皮细胞炎症模型,同时使用相思子碱、P53抑制剂(Pifithrin-μ)、P53激活剂(Kevetrin hydrochloride)与水牛乳腺上皮细胞共同孵育12 h;通过HE染色观察水牛乳腺组织形态,采用CCK-8法检测细胞活性,使用EliSA试剂盒检测水牛乳腺上皮细胞β-酪蛋白分泌水平,利用实时荧光定量PCR测定NF-κB、IL-1β、TNFα、β-casein、mTOR、P53、JAK2、STAT5和AKT1基因相对表达量,并以Western blotting检测NF-κB、IL-1β、TNFα、β-casein、mTOR和P53蛋白相对表达量。【结果】临床型乳房炎乳腺组织间隙及腺泡腔被大量中性粒细胞浸润,部分腺泡结构呈现一定程度的萎缩。经1.0μg/mL脂多糖处理,水牛乳腺上皮细胞活力显著降低(P<0.05,下同),2.0、4.0和8.0μg/mL脂多糖处理,水牛乳腺上皮细胞活力极显著降低(P<0.001),25、50、100和200μmol/L相思子碱处理能缓解脂多糖的影响,水牛乳腺上皮细胞活力明显升高。与空白对照组相比,脂多糖处理组水牛乳腺上皮细胞NF-κB、IL-1β、TNFα和P53基因和蛋白相对表达量显著升高,β-酪蛋白表达水平显著降低;与脂多糖组相比,相思子碱+脂多糖处理组水牛乳腺上皮细胞β-酪蛋白水平显著升高,NF-κB、IL-1β、TNFα、P53基因和蛋白相对表达量显著降低;与脂多糖组相比,脂多糖+Pifithrin-μ组水牛乳腺上皮细胞β-酪蛋白表达水平显著升高,与相思子碱组相比,相思子碱+Kevetrin hydrochloride组水牛乳腺上皮细胞β-酪蛋白表达水平显著降低。【结论】脂多糖降低了水牛乳腺上皮细胞β-酪蛋白合成水平与β-酪蛋白合成相关基因及其编码蛋白的表达水平;相思子碱能通过抑制NF-κB通路缓解脂多糖诱导的水牛乳腺上皮细胞炎症,并通过调节P53/mTOR通路缓解脂多糖诱导的水牛乳腺上皮细胞β-酪蛋白合成减少。
